    Retroperitoneal smooth muscle tumor of uncertain malignant potential after hysterectomy: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Smooth muscle tumors of uncertain malignant potential represent a histologically heterogeneous group of uterine smooth muscle tumors that cannot be diagnosed as either benign or malignant. Smooth muscle tumors of uncertain malignant potential are usually clinically benign, but should be considered tumors of low malignant potential because they can occasionally recur or metastasize to distant sites.</p> <p>Case presentation</p> <p>We report the case of a 62-year-old Mongol woman diagnosed with a retroperitoneal smooth muscle tumor of uncertain malignant potential and lung metastasis, with a history of prior hysterectomy. The case was initially misdiagnosed as retroperitoneal sarcoma, and our patient received chemotherapy. However, no interval change in the size of the retroperitoneal mass and metastatic lung nodules was seen over a period of at least five years. She underwent partial resection of the retroperitoneal mass for the purposes of debulking and establishing a histopathological diagnosis. The diagnosis of the retroperitoneal mass was then confirmed as a smooth muscle tumor of uncertain malignant potential.</p> <p>Conclusion</p> <p>Smooth muscle tumors of uncertain malignant potential have an unpredictable clinical course, and relapses generally appear to occur after a long disease-free interval of up to several years. Therefore, patients diagnosed with smooth muscle tumors of uncertain malignant potential should receive long-term follow-up.</p

    Submerzni uzgoj i karakterizacija polisaharida iz gljive Grifola frondosa – primjena u kozmetičkoj industriji

    Grifola frondosa (maitake) is traditionally called \u27the king of mushrooms\u27 and \u27the hen of the woods\u27. Both the fruiting bodies and the mycelium of maitake have been reported to have antitumor and antiviral activities. Recently, submerged culture processes have been developed, with the intention of providing opportunities for increased economic exploitation of maitake. Commonly the aim of these processes is to produce extracellular polysaccharides (EPS), mostly glucans, and to explore their applications, particularly in the cosmetic industry. A wide variety of EPS with different molecular chain length and chemical compositions are produced under different culture conditions. In this article, various biological and physicochemical properties of the EPS of G. frondosa (GF-EPS) are described, with a view to applications in the area of functional cosmeceuticals. The GF-EPS, together with GF mycelial extract (GF-MPS), showed antioxidative activity, stimulation of collagen biosynthetic activity, cell proliferation activity, and inhibitory activity of melanogenesis, without significant cytotoxicity. These diverse functionalities suggest that both GF-EPS and GF-MPS can be promising cosmetic ingredients.Gljivu Grifola frondosa (maitake) u Koreji tradicionalno zovu kraljicom gljiva i šumskim bogatstvom. Dokazano je da plodište i micelij gljiva maitake imaju antitumorski i antivirusni učinak. Ekonomsko iskorištenje te gljive omogućeno je submerznim uzgojem. Uobičajeni je cilj uzgoja proizvodnja ekstracelularnih polisaharida (EPS), uglavnom glukana i istraživanje mogućnosti njihove primjene u kozmetičkoj industriji. Pri raznim uvjetima proizvodnje dobivaju se ekstracelularni polisaharidi različitih duljina lanaca i kemijskoga sastava. U ovom su radu opisana razna biološka i fizikalno-kemijska svojstva ekstracelularnih polisaharida gljive Grifola frondosa (GF-EPS), te primjena u proizvodnji kozmetičkih preparata s povoljnim utjecajem na ljudsko zdravlje. Ekstracelularni polisaharidi i ekstrakt micelija gljive Grifola frondosa (GFMPS) imaju antioksidativni učinak, stimuliraju biosintezu kolagena, bujanje stanica, sprečavaju nastanak melanoma i nemaju jače citotoksično djelovanje, zbog čega su prikladni za primjenu u kozmetičkoj industriji

    Using In Vitro Dynamic Models To Evaluate Fluoroquinolone Activity against Emergence of Resistant Salmonella enterica Serovar Typhimurium

    The objectives of this study were to determine pharmacokinetic/pharmacodynamic (PK/PD) indices of fluoroquinolones that minimize the emergence of resistant Salmonella enterica serovar Typhimurium (S. Typhimurium) using in vitro dynamic models and to establish mechanisms of resistance. Three fluoroquinolones, difloxacin (DIF), enrofloxacin (ENR), and marbofloxacin (MAR), at five dose levels and 3 days of treatment were simulated. Bacterial killing-regrowth kinetics and emergence of resistant bacteria after antibacterial drug exposure were quantified. PK/PD indices associated with different levels of antibacterial activity were computed. Mechanisms of fluoroquinolone resistance were determined by analyzing target mutations in the quinolone resistance-determining regions (QRDRs) and by analyzing overexpression of efflux pumps. Maximum losses in susceptibility of fluoroquinolone-exposed S. Typhimurium occurred at a simulated AUC/MIC ratio (area under the concentration-time curve over 24 h in the steady state divided by the MIC) of 47 to 71. Target mutations in gyrA (S83F) and overexpression of acrAB-tolC contributed to decreased susceptibility in fluoroquinolone-exposed S. Typhimurium. The current data suggest AUC/MIC (AUC/mutant prevention concentration [MPC])-dependent selection of resistant mutants of S. Typhimurium, with AUC/MPC ratios of 69 (DIF), 62 (ENR), and 39 (MAR) being protective against selection of resistant mutants. These values could not be achieved in veterinary clinical areas under the current recommended therapeutic doses of the fluoroquinolones, suggesting the need to reassess the current dosing regimen to include both clinical efficacy and minimization of emergence of resistant bacteria

    Semaphorin 7A plays a critical role in TGF-β1–induced pulmonary fibrosis

    Semaphorin (SEMA) 7A regulates neuronal and immune function. In these studies, we tested the hypothesis that SEMA 7A is also a critical regulator of tissue remodeling. These studies demonstrate that SEMA 7A and its receptors, plexin C1 and β1 integrins, are stimulated by transforming growth factor (TGF)-β1 in the murine lung. They also demonstrate that SEMA 7A plays a critical role in TGF-β1–induced fibrosis, myofibroblast hyperplasia, alveolar remodeling, and apoptosis. TGF-β1 stimulated SEMA 7A via a largely Smad 3–independent mechanism and stimulated SEMA 7A receptors, matrix proteins, CCN proteins, fibroblast growth factor 2, interleukin 13 receptor components, proteases, antiprotease, and apoptosis regulators via Smad 2/3–independent and SEMA 7A–dependent mechanisms. SEMA 7A also played an important role in the pathogenesis of bleomycin-induced pulmonary fibrosis. TGF-β1 and bleomycin also activated phosphatidylinositol 3-kinase (PI3K) and protein kinase B (PKB)/AKT via SEMA 7A–dependent mechanisms, and PKB/AKT inhibition diminished TGF-β1–induced fibrosis. These observations demonstrate that SEMA 7A and its receptors are induced by TGF-β1 and that SEMA 7A plays a central role in a PI3K/PKB/AKT-dependent pathway that contributes to TGF-β1–induced fibrosis and remodeling. They also demonstrate that the effects of SEMA 7A are not specific for transgenic TGF-β1, highlighting the importance of these findings for other fibrotic stimuli

    Comparison of MRI features and surgical outcome among the subtypes of focal cortical dysplasia

    AbstractPurposeFocal cortical dysplasia (FCD) is the most common pathological diagnosis in patients who have undergone surgical treatment for intractable neocortical epilepsy. However, presurgical identification of MRI abnormalities in FCD patients remains difficult, and there are no highly sensitive imaging parameters available that can reliably differentiate among FCD subtypes. The purpose of our study was to investigate the surgical outcome in FCD patients with identifiable MRI abnormalities and to evaluate the prognostic role of the various MRI features and the characteristics of FCD pathology.MethodsWe retrospectively recruited epilepsy patients who had undergone surgical treatment for refractory epilepsy with focal MRI abnormalities and the pathological diagnosis of FCD. We evaluated the surgical outcome according to the pathological subtypes, and studied the prognostic roles of various MRI features. We used recently proposed three-tiered FCD classification system which included FCD type III when FCD occurs in association with other potentially epileptogenic pathologies.ResultsA total of 69 patients were included, and 68.1% of patients became seizure free. Patients with FCD type III had a lower chance for achieving seizure freedom (7/15) than in patients with isolated FCD (FCD types I and II) (40/54, p=0.044). Cortical thickness and blurring of gray–white matter junction were more common in isolated FCD than in FCD type III, but most MRI features failed to differentiate between FCD types I and II, and only the transmantle sign was specific for FCD type II. We failed to find a prognostic value of specific MRI abnormalities of prognostic value in terms of post-epilepsy surgery outcome in FCD patients.ConclusionsOur study showed that patients with FCD III have poor surgical outcome. Typical MRI features of isolated FCD such as cortical thickness and blurring of gray–white matter junction were less common in FCD type III and only transmantle sign was helpful in differentiating between FCD types I and II

    A Photometric and Spectroscopic Study of the Short-Period Algol EW Bo\"{o}tis with a δ\delta Sct Pulsator

    In this paper, we present TESS photometry and high-resolution spectra of the short-period Algol EW Boo. We obtained double-lined radial velocities (RVs) from the time-series spectra and measured the effective temperature of the primary star as Teff,1T_{\rm{eff,1}} = 8560 ±\pm 118 K. For the orbital period study, we collected all times of minima available for over the last 30 years. It was found that the eclipse timing variation of the system could be represented by a periodic oscillation of 17.6 ±\pm 0.3 years with a semi-amplitude of 0.0041 ±\pm 0.0001 d. The orbital and physical parameters were derived by simultaneously analyzing the TESS light and RV curves using the Wilson-Devinney (WD) binary star modeling code. The component masses and radii were showed over 3% precision: M1M_{1} = 2.67 ±\pm 0.08 M_{\odot}, M2M_{2} = 0.43 ±\pm 0.01 M_{\odot}, R1R_{1} = 2.01 ±\pm 0.02 R_{\odot}, and R2R_{2} = 1.35 ±\pm 0.01 R_{\odot}. Furthermore, multiple frequency analyses were performed for the light-curve residuals from the WD model. As a result, we detected 17 pressure-mode pulsations in the region of 40.15 - 52.37 d1^{-1}. The absolute dimensions and pulsation characteristics showed that the δ\delta Sct pulsator was the more massive and hotter primary star of the EW Boo.Comment: 27 pages, 8 figures, Accepted for publication in A

    A Case of a Subepidermal Calcified Nodule on the Sole without Trauma

    Subepidermal calcified nodule is an uncommon form of calcinosis cutis, which most commonly occurs in children. It usually presents as an asymptomatic, solitary verrucous nodule on the head and neck region, but occasionally as multiple lesions. Serum calcium and phosphorus levels are usually normal. Histopathology shows well-formed homogeneous eosinophilic material and granules in the upper dermis. Material in the dermis stained with von Kossa was positive. We report on an unusual case of a subepidermal calcified nodule occurring on the sole. A 21-month-old male presented with an oval-shaped, whitish, hard nodule measuring 5×5 mm on the left sole, without any previous history of trauma

    Epidermal Cysts in a Tacrolimus Treated Renal Transplant Recipient

    Tacrolimus, a calcineurin inhibitor, formerly also known as FK506, is a macrolactam drug isolated from Streptomyces tsukubaensis. Its mode of action closely parallels the action of cyclosprorin A (CsA) and can be used for the treatment of inflammatory and autoimmune skin diseases in which systemic CsA has proved effective against psoriasis, pyoderma gangrenosum, atopic dermatitis, lupus erythematosus and graft versus host disease (GVHD). Although several cases of epidermal cysts have been reported in patients using cyclosporine and other immunosuppressants after organ transplantation; such types of cases have yet not been reported after administration of tacrolimus.
