Neurocutanoussarcoidosis: a rare entity
Neurosarcoidosis is a granulomatous disorder considered to be a great imitator. We report a case of young lady who presented with gradual onset flexed posturing of fingers of left hand for four years, followed by similar pattern of weakness involving the right hand for three years along with slipping of slippers for one year. Neurologic examination showed distal wasting & weakness in upper limbs with normal jerks and sensory impairment in gloves and stocking distribution. On skin examination there were hypopigmented macules over face, neck and back. Electro diagnostic studies showed sensory motor axonal polyneuropathy, while skin biopsy reported as naked noncaseating granulomas suggestive of sarcoidosis. No such case reported yet in Pakistan with sarcoidperipheral polyneuropathy and rare hypopigmented form of Sarcoidosis without pulmonary involvement
Eight – and - half syndrome: a rare neuro-ophthalmic syndrome “Possible Nine Syndrome”
“Eight-and-half” syndrome is “one-and-a-half” syndrome characterized by conjugated horizontal gaze palsy and internuclearopthalmoplegia plus ipsilateral fascicular seventh nerve palsy. We report a case of 50 year old woman who presented with right eight and half syndrome together with contralateral hemiparesis and hemihypesthesia. Non contrast CT scan brain showed hyperdensesignal on right pontine region. MRI brain plain revealed hemorrhagicpontinetegmentuminfarction as well as corticospinal tract and medial meniscus. This spectrum of presentation suggests a possible “Nine syndrome” as a novel neuropthalmicpontine syndrome. The first of its kind ever reported from Pakista
Effect of river indus sand on concrete tensile strength
In the development of Pakistan construction industry, the utilization of River Indus sand in concrete as fine aggregate has expanded tremendously. The aim of this research is to study the effect of Indus River sand on the tensile strength of various grades of concrete when it is utilized as fine aggregate. Concrete Samples of M15, M20 and M25 grade concrete were cured for 7, 14, 21 and 28 days. Based on the results, it is found that concrete became less workable when Indus river sand was utilized. It is recorded that tensile strength of concrete is decreased from 5% up to 20% in comparison with hill sand. The results were derived from various concrete grades
Comparative Study on Flexural Performance of Foamed Concrete Beam Containing Plastic Fibres
Foamed concrete is considered as light weight concrete which possesses lot of benefits in context of speedy construction, light weight and durability. In this study, foamed concrete beam, containing the plastic fibers were analyzed under flexural load by experimentally and also by finite element analysis in terms of load bearing capacity, load deflection profile and cracking pattern. The Finite element analysis (FEA) was conducted by utilizing ABAQUS software to simulate the foamed concrete beam under flexure load. The beam consist the dimension was 100mm thickness, 200mm breadth, 1000 mm length. The experimental and FEA outcomes were compared in context of the beam’s ultimate load, load deflection and cracking pattern. The outcomes demonstrate that the difference between ultimate loads was 5 to 8% between FEA and experimental results whereas the maximum deflection was 3 to 7 % decreased in FEA compare to the experimental analysis. The parametric was conducted by changing the reinforcement area the results demonstrates that by increasing the main reinforcement area the ultimate load was rises and deflection was decreases compare to the control sample.

Frequency of peripheral neuropathy in newly diagnosed patients of diabetes mellitus iion clinical and electrophysiological basis
To determine the frequency of peripheral neuropathy in newly diagnosed patients of Diabetes Mellitus type II on clinical and electrophysiological basis. Methods: This is Hospital based descriptive cross sectional study conducted at department of Neurology Civil Hospital Karachi/ Dow University of Health Sciences from 20th December 2011 to 20th June 2012. Data was collected from Neurology OPD, Medicine OPD and Diabetic Clinic Civil Hospital Karachi. Results: Total no of patients were 107 who were already diagnosed as type 2 Diabetes Mellitus. Out of these, there were 57 (53.3%) male patients and 50 (46.7%) female patients, with ratio of males to female was . 1.14The age group distribution of study population ranged from 30-60 years and mean age was 45.19 with standard deviation of 7.38. It was observed that 35 (32.7%) patients had symptoms of peripheral neuropathy as compared to this 72 (67.3.2%) patients did not have symptoms of peripheral neuropathy while mean ± standard deviation of duration of symptoms was 2.42±1.95 months and 18 (16.8%) patients had peripheral neuropathy as compared to these 89 (83.2%) patients did not have peripheral neuropathy, clinical or electrophysiological. Conclusion: Diabetic Peripheral Neuropathy is a fairly common entity in our patients with type 2 Diabetes Mellitus and was seen as early as within four weeks of diagnosis in 16.8% of patients
Utilization of palm oil fuel ash and eggshell powder as partial cement replacement - a review
The increase in population leads to increase in construction of houses and other buildings to accommodate these people. The extensive use of concrete for constructional purposes leads to release of Carbon Dioxide (CO2) gas into the atmosphere which adds to the already increased global warming. The increase in urbanization has also lead to increased generation of waste materials. These waste materials are by-products, which are disposed in landfills causing environmental and health issues. The utilization of agricultural wastes as cement substitute is a great alternative for reducing the use and production of cement, which contributes to 5% to 7% of global CO2 emissions alone. Palm Oil Fuel Ash (POFA) Eggshells are two major agricultural wastes, which are generated in abundance in Malaysia. This paper reviews the combined utilization of Eggshells Powder (ESP) and POFA as potential partial cement replacement material and development of bio-concrete, which may help in reducing the environmental issues that are caused by the agricultural by-products. They have been used successfully but individually in concrete. The pozzolanic activity triggered by POFA requires Calcium Hydroxide which cement provides to a limit. Chemical and fresh state properties of foamed concrete incorporating palm oil fuel ash and eggshell ash as cement replacement
Malaysia faces three major environmental problems, out of which solid waste and management is one of them. Palm Oil Fuel Ash (POFA) and eggshells are two agro-food waste materials which are produced in enormous quantities in Malaysia. Due to the characteristics possessed by eggshells and POFA, these waste materials can potentially be utilized in the production as cement replacement, reducing the use of cement which is one of the major production of Carbon Dioxide (CO2) gas emissions. This study was conducted to determine the chemical and fresh state properties of foamed concrete incorporating POFA and eggshell ash (ESA) as cement replacement. Based upon the results, it was observed that the increase in usage amount of POFA and ESA as cement replacement, the workability of foamed concrete reduced without blocking. For the chemical analysis result shows the POFA which had high amount of silicon dioxide and ESA having large amount of calcium oxide were compatible and could be used together as cement replacement.
