    Adolescência em conflito com a lei:: a intensidade da história de vida em ato

    This study approaches infractional acts of teenagers from the perspective of their life history of conflicts withthe law and the meanings assigned by them to the perpetrated acts. Semi-structured interviews were performed with fourteenagers with lawsuit in process for having committed infractional acts. The data were analyzed using content analysis andinterpreted with psychoanalysis contributions. In the life history of these teenagers four final categories showed evidence ofextreme and recurring experiences of neglect and violence towards parental figures. The fragility of teenager’s psychologicalresources to assign meaning to the experience of being confronted with boundaries and the impossibility to experience andrecognize the law as a protective condition contribute to the production of transgressive acts.Nesta pesquisa o ato infracional é abordado a partir da história de vida de adolescentes em conflito com a lei e dos significados atribuídos por eles aos atos cometidos. Realizaram-se entrevistas semiestruturadas com quatro adolescentes em julgamento por terem cometido atos transgressivos. Os dados foram analisados por meio da Análise de Conteúdo e interpretados com aportes da Psicanálise. Quatro categorias finais permitiram constatar, na história de vida dos jovens, vivências extremas e recorrentes de descuido e violência no encontro com as figuras parentais. A fragilidade de recursos dos jovens para atribuir sentido à experiência de serem confrontados com limites, bem como a impossibilidade de experenciar e reconhecer a Lei como condição de proteção contribuem para a produção do ato transgressivo. &nbsp

    Coalizão talonavicular parcial: Partial talonavicular coalition

    Introdução: Coalizão tarsal se refere à fusão congênita entre dois ou mais ossos do mediopé ou retropé, podendo ser de ordem óssea, cartilaginosa ou fibrosa. O subtipo talonavicular é menos prevalente, já que a coalizão talocalcânea e calcâneonavicular são responsáveis por mais de 90% de todos os casos de coalização tarsal. Apresentação do Caso: LVP, feminino, 24 anos, com queixa de dor crônica em pé direito e dificuldade de deambulação há 10 anos. Clinicamente, observou-se dor à mobilização passiva e diminuição da amplitude de movimento, sendo realizada tomografia computadorizada (TC) de pé direito, que evidenciou coalizão talonavicular parcial. Discussão: A apresentação clínica é frequentemente assintomática, favorecendo maior progressão de doença e evolução para complicações que acarretam maior morbidade, como a osteoartrite de mediopé. O tratamento conservador deve ser indicado inicialmente e, em casos de refratariedade à abordagem clínica, recomenda-se a ressecção cirúrgica da coalizão com interposição de enxerto tecidual. Conclusão: A coalizão talonavicular é um subtipo raro e infrequente dentre as coalizões tarsais, sendo uma causa subdiagnosticada de dor crônica no tornozelo e pé, associando-se, portanto, com maior morbidade em virtude do diagnóstico tardio.&nbsp

    Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19

    IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19. Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19. DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022). INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes. RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively). CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.     Este estudo teve por objetivo verificar a relação entre a presença de transtornos de ansiedade e risco de suicídio em jovens. Em delineamento transversal de base populacional, os jovens respondiam a um questionário sobre questões sociodemográficas, comportamentais e de saúde. Avaliou-se o bem-estar psicológico através da Escala de Faces de Andrews e consumo de substâncias foi efetuado pelo Teste de Triagem do Envolvimento com Álcool, Cigarro e Outras Substâncias, (Alcohol, Smoking and Substance Involviment Screening Test - ASSIST). A Mini Internacional Neuropsychiatric Interview 5.0 avaliou transtornos de ansiedade e o risco de suicídio. Dos 1.621 jovens entrevistados, 20,9% apresentaram algum transtorno de ansiedade e 8,6% risco de suicídio. A presença de algum transtorno de ansiedade esteve significativamente associada ao risco de suicídio (RP 6,10 IC95% 3,95 a 9,43). Dessa forma, salienta-se a importância de maior atenção ao risco de suicídio também em pacientes com transtornos de ansiedade.<br>Este estudio tubo por objetivo verificar la relación entre la presencia de trastornos de ansiedad y riesgo de suicidio en jóvenes. De los 1.621 jóvenes entrevistados, 20,9% presentaron algún trastorno de ansiedad y 8,6% riesgo de suicidio. La presencia de algún trastorno de ansiedad estuvo significativamente asociada al riesgo de suicidio (RP 6,10 IC95% 3,95 a 9,43). De esa forma, se resalta la importancia de mayor atención al riesgo de suicidio también en pacientes con trastornos de ansiedad.<br>This study aimed at investigating the relationship between the presence of anxiety disorders and suicide risk in young adults. In a cross-sectional population-based design young adults answered a questionnaire about sociodemographic, behavioral and health characteristics. The measure of psychological well-being was made by Faces Scale of Andrews while the substances use evaluation was performed according to the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST). The Mini International Neuropsychiatric Interview 5.0 evaluated anxiety disorders and suicide risk. Among the 1.621 young adults interviewed, 20.9% had some anxiety disorders and 8.6% suicide risk. After multivariate analysis, the presence of any anxiety disorder was significantly associated with suicide risk (PR 6.10 95% CI 3.95 to 9.43). Thus, it is highlighted the importance of greater attention to suicide risk in those patients with anxiety disorders

    NEOTROPICAL CARNIVORES: a data set on carnivore distribution in the Neotropics

    Mammalian carnivores are considered a key group in maintaining ecological health and can indicate potential ecological integrity in landscapes where they occur. Carnivores also hold high conservation value and their habitat requirements can guide management and conservation plans. The order Carnivora has 84 species from 8 families in the Neotropical region: Canidae; Felidae; Mephitidae; Mustelidae; Otariidae; Phocidae; Procyonidae; and Ursidae. Herein, we include published and unpublished data on native terrestrial Neotropical carnivores (Canidae; Felidae; Mephitidae; Mustelidae; Procyonidae; and Ursidae). NEOTROPICAL CARNIVORES is a publicly available data set that includes 99,605 data entries from 35,511 unique georeferenced coordinates. Detection/non-detection and quantitative data were obtained from 1818 to 2018 by researchers, governmental agencies, non-governmental organizations, and private consultants. Data were collected using several methods including camera trapping, museum collections, roadkill, line transect, and opportunistic records. Literature (peer-reviewed and grey literature) from Portuguese, Spanish and English were incorporated in this compilation. Most of the data set consists of detection data entries (n = 79,343; 79.7%) but also includes non-detection data (n = 20,262; 20.3%). Of those, 43.3% also include count data (n = 43,151). The information available in NEOTROPICAL CARNIVORES will contribute to macroecological, ecological, and conservation questions in multiple spatio-temporal perspectives. As carnivores play key roles in trophic interactions, a better understanding of their distribution and habitat requirements are essential to establish conservation management plans and safeguard the future ecological health of Neotropical ecosystems. Our data paper, combined with other large-scale data sets, has great potential to clarify species distribution and related ecological processes within the Neotropics. There are no copyright restrictions and no restriction for using data from this data paper, as long as the data paper is cited as the source of the information used.
