    The prevalence of Helicobacter pylori carrier rates among the healthy blood donors in Hong Kong

    A serological assay was employed in this study to assess the Helicobacter pylori carrier rates among the healthy blood donors (all Chinese) in Hong Kong. The commercial kit for detecting anti-H. pylori antibody titres was found to have a sensitivity of 84% and a specificity of 85% by using the histochemistry results as the gold standard. Elevated anti-H. pylori antibody titres were observed in 42.4%, 53.2% and 72.2% of the healthy blood donors of age below 20, 21 to 30 and 31 to 40 years respectively. This indicates a steady rise of H. pylori carrier rates with age. The overall H. pylori prevalence rate was 54.9%. The positivity of H. pylori in teenagers appeared to be double that reported in Western countries. Whether this is related to the younger age of peptic ulcer presentation in Hong Kong compared with Western countries is not known. However, there was no significant difference of the H. pylori rates between males and females of each age group although a male predominance has been well established for peptic ulcer in Hong Kong.published_or_final_versio

    Control of chaos in nonlinear circuits and systems

    Nonlinear circuits and systems, such as electronic circuits (Chapter 5), power converters (Chapter 6), human brains (Chapter 7), phase lock loops (Chapter 8), sigma delta modulators (Chapter 9), etc, are found almost everywhere. Understanding nonlinear behaviours as well as control of these circuits and systems are important for real practical engineering applications. Control theories for linear circuits and systems are well developed and almost complete. However, different nonlinear circuits and systems could exhibit very different behaviours. Hence, it is difficult to unify a general control theory for general nonlinear circuits and systems. Up to now, control theories for nonlinear circuits and systems are still very limited. The objective of this book is to review the state of the art chaos control methods for some common nonlinear circuits and systems, such as those listed in the above, and stimulate further research and development in chaos control for nonlinear circuits and systems. This book consists of three parts. The first part of the book consists of reviews on general chaos control methods. In particular, a time-delayed approach written by H. Huang and G. Feng is reviewed in Chapter 1. A master slave synchronization problem for chaotic Lur’e systems is considered. A delay independent and delay dependent synchronization criteria are derived based on the H performance. The design of the time delayed feedback controller can be accomplished by means of the feasibility of linear matrix inequalities. In Chapter 2, a fuzzy model based approach written by H.K. Lam and F.H.F. Leung is reviewed. The synchronization of chaotic systems subject to parameter uncertainties is considered. A chaotic system is first represented by the fuzzy model. A switching controller is then employed to synchronize the systems. The stability conditions in terms of linear matrix inequalities are derived based on the Lyapunov stability theory. The tracking performance and parameter design of the controller are formulated as a generalized eigenvalue minimization problem which is solved numerically via some convex programming techniques. In Chapter 3, a sliding mode control approach written by Y. Feng and X. Yu is reviewed. Three kinds of sliding mode control methods, traditional sliding mode control, terminal sliding mode control and non-singular terminal sliding mode control, are employed for the control of a chaotic system to realize two different control objectives, namely to force the system states to converge to zero or to track desired trajectories. Observer based chaos synchronizations for chaotic systems with single nonlinearity and multi-nonlinearities are also presented. In Chapter 4, an optimal control approach written by C.Z. Wu, C.M. Liu, K.L. Teo and Q.X. Shao is reviewed. Systems with nonparametric regression with jump points are considered. The rough locations of all the possible jump points are identified using existing kernel methods. A smooth spline function is used to approximate each segment of the regression function. A time scaling transformation is derived so as to map the undecided jump points to fixed points. The approximation problem is formulated as an optimization problem and solved via existing optimization tools. The second part of the book consists of reviews on general chaos controls for continuous-time systems. In particular, chaos controls for Chua’s circuits written by L.A.B. Tîrres, L.A. Aguirre, R.M. Palhares and E.M.A.M. Mendes are discussed in Chapter 5. An inductorless Chua’s circuit realization is presented, as well as some practical issues, such as data analysis, mathematical modelling and dynamical characterization, are discussed. The tradeoff among the control objective, the control energy and the model complexity is derived. In Chapter 6, chaos controls for pulse width modulation current mode single phase H-bridge inverters written by B. Robert, M. Feki and H.H.C. Iu are discussed. A time delayed feedback controller is used in conjunction with the proportional controller in its simple form as well as in its extended form to stabilize the desired periodic orbit for larger values of the proportional controller gain. This method is very robust and easy to implement. In Chapter 7, chaos controls for epileptiform bursting in the brain written by M.W. Slutzky, P. Cvitanovic and D.J. Mogul are discussed. Chaos analysis and chaos control algorithms for manipulating the seizure like behaviour in a brain slice model are discussed. The techniques provide a nonlinear control pathway for terminating or potentially preventing epileptic seizures in the whole brain. The third part of the book consists of reviews on general chaos controls for discrete-time systems. In particular, chaos controls for phase lock loops written by A.M. Harb and B.A. Harb are discussed in Chapter 8. A nonlinear controller based on the theory of backstepping is designed so that the phase lock loops will not be out of lock. Also, the phase lock loops will not exhibit Hopf bifurcation and chaotic behaviours. In Chapter 9, chaos controls for sigma delta modulators written by B.W.K. Ling, C.Y.F. Ho and J.D. Reiss are discussed. A fuzzy impulsive control approach is employed for the control of the sigma delta modulators. The local stability criterion and the condition for the occurrence of limit cycle behaviours are derived. Based on the derived conditions, a fuzzy impulsive control law is formulated so that the occurrence of the limit cycle behaviours, the effect of the audio clicks and the distance between the state vectors and an invariant set are minimized supposing that the invariant set is nonempty. The state vectors can be bounded within any arbitrary nonempty region no matter what the input step size, the initial condition and the filter parameters are.     Effects of beam-column depth ratio on seismic behaviour of non-seismic detailed reinforced concrete beam-column joints

    Four reinforced concrete exterior beam-column joints with open anchorage beam reinforcement, which are manufactured to simulate those in existing reinforced concrete framed buildings, are tested under reversed cyclic loads simulating earthquake excitation. The particular emphasis of this project is given to the effects of the beam-column depth ratio and the stirrup ratio in joints on the shear strength and seismic behaviour of the exterior joints without seismically designed details. The experimental results indicate that the stirrup placed in the beam-column joint cores can effectively improve the shear strength of the joint and enhance the seismic performance, and the shear strength of the joints decreases when the beam-column depth ratio increases. The experimental results are also compared with the results predicted by two non-seismic design codes (Eurocode 2 and HK code 2013) and three codes for seismic design (Eurocode 8, ACI 318-14 and NZS 3101). In general, the current non-seismic design codes and seismic design codes of practice cannot accurately predict the shear strength of the exterior joints with non-seismically designed details. It is shown that neglecting the seismic design of beam-column joints may lead to potential damage of reinforced concrete framed buildings in unexpected moderate or low seismic areas

    Validity of self-measured waist and hip circumferences: results from a community study in Malaysia

    Background: Measures of central adiposity are better predictors of adverse health events than BMI. Nonetheless, BMI is more widely used in health research. One reason for this may be the limited research supporting the self-measurement of waist and hip circumference. The lack of validity studies is particularly acute in Asia. The main objective was to establish the validity of self-measurement of waist and hip circumference in a community setting and the correlation of those measures with BMI, blood pressure, and blood glucose levels. Methods. A community based, cross-sectional survey. A "healthy living expo" at a shopping mall in a rural town on peninsular Malaysia One hundred and thirty six (136) individuals volunteered to participate in the study, 125 of whom met the inclusion criteria. The ethnic distribution of the participants was 80% Chinese, 17% Malay and 3% Indian. Most participants were female (60%), with participants' ages ranging from 18 to 78 years (mean, 47.2). Self and assisted measurements of waist and hip were taken. Blood pressure, non-fasting blood glucose, height, and weight were also measured. Bland Altman plots and Lin's concordance coefficient were used to measure agreement between self and assisted measures. Pearson's correlation was used to examine the association of self and assisted measures with blood pressure, blood glucose, and BMI. Results: There was a downwards bias in self measured waist (-0.81 cm) and hip (-1 cm) circumferences compared with assisted measures. The concordance for the self and assisted measures of waist, hip and the ratio of the two were, respectively,.96,.93, and.84. The correlation between measures of central adiposity and BMI, blood pressure and blood glucose were similar for self and assisted measures. Conclusion: The results provide additional support for the use of self-measurement of waist and hip circumference studies of central adiposity, but is limited by the specificity of the setting. © 2013 Reidpath et al.; licensee BioMed Central Ltd

    Optimising antimicrobial prescription in hospitals by introducing an antimicrobial stewardship programme in Hong Kong: Consensus statement

    Objective. To discuss the implementation of an 'antimicrobial stewardship programme' as a means to improve the quality of antimicrobial use in a hospital setting in Hong Kong. Participants. Consensus working group on 'antimicrobial stewardship programme', The Scientific Committee on Infection Control, Centre for Health Protection, Department of Health, comprised 11 experts. The remit of the working group was to discuss the rationale and requirement for optimising antimicrobial prescriptions in hospitals by the introduction of an 'antimicrobial stewardship programme'. Evidence. PubMed articles, national and international guidelines, and abstracts of international meetings published between January 2000 and December 2004 on programmes for improving the use of antimicrobials in hospitals. Only English medical literature was reviewed. Consensus process. Data search was performed independently by three members of the working group. They met on three occasions before the meeting to discuss all collected articles. A final draft was circulated to the working group before a meeting on 3 January 2005. Five commonly asked questions about an 'antimicrobial stewardship programme' were selected for discussion by the participants. Published information on the rationale, components, outcome measures, advantages, and disadvantages of the programme was reviewed. Recent unpublished data from local studies of an 'antimicrobial stewardship programme' were also discussed. The timing, potential problems, and practical issues involved in the implementation of an 'antimicrobial stewardship programme' in Hong Kong were then considered. The consensus statement was circulated to and approved by all participants. Conclusion. The continuous indiscriminate and excessive use of antimicrobial agents promotes the emergence of antibiotic-resistant organisms. Antimicrobial resistance substantially raises already-rising health care costs and increases patient morbidity and mortality. Pattern of prescriptions in hospitals can be improved through the implementation of an 'antimicrobial stewardship programme'. A 'universal' and 'continuous' 'antimicrobial stewardship programme' should now be established in Hong Kong hospitals.published_or_final_versio

    The photometric observation of the quasi-simultaneous mutual eclipse and occultation between Europa and Ganymede on 22 August 2021

    Mutual events (MEs) are eclipses and occultations among planetary natural satellites. Most of the time, eclipses and occultations occur separately. However, the same satellite pair will exhibit an eclipse and an occultation quasi-simultaneously under particular orbital configurations. This kind of rare event is termed as a quasi-simultaneous mutual event (QSME). During the 2021 campaign of mutual events of jovian satellites, we observed a QSME between Europa and Ganymede. The present study aims to describe and study the event in detail. We observed the QSME with a CCD camera attached to a 300-mm telescope at the Hong Kong Space Museum Sai Kung iObservatory. We obtained the combined flux of Europa and Ganymede from aperture photometry. A geometric model was developed to explain the light curve observed. Our results are compared with theoretical predictions (O-C). We found that our simple geometric model can explain the QSME fairly accurately, and the QSME light curve is a superposition of the light curves of an eclipse and an occultation. Notably, the observed flux drops are within 2.6% of the theoretical predictions. The size of the event central time O-Cs ranges from -14.4 to 43.2 s. Both O-Cs of flux drop and timing are comparable to other studies adopting more complicated models. Given the event rarity, model simplicity and accuracy, we encourage more observations and analysis on QSMEs to improve Solar System ephemerides.Comment: 23 pages, 5 appendixes, 16 figures, 7 table

    Adaptation and psychometric evaluation of the Chinese version of the functional assessment of chronic illness therapy spiritual well-being scale among Chinese childhood cancer patients in China

    BackgroundSpiritual well-being is a strength for childhood cancer patients to cope with cancer. The availability of a valid and reliable instrument for assessing spiritual well-being is crucial. This study translated and adapted the Functional Assessment of Chronic Illness Therapy Spiritual Well-being scale (FACIT-Sp) for Chinese childhood cancer patients and examined the psychometric properties and factor structure in this population.MethodsThis was a methodological study. The FACIT-Sp was translated into Chinese. Adaptation was based on our qualitative study. For psychometric evaluation, a convenience sample of 412 were recruited based on the suggested sample size for the exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). Childhood cancer patients were included if they aged 8–17 years, with parental consent to participate, able to communicate that they were being treated for cancer, and able to communicate and read Chinese. Participants answered the Chinese version of the adapted FACIT-Sp, the Center for Epidemiology Studies Depression Scale for Children (CES-DC), and the Pediatric Quality of Life Inventory 3.0 Cancer Module (PedsQL). Content validity, convergent validity, internal consistency and test–retest reliability were examined. Both EFA and CFA assessed the structural validity of the adapted FACIT-Sp.ResultsThe content validity index values for items ranged 0.8–1.0 and that for the scale was 0.84, indicating appropriate content validity. The scale had good internal consistency, with a Cronbach’s alpha of 0.815. The FACIT-Sp scores positively correlated with the CES-DC scores, and negatively correlated with PedsQL scores, suggesting that the Chinese version of the adapted FACIT-Sp had reasonable convergent validity. EFA yielded a four-factor (meaning, peace, faith, and connection with others) model. The CFA results revealed that the four-factor model achieved a better fit than the original three-factor model (Chi-Square Mean/Degree of Freedom = 2.240 vs. 3.557, Comparative Fit Index = 0.953 vs. 0.916, Goodness of Fit Index = 0.909 vs. 0.884, Root Mean Square Error of Approximation = 0.078 vs. 0.112).ConclusionThe Chinese version of the adapted FACIT-Sp is a reliable and valid instrument for assessing spiritual well-being among Chinese childhood cancer patients. This instrument can be applied in clinical settings for routine assessment

    Insights on Distinct Left Atrial Remodeling Between Atrial Fibrillation and Heart Failure With Preserved Ejection Fraction

    BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) and atrial fibrillation (AF) commonly coexist with overlapping pathophysiology like left atrial (LA) remodeling, which might differ given different underlying mechanisms. OBJECTIVES: We sought to investigate the different patterns of LA wall remodeling in AF vs. HFpEF. METHODS: We compared LA wall characteristics including wall volume (LAWV), wall thickness (LAWT), and wall thickness heterogeneity (LAWT[SD]) and LA structure, function among the controls (without AF or HFpEF, n = 115), HFpEF alone (n = 59), AF alone (n = 37), and HFpEF+AF (n = 38) groups using multi-detector computed tomography and echocardiography. RESULTS: LA wall remodeling was most predominant and peak atrial longitudinal strain (PALS) was worst in HFpEF+AF patients as compared to the rest. Despite lower E/e' (9.8 ± 3.8 vs. 13.4 ± 6.4) yet comparable LA volume, LAWT and PALS in AF alone vs. HFpEF alone, LAWV [12.6 (11.6–15.3) vs. 12.0 (10.2–13.7); p = 0.01] and LAWT(SD) [0.68 (0.61–0.71) vs. 0.60 (0.56–0.65); p < 0.001] were significantly greater in AF alone vs. HFpEF alone even after multi-variate adjustment and propensity matching. After excluding the HFpEF+AF group, both LAWV and LAWT [SD] provided incremental values when added to PALS or LAVi (all p for net reclassification improvement <0.05) in discriminating AF alone, with LAWT[SD] yielding the largest C-statistic (0.78, 95% CI: 0.70–0.86) among all LA wall indices. CONCLUSIONS: Despite a similar extent of LA enlargement and dysfunction in HFpEF vs. AF alone, larger LAWV and LAWT [SD] can distinguish AF from HFpEF alone, suggesting the distinct underlying pathophysiological mechanism of LA remodeling in AF vs. HFpEF
