âIslands in the streamâ â causeways or compromise?
In recent years, policy drivers have given a strategic push towards encouraging âemployer-ledâ work based learning in Higher Education. For example, Leitch ( 2006?) and other key policy makers advocate institutional change and reform in HE to respond to market needs; HEFCE encourages HEIâs âTowards a strategy for work based learningâ; the QAA has reflected most recently on âemployer-responsive provisionâ. This paper sets out to explore the impact of these strategic objectives and some issues which emerge from the rapprochement of stakeholders and providers. It is based on experience in an institution where challenges and tensions are being met and overcome. The case example is part of a Higher Level Skills Pathway (HLSP) Project whose lead partner is the North West Universities Association (NWUA) in North West England. Learning Pathway provision for Housing Practitioners (via a Professional Certificate in Leadership) has been developed in conjunction with employers using the WBIS (Work based and Integrative Studies) framework at the University of Chester. This flexible modular framework puts knowledge and experiential learning gained in the work context at the core of learning activity. This paper uses the example to characterise the power relationships and tensions. Reflecting on the case study, it seems that by attending to such policy drivers, much compromise is required from both parties in terms of curriculum design and the relationships being built between Higher Education Institutions (HEIâs) and employers. The term âemployer-ledâ denotes an uneven power relationship and this may in the long run serve to undermine the hallmark of HE provision â quality and standards. In conclusion we suggest that the whole relationship needs to be predicated on co-produced provision in order to build sustainable relationships between employers and HEIâs. The term âco-productionâ equalises the power relationship, encouraging the goal of dynamic interaction, mutual respect and benefits based on the expertise and knowledge of each party

Alignment apparatus using a laser having a gravitationally sensitive cavity reflector
A description is given of a device for determining a true gravitational vertical. The apparatus is composed of a vertically disposed laser with a gravitationally sensitive lower regeneration reflector. The reflector enables an output beam to be developed only when the optical axis of the laser is disposed normal to the gravitationally sensitive reflective surface. In an alternative embodiment, the devices is combined with a servo system to provide a gravitationally stabilized horizontal platform
Functional Assessment of Heart Failure Patients
Heart failure (HF) is the condition characterized by the inability of the heart to pump sufficient blood to meet the demands of the body. It has been well established that both the prevalence and incidence of HF is increasing.1 There are 2 primary types of HF, categorized by ejection fraction: Reduced ejection fraction and preserved ejection fraction.2 Additionally, HF is commonly classified into stages from mild to severe using a symptom-based scale related to functional limitations.
One of the hallmark features of HF is exercise intolerance, which is accompanied by symptoms of fatigue and shortness of breath.3 As the disease progresses, patients experience a downward spiral as these symptoms typically result in reduced physical activity, which leads to progressively worsening exercise intolerance. Typically, patients with HF are faced with what can be termed a functional disability. Often, their reduced functional abilities restrict or may even prevent them from performing occupational tasks, which may result in loss of work. Additionally, it is well known that patients with HF experience impairment in the ability to carry out activities of daily living and suffer from reduced quality of life.
The objective of this paper was to provide an overview of assessments of functional ability of patients with HF. Two categories of assessment are reviewed: Cardiovascular function and muscular function. The review includes procedural guidance on how to administer the assessments and information related to the advantages and disadvantages of each method. Because both HF types (reduced ejection fraction and preserved ejection fraction) are characterized by exercise intolerance, the procedures can be used effectively with either type of HF
