    Obesity and the Mediterranean Diet: A Review of Evidence of the Role and Sustainability of the Mediterranean Diet

    Several different socio-economic factors have caused a large portion of the population to adopt unhealthy eating habits that can undermine healthcare systems, unless current trends are inverted towards more sustainable lifestyle models. Even though a dietary plan inspired by the principles of the Mediterranean Diet is associated with numerous health benefits and has been demonstrated to exert a preventive effect towards numerous pathologies, including obesity, its use is decreasing and it is now being supplanted by different nutritional models that are often generated by cultural and social changes. Directing governments' political actions towards spreading adherence to the Mediterranean Diet's principles as much as possible among the population could help to tackle the obesity epidemic, especially in childhood. This document intends to reiterate the importance of acting in certain age groups to stop the spread of obesity and proceeds with a critical review of the regulatory instruments used so far, bearing in mind the importance of the scientific evidence that led to the consideration of the Mediterranean Diet as not just a food model, but also as the most appropriate regime for disease prevention, a sort of complete lifestyle plan for the pursuit of healthcare sustainability

    Impact of rhinovirus infections in children

    Rhinovirus (RV) is an RNA virus that causes more than 50% of upper respiratory tract infections in humans worldwide. Together with Respiratory Syncytial Virus, RV is one of the leading causes of viral bronchiolitis in infants and the most common virus associated with wheezing in children aged between one and two years. Because of its tremendous genetic diversity (>150 serotypes), the recurrence of RV infections each year is quite typical. Furthermore, because of its broad clinical spectrum, the clinical variability as well as the pathogenesis of RV infection are nowadays the subjects of an in-depth examination and have been the subject of several studies in the literature. In fact, the virus is responsible for direct cell cytotoxicity in only a small way, and it is now clearer than ever that it may act indirectly by triggering the release of active mediators by structural and inflammatory airway cells, causing the onset and/or the acute exacerbation of asthmatic events in predisposed children. In the present review, we aim to summarize the RV infection’s epidemiology, pathogenetic hypotheses, and available treatment options as well as its correlation with respiratory morbidity and mortality in the pediatric population

    Volume unbalance on the 2016 Amatrice - Norcia (Central Italy) seismic sequence and insights on normal fault earthquake mechanism

    We analyse the M w 6.5, 2016 Amatrice-Norcia (Central Italy) seismic sequence by means of InSAR, GPS, seismological and geologic data. The >1000 km 2 area affected by deformation is involving a volume of about 6000 km 3 and the relocated seismicity is widely distributed in the hangingwall of the master fault system and the conjugate antithetic faults. Noteworthy, the coseismically subsided hangingwall volume is about 0.12 km 3 , whereas the uplifted adjacent volumes uplifted only 0.016 km 3 . Therefore, the subsided volume was about 7.5 times larger than the uplifted one. The coseismic motion requires equivalent volume at depth absorbing the hangingwall downward movement. This unbalance regularly occurs in normal fault-related earthquakes and can be inferred as a significant contribution to coseismic strain accomodated by a stress-drop driven collapse of precursory dilatancy. The vertical coseismic displacement is in fact larger than the horizontal component, consistent with the vertical orientation of the maximum lithostatic stress tensor

    An inverse modeling approach to obtain P-T conditions of metamorphic stages involving garnet growth and resorption

    This contribution presents an approach and a computer program (GRTMOD) for numerical simulation of garnet evolution based on compositions of successive growth zones in natural samples. For each garnet growth stage, a new local effective bulk composition is optimized, allowing for resorption and/or fractionation of previously crystallized garnet. The successive minimizations are performed using the Nelder-Mead algorithm; a heuristic search method. An automated strategy including two optimization stages and one refinement stage is described and tested. This program is used to calculate pressure-temperature (P-T) conditions of crystal growth as archived in garnet from the Sesia Zone (Western Alps). The compositional variability of successive growth zones is characterized using standardized X-ray maps and the program XMapTools. The model suggests that Permian garnet cores crystallized under granulite-facies conditions at T>800 °C and P = 6 kbar. During Alpine times, a first garnet rim grew at eclogite-facies conditions (650 °C, 16 kbar) at the expense of the garnet core. A second rim was added at lower P (∌11 kbar) and 630 °C. In total, garnet resorption is modeled to amount to ∌9 vol% during the Alpine evolution; this value is supported by our observations in X-ray compositional maps

    Using groundwater monitoring wells for rapid application of soil gas radon deficit technique to evaluate residual LNAPL

    The application of the 222Radon (Rn) deficit technique using subsurface soil gas probes for the identification and quantification of light non-aqueous phase liquids (LNAPL) has provided positive outcomes in recent years. This study presents an alternative method for applying this technique in the headspace of groundwater monitoring wells. The developed protocol, designed for groundwater monitoring wells with a portion of their screen in the vadose zone, is based on the use of portable equipment that allows rapid measurement of the Rn soil gas activity in the vadose zone close to the water table (i.e., smear zone) where LNAPL is typically expected. The paper first describes the step-by-step procedure to be followed for the application of this method. Then, a preliminary assessment of the potential of the method was carried out at two Italian sites characterized by accidental gasoline and diesel spills into the subsurface from underground storage tanks. Although the number of tests conducted does not allow for definitive conclusions, the results obtained suggest that, from a qualitative point of view, Rn monitoring in the headspace of monitoring wells is a promising, fast, and minimally invasive screening method that could also potentially reduce the costs associated with field data acquisition. This method proves to be suitable for detecting the presence of LNAPL in both the mobile and residual phases with results consistent with the other lines of evidence available at the sites, such as groundwater and soil gas monitoring. Future efforts should be directed toward evaluating the accuracy of this method for a quantitative assessment of residual LNAPL saturations

    Social Determinants of Health and Distance Learning in Italy in the Era of the SARS-CoV-2 Pandemic

    Objectives: To investigate the experiences by distance learning (DL) method during the first wave of the SARS-CoV-2 pandemic in Italy, and to search for correlations with purported experiences and respective levels of social determinants of health (SDH). Study design and methods: Cross-sectional online survey, investigating various SDH and parents’ attitude towards DL, proposed 6 months after the beginning of the pandemic to a sample population of parents with school-aged children throughout Italy. Results: A total of 3791 questionnaires were analyzed. Non-Italian parents complained more frequently of difficulties in providing support to their children in DL due to poor digital skills (p = 0.01), lack of good-quality digital equipment (p = 0.01), problems with the Italian language (p < 0.001), and a lower level of education (p < 0.001). When parents lived apart, greater difficulties in concentration in children using DL (p = 0.05) and a lower parental capacity to support DL (p = 0.002) were reported. Adequate digital structures appeared related to living in owned compared to rented property, higher levels of parental education, and better familial financial situations. Conclusions: Students from families with financial difficulties and low levels of parental education, or even those living in houses for rent or having separated parents, may be disadvantaged in an educational context since the introduction of DL

    The Relationship between Gut Microbiota and Respiratory Tract Infections in Childhood: A Narrative Review

    Respiratory tract infections (RTIs) are common in childhood and represent one of the main causes of hospitalization in this population. In recent years, many studies have described the association between gut microbiota (GM) composition and RTIs in animal models. In particular, the “inter-talk” between GM and the immune system has recently been unveiled. However, the role of GM in human, and especially infantile, RTIs has not yet been fully established. In this narrative review we provide an up-to-date overview of the physiological pathways that explain how the GM shapes the immune system, potentially influencing the response to common childhood respiratory viral infections and compare studies analysing the relationship between GM composition and RTIs in children. Most studies provide evidence of GM dysbiosis, but it is not yet possible to identify a distinct bacterial signature associated with RTI predisposition. A better understanding of GM involvement in RTIs could lead to innovative integrated GM-based strategies for the prevention and treatment of RTIs in the paediatric population

    Morpho-Tectonic Evolution of the Southern Apennines and Calabrian Arc: Insights From Pollino Range and Surrounding Extensional Intermontane Basins

    The evolution of topography in forearc regions results from the complex interplay of crustal and mantle processes. The Southern Apennines represent a well-studied forearc region that experienced several tectonic phases, initially marked by compressional deformation followed by extension and large-scale uplift. We present a new structural, geomorphic and fluvial analysis of the Pollino Massif and surrounding intermontane basins (Mercure, Campotenese and Castrovillari) to unravel their evolution since the Pliocene. We constrain multiple tectonic transport directions, evolution of the drainage, and magnitude and timing of long-term incision following base level falls. Two sets of knickpoints suggest two phases of base level lowering and allow to estimate similar to 500 m of long-term uplift (late Pleistocene), as observed in the Sila Massif. On a smaller spatial scale, the evolution and formation of topographic relief, sedimentation, and opening of intermontane basins is strongly controlled by the recent increase in rock uplift rate and fault activity. At the regional scale, an along-strike, long-wavelength uplift pattern from north to south can be explained by progressive lateral slab tearing and inflow of asthenospheric mantle beneath Pollino and Sila, which in turn may have promoted extensional tectonics. The lower uplift of Le Serre Massif may be explained as result of weak plate coupling due to narrowing of the Calabrian slab. The onset of uplift in the Pollino Massif, ranging from 400 to 800 ka, is consistent with that one proposed in the southern Calabrian forearc, suggesting a possible synchronism of uplift, and lateral tearing of the Calabrian slab.Topographic evolution constrained by structural, geomorphic and river analysis of the Pollino range and surrounding extensional basins At short spatial scale, increase in rock uplift and fault activity controls the endorheic-exorheic transition At regional scale, uplift increases between 400 and 800 ka, due to progressive lateral slab tearing, and inflow asthenospheric mantl

    Passive immunoprophylaxis against respiratory syncytial virus in children: Where are we now?

    Respiratory syncytial virus (RSV) represents the main cause of acute respiratory tract infections in children worldwide and is the leading cause of hospitalization in infants. RSV infection is a self-limiting condition and does not require antibiotics. However hospitalized infants with clinical bronchiolitis often receive antibiotics for fear of bacteria coinfection, especially when chest radiography is performed due to similar radiographic appearance of infiltrate and atelectasis. This may lead to unnecessary antibiotic prescription, additional cost, and increased risk of development of resistance. Despite the considerable burden of RSV bronchiolitis, to date, only symptomatic treatment is available, and there are no commercially available vaccines. The only licensed passive immunoprophylaxis is palivizumab. The high cost of this monoclonal antibody (mAb) has led to limiting its prescription only for high-risk children: infants with chronic lung disease, congenital heart disease, neuromuscular disorders, immunodeficiencies, and extreme preterm birth. Nevertheless, it has been shown that the majority of hospitalized RSV-infected children do not fully meet the criteria for immune prophylaxis. While waiting for an effective vaccine, passive immune prophylaxis in children is mandatory. There are a growing number of RSV passive immunization candidates under development intended for RSV prevention in all infants. In this review, we describe the state-of-the-art of palivizumab’s usage and summarize the clinical and preclinical trials regarding the development of mAbs with a better cost-effectiveness ratio

    Inoculated mammary carcinoma-associated fibroblasts: contribution to hormone independent tumor growth

    <p>Abstract</p> <p>Background</p> <p>Increasing evidence has underscored the role of carcinoma associated fibroblasts (CAF) in tumor growth. However, there are controversial data regarding the persistence of inoculated CAF within the tumors. We have developed a model in which murine metastatic ductal mammary carcinomas expressing estrogen and progesterone receptors transit through different stages of hormone dependency. Hormone dependent (HD) tumors grow only in the presence of progestins, whereas hormone independent (HI) variants grow without hormone supply. We demonstrated previously that CAF from HI tumors (CAF-HI) express high levels of FGF-2 and that FGF-2 induced HD tumor growth <it>in vivo</it>. Our main goal was to investigate whether inoculated CAF-HI combined with purified epithelial (EPI) HD cells can induce HD tumor growth.</p> <p>Methods</p> <p>Purified EPI cells of HD and HI tumors were inoculated alone, or together with CAF-HI, into female BALB/c mice and tumor growth was evaluated. In another set of experiments, purified EPI-HI alone or combined with CAF-HI or CAF-HI-GFP were inoculated into BALB/c or BALB/c-GFP mice. We assessed whether inoculated CAF-HI persisted within the tumors by analyzing inoculated or host CAF in frozen sections of tumors growing in BALB/c or BALB/c-GFP mice. The same model was used to evaluate early stages of tumor development and animals were euthanized at 2, 7, 12 and 17 days after EPI-HI or EPI-HI+CAF-HI inoculation. In angiogenesis studies, tumor vessels were quantified 5 days after intradermal inoculation.</p> <p>Results</p> <p>We found that admixed CAF-HI failed to induce epithelial HD tumor growth, but instead, enhanced HI tumor growth (p < 0.001). Moreover, inoculated CAF-HI did not persist within the tumors. Immunofluorescence studies showed that inoculated CAF-HI disappeared after 13 days. We studied the mechanisms by which CAF-HI increased HI tumor growth, and found a significant increase in angiogenesis (p < 0.05) in the co-injected mice at early time points.</p> <p>Conclusions</p> <p>Inoculated CAF-HI do not persist within the tumor mass although they play a role during the first stages of tumor formation promoting angiogenesis. This angiogenic environment is unable to replace the hormone requirement of HD tumors that still need the hormone to recruit the stroma from the host.</p
