    Energy and seismic drawbacks of masonry: a unified retrofitting solution

    All over the world, a large part of existing buildings is not adequate to satisfy the safety requirement and the thermal comfort criteria. For this reason, the interest in structural and energy retroftting systems has steadily grown in the last decades. In this scenario, an innovative thermal resistant geopolymer mortar has been developed and used for Inorganic Matrix Composite (IMC) systems aimed to a combined seismic and energy new retroftting technique. The geopolymer-based IMC is able to ensure competitive mechanical properties with respect to the traditional lime-based IMCs and, at the same time, a signifcant reduction in thermal conductivity. In this paper, an experimental program is reported considering small-scaled masonry panels with double-side IMC-retroftting and determining both the in-plane shear strength and the thermal resistance. The experimental shear tests are aimed to compare the mechanical performance of the geopolymer innovative systems with those of the traditional lime-based ones. Moreover, the thermal resistance gain of the innovative solutions was measured and compared with traditional systems. The results evidenced the efectiveness of the proposed technique that signifcantly improved the performances of masonry walls from both the thermal and the mechanical point of view

    A practical approach to the use of low molecular weight heparins in VTE treatment and prophylaxis in children and newborns.

    Low-molecular weight heparins are currently the most commonly used anticoagulants in children and newborns. However, since thrombotic complications rarely occur outside large children’s hospitals, physicians often encounter some practical problems inmanaging these treatments when a pediatric thrombosis specialist is not available. The drug of choice is enoxaparin, due to its favorable FXa/FIIa ratio and the availability of pharmacokinetic and pharmacodynamic data. The treatment of acute thrombosis should be started with two daily injections but when compliance is an issue, a single daily administration schedule could be chosen for secondary prophylaxis ensuring careful measurement of the post 24-hour anti-FXa activity. Furthermore, a subcutaneous device may be a useful tool and a topical dermal anesthetic could be effective in controlling pain without affecting anti-FXa levels. In neonate and toddlers, where mini doses are frequently needed, the dead space of syringes and needles could represent an issue and therefore the use of insulin syringes without dead space is advisable,while a dilution of the drug is useful with other syringes. This article derives froma nonsystematic review of the available literature, with special attention to recent international guidelines and expert recommendations, combined to authors’ clinical practice in large tertiary pediatric hospitals and will provide concise and practical information for the use of low-molecular weight heparin in childhood and infancy in a sort of “answering frequently asked questions.

    3D MODELLING AND MEDIEVAL LIGHTING RECONSTRUCTION FOR RUPESTRIAN CHURCHES

    The aim of this research is to define a method, able to analyse the systems of illumination in the medieval period, through a 3D virtual model reconstruction. It specifically needs to deepen the interactions between natural and artificial illumination in confined spaces. The study describes and examines the best survey technique to obtain a 3D model reconstruction, according to the achievement of such lighting scenarios, and the most suitable tools for lighting analysis. Thus, it is possible to carry out accurate interpretations of the past starting from historical sources and scientific data about lighting. This method was validated on a case study of a rupestrian church in Matera. The rupestrian contexts are less investigated in relation to mediaeval artificial lighting even if there are recognitions and comparative studies of widespread contexts, not only in Puglia and Basilicata, but also in Mediterranean area (e.g. in Cappadocia and Cyprus) which allow identifying evidences related to artificial lighting systems. The architectural signs (hole, niches, etc.) for ancient luminaries can be studied in 3D and lighting prospective. Lastly, the virtual model of the rupestrian church was imported into an opensource visualisation software (3D Heritage Online Presenter) to be appreciable on line and to promote historical heritage knowledge

    Comparative analysis of density histograms and visual scores in incremental and volumetric high-resolution computed tomography of the chest in idiopathic pulmonary fibrosis patients

    Background: Volumetric high-resolution computed tomography (HRCT) of the chest has recently replaced incremental CT in the diagnostic workup of idiopathic pulmonary fibrosis (IPF). Concomitantly, visual and quantitative scores have been proposed for disease extent assessment to ameliorate disease management. Purpose: To compare the performance of density histograms (mean lung attenuation, skewness, and kurtosis) and visual scores, along with lung function correlations, in IPF patients submitted to incremental or volumetric thorax HRCT. Material and methods: Clinical data and CT scans of 89 newly diagnosed and therapy-naive IPF patients were retrospectively evaluated. Results: Forty-six incremental and 43 volumetric CT scans were reviewed. No differences of density histograms and visual scores estimates were found by comparing two HRCT techniques, with an optimal inter-operator agreement (concordance correlation coefficient >0.90 in all instances). Single-breath diffusing lung capacity for carbon monoxide (DLCOsb) was inversely related with the Best score (r = −00.416; p = 0.014), the Kazerooni fibrosis extent (r = −0.481; p = 0.004) and the mean lung attenuation (r = −0.382; p = 0.026), while a positive correlation was observed with skewness (r = 0.583; p = 0.001) and kurtosis (r = 0.543; p = 0.001) in the incremental HRCT sub-group. Similarly, in the volumetric CT sub-cohort, DLCOsb was significantly associated with skewness (r = 0.581; p = 0.007) and kurtosis (r = 0.549; p = 0.018). Correlations with visual scores were not confirmed. Forced vital capacity significantly related to all density indices independently on HRCT technique. Conclusions: Density histograms and visual scores similarly perform in incremental and volumetric HRCT. Density quantification displays an optimal reproducibility and proves to be superior to visual scoring as more strongly correlated with lung function

    Primary pleural haemangioendothelioma in an Italian female patient: a case report and review of the literature

    Primary epithelioid haemangioendothelioma (EHE) of the pleura is a rare vascular tumour that occurs mainly in men. Pleural effusion and thickening are the most common clinical presentations. A 58 year old female, nonsmoking patient presented to us with dry cough, dyspnoea and left chest pain for several weeks (no asbestos exposure). Standard chest X-ray and contrast enhanced multislice computed tomography revealed a large-size lobulated mass originating from the pleura which was diagnosed as primary pleural haemangioendothelioma (PHE) by histology and immunohistochemistry (reactivity for vimentin, CD31, CD34, Factor VIII and ulex europeaus). No metastases were detected. The patient refused treatment and died three months later due to the onset of acute and progressive respiratory failure. Despite the lack of highgrade malignancy, primary PHE displays a poor prognosis while curative therapies are actually not available. To our knowledge, this is the first case of primary PHE in a female patient occurring in Italy and the third one to have been reported in English literature. Difficulties in diagnosis and treatment management are discussed below

    Non-alcoholic fatty liver disease is associated with early left ventricular dysfunction in childhood acute lymphoblastic leukaemia survivors.

    Background: Childhood acute lymphoblastic leukaemia (ALL) survivors have an increased risk of metabolic and cardiovascular disease. We aimed to assess the presence of non-alcoholic fatty liver disease (NAFLD) in childhood ALL and if it is associated with early cardiovascular dysfunction. Methods: In total, 53 childhood ALL survivors and 34 controls underwent auxological evaluation, biochemical assay, liver, heart and vascular ultrasound study. Results: NAFLD was more frequent in ALL patients than in controls (39.6% vs 11.7%, P < 0.01). Patients with NAFLD were more obese and insulin resistant than patients without NAFLD. Flow-mediated dilatation and interventricular septum were lower in the ALL group than those in the control group (P < 0.001 for both). The patients with NAFLD showed lower left ventricular ejection fraction than those without NAFLD (P = 0.011). In ALL survivors, BMI-SDS and subcutaneous fat were the strongest predictors of NAFLD, whereas preperitoneal adipose tissue and C-reactive protein were the strongest predictors of left ventricular ejection fraction. Conclusions: Childhood ALL survivors had higher prevalence of NAFLD than healthy controls, which is associated with early left ventricular impairment. In the case of fatty liver, a comprehensive heart evaluation is mandatory. We strongly recommend to prevent visceral adiposity in ALL survivors, to search for metabolic syndrome or its components and to reinforce the need of intervention on diet and lifestyle during the follow-up of these patients

In altri passaggi il nostro autore polemizza contro diverse dottrine, di origine platonica o gnostica, sostenendo la veridicit\ue0 della percezione sensoriale (Marc 4, 8, 2-3 ; ma soprattutto An 17) : in questo caso l\u2019antecedente \ue8 il testo aristotelico, che veicola le dottrine dei presocratici (De An. 3, 3, 427 a).
