    Policy Formulation and Implementation on Participatory Budgeting in Seoul, South Korea

    This research analyzes the policy formulation and implementation of participatory budgeting in Seoul by using qualitative methods focusing on document research. The target time for this research is from 2010 until May 2012 when the Seoul government enacted the PB regulation for policy formulation, and from 2012 until 2016 for policy implementation. The results of the research show that Seoul government had already faced many demands for the implementation of PB before the regulation on PB was enacted. These demands are the result of two flows, that is, a political flow that regards citizen participation in the process of public policy as important and the worsening financial condition of the local government. In this situation, the change of Seoul mayor in 2011 has led the Seoul government to start policy formulation for PB. In the process of making the regulations, the CSOs that already studied a lot about PB also participated in this process. So, Seoul government has made regulation on PB with high level of citizen participation. After regulation on PB was enacted, during the implementation of PB, Seoul government seeks to increase the level of citizen participation by enhancing its representativeness and expertise. For example, Seoul government randomly select most of the PB committee members through open recruitment for representatives, make compulsory lessons in budgets school for expertise, and disclose all information and provide more opportunities for participation such as e-voting to increase the number of participants. As a result, the number of e-voting participants reached about 1% among the total population of Seoul

    Magnon topology and thermal Hall effect in trimerized triangular lattice antiferromagnet

    The non-trivial magnon band topology and its consequent responses have been extensively studied in two-dimensional magnetisms. However, the triangular lattice antiferromagnet (TLAF), the best-known frustrated two-dimensional magnet, has received less attention than the closely related Kagome system, because of the spin-chirality cancellation in the umbrella ground state of the undistorted TLAF. In this work, we study the band topology and the thermal Hall effect (THE) of the TLAF with (anti-)trimerization distortion under the external perpendicular magnetic field using the linearized spin wave theory. We show that the spin-chirality cancellation is removed in such case, giving rise to the non-trivial magnon band topology and the finite THE. Moreover, the magnon bands exhibit band topology transitions tuned by the magnetic field. We demonstrate that such transitions are accompanied by the logarithmic divergence of the first derivative of the thermal Hall conductivity.     Effect of RGD Peptide-Coated TiO 2

    The purpose of this research was to characterize an Arg-Gly-Asp (RGD) peptide immobilized on TiO2 nanotubes. In addition, we investigated the effects of the RGD peptide-coated TiO2 nanotubes on the cellular response, proliferation, and functionality of osteogenic-induced human mesenchymal stem cells (hMSCs), which are osteoclasts that have been induced by bone marrow macrophages. The RGD peptide was grafted covalently onto the surface of TiO2 nanotubes based on the results of SEM, FT-IR, and XPS. Furthermore, the RGD peptide promoted the initial attachment and proliferation of the hMSCs, regardless of the size of the TiO2 nanotubes. However, the RGD peptide did not prominently affect the osteogenic functionality of the hMSCs because the peptide suppressed hMSC motility associated with osteogenic differentiation. The result of an in vitro osteoclast test showed that the RGD peptide accelerated the initial attachment of preosteoclasts and the formation of mature osteoclasts, which could resorb the bone matrix. Therefore, we believe that an RGD coating on TiO2 nanotubes synthesized on Ti implants might not offer significant acceleration of bone formation in vivo because osteoblasts and osteoclasts reside in the same compartment

    Short term outcomes of topiramate monotherapy as a first-line treatment in newly diagnosed West syndrome

    PurposeTo investigate the efficacy of topiramate monotherapy in West syndrome prospectively.MethodsThe study population included 28 patients (15 male and 13 female children aged 2 to 18 months) diagnosed with West syndrome. After a 2-week baseline period for documentation of the frequency of spasms, topiramate was initiated at 2 mg/kg/day. The dose was increased by 2 mg/kg every week to a maximum of 12 mg/kg/day. Clinical assessment was based on the parents' report and a neurological examination every 2 weeks for the first 2 months of treatment. The baseline electroencephalograms (EEGs) were compared with the post-treatment EEGs at 2 weeks and 1 month.ResultsWest syndrome was considered to be cryptogenic in 7 of the 28 patients and symptomatic in 21 patients. After treatment, 11 patients (39%) became spasm-free, 6 (21%) had more than 50% spasmsreduction, 3 (11%) showed less than 50% reduction, and 8 (29%) did not respond. The effective daily dose for achieving more than 50% reduction in spasm frequency, including becoming spasm-free, was found to be 5.8±1.1 mg/kg/day. Nine patients (32%) showed complete disappearance of spasms and hypsarrhythmia, and 11 (39%) showed improved EEG results. Despite adverse events (4 instances of irritability, 3 of drowsiness, and 1 of decreased feeding), no patients discontinued the medication.ConclusionTopiramate monotherapy seems to be effective and well tolerated as a first line therapy for West syndrome and is not associated with serious adverse effects

    Impacts of gender, weather, and workplace differences in farm worker's gear

    Background: The farmers cannot help working in outdoor conditions which have high humidity and solar radiation during the harvest period. Wearable items including clothing are the nearest environment of human body, and to understand the current state of them can be a way to set up an active prevention strategy against the health risk from heat stress in summertime agriculture. The aim of this study was to investigate the work wear and accessories which the elderly farmers used during agricultural working. Methods: One hundred twenty farmers (49 males and 71 females) working in nine separate sites on different days took part in this study. The average age of subjects was 61 years old. We examined the types of working posture, clothing, and items that the farmers used and/or wore. We also interviewed the farmers to know why they used such items while working. Results: The results of this study were as follows: (1) Farmers worked in the thermal environment which was over wet bulb globe temperature (WBGT) reference value, and the farmers could suffer heat stress due to workload induced from wearing conventional long-sleeved shirts and long trousers which were 0.66 clo in average under this summertime working thermal condition. (2) The farmers tended to change the layer of upper clothing for adapting to weather condition. (3) The types of footwear used seemed to be related with facilities as well as weather, and farmers tended to wear lighter footwear when the weather is hotter or when they work in PVC greenhouse. The majority of elderly farmers wore loafers and rubber shoes which had indistinguishable thin soles. (4) The types of hats showed the difference between facilities as well as gender and only 31.7 % of all participants used long brims. (5) Korean elderly farmers did not use any active cooling item as agricultural auxiliary tools in summer harvesting time. Conclusions: Korean elderly farmers worked in poor surroundings which could threaten their health and safety and seemed not to adjust their workload and clothing during summer harvest season. Thus, it would be necessary to monitor individual responses in order to ensure that the risk of heat stress is preventedopen

    Liver transplantation for advanced hepatocellular carcinoma

    There has been ongoing debate that the Milan criteria may be too strict that a significant number of patients who could benefit from liver transplantation (LT) might have been excluded. Based on this idea, various studies have been conducted to further expand the Milan criteria and give more HCC patients a chance of cure. In deceased donor LT (DDLT) setting, expansion of the criteria is relatively tempered because the results of LT for HCC should be comparable to those of patients with non-malignant indications. On the other hand, in living donor LT (LDLT) situation, liver grafts are not public resources. The acceptable target outcomes for LDLT might be much lower than those for DDLT. Patients with biologically favorable tumors might have excellent survivals after LT despite morphological advanced HCCs. Therefore, the significance and utility of biological tumor parameters for selecting suitable LT candidates have been increased to predict HCC recurrence after LT. Although there is no consensus regarding the use of prognostic biomarkers in LT selection criteria for HCC, the combination of conventional morphological parameters and new promising biomarkers could help us refine and expand the LT criteria for HCC in the near future

    Concomitant use of corticosteroid and antimicrobials for liver abscesses in patients with chronic granulomatous disease

    Chronic granulomatous disease (CGD) is a rare inherited disorder caused by defective nicotinamide adenine dinucleotide phosphate oxidase enzyme and characterized by recurrent bacterial and fungal infections. Although liver abscess is a common manifestation of CGD, its management in CGD patients is not well-defined. In addition, the generalized guidelines for treating liver abscesses do not necessarily apply to CGD patients. Corticosteroids are commonly used to control granulomatous complications, such as inflammatory gastrointestinal and genitourinary lesions, in patients with CGD, Corticosteroids have also been used in combination with antimicrobials to treat refractory infections in patients with CGD. Because corticosteroids are capable of suppressing symptomatic inflammation, all potential infections must be adequately controlled prior to corticosteroid initiation. We report 3 typical CGD cases with liver abscesses refractory to conventional treatments that were successfully treated with the concomitant use of corticosteroid and antimicrobials. It remains unclear whether corticosteroid therapy is required for liver abscesses in CGD refractory to conventional treatments. However, based on our observations, use of corticosteroids in combination with optimal antimicrobials should be considered for refractory liver abscesses in CGD
