    BACKGROUND: Observational research suggests that combined therapy with Vitamin C, thiamine and hydrocortisone may reduce mortality in patients with septic shock. METHODS AND DESIGN: The Vitamin C, Thiamine and Steroids in Sepsis (VICTAS) trial is a multicenter, double-blind, adaptive sample size, randomized, placebo-controlled trial designed to test the efficacy of combination therapy with vitamin C (1.5 g), thiamine (100 mg), and hydrocortisone (50 mg) given every 6 h for up to 16 doses in patients with respiratory or circulatory dysfunction (or both) resulting from sepsis. The primary outcome is ventilator- and vasopressor-free days with mortality as the key secondary outcome. Recruitment began in August 2018 and is ongoing; 501 participants have been enrolled to date, with a planned maximum sample size of 2000. The Data and Safety Monitoring Board reviewed interim results at N = 200, 300, 400 and 500, and has recommended continuing recruitment. The next interim analysis will occur when N = 1000. This update presents the statistical analysis plan. Specifically, we provide definitions for key treatment and outcome variables, and for intent-to-treat, per-protocol, and safety analysis datasets. We describe the planned descriptive analyses, the main analysis of the primary end point, our approach to secondary and exploratory analyses, and handling of missing data. Our goal is to provide enough detail that our approach could be replicated by an independent study group, thereby enhancing the transparency of the study.     Molecular Design Parameters of Anthraquinone Dyes for Guest-Host Liquid-Crystal Applications : Experimental and Computational Studies of Spectroscopy, Structure and Stability

    A set of five anthraquinone dyes with bis(4-propylphenyl) substituent groups, connected via sulfide or amine linkages at the 1,5-positions or directly at the 2,6-positions, have been studied in solution by UV-vis spectroscopy and electrochemistry, allied with density functional theory calculations of structures, electronic transitions, and redox potentials. The visible transitions and redox potentials are shown to vary with the HOMO and LUMO energies, with the variation in both color and redox stability between the dyes being attributable principally to variations in the HOMOs located mainly on the substituents and outer anthraquinone rings. The calculated molecular structures and visible transition dipole moments are shown to vary subtly with substituent, giving variations in the molecular aspect ratios, minimum moment of inertia axes, and transition dipole moment vector orientations that can rationalize the alignment trends reported in the literature for such anthraquinone dyes in liquid crystal hosts, showing why 1,5-disulfide and 2,6-diphenyl substituents give better designs than 1,5-diamine substituents. The computational approaches reported here are shown to give good matches with experimental trends, indicating that they may be used more generally to aid the rational molecular design of dyes for applications as guests in liquid crystal hosts

    Massive scalar field in multiply connected flat spacetimes

    The vacuum expectation value of the stress-energy tensor ⟨0∣Tμν∣0⟩\left\langle 0\left| T_{\mu\nu} \right|0\right\rangle is calculated in several multiply connected flat spacetimes for a massive scalar field with arbitrary curvature coupling. We find that a nonzero field mass always decreases the magnitude of the energy density in chronology-respecting manifolds such as R3×S1R^3 \times S^1, R2×T2R^2 \times T^2, R1×T3R^1 \times T^3, the M\"{o}bius strip, and the Klein bottle. In Grant space, which contains nonchronal regions, whether ⟨0∣Tμν∣0⟩\left\langle 0\left| T_{\mu\nu} \right|0\right\rangle diverges on a chronology horizon or not depends on the field mass. For a sufficiently large mass ⟨0∣Tμν∣0⟩\left\langle 0\left| T_{\mu\nu} \right|0\right\rangle remains finite, and the metric backreaction caused by a massive quantized field may not be large enough to significantly change the Grant space geometry.Comment: 19 pages, REVTeX, 5 figures in separate uuencoded compressed fil

    Use of P wave configuration during atrial tachycardia to predict site of origin

    Objectives.This study sought to construct an algorithm to differentiate left atrial from right atrial tachycardia foci on the basis of surface electrocardiograms (ECGs).Background.Atrial tachycardia is an uncommon form of supraventricular tachycardia, often resistant to drug therapy.Methods.A total of 31 consecutive patients with atrial tachycardia due to either abnormal automaticity or triggered rhythm underwent detailed atrial endocardial mapping and successful radiofrequency catheter ablation of a single atrial focus. P wave configuration was analyzed from 12-lead ECGs during tachycardia during either spontaneous or pharmacologically induced atrioventricular block. P waves inscribed above the isoelectric line (TP interval) were classified as positive, below as negative, above and below (or conversely, below and above) as biphasic and flat P waves as isoelectric (0). In 17 patients the tachycardia was located in the right atrium: crista terminalis (n = 4); right atrial appendage (n = 4); lateral wall (n = 4); posteroinferior right atrium (n = 3); tricuspid annulus (n = 1); and near the coronary sinus (n = 1). In 14 patients, atrial tachycardia was located in the left atrium: at the entrance of the right (n = 6) or left (n = 4) superior pulmonary veins; left inferior pulmonary vein (n = 1); inferior left atrium (n = 1); base of left atrial appendage (n = 1); and high lateral left atrium (n = 1).Results.There were no differences in P wave vectors between sites at the right atrial lateral wall versus the right atrial appendage or between sites at the entrance of right versus left superior pulmonary veins. However, analysis of P wave configuration showed that leads aVL and V1were most helpful in distinguishing right atrial from left atrial foci. The sensitivity and specificity of using a positive or biphasic P wave in lead aVL to predict a right atrial focus was 88% and 79%, respectively. The sensitivity and specificity of a positive P wave in lead V1in predicting a left atrial focus was 93% and 88%, respectively.Conclusions.1) Analyses of surface P wave configuration proved to be reasonably good in differentiating right atrial from left atrial tachycardia foci. 2) Leads II, III and aVF were helpful in providing clues for differentiating superior from inferior foci

    Place attachment in deprived neighbourhoods: The impacts of population turnover and social mix

    This paper examines the determinants of individual place attachment, focussing in particular on differences between deprived and others neighbourhoods, and on the impacts of population turnover and social mix. It uses a multi-level modelling approach to take account of both individual- and neighbourhood-level determinants. Data are drawn from a large sample government survey, the Citizenship Survey 2005, to which a variety of neighbourhood-level data have been attached. The paper argues that attachment is significantly lower in more deprived neighbourhoods primarily because these areas have weaker social cohesion but that, in other respects, the drivers of attachment are the same. Turnover has modest direct impacts on attachment through its effect on social cohesion. Social mix has very limited impacts on attachment and the effects vary between social groups. In general, higher status or more dominant groups appear less tolerant of social mix

    Effective theories for real-time correlations in hot plasmas

    We discuss the sequence of effective theories needed to understand the qualitative, and quantitative, behavior of real-time correlators in ultra-relativistic plasmas. We analyze in detail the case where A is a gauge-invariant conserved current. This case is of interest because it includes a correlation recently measured in lattice simulations of classical, hot, SU(2)-Higgs gauge theory. We find that simple perturbation theory, free kinetic theory, linearized kinetic theory, and hydrodynamics are all needed to understand the correlation for different ranges of time.     Nitrogen-induced terrestrial eutrophication: cascading effects and impacts on ecosystem services

    Human activity has significantly increased the deposition of nitrogen (N) on terrestrial ecosystems over pre-industrial levels leading to a multitude of effects including losses of biodiversity, changes in ecosystem functioning, and impacts on human well-being. It is challenging to explicitly link the level of deposition on an ecosystem to the cascade of ecological effects triggered and ecosystem services affected, because of the multitude of possible pathways in the N cascade. To address this challenge, we report on the activities of an expert workshop to synthesize information on N-induced terrestrial eutrophication from the published literature and to link critical load exceedances with human beneficiaries by using the STressor–Ecological Production function–final ecosystem Services Framework and the Final Ecosystem Goods and Services Classification System (FEGS-CS). We found 21 N critical loads were triggered by N deposition (ranging from 2 to 39 kg N·ha−1·yr−1), which cascaded to distinct beneficiary types through 582 individual pathways in the five ecoregions examined (Eastern Temperate Forests, Marine West Coast Forests, Northwestern Forested Mountains, North American Deserts, Mediterranean California). These exceedances ultimately affected 66 FEGS across a range of final ecosystem service categories (21 categories, e.g., changes in timber production, fire regimes, and native plant and animal communities) and 198 regional human beneficiaries of different types. Several different biological indicators were triggered in different ecosystems, including grasses and/or forbs (33% of all pathways), mycorrhizal communities (22%), tree species (21%), and lichen biodiversity (11%). Ecoregions with higher deposition rates for longer periods tended to have more numerous and varied ecological impacts (e.g., Eastern Temperate Forests, eight biological indicators) as opposed to other ecoregions (e.g., North American Deserts and Marine West Coast Forests each with one biological indicator). Nonetheless, although ecoregions differed by ecological effects from terrestrial eutrophication, the number of FEGS and beneficiaries impacted was similar across ecoregions. We found that terrestrial eutrophication affected all ecosystems examined, demonstrating the widespread nature of terrestrial eutrophication nationally. These results highlight which people and ecosystems are most affected according to present knowledge, and identify key uncertainties and knowledge gaps to be filled by future research
