    Clinical factors associated with fatigue over time in paediatric oncology patients receiving chemotherapy

    The purpose of this study was to investigate the relationships between clinical factors (including haemoglobin value, chemotherapeutic agents, and corticosteroid use) and changing patterns of fatigue before and for the next 10 days following the start of a new round of chemotherapy in children with cancer. A prospective longitudinal design was used to collect data from 48 paediatric oncology patients who were about to begin a new round of chemotherapy and their parents. Fatigue levels were assessed using multidomain questionnaires with three categories of patient self-report (including ā€˜General Fatigue', ā€˜Sleep/Rest Fatigue', and ā€˜Cognitive Fatigue') and four categories of parent proxy-report (including ā€˜Lack of Energy', ā€˜Unable to Function', ā€˜Altered Sleep', and ā€˜Altered Mood'). The findings suggest that fatigue from both patient self-report and parent proxy-report changed significantly over time. The major findings from this study are that patients have more problems with fatigue in the first few days after the start of a cycle of chemotherapy. Corticosteroid use and haemoglobin value were associated with significant increases in fatigue that were sustained for several days and reached the highest level of fatigue at day 5 for those receiving concurrent steroids. The association of chemotherapeutic agents with fatigue varied between patient self-report and parent report, but the type of chemotherapeutic agents used was not associated with most changes in fatigue

    Micronutrient status in lactating mothers before and after introduction of fortified flour: cross-sectional surveys in Maela refugee camp

    Background Deficiency of micronutrients is common in refugee populations. Objectives Identify deficiencies and whether provided supplements and wheat flour fortified with 10 micronutrients impacts upon status among breast-feeding women from Maela refugee camp. Methods Two sequential cross-sectional studies were conducted in different groups of lactating mothers at 12 weeks postpartum. The first survey was before and the second 4-5 months after micronutrient fortified flour (MFF) had been provided to the camp (in addition to the regular food basket). Iron status and micronutrients were measured in serum, whole blood, and in breast milk samples. Results Iron and zinc deficiency and anemia were highly prevalent while low serum retinol and thiamine deficiency were rarely detected. Iron and zinc deficiency were associated with anemia, and their proportions were significantly lower after the introduction of MFF (21 vs. 35% with soluble transferrin receptor (sTfR)>8.5 mg/L, P = 0.042, and 50 vs. 73% with serum zinc<0.66 mg/L, P = 0.001). Serum sTfR, whole-blood thiamine diphosphate (TDP) and serum Ī²-carotene were significant predictors (P<0.001) of milk iron, thiamine and Ī²-carotene, respectively. Lower prevalence of iron deficiency in the MFF group was associated with significantly higher iron and thiamine in breast milk. Conclusions High whole-blood TDP and breast milk thiamine reflected good compliance to provided thiamine; high prevalence of iron deficiency suggested insufficient dietary iron and low acceptance to ferrous sulfate supplements. MFF as an additional food ration in Maela refugee camp seemed to have an effect in reducing both iron and zinc deficiency postpartum. Ā© Springer-Verlag 2012

    Enhancing nutritional research within an aged care facility

    Aim: The present study examined the barriers to the implementation of a nutritional intervention project and sought ways to enhance future projects.Methods: The study, utilising a qualitative approach, included participants from key areas involved in the project and who were employed by either the university or regional aged care facility.Results: Through analysis the themes \u27successes\u27, \u27stumbling blocks\u27 and \u27solutions\u27 emerged. Within these themes descriptive stories of what worked, what did not work and suggestions for future projects are depicted.Conclusions: Communication was the connecting element of each theme, suggesting effective verbal and written communication that ensures ownership by all involved is essential for successful clinical research projects. Strategies to enhance this process have been included to assist future researchers.<br /
