The end of the era of generosity? Global health amid economic crisis
In the past decade donor commitments to health have increased by 200 percent. Correspondingly, there has been a swell of new players in the global health landscape. The unprecedented, global response to a single disease, HIV/AIDS, has been responsible for a substantial portion of this boon. Numerous health success have followed this windfall of funding and attention, yet the food, fuel, and economic crises of 2008 have shown the vulnerabilities of health and development initiatives focused on short term wins and reliant on a constant flow of foreign funding. For too long, the international community has responded to global health and development challenges with emergency solutions that often reflect the donor's priorities, values, and political leanings, rather than funding durable health systems that can withstand crises. Progress towards achieving the Millennium Development Goals has stalled in many countries. Disease specific initiatives have weakened health systems and limited efforts to improve maternal and child health. As we enter this era of scarce resources, there is a need to return to the foundations of the Alma Ata Declaration signed thirty years ago with the goal of providing universal access to primary healthcare. The global health community must now objectively evaluate how we can most effectively respond to the crises of 2008 and take advantage of this moment of extraordinary attention for global health and translate it into long term, sustainable health improvements for all

Communicable Ulcerative Colitis Induced by T-bet Deficiency in the Innate Immune System
Inflammatory bowel disease (IBD) has been attributed to overexuberant host immunity or the emergence of harmful intestinal flora. The transcription factor T-bet orchestrates inflammatory genetic programs in both adaptive and innate immunity. We describe a profound and unexpected function for T-bet in influencing the behavior of host inflammatory activity and commensal bacteria. T-bet deficiency in the innate immune system results in spontaneous and communicable ulcerative colitis in the absence of adaptive immunity and increased susceptibility to colitis in immunologically intact hosts. T-bet controls the response of the mucosal immune system to commensal bacteria by regulating TNF-α production in colonic dendritic cells, critical for colonic epithelial barrier maintenance. Loss of T-bet influences bacterial populations to become colitogenic, and this colitis is communicable to genetically intact hosts. These findings reveal a novel function for T-bet as a peacekeeper of host-commensal relationships and provide new perspectives on the pathophysiology of IBD
Main-Belt Comet P/2012 T1 (PANSTARRS)
We present initial results from observations and numerical analyses aimed at
characterizing main-belt comet P/2012 T1 (PANSTARRS). Optical monitoring
observations were made between October 2012 and February 2013 using the
University of Hawaii 2.2 m telescope, the Keck I telescope, the Baade and Clay
Magellan telescopes, Faulkes Telescope South, the Perkins Telescope at Lowell
Observatory, and the Southern Astrophysical Research (SOAR) telescope. The
object's intrinsic brightness approximately doubles from the time of its
discovery in early October until mid-November and then decreases by ~60%
between late December and early February, similar to photometric behavior
exhibited by several other main-belt comets and unlike that exhibited by
disrupted asteroid (596) Scheila. We also used Keck to conduct spectroscopic
searches for CN emission as well as absorption at 0.7 microns that could
indicate the presence of hydrated minerals, finding an upper limit CN
production rate of QCN<1.5x10^23 mol/s, from which we infer a water production
rate of QH2O<5x10^25 mol/s, and no evidence of the presence of hydrated
minerals. Numerical simulations indicate that P/2012 T1 is largely dynamically
stable for >100 Myr and is unlikely to be a recently implanted interloper from
the outer solar system, while a search for potential asteroid family
associations reveal that it is dynamically linked to the ~155 Myr-old Lixiaohua

Negative pressure wound therapy with instillation: International consensus guidelines update.
The use of negative pressure wound therapy with instillation and dwell time (NPWTi-d) has gained wider adoption and interest due in part to the increasing complexity of wounds and patient conditions. Best practices for the use of NPWTi-d have shifted in recent years based on a growing body of evidence and expanded worldwide experience with the technology. To better guide the use of NPWTi-d with all dressing and setting configurations, as well as solutions, there is a need to publish updated international consensus guidelines, which were last produced over 6âyears ago. An international, multidisciplinary expert panel of clinicians was convened on 22 to 23 February 2019, to assist in developing current recommendations for best practices of the use of NPWTi-d. Principal aims of the meeting were to update recommendations based on panel members\u27 experience and published results regarding topics such as appropriate application settings, topical wound solution selection, and wound and patient characteristics for the use of NPWTi-d with various dressing types. The final consensus recommendations were derived based on greater than 80% agreement among the panellists. The guidelines in this publication represent further refinement of the recommended parameters originally established for the use of NPWTi-d.
