Evidence-informed health policy 1 – Synthesis of findings from a multi-method study of organizations that support the use of research evidence
Background: Organizations have been established in many countries and internationally to support the use of research evidence by producing clinical practice guidelines, undertaking health technology assessments, and/or directly supporting the use of research evidence in developing health policy on an international, national, and state or provincial level. Learning from these organizations can reduce the need to 'reinvent the wheel' and inform decisions about how best to organize support for such organizations, particularly in low- and middle-income countries (LMICs). Methods: We undertook a multi-method study in three phases – a survey, interviews, and case descriptions that drew on site visits – and in each of the second and third phases we focused on a purposive sample of those involved in the previous phase. We used the seven main recommendations that emerged from the advice offered in the interviews to organize much of the synthesis of findings across phases and methods. We used a constant comparative method to identify themes from across phases and methods. Results: Seven recommendations emerged for those involved in establishing or leading organizations that support the use of research evidence in developing health policy: 1) collaborate with other organizations; 2) establish strong links with policymakers and involve stakeholders in the work; 3) be independent and manage conflicts of interest among those involved in the work; 4) build capacity among those working in the organization; 5) use good methods and be transparent in the work; 6) start small, have a clear audience and scope, and address important questions; and 7) be attentive to implementation considerations, even if implementation is not a remit. Four recommendations emerged for the World Health Organization (WHO) and other international organizations and networks: 1) support collaborations among organizations; 2) support local adaptation efforts; 3) mobilize support; and 4) create global public goods. Conclusion: This synthesis of findings from a multi-method study, along with the more detailed findings from each of the three phases of the study (which are reported in the three following articles in the series), provide a strong basis on which researchers, policymakers, international organizations (and networks) like WHO can respond to the growing chorus of voices calling for efforts to support the use of research evidence in developing health policy
Effects of an evidence service on health-system policy makers' use of research evidence: A protocol for a randomised controlled trial
<p>Abstract</p> <p>Background</p> <p>Health-system policy makers need timely access to synthesised research evidence to inform the policy-making process. No efforts to address this need have been evaluated using an experimental quantitative design. We developed an evidence service that draws inputs from Health Systems Evidence, which is a database of policy-relevant systematic reviews. The reviews have been (a) categorised by topic and type of review; (b) coded by the last year searches for studies were conducted and by the countries in which included studies were conducted; (c) rated for quality; and (d) linked to available user-friendly summaries, scientific abstracts, and full-text reports. Our goal is to evaluate whether a "full-serve" evidence service increases the use of synthesized research evidence by policy analysts and advisors in the Ontario Ministry of Health and Long-Term Care (MOHLTC) as compared to a "self-serve" evidence service.</p> <p>Methods/design</p> <p>We will conduct a two-arm randomized controlled trial (RCT), along with a follow-up qualitative process study in order to explore the findings in greater depth. For the RCT, all policy analysts and policy advisors (n = 168) in a single division of the MOHLTC will be invited to participate. Using a stratified randomized design, participants will be randomized to receive either the "full-serve" evidence service (database access, monthly e-mail alerts, and full-text article availability) or the "self-serve" evidence service (database access only). The trial duration will be ten months (two-month baseline period, six-month intervention period, and two month cross-over period). The primary outcome will be the mean number of site visits/month/user between baseline and the end of the intervention period. The secondary outcome will be participants' intention to use research evidence. For the qualitative study, 15 participants from each trial arm (n = 30) will be purposively sampled. One-on-one semi-structured interviews will be conducted by telephone on their views about and their experiences with the evidence service they received, how helpful it was in their work, why it was helpful (or not helpful), what aspects were most and least helpful and why, and recommendations for next steps.</p> <p>Discussion</p> <p>To our knowledge, this will be the first RCT to evaluate the effects of an evidence service specifically designed to support health-system policy makers in finding and using research evidence.</p> <p>Trial registration</p> <p>ClinicalTrials.gov: <a href="http://www.clinicaltrials.gov/ct2/show/NCT01307228">NCT01307228</a></p
Two-dimensional lattice-fluid model with water-like anomalies
We investigate a lattice-fluid model defined on a two-dimensional triangular
lattice, with the aim of reproducing qualitatively some anomalous properties of
water. Model molecules are of the "Mercedes Benz" type, i.e., they possess a D3
(equilateral triangle) symmetry, with three bonding arms. Bond formation
depends both on orientation and local density. We work out phase diagrams,
response functions, and stability limits for the liquid phase, making use of a
generalized first order approximation on a triangle cluster, whose accuracy is
verified, in some cases, by Monte Carlo simulations. The phase diagram displays
one ordered (solid) phase which is less dense than the liquid one. At fixed
pressure the liquid phase response functions show the typical anomalous
behavior observed in liquid water, while, in the supercooled region, a
reentrant spinodal is observed.Comment: 9 pages, 1 table, 7 figure
Determining research knowledge infrastructure for healthcare systems: a qualitative study
<p>Abstract</p> <p>Background</p> <p>This study examines research knowledge infrastructures (RKIs) found in health systems. An RKI is defined as any instrument (<it>i.e</it>., programs, interventions, tools) implemented in order to facilitate access, dissemination, exchange, and/or use of evidence in healthcare organisations. Based on an environmental scan (17 key informant interviews) and scoping review (26 studies), we found support for a framework that we developed that outlines components that a health system can have in its RKI. The broad domains are climate for research use, research production, activities used to link research to action, and evaluation.</p> <p>The objective of the current study is to profile the RKI of three types of health system organisations--regional health authorities, primary care practices, and hospitals--in two Canadian provinces to determine the current mix of components these organisations have in their RKI, their experience with these components, and their views about future RKI initiatives.</p> <p>Methods</p> <p>This study will include semistructured telephone interviews with a purposive sample region of a senior management team member, library/resource centre manager, and a 'knowledge broker' in three regional health authorities, five or six purposively sampled hospitals, and five or six primary care practices in Ontario and Quebec, for a maximum of 71 interviewees. The interviews will explore (a) which RKI components have proven helpful, (b) barriers and facilitators in implementing RKI components, and (c) views about next steps in further development of RKIs.</p> <p>Discussion</p> <p>This is the first qualitative examination of potential RKI efforts that can increase the use of research evidence in health system decision making. We anticipate being able to identify broadly applicable insights about important next steps in building effective RKIs. Some of the identified RKI components may increase the use of research evidence by decision makers, which may then lead to more informed decisions.</p
Scaling Relations for Logarithmic Corrections
Multiplicative logarithmic corrections to scaling are frequently encountered
in the critical behavior of certain statistical-mechanical systems. Here, a
Lee-Yang zero approach is used to systematically analyse the exponents of such
logarithms and to propose scaling relations between them. These proposed

Evidence-informed health policy 3 – Interviews with the directors of organizations that support the use of research evidence
Background: Previous surveys of organizations that support the development of evidence-informed health policies have focused on organizations that produce clinical practice guidelines (CPGs) or undertake health technology assessments (HTAs). Only rarely have surveys focused at least in part on units that directly support the use of research evidence in developing health policy on an international, national, and state or provincial level (i.e., government support units, or GSUs) that are in some way successful or innovative or that support the use of research evidence in low- and middle-income countries (LMICs). Methods: We drew on many people and organizations around the world, including our project reference group, to generate a list of organizations to survey. We modified a questionnaire that had been developed originally by the Appraisal of Guidelines, Research and Evaluation in Europe (AGREE) collaboration and adapted one version of the questionnaire for organizations producing CPGs and HTAs, and another for GSUs. We sent the questionnaire by email to 176 organizations and followed up periodically with non-responders by email and telephone. Results: We received completed questionnaires from 152 (86%) organizations. More than one-half of the organizations (and particularly HTA agencies) reported that examples from other countries were helpful in establishing their organization. A higher proportion of GSUs than CPG-or HTA-producing organizations involved target users in the selection of topics or the services undertaken. Most organizations have few (five or fewer) full-time equivalent (FTE) staff. More than four-fifths of organizations reported providing panels with or using systematic reviews. GSUs tended to use a wide variety of explicit valuation processes for the research evidence, but none with the frequency that organizations producing CPGs, HTAs, or both prioritized evidence by its quality. Between one-half and two-thirds of organizations do not collect data systematically about uptake, and roughly the same proportions do not systematically evaluate their usefulness or impact in other ways. Conclusion: The findings from our survey, the most broadly based of its kind, both extend or clarify the applicability of the messages arising from previous surveys and related documentary analyses, such as how the 'principles of evidence-based medicine dominate current guideline programs' and the importance of collaborating with other organizations. The survey also provides a description of the history, structure, processes, outputs, and perceived strengths and weaknesses of existing organizations from which those establishing or leading similar organizations can draw

Monte Carlo Study of an Extended 3-State Potts Model on the Triangular Lattice
By introducing a chiral term into the Hamiltonian of the 3-state Potts model
on a triangular lattice additional symmetries are achieved between the
clockwise and anticlockwise states and the ferromagnetic state. This model is
investigated using Monte Carlo methods. We investigate the full phase diagram
and find evidence for a line tricritical points separating the ferromagnetic

From one cell to the whole froth: a dynamical map
We investigate two and three-dimensional shell-structured-inflatable froths,
which can be constructed by a recursion procedure adding successive layers of
cells around a germ cell. We prove that any froth can be reduced into a system
of concentric shells. There is only a restricted set of local configurations
for which the recursive inflation transformation is not applicable. These
configurations are inclusions between successive layers and can be treated as
vertices and edges decorations of a shell-structure-inflatable skeleton. The
recursion procedure is described by a logistic map, which provides a natural
classification into Euclidean, hyperbolic and elliptic froths. Froths tiling
manifolds with different curvature can be classified simply by distinguishing
between those with a bounded or unbounded number of elements per shell, without
any a-priori knowledge on their curvature. A new result, associated with
maximal orientational entropy, is obtained on topological properties of natural
cellular systems. The topological characteristics of all experimentally known

A translational framework for public health research
<p><b>Background</b></p>
<p>The paradigm of translational medicine that underpins frameworks such as the Cooksey report on the funding of health research does not adequately reflect the complex reality of the public health environment. We therefore outline a translational framework for public health research.</p>
<p><b>Discussion</b></p>
<p>Our framework redefines the objective of translation from that of institutionalising effective interventions to that of improving population health by influencing both individual and collective determinants of health. It incorporates epidemiological perspectives with those of the social sciences, recognising that many types of research may contribute to the shaping of policy, practice and future research. It also identifies a pivotal role for evidence synthesis and the importance of non-linear and intersectoral interfaces with the public realm.</p>
<p><b>Summary</b></p>
<p>We propose a research agenda to advance the field and argue that resources for 'applied' or 'translational' public health research should be deployed across the framework, not reserved for 'dissemination' or 'implementation'.</p>
