    Epidemiological Aspects of Human Brucellosis and Leptospirosis Outbreaks in Korea

    In order to compare the epidemiological aspects of human brucellosis (HB) and human leptospirosis (HL) outbreaks in Korea, we have analyzed the current state both of the disease incidence and related risk factors. A total 651 cases of HB occurred between 2001 and 2010 in Korea, and the average incidence rate per 100,000 populations was 0.15. A total 1,153 of HL cases occurred during the same period, and its rate was 0.24. While most of the HB prevalence occurred in the whole year round, prevalence in summer was more prominent for HB (p < 0.01), while outbreaks in autumn was more frequent for HL (p < 0.01). Geographical distribution HB cases were eastern and western regions of the rural (88.6% of total) in the Korean peninsula, showing higher outbreaks than other areas, while HL occurred in easterly regions (64.5%). Significantly more males were infected in both HB (86.8%) and HL (59.5%) than those of females in both HB (13.1%) and HL (41.1%), respectively (p < 0.01). The distribution by age groups were different between HB and HL, while the outbreaks over 62.1% of the cases of HB occurred in 40- to 59-year-old age group, and that of HL was clearly showing a high incidence in the elderly age over 60-year-old (59.6%) (p < 0.01). In both diseases, elderly people especially in famers showed a very high incidence rate (57.9% of HB and 52.2% of HL), which is possible due to increased outdoor activities and a decreased number of young people in those areas. The occupational distribution of HB cases was famers, veterinarians, dairyman and others, and those of HL cases were broad. In conclusion, the difference between HB and HL risk factors reflects the different influence of host/vector, climate, and geographical and environmental characteristics in the epidemiological patterns

    Dispersion of Vascular Plant in Kumo-do, Korea

    AbstractThe vascular plants observed in the area were composed of a total of 228 taxa; 72 families, 172 genus, 201 species, 25 varieties, 1 sub-species and 1 cross species. The only endangered plants found in the area were Milletia japonica (Siebold & Zucc.) A.Gray. The endemic plants growing in the Geumodo except transplanted plants were Lespedeza x maritima Nakai and Carpinus coreana Nakai. which accounted for 0.8% of the vascular plants in Geumodo, 228 taxa. Specialized plants of Geumodo were a total of 41 species; 30 taxa in Grade I, 1 taxon in Grade II, 9 taxa in Grade III and 1 taxon in Grade V. Milletia japonica (Siebold & Zucc.) A.Gray was the only species found in important Grade IV to V. Currently, ferries ply to the island, attracting many tourists. This poses a threat to the rare plants living in the island and presses down the island to develop. Therefore, in the long-term perspective, the conservation plan such as comprehensive research and monitoring on the ecosystem shall be established to protect evergreen broad-leaved forests

    Carbonic anhydrase XII expression is associated with histologic grade of cervical cancer and superior radiotherapy outcome

    BACKGROUND: To investigate whether expression of carbonic anhydrase XII (CA12) is associated with histologic grade of the tumors and radiotherapy outcomes of the patients with invasive cervical cancer. METHODS: CA12 expression was examined by immunohistochemical stains in cervical cancer tissues from 183 radiotherapy patients. Histological grading was classified as well (WD), moderately (MD) or poorly differentiated (PD). Oligonucleotide microarray experiment was performed using seven cervical cancer samples to examine differentially expressed genes between WD and PD cervical cancers. The association between CA12 and histological grade was analyzed by chi-square test. CA12 and histological grades were analyzed individually and as combined CA12 and histologic grade categories for effects on survival outcome. RESULTS: Immunohistochemical expression of CA12 was highly associated with the histologic grade of cervical cancer. Lack of CA12 expression was associated with PD histology, with an odds ratio of 3.9 (P = 0.01). Microarray analysis showed a fourfold reduction in CA12 gene expression in PD tumors. CA12 expression was marginally associated with superior disease-free survival. Application of the new combined categories resulted in further discrimination of the prognosis of patients with moderate and poorly differentiated tumor grade. CONCLUSIONS: Our study indicates that CA12 may be used as a novel prognostic marker in combination with histologic grade of the tumors

    Satellite radiance data assimilation for binary tropical cyclone cases over the western North Pacific

    A total of three binary tropical cyclone (TC) cases over the Western North Pacific are selected to investigate the effects of satellite radiance data assimilation on analyses and forecasts of binary TCs. Two parallel cycling experiments with a 6 h interval are performed for each binary TC case, and the difference between the two experiments is whether satellite radiance observations are assimilated. Satellite radiance observations are assimilated using the Weather Research and Forecasting Data Assimilation (WRFDA)&apos;s three-dimensional variational (3D-Var) system, which includes the observation operator, quality control procedures, and bias correction algorithm for radiance observations. On average, radiance assimilation results in slight improvements of environmental fields and track forecasts of binary TC cases, but the detailed effects vary with the case. When there is no direct interaction between binary TCs, radiance assimilation leads to better depictions of environmental fields, and finally it results in improved track forecasts. However, positive effects of radiance assimilation on track forecasts can be reduced when there exists a direct interaction between binary TCs and intensities/structures of binary TCs are not represented well. An initialization method (e.g., dynamic initialization) combined with radiance assimilation and/or more advanced DA techniques (e.g., hybrid method) can be considered to overcome these limitations

    Learning Curve Associated with Complications in Biportal Endoscopic Spinal Surgery: Challenges and Strategies

    Study DesignDescriptions of technical strategies to overcome pitfalls associated with early learning periods in biportal endoscopic spinal surgery (BESS).PurposeTo introduce BESS for lumbar spinal diseases (LSDs) and to inform certain challenges to be overcome in mastering the technique.Overview of LiteratureBESS has shown superior benefits including excellent magnification, a wider range of view by dynamic handling of an endoscope and instruments. Clinical reports, however, have not yet been very revealing for its new introduction into minimally invasive spine surgery.MethodsTo evaluate the learning curve for BESS, the procedures for various LSDs by one surgeon were analyzed in the view of shortening of the operating times and reduction of complications. Reviewing of recorded procedures helped in finding the reasons and the implemented solutions.ResultsThe 68 cases included 25 for lumbar disc herniation (LDH), 3 for revision for recurred LDH, 39 for lumbar spinal stenosis (LSS) and 1 for synovial cyst. The operation time for the total cases averaged 83.7±33.6 minutes. According to diagnosis, it was 68.2±23.7 minutes for LDH. After the 14th case of LDH, it was nearly constant and close to the average time. One level of LSS needed 110.4±34.4 minutes. Prolonged operation times even in some later cases of LSS were mainly from struggling against blurred vision due to epidural bleeding. There were 7 cases of complications (10.3%) including 2 cases of dural tear, 1 case of root injury, and 4 cases of incomplete decompression on postoperative magnetic resonance imaging. There was no case of symptomatic hematoma or wound infection.ConclusionsBESS seemed to have a relatively short learning curve period. The overall complication rate in early learning period was 10.3%. These could be avoided by magnified regional views on an endoscope and a clear surgical field by controlling epidural bleeding

    Factors associated with cervical cancer screening behaviors among young married female (aged 20–29) immigrants in South Korea

    Objective To identify factors that affect the participation of female immigrants in their 20s in the national cervical cancer screening programs. Methods Data were obtained from the National Health Insurance Services from 2016 to 2017. A total of 17,730 women who agreed to undergo cervical cancer screening during 2016–2017 were included in the study. Results Of the 17,730 women, 8,149 (46%) participated in cervical cancer screening, whereas, 9,581 (54%) did not. Logistic regression analysis of factors related to cervical cancer screening showed that the odds ratio (OR) of screening was higher in short duration of stay (OR, 1.18; 95% confidence interval [CI], 1.03–1.35), Chinese nationality (OR, 1.43; 95% CI, 1.28–1.59), unemployment (OR, 1; 95% CI, reference), participation in general health screening (OR, 4.16; 95% CI, 3.24–5.33), and comorbidities (OR, 1.16; 95% CI, 1.09–1.24) when compared to the other populations. The highest OR was associated with participation in general health screening. Conclusion Appropriate programs should be developed to increase participation of socially vulnerable groups in cervical cancer screening. Such programs will improve awareness regarding cervical cancer screening and reduce disparities in healthcare

    Systematic and effective welfare for the disabled is possible when there are scientific and objective criteria demonstrating either presence or severity of the impairment. We need our own scientific criteria suitable for our culture and society, since the impairment is influenced by them. In 2007, we established the Developing Committee of Korean Academy of Medical Sciences (KAMS) Guideline for Impairment Rating under KAMS supervision. We included all fixed and permanent physical impairments after a sufficient medical treatment. The impairment should be stable and medically measurable. If not, it should be reevaluated later. We benchmarked the American Medical Association Guides. The KAMS Guideline should be scientific, objective, valid, reasonable and practical. In particular, we tried to secure objectivity. We developed the KAMS Guideline for Impairment Rating

    Castleman disease is a relatively rare disorder of lymphoid tissue that involves the gastrointestinal tract in a variety of clinical and pathologic manifestations. A submucosal location has never been described in the medical literature. We report a case of esophageal Castleman disease involving thesubmucosal layer in a 62-year-old man, which was confirmed on pathology. Esophagography and CT demonstrated an intramural tumor, and a leiomyoma or leiomyosarcoma was suspected based on the known incidence of such tumors

    Cognitive-Enhancing Effect of Aronia melanocarpa

    Aronia melanocarpa (A. melanocarpa) berries are a fruit with a marked antioxidant effect. The objective of this study was to confirm the effect of A. melanocarpa berries extract against scopolamine-induced memory impairment in mice using the Morris water maze and passive avoidance test. Moreover, we determined a possible mechanism of the cognitive-enhancing effect involving AChE activity and BDNF and p-CREB expression in the hippocampus of mice. A. melanocarpa berries extract attenuated the learning and memory impairment induced by scopolamine in the Morris water maze (79.3 ± 0.8 s of 200 mg/kg and 64.4 ± 10.7 s of 400 mg/kg on day 4) and passive avoidance tests (46.0 ± 41.1 s of 200 mg/kg and 25.6 ± 18.7 s of 400 mg/kg). A. melanocarpa berries extract reduced the acetylcholinesterase level in the hippocampus of scopolamine-injected mice and increased BDNF and p-CREB expression in the hippocampus. The major compound, cyanidin-3-O-galactoside, also reversed memory impairment. These results showed that A. melanocarpa berries extract improved memory impairment by inhibiting AChE and increasing BDNF and p-CREB expression, and cyanidin-3-O-galactoside may be responsible for the effect of A. melanocarpa berries extract
