    Experiences of International Students in Practicum and Internship Courses: A Consensus Qualitative Research

    This qualitative study explores the practicum and/or internship experiences of international students in counseling. Based on the foundation of phenomenological research, this study uses a consensual qualitative research method. Semi-structured interview questionnaires asked ten participants regarding their experiences of practica and/or internships (including their fears, challenges, and support from training programs). Results revealed that the fears and challenges that international students face during the practicum and/or internship training primarily stemmed from their language barrier and/or a lack of understanding of the American counseling system. Our findings indicated that providing practical information, such as sites, the American counseling system, insurance, and cultures, in addition to ensuring that supervisors and faculty members increase in multicultural competency and sensitivity about international students, would improve international students’ preparation for their practica and/or internships

    Multiple Sclerosis and Peripheral Multifocal Demyelinating Neuropathies Occurring in a Same Patient

    The co-occurrence of multiple sclerosis and peripheral demyelinating neuropathy is rare. It has been disputed whether these are pathologically related or coincidental findings. We report a 36-year-old woman who presented with diplopia, right facial palsy and left-sided weakness. Brain magnetic resonance imaging showed a lesion indicative of central demyelinating disease. Nerve conduction studies revealed peripheral multifocal demyelinating neuropathies. We suggest that the central and the peripheral lesions may be continua of a demyelinating process

    Connexin32 inhibits gastric carcinogenesis through cell cycle arrest and altered expression of p21Cip1 and p27Kip1

    Gap junctions and their structural proteins, connexins (Cxs), havebeen implicated in carcinogenesis. To explore the involvement ofCx32 in gastric carcinogenesis, immunochemical analysis of Cx32and proliferation marker Ki67 using tissue-microarrayed humangastric cancer and normal tissues was performed. In addition, afterCx32 overexpression in the human gastric cancer cell line AGS,cell proliferation, cell cycle analyses, and p21Cip1 and p27Kip1expression levels were examined by bromodeoxyuridine assay,flow cytometry, real-time RT-PCR, and western blotting.Immunohistochemical study noted a strong inverse correlationbetween Cx32 and Ki67 expression pattern as well as theirlocation. In vitro, overexpression of Cx32 in AGS cells inhibitedcell proliferation significantly. G1 arrest, up-regulation of cellcycle-regulatory proteins p21Cip1 and p27Kip1 was also found atboth mRNA and protein levels. Taken together, Cx32 plays someroles in gastric cancer development by inhibiting gastric cancercell proliferation through cell cycle arrest and cell cycle regulatoryproteins.     Maternal Psychosocial Factors that Affect Breastfeeding Adaptation and Immune Substances in Human Milk

    PURPOSE: This study was to identify relationships of maternal psychosocial factors including mother's mood state, childcare stress, social support and sleep satisfaction with breastfeeding adaptation and immune substances in breast milk, especially secretory immunoglobulin A (sIgA) and transforming growth factor-beta 2 (TGF-beta2). METHODS: Data were collected from 84 mothers who delivered full-term infants by natural childbirth. Structured questionnaires and breast milk were collected at 2~4 days and 6 weeks postpartum. Data were analyzed using descriptive statistics, Pearson's correlation, multiple linear regression, and generalized estimating equation (GEE). RESULTS: Scores for the breastfeeding adaptation scale were significantly related with child care stress, mood state and social support. Mother's anger was positively correlated with the level of sIgA in colostrum (p<.01). Immune substances of breastmilk was significantly influenced by time for milk collection (p<.001) and the type of breastfeeding (sIgA, p<.001, TGF-beta2, p=.003). Regression analysis showed that breastfeeding adaptation could be explained 59.1% by the type of breastfeeding, childcare stress, the Profile of Mood States, emotional support and sleep quality (F=16.67, p<.001). CONCLUSION: The findings from this study provide important concepts of breastfeeding adaptation program and explanation of psychosocial factors by immune substances in breast milk. Future research, specially, bio-maker research on breast milk should focus on the ways to improve breastfeeding adaptation

    Multiplex Analysis of Cytokines in the Serum and Cerebrospinal Fluid of Patients With Alzheimer's Disease by Color-Coded Bead Technology

    Background and purpose: The availability and promise of effective treatments for neurodegenerative disorders are increasing the importance of early diagnosis. Having molecular and biochemical markers of Alzheimer’s disease (AD) would complement clinical approaches, and further the goals of early and accurate diagnosis. Combining multiple biomarkers in evaluations significantly increases the sensitivity and specificity of the biochemical tests. Methods: In this study, we used color-coded bead-based Luminex technology to test the potential of using chemokines and cytokines as biochemical markers of AD. We measured the levels of 22 chemokines and cytokines in the serum and cerebrospinal fluid (CSF) of 32 de novo patients (13 controls, 11 AD, and 8 Parkinson’s disease [PD]). Results: MCP-1 was the only cytokine detectable in CSF, and its levels did not differ between control and disease groups. However, the serum concentration of eotaxin was significantly higher in AD patients than in the control group. Conclusions: The analysis of multiple inflammatory mediators revealed marginal differences in their CSF and serum concentrations for the differential diagnosis of AD and PD.     Brain abscess in Korean children: A 15-year single center study

    PurposeA brain abscess is a serious disease of the central nerve system. We conducted this study to summarize the clinical manifestations and outcomes of brain abscesses.MethodsA retrospective chart review of pediatric patients diagnosed with brain abscesses from November 1994 to June 2009 was performed at Samsung Medical Center, Seoul, Korea.ResultsTwenty-five patients were included in this study. On average, 1.67 cases per year were identified and the median age was 4.3 years. The common presenting clinical manifestations were fever (18/25, 72%), seizure (12/25, 48%), altered mental status (11/25, 44%), and signs of increased intracranial pressure (9/25, 36%). A total of 14 (56%) patients had underlying illnesses, with congenital heart disease (8/25, 32%) as the most common cause. Predisposing factors were identified in 15 patients (60%). The common predisposing factors were otogenic infection (3/25, 12%) and penetrating head trauma (3/25, 12%). Causative organisms were identified in 64% of patients (16/25). The causative agents were S. intermedius (n=3), S. aureus (n=3), S. pneumoniae (n=1), Group B streptococcus (n=2), E. coli (n=1), P. aeruginosa (n=1), and suspected fungal infection (n=5). Seven patients received medical treatment only while the other 18 patients also required surgical intervention. The overall fatality rate was 16% and 20% of patients had neurologic sequelae. There was no statistical association between outcomes and the factors studied.ConclusionAlthough uncommon, a brain abscess is a serious disease. A high level of suspicion is very important for early diagnosis and to prevent serious consequences
