Effect of spray air settings of speed-increasing contra-angle handpieces on intrapulpal temperatures, drilling times, and coolant spray pattern
OBJECTIVES: Decreasing aerosol leaks are of great interest, especially in the recent era of COVID-19. The aim was to investigate intrapulpal heat development, coolant spray patterns, and the preparation efficiency of speed-increasing contra-angle handpieces with the spray air on (mist) or off (water jet) settings during restorative cavity preparations. METHODS: Standard-sized cavities were prepared in 80 extracted intact human molar teeth using diamond cylindrical drills with a 1:5 speed-increasing contra-angle handpiece. A custom-made device maintained the standardized lateral drilling force (3 N) and predetermined depth. Temperatures were measured using intrapulpal thermocouple probes. The four experimental groups were as follows: mist cooling mode at 15 mL/min (AIR15), water jet cooling mode at 15 mL/min (JET15), mist cooling mode at 30 mL/min (AIR30), and water jet cooling mode at 30 mL/min (JET30). The coolant spray pattern was captured using macro-photo imaging. RESULTS: The JET15 group had the highest increase in temperature (ΔT = 6.02 °C), while JET30 (ΔT = 2.24 °C; p < 0.001), AIR15 (ΔT = 3.34 °C; p = 0.042), and AIR30 (ΔT = 2.95 °C; p = 0.003) had significantly lower increases in temperature. Fine mist aerosol was formed in the AIR15 and AIR30 preparations but not in the JET15 and JET30 preparations (p < 0.001). The irrigation mode had no influence on the preparation time (p = 0.672). CONCLUSIONS: Water jet irrigation using coolant at 30 mL/min appeared to be the optimal mode. Considering the safe intrapulpal temperatures and the absence of fine mist aerosols, this mode can be recommended for restorative cavity preparations. CLINICAL SIGNIFICANCE: To increase infection control in dental practices, the water jet irrigation mode of speed-increasing handpieces with coolant flow rates of 30 mL/min should be considered for restorative cavity preparations. Absztrakt:
Az alsó bölcsességfogak coronectomiája a bölcsességfog részleges eltávolításával
hivatott az esetleges nervus alveolaris inferior sérüléseit kiküszöbölni. Az
eljárásnak van néhány sarkalatos pontja, úgymint a fog pulpalis, periapicalis
preoperatív állapota, a koronai sectio és koronaeltávolítás, illetve a sebzárás
mikéntje, valamint a páciens posztoperatív követése és az esetlegesen felmerülő
szövődmények ismerete és kezelése. Szakirodalmi adatok szerint a coronectomia
kevesebb szövődménnyel bír, mint a hagyományos, teljes fogeltávolítások, és a
nervus alveolaris inferior sérülések szempontjából a coronectomia egyértelműen
és kifejezetten a biztonságosabb eljárás. A szerzők célja volt a szakirodalmat
áttekintve, az eljárás indikációiról és kontraindikációiról, korrekt
kivitelezéséről és az esetleges sikertelenségének okairól, gyakoriságairól

Abstract:
Coronectomy of lower impacted wisdom teeth is the partial removal of third
molars, aiming to avoid inferior alveolar nerve injuries. The coronectomy
procedure has several crucial points, such as the pulpal, periapical
preoperative conditions of the tooth, the way of crown sectioning and
decoronation, the wound closure and the length of patients’ follow up. In
addition it is important to identify and manage possible intraoperative or
postoperative complications correctly. According to the literature, the
complication rate of coronectomy is usually lower, than that of total teeth
removal, furthermore considering inferior alveolar nerve injuries, coronectomy
is significantly the safer procedure. The aim of the authors was to review the
relevant literature, defining the indications, contraindications and the correct
implementation of the coronectomy and to demonstrate the causes and incidences
of failures and complications. Further aim was to promote the domestic

A jellegzetes panorámaröntgen-jelek szerepe a nervus alveolaris inferior sérülések várható megítélésében alsó bölcsességfogak eltávolításakor = The specific panoramic radiographic signs and their relation with inferior alveolar nerve injuries after mandibular third molar surgery
Az impaktált alsó bölcsességfogak műtéti eltávolításakor fellépő egyik legkevésbé kívánatos szövődmény a nervus
alveolaris inferior sérülése. A szerzők célja volt, hogy bemutassák a jellegzetes (ún. klasszikus) panorámaröntgenjeleket, illetve felhívják a figyelmet azoknak a nervus alveolaris inferior (IAN) sérülésekkel való összefüggéseire. Korábbi esetkontroll-vizsgálat során szerzett adatok alapján bemutatják az egyes jelek prediktív értékét (ún. „jóslóerejét”) valamint egy esetismertetés kapcsán a röntgendiagnosztikai és a műtéti megfigyelések kapcsolatát. A szerzők véleménye szerint a műtétet megelőző panorámaröntgen mindenképpen fontos rutin diagnosztikai eljárás, és a jellegzetes panorámaröntgen-jelek alapján nagy segítség a fokozott kockázatú esetek szűrésében, illetve a további 3 dimenziós képalkotó eljárások indikációs körének meghatározásában
Intrapulpal temperature changes during the cementation of ceramic veneers
Adhesive cementation of ceramic veneers may increase pulpal temperature (PT) due to the combined
efect of heat generated by the curing unit and the exothermic reaction of the luting agent (LA).
PT increase may induce pulpal damage. The aim was to determine the PT rise during the luting of
ceramic veneers (CV) of diferent thicknesses with light- or dual-curing (LC, DC) adhesive cements as
well as pre-heated restorative resin-based composites (PH-RBC). For this a thermocouple sensor was
positioned in the pulp chamber of a prepared maxillary central incisor. LC, DC adhesive cements and
PH-RBCs heated to 55 °C were used for the luting of CVs of 0.3, 0.5, 0.7, and 1.0 mm thicknesses. The
exothermic reaction of LAs added signifcantly to the thermal efect of the curing unit. PT change
ranged between 8.12 and 14.4 °C with the investigated combinations of LAs and ceramic thicknesses
(p≤ 0.01). The increase was inversely proportional to the increasing CV thicknesses. The highest rise
(p≤ 0.01) was seen with the polymerization of PH-RBCs. Temperature changes were predominantly
infuenced by the composition of the LA, which was followed by CV thickness
