Prevalence of Dyskinesia and OFF by 30-Minute Intervals Through the Day and Assessment of Daily Episodes of Dyskinesia and OFF: Novel Analyses of Diary Data from Gocovri Pivotal Trials.
BACKGROUND: Parkinson\u27s disease (PD) patients using levodopa commonly develop dyskinesia and OFF episodes that reduce quality of life.
OBJECTIVE: Evaluate prevalence of troublesome dyskinesia and OFF through the day, assessed by 30-minute intervals, as well as the mean number and duration of troublesome dyskinesia and OFF episodes, transitions between PD states, and effects of GocovriĀ® (amantadine) extended release capsules on these episodes.
METHODS: Evaluate diary data from pooled Gocovri phase 3, placebo-controlled trials-analyzed for 17 hours following wake-up-at baseline and week 12.
RESULTS: Diaries were evaluable for 162 patients. At baseline, 67% of patients woke up OFF, with prevalence decreasing to 13% at 2 hours and then remaining relatively steady at ā¼12% (range, 6-17%) across half-hour intervals thereafter. Troublesome dyskinesia prevalence rose steadily from 5% to 24% over the first 2 hours, then fluctuated between 20% and 44% through the rest of the waking day. At baseline, patients experienced a mean of 3.0 daily episodes of troublesome dyskinesia (average duration 2.0 hours each), and 2.2 daily episodes of OFF (average duration 1.1 hour each). At week 12, Gocovri-treated patients showed greater reductions than placebo in troublesome dyskinesia and OFF episodes per day (treatment difference: -1.0 episodes and -0.4 episodes, respectively) and average episode duration (treatment difference: -0.6 hours and -0.3 hours, respectively). Mean duration of individual episodes of ON without troublesome dyskinesia (Good ON) increased by 5.0 hours for Gocovri, compared with 2.0 hours for placebo. Patients taking Gocovri experienced 2.2 fewer transitions between states than patients taking placebo.
CONCLUSIONS: Troublesome dyskinesia and OFF occurred in the morning and throughout the waking day. Gocovri-treated patients experienced fewer, shorter episodes of both troublesome dyskinesia and OFF, thereby increasing the duration of continuous Good ON episodes and reducing the frequency of transitions between motor states

The Persistent Southern Disadvantage in Us Early Life Mortality, 1965ā2014
Background: Recent studies of US adult mortality demonstrate a growing disadvantage among southern states. Few studies have examined long-term trends and geographic patterns in US early life (ages 1 to 24) mortality, ages at which key risk factors and causes of death are quite different than among adults. Objective: This article examines trends and variations in early life mortality rates across US states and census divisions. We assess whether those variations have changed over a 50-year time period and which causes of death contribute to contemporary geographic disparities. Methods: We calculate all-cause and cause-specific death rates using death certificate data from the Multiple Cause of Death files, combining public-use files from 1965ā2004 and restricted data with state geographic identifiers from 2005ā2014. State population (denominator) data come from US decennial censuses or intercensal estimates. Results: Results demonstrate a persistent mortality disadvantage for young people (ages 1 to 24) living in southern states over the last 50 years, particularly those located in the East South Central and West South Central divisions. Motor vehicle accidents and homicide by firearm account for most of the contemporary southern disadvantage in US early life mortality. Contribution: Our results illustrate that US children and youth living in the southern United States have long suffered from higher levels of mortality than children and youth living in other parts of the country. Our findings also suggest the contemporary southern disadvantage in US early life mortality could potentially be reduced with state-level policies designed to prevent deaths involving motor vehicles and firearms
The VISTA Science Archive
We describe the VISTA Science Archive (VSA) and its first public release of
data from five of the six VISTA Public Surveys. The VSA exists to support the
VISTA Surveys through their lifecycle: the VISTA Public Survey consortia can
use it during their quality control assessment of survey data products before
submission to the ESO Science Archive Facility (ESO SAF); it supports their
exploitation of survey data prior to its publication through the ESO SAF; and,
subsequently, it provides the wider community with survey science exploitation
tools that complement the data product repository functionality of the ESO SAF.
This paper has been written in conjunction with the first public release of
public survey data through the VSA and is designed to help its users understand
the data products available and how the functionality of the VSA supports their
varied science goals. We describe the design of the database and outline the
database-driven curation processes that take data from nightly
pipeline-processed and calibrated FITS files to create science-ready survey
datasets. Much of this design, and the codebase implementing it, derives from
our earlier WFCAM Science Archive (WSA), so this paper concentrates on the
VISTA-specific aspects and on improvements made to the system in the light of

Kinematics of XāRayāEmitting Components in Cassiopeia A
We present high-resolution X-ray proper-motion measurements of Cassiopeia A using Chandra X-Ray Observatory observations from 2000 and 2002. We separate the emission into four spectrally distinct classes: Si-dominated, Fe-dominated, low-energy-enhanced, and continuum-dominated. These classes also represent distinct spatial and kinematic components. The Si- and Fe-dominated classes are ejecta and have a mean expansion rate of 0.2% yr-1. This is the same as for the forward shock filaments but less than the 0.3% yr-1 characteristic of optical ejecta. The low-energy-enhanced spectral class possibly illuminates a clumpy circumstellar component and has a mean expansion rate of 0.05% yr-1. The continuum-dominated emission likely represents the forward shock and consists of diffuse circumstellar material, which is seen as a circular ring around the periphery of the remnant as well as projected across the center
An Empirical Examination of Symptom Substitution Associated with Behavior Therapy for Tourette\u27s Disorder
Over the past six decades, behavior therapy has been a major contributor to the development of evidence-based psychotherapy treatments. However, a long-standing concern with behavior therapy among many nonbehavioral clinicians has been the potential risk for symptom substitution. Few studies have been conducted to evaluate symptom substitution in response to behavioral treatments, largely due to measurement and definitional challenges associated with treated psychiatric symptoms. Given the overt motor and vocal tics associated with Touretteās disorder, it presents an excellent opportunity to empirically evaluate the potential risk for symptom substitution associated with behavior therapy. The present study examined the possible presence of symptom substitution using four methods: (a) the onset of new tic symptoms, (b) the occurrence of adverse events, (c) change in tic medications, and (d) worsening of co-occurring psychiatric symptoms. Two hundred twenty-eight participants with Touretteās disorder or persistent motor or vocal tic disorders were randomly assigned to receive behavioral therapy or supportive therapy for tics. Both therapies consisted of eight sessions over 10 weeks. Results indicated that participants treated with behavior therapy were not more likely to have an onset of new tic symptoms, experience adverse events, increase tic medications, or have an exacerbation in co-occurring psychiatric symptoms relative to participants treated with supportive therapy. Further analysis suggested that the emergence of new tics was attributed with the normal waxing and waning nature of Touretteās disorder. Findings provide empirical support to counter the long-standing concern of symptom substitution in response to behavior therapy for individuals with Tourette\u27s disorder
Auditory In-Vehicle Technologies to Support Older Drivers
OBJECTIVES Population aging, in combination with improved health care and more active lifestyles well into advanced age, have resulted in an increased number of older adults driving more miles than ever before. Unfortunately, these older drivers are over-represented in motor vehicle crashes and crash-related fatalities. Rather than the risk-tasking behaviors observed in young drivers, the collisions of older drivers frequently involve perceptual-cognitive errors. Advanced in-vehicle technologies have the potential to function as sensory-cognitive aids and may offset the negative impact of age-related changes in sensory and cognitive abilities. Collision Avoidance Systems (CASs) function as sensory aids to augment hazard detection capabilities, and therefore may be of particular benefit to older drivers. Navigation aids can offset the working memory requirements of wayfinding, and auditory guidance directions may reduce the visual demands of searching for street signs and reading maps. However, these advanced systems also have the potential to increase the information processing demands of the driving task or distract drivers, particularly if they are not designed in accordance with the sensory and perceptual capabilities of older adults. A series of experiments aimed at examining the impact of sensory-cognitive characteristics of auditory navigational aids on driver wayfinding, performance on a visual peripheral detection task, and neurophysiological, behavioral and subjective indices of driver mental workload and performance were conducted. METHODS Results of two investigations will be discussed. The first investigation examined the impact of amplitude level on working memory. Older adults frequently exhibit reduced complex working memory span. However, recent evidence indicates that increasing a soundās amplitude increases its duration in echoic memory (Baldwin, in press). Based on these findings, we hypothesized that increasing the amplitude of verbal material would improve working memory efficiency. RESULTS In support of this hypothesis, a strong positive correlation between the amplitude level at which the verbal material was presented and complex working memory span as measured by a version of Daneman and Carpenterās (1980) Listening span task was observed. HST Imaging Polarimetry of the Gravitational Lens FSC10214+4724
We present imaging polarimetry of the extremely luminous, redshift 2.3 IRAS
source FSC10214+4724. The observations were obtained with HST's Faint Object
Camera in the F437M filter, which is free of strong emission lines. The 0.7
arcsec long arc is unresolved to 0.04 arcsec FWHM in the transverse direction,
and has an integrated polarization of 28 +/- 3 percent, in good agreement with
ground-based observations. The polarization position angle varies along the arc
by up to 35 deg. The overall position angle is 62 +/- 3 deg east of north. No
counterimage is detected to B = 27.5 mag (), giving an observed arc to
counterimage flux ratio greater than 250, considerably greater than the flux
ratio of 100 measured previously in the I-band. This implies that the
configuration of the object in the source plane at the B-band is different from
