Forecasting of sports fields construction costs aided by ensembles of neural networks
The paper presents an original approach to construction cost analysis and development of predictive models based on ensembles of artificial neural networks. The research was focused on the application of two alternative approaches of ensemble averaging that allow for combining a number of multilayer perceptron neural networks and developing effective models for cost predictions. The models have been developed for the purpose of forecasting construction costs of sports fields as a specific type of construction objects. The research included simulation and selection of numerous neural networks that became the members of the ensembles. The ensembles included either the networks of different types in terms of their structure and activation functions or the networks of the same type. The research also included practical implementation of the developed models for cost analysis based on a sports field BIM model. This case study examined and confirmed all of the four models’ predictive capabilities and superiority over models based on single networks for the particular problem. Verification including testing and the case study enabled selection of the best ensemble-based model that combined ten networks of different types. The proposed approach is prospective for fast cost analyses and conceptual estimates in construction projects

Anomalous origin of culprit coronary arteries in acute coronary syndromes
Background: The aim of the study was to describe a series of acute coronary syndrome (ACS) patients in whom anomalous origin of culprit coronary artery (AOCCA) was diagnosed. Percutaneous coronary interventions (PCI) in AOCCA are performed very infrequently.
Methods: Electronic databases from three high-volume tertiary cardiac centers were retrospectively searched for the presence of AOCCA in ACS.
Results: Different types of AOCCA in ACS were identified in 20 patients. The most frequent AOCCA was left circumflex coronary artery (LCx) originating from right coronary artery (RCA) or directly from the right coronary sinus (RCS), n = 13, followed by high/atypical RCA, n = 3, left coronary artery (LCA) originating from RCS (n = 3) with either RCA-AOCCA (n = 1) or left anterior descending coronary artery (LAD)-AOCCA (n = 1) or RCA originating from left sinus of Valsalva, (n = 1), LAD originating from RCA (n = 1). In 1 ST-segment elevation myocardial infarction (STEMI)-patient RCA-AOCCA cannulation was unsuccessful, in 1 non-STEMI-patient AOCCA was missed, 1 ACS- -patient was treated surgically and 1 ACS-patient was treated conservatively (both patients with non- STEMI). In the remaining patients PCI was successfully performed.
Conclusions: The most frequently encountered AOCCA is LCx branching-off from RCA. AOCCA may either be difficult to cannulate and PCI aborted even in STEMI, or missed, especially when the intermediate branch from LCA is mimicking proper LCx

NT-proBNP level in the diagnosis of isolated left ventricular diastolic dysfunction in patients with documented coronary artery disease
Background: The diagnostic value of NT-proBNP for left ventricular (LV) systolic dysfunction
is well established. However, its role for diastolic dysfunction (DD) diagnosis in patients
with preserved systolic function has not been clearly defined.
Methods: A total of 83 patients with documented coronary arterial disease following anterior
myocardial infarction and with a left ventricular ejection fraction (LVEF) > 45% were
enrolled. According to echocardiographic mitral inflow and right upper pulmonary vein flow,
DD was excluded in 32 patients (group A). The patients with DD were divided into three
subgroups: B1 - 38 patients with impaired relaxation, B2 - 8 patients with
pseudonormalisation and B3 - 7 patients with restrictive inflow. In all patients E-wave
propagation (Vp) and NT-proBNP were determined.
Results: Mean LVEF was 56.2 ± 9% and did not differ between the subgroups. NT-proBNP
levels were 107 ± 101 pg/ml in group A, 299 ± 281 pg/ml in B1, 734 ± 586 pg/ml in B2
(p < 0.05 vs. A) and 2322 ± 886 pg/ml in B3 (p < 0.01 vs. A and p < 0.01 vs. B2).
Propagation Vp was 69 ± 21 cm/s, 56 ± 20 cm/s, 53 ± 17 cm/s (p < 0.05 vs. A) and 44 ± 11 cm/s
(p < 0.01 vs. A) respectively. A positive correlation was found for DD degree with NT-proBNP
level (r = 0.66; p < 0.001) and negative with Vp (r = –0.41; p < 0.001). ROC curves were
constructed to determine the NT-proBNP level cut-off point for DD (> 131 pg/ml, area under
the curve: 0.63) and advanced restrictive DD (> 1670 pg/ml, area under the curve: 0.83)
diagnosis. Sensitivity, specificity, accuracy and positive and negative predictive values were
71%, 50%, 63%, 69%, 52% and 57%, 99%, 95%, 80%, 96% respectively.
Conclusions: In patients with coronary artery disease and preserved LV systolic function
a single NT-proBNP measurement helps to identify those with isolated DD, especially those with
advanced restriction
Risk factors of asymptomatic restenosis in patients with first anterior ST elevation myocardial infarction treated by primary percutaneous coronary intervention
Background: The issue of predicting coronary artery restenosis, especially silent, in patients following primary percutaneous
coronary intervention (PCI) has been extensively studied, however, risk factors have not been fully defined.
Aim: To asses the frequency of silent restenosis and its predictors in patients with anterior ST elevation myocardial infarction
(STEMI) treated with primary PCI and implantation of bare metal stents (BMS).
Methods: We recruited a cohort of 114 patients with first anterior STEMI treated with primary PCI within 12 hours of the
onset of symptoms, and with the left anterior descending coronary artery occlusion (TIMI 0) and successful flow restoration
(TIMI 3). A 12-lead ECG was performed before and 60 minutes after PCI. Troponin I and CK-MB were measured on admission
and after six, 12 and 24 hours. Transthoracic echocardiography (TTE) was performed at discharge. Resting TTE and
coronary angiography were performed after a six month follow-up in asymptomatic patients.
Results: The frequency of silent restenosis in our study group was 23.9%. The best multivariate models in logistic regression
of restenosis prediction were: lower end-systolic volume of the left ventricle assessed two days after infarction longer lesion
and smaller reference diameter of the stented vessel.
Conclusions: Silent restenosis in patients with first anterior STEMI treated by primary PCI with the use of BMS is still frequent. The
best ways to identify patients with silent restenosis at six month follow-up, apart from the lower end systolic volume in the echocardiographic
study, are longer narrowing in the infarct-related artery and lower reference diameter of the treated vessel.

wieńcowej (PCI), mimo adekwatnej farmakoterapii, problem określenia czynników ryzyka restenozy po skutecznej pierwotnej
PCI jest w ostatnim czasie intensywnie badany. Nawrót zwężenia może powodować objawy kliniczne, jednak u wielu
pacjentów jest bezobjawowy, co znacznie utrudnia diagnostykę i opóźnia wdrożenie właściwego leczenia.
Cel: W niniejszej pracy w 6-miesięcznej obserwacji oceniano częstość i czynniki ryzyka wystąpienia bezobjawowej restenozy
u chorych z pierwszym w życiu zawałem z uniesieniem odcinka ST (STEMI) ściany przedniej, leczonych pierwotną PCI
z implantacją stentów nieuwalniających leku (BMS).
Metody: Do badania włączono 114 osób z pierwszym w życiu STEMI ściany przedniej, z zamknięciem tętnicy przedniej
zstępującej (TIMI 0), leczonych pierwotną PCI w ciągu 12 godzin od początku objawów, skutecznym udrożnieniu tętnicy
(TIMI 3) i implantacją BMS. Badanie EKG wykonywano przed i 60 minut po PCI, wartości troponiny I i CK-MB oznaczano
przy przyjęciu, a następnie po 6, 12 i 24 godzinach. Przezklatkowe badanie echokardiograficzne wykonano 2 dni po zawale
- przed wypisem ze szpitala. Po 6-miesięcznej obserwacji u pacjentów bez objawów klinicznych ponownie wykonano
spoczynkowe przezklatkowe badanie echokardiograficzne i angiografię tętnic wieńcowych.
Wyniki: Częstość występowania bezobjawowej restenozy w badanej grupie pacjentów wynosiła 23,9%. W analizie wieloczynnikowej
metodą logistycznej regresji najlepiej przewidywały nawrót zwężenia w tętnicy dozawałowej (IRA) - niska
końcowoskurczowa objętość lewej komory oceniana 2 dni po zawale, dłuższa blaszka miażdżycowa i mniejsza średnica IRA.
Wnioski: Częstość występowania bezobjawowego nawrotu zwężenia u pacjentów po pierwszym w życiu STEMI ściany
przedniej leczonych skuteczną PCI z użyciem BMS jest wciąż wysoka. Najlepszymi czynnikami predykcyjnymi identyfikującymi
chorych z nawrotem zwężenia, poza niską objętością końcowoskurczową lewej komory w badaniu echokardiograficznym,
jest dłuższe zwężenie i mała średnica IRA.

Wartość diagnostyczna pomiaru NT-proBNP w rozpoznawaniu izolowanej dysfunkcji rozkurczowej lewej komory wśród pacjentów z udokumentowaną chorobą wieńcową
Wstęp: Wartość diagnostyczna pomiaru NT-proBNP w rozpoznawaniu skurczowej dysfunkcji
lewej komory jest powszechnie znana i dobrze udokumentowana. Rola NT-proBNP w diagnostyce
rozkurczowej dysfunkcji (DD) wśród pacjentów z chorobą wieńcową i zachowaną funkcją
skurczową lewej komory jest mniej poznana.
Metody: Do badania zakwalifikowano kolejnych 83 pacjentów z udokumentowaną angiograficznie
chorobą wieńcową, po przebytym zawale serca ściany przedniej i z frakcją wyrzutową
lewej komory większą od 45%. Na podstawie typowych wskaźników napływu mitralnego
i oceny napływu w żyle płucnej prawej górnej podzielono chorych na 2 grupy: grupę A bez DD
- 32 osoby i grupę B z DD, w której wyodrębniono 3 podgrupy: podgrupę B1 z profilem zaburzonej
relaksacji - 38 chorych, podgrupę B2 z profilem pseudonormalizacji - 8 osób oraz
podgrupę B3 z napływem restrykcyjnym - 7 osób. Ponadto u wszystkich pacjentów oznaczono
propagację fali E (Vp) i NT-proBNP.
Wyniki: Frakcja wyrzutowa lewej komory wynosiła średnio 56,2 ± 9% i nie różniła się
znamiennie statystycznie między poszczególnymi podgrupami. Stężenie NT-proBNP było równe
107 ± 101 pg/ml w grupie A, 299 ± 281 pg/ml w B1, 734 ± 586 pg/ml w B2, 2322 ± 886
pg/ml w B3, a różnice istotnie statystycznie wystąpiły między grupami A i B2 (p < 0,05), A i B3
(p < 0,01), B2 i B3 (p < 0,01). Z kolei prędkość propagacji fali E wynosiła odpowiednio 69 ± 21 cm/s, 56 ± 20 cm/s, 53 ± 17 cm/s, 44 ± 11 cm/s, a różnice istotnie statystycznie
wystąpiły między grupami A i B2 (p < 0,05) oraz A i B3 (p < 0,01). Stwierdzono istotną dodatnią korelację między stopniem klasy DD a wartością NT-proBNP (r = 0,66; p < 0,001)
i ujemną korelację z wartością Vp (r = -0,41; p < 0,001). Opracowano krzywe ROC w celu
określenia punktu odcięcia stężenia NT-proBNP determinującego rozpoznanie izolowanej DD
lub zaawansowanej DD o typie restrykcji. Wartość najlepiej dyskryminująca rozpoznanie DD
wynosiła ponad 131 pg/ml (pole pod krzywą ROC: 0,63), a rozpoznanie restrykcji powyżej 1670
pg/ml (0,83). Czułość, swoistość, dokładność, pozytywna i negatywna wartość predykcyjna
wynosiły odpowiednio: 71%, 50%, 63%, 69%, 52% i 57%, 99%, 95%, 80%, 96%.
Wnioski: U pacjentów z chorobą niedokrwienną serca i zachowaną funkcją skurczową lewej
komory jednorazowy pomiar NT-proBNP identyfikuje osoby z izolowaną rozkurczową dysfunkcją
lewej komory, szczególnie w najbardziej zaawansowanym stadium, jakim są zaburzenia
restrykcyjne
