    The assessment of postpartum mood disorders and its risk factors

    Wstęp: Celem pracy jest ocena rozpowszechnienia zaburzeń nastroju z uwzględnieniem występowania depresji poporodowej oraz analiza czynników predysponujących do ich występowania. Materiał i metody: Grupą badaną były pacjentki Ginekologiczno-Położniczego Szpitala Klinicznego Uniwersytetu Medycznego w Poznaniu, przebywające na oddziałach poporodowych. Badanie miało charakter kwestionariuszowy. W pracy wykorzystano skalę depresji Becka, Edynburską Skalę Depresji Poporodowej (EPDS) i kwestionariusz własny. Pacjentki wypełniły anonimowy kwestionariusz i skalę depresji Becka w 1.−7. dni po porodzie (n = 184), a następnie przesłano im skalę depresji Becka oraz EPDS po 30 dniach od porodu w celu ponownej oceny nastroju. Uzyskano 41 odpowiedzi zwrotnych. Badanie uzyskało zgodę Lokalnej Komisji Bioetycznej. Wyniki: Wśród badanych pacjentek będących w 1.−7. dniu po porodzie: 11,96% uzyskało wynik świadczący o łagodnej depresji, a 0,54% o umiarkowanej według skali Becka. Po około 30 dniach od porodu wynik ten zwiększył się do 19,51% dla łagodnej i odpowiednio 2,44% dla umiarkowanej depresji. Wnioski: Na podstawie uzyskanych wyników czynnikami predysponującymi do wystąpienia zaburzeń nastroju w 1.−7. dni po porodzie były: nieplanowana ciąża, komplikacje zdrowotne podczas trwania ciąży, pobyt w szpitalu z powodu zagrożenia ciąży, epizody depresji w przeszłości oraz wystąpienie myśli rezygnacyjnych lub samobójczych w przeszłości. Wystąpienie w przeszłości stresującego wydarzenia w postaci poronienia, martwego porodu lub śmierci dziecka wiązało się z uzyskiwaniem niższych wyników w skali Becka przez badane pacjentki. W pracy nie wykazano istotnej statystycznie zależności pomiędzy uzyskanymi wynikami w skali Becka a badanymi czynnikami ocenianymi miesiąc od porodu, analizowana grupa była istotnie mniejsza (n = 41).Introduction: The aim of our study was to estimate the prevalence of mood disorders including postpartum depression and to analyze its potential risk factors. Material and methods: The patients staying in maternity wards at the Gynecological Clinical Hospital of PUMS were examined during their postpartum period. Depressive symptoms were measured using the Edinburgh Postnatal Depression Scale (EPDS), Beck Depression Inventory (BDI) and our own questionnaire. The anonymous questionnaire and BDI were filled by the patients between 1st and 7th day after the delivery (n = 184), then they were asked to fill the BDI and EPDS after 30 days of the childbed (n = 41 patients refilled the survey). The research has obtained permission of the Local Bioethical Commission. Results: During the first week of the childbed, 11.96% of the patients achieved score, which might imply they suffer from mild depression and 0.54% from major depression according to BDI. After the second examination the score increased to 19.51% and 2.44%, respectively. Conclusions: The unplanned pregnancy, health issues during the gestation, hospitalization due to pregnancy complications, no stressful factors in the past like: history of miscarriage, stillbirth or death of the child, episodes of depression in the past, suicidal ideation in the history can be bracketing the risk factors of mood disorders after 1 to 7 days of the childbed. The significant correlation between score of the BDI and the answers in the questionnaire wasn’t demonstrated after 30 days of the postpartum period, however the analyzed group was significantly lower (n = 41)

    A Downstream CpG Island Controls Transcript Initiation and Elongation and the Methylation State of the Imprinted Airn Macro ncRNA Promoter

    A CpG island (CGI) lies at the 5′ end of the Airn macro non-protein-coding (nc) RNA that represses the flanking Igf2r promoter in cis on paternally inherited chromosomes. In addition to being modified on maternally inherited chromosomes by a DNA methylation imprint, the Airn CGI shows two unusual organization features: its position immediately downstream of the Airn promoter and transcription start site and a series of tandem direct repeats (TDRs) occupying its second half. The physical separation of the Airn promoter from the CGI provides a model to investigate if the CGI plays distinct transcriptional and epigenetic roles. We used homologous recombination to generate embryonic stem cells carrying deletions at the endogenous locus of the entire CGI or just the TDRs. The deleted Airn alleles were analyzed by using an ES cell imprinting model that recapitulates the onset of Igf2r imprinted expression in embryonic development or by using knock-out mice. The results show that the CGI is required for efficient Airn initiation and to maintain the unmethylated state of the Airn promoter, which are both necessary for Igf2r repression on the paternal chromosome. The TDRs occupying the second half of the CGI play a minor role in Airn transcriptional elongation or processivity, but are essential for methylation on the maternal Airn promoter that is necessary for Igf2r to be expressed from this chromosome. Together the data indicate the existence of a class of regulatory CGIs in the mammalian genome that act downstream of the promoter and transcription start

    This paper offers an insight into the phenomenon of mode(l)s identities examples occurrence in contemporary culture by analyzing the fashion blog. The work is based on Z. Bauman's, B. Rejakowa's and T. Leszniewski's conceptual analysis of identities examples as a framework and further on expanded by a problem of the consumption society.     Praca stanowi analizę procesu adaptacji Trylogii Henryka Sienkiewicza, na spektakl Jana Klaty. Praca ma na celu przybliżenie czytelnikowi pracy twórczej reżysera, dramaturga, aktorów i innych twórców spektaklu, oraz przybliżenie procesu adaptowania tekstu literackiego na scenariusz spektaklu i sam spektakl. W pierwszym rozdziale opisuję, analizuję oraz porównuję narrację w powieści Sienkiewicza i w spektaklu Jana Klaty. W drugim rozdziale opisuję i analizuję postacie literackie oraz porównuję je z postaciami scenicznymi stworzonymi przez reżysera. W trzecim rozdziale analizuję zagadnienia przestrzeni, miejsca i czasu, zarówno w powieści, jak i w spektaklu. Czwarty rozdział dotyczy muzyki występującej w przedstawieniu. W piątym, ostatnim rozdziale opisuję oraz analizuję mity Sienkiewicza oraz mity Jana Klaty w kontekście Trylogii. Zakończenie stanowi podsumowanie mojej pracy oraz wnioski.The work constitutes analysis of the adaptation process of the Trilogy of Henryk Sienkiewicz, to the performance of Jan Klata. The work is aimed at introducing the reader to creative work of the director, dramatist, actors and other authors of the performance, and moving closer the process of adapting literary texts to the script of the performance and alone performance. In the first chapter I am describing, I am analysing as well as I am comparing the narration in the novel of Sienkiewicz and in the performance of Jan Klata. In the second chapter I am describing and I am analysing literary forms as well as I am comparing them with stage figures created by the director. In the third chapter I am analysing issues of the space, of place and time, both in the novel, and in the performance. The fourth chapter is regarding the music acting in the show. In the fifth, last chapter I am describing as well as I am analysing myths of Sienkiewicz and myths of Jan Klata in the context of the Trilogy. The end constitutes recapitulating my work and conclusions

    Genetic and Environmental Risk Factors for Isolated Hemangiomas in Infants

    The goal of this analysis is to identify risk factors for infantile hemangiomas (IH) to better delineate hemangioma predisposition. We analyzed live birth children with isolated cutaneous hemangioma that were reported to the Polish Registry of Congenital Malformations from across Poland between the years 1998 and 2016. Lower birthweight and gestational age were the most significant risk factors associated with IH. We also observed a trend for a higher risk of IH with a lower level of maternal and paternal education. Moreover, mothers with IH have a higher probability of having a child with IH compared to fathers. However, this association is only present when the child is female. Similarly, a higher risk of hemangioma in a female child is found among mothers having relatives of the first degree with IH, compared to fathers with a similar pedigree. Our results suggest the role of exogenous factors in the etiology of IH. The analysis of familial cases suggests a multifactorial model of inheritance. The study indicates that female gender is an important risk factor for the expression of familial IH. Potential interaction of genetic risk factors with exposure to female sex hormones may play a role in the development of IH

    Influence of the Physical Training on Muscle Function and Walking Distance in Symptomatic Peripheral Arterial Disease in Elderly

    Introduction. A typical symptom of chronic lower-limb ischaemia is lower-limb pain, which occurs during walking forcing the patient to stop, intermittent claudication (IC). Exercise rehabilitation is the basic form of treatment for these patients. Aim. The aim of this study was to compare the effectiveness of three types of physical training programmes conducted over a 12-week period in patients with chronic lower-limb arterial insufficiency. Materials and Methods. Ninety-five people qualified for the 3-month supervised motor rehabilitation programme, conducted three times a week. The respondents were assigned to three types of rehabilitation programmes using a pseudo-randomization method: Group I (TW), subjects undertaking treadmill walking training; Group II (NW), subjects undertaking Nordic walking training; Group III (RES+NW), subjects undertaking resistance and Nordic walking training. Treadmill test, 6 Minute Walk Test (6MWT), and isokinetic test were repeated after 3 months of rehabilitation, which 80 people completed. Results. Combined training (RES+NW) is more effective than Nordic walking alone and supervised treadmill training alone for improving ankle force-velocity parameters (p<0.05) in patients with intermittent claudication. Each of the proposed exercise rehabilitation programmes increased walking distance of patients with intermittent claudication (p<0.05), especially in 6MWT (p=0.001). Significant relationships of force-velocity parameters are observed in the maximum distance obtained in 6MWT, both in Group III (RES + NW) and in Group II (NW) at the level of moderate and strong correlation strength, which indicates that if the lower limbs are stronger the walking distance achieved in 6MWT is longer. Conclusions. Given both the force-velocity parameters and the covered distance, the training RES + NW gives the most beneficial changes compared to training TW alone and NW alone. All types of training increased walking distance, which is an important aspect of the everyday functioning of people with IC

    Geospatial clustering of gastroschisis in Poland: Data from the Polish Registry of Congenital Malformations (PRCM)

    Objectives: The aims of this study were: to evaluate the prevalence of abdominal wall defects in the Polish population, to analyze temporal trends in the prevalence, to identify areas (clusters) of high risk of abdominal wall defects, and to characterize, with respect to epidemiology, children with abdominal wall defects and their mothers in the area defined as a cluster. Material and Methods: We used isolated congenital malformations (gastroschisis Q79.3 and omphalocele Q79.2 according to the International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10, the extended version)) data reported to the Polish Registry of Congenital Malformations (PRCM) over the years 1998– 2008 based on the population of 2 362 502 live births. We analyzed 11 administrative regions of Poland with complete epidemiologic data. Results: Of 11 regions, 2 had a significantly higher standardized prevalence of isolated gastroschisis: Dolnośląskie (1.7/10 000 live births, p = 0.0052) and Śląskie (1.9/10 000 live births, p < 0.0001). Furthermore, within the region of Dolnośląskie, we defined a clear prevalence of the isolated gastroschisis cluster (p = 0.023). We comprehensively examined demographic and socio-economic risk factors for abdominal wall defects in this area, and we found that these factors failed to account for the cluster. Conclusions: We identified a distinct prevalence cluster for isolated gastroschisis, although a precise reason for the disease clustering in this region remains unknown. Cluster identification enables more focused research aimed at identification of specific factors with teratogenic effects
