The issue of digital piracy as violation of intellectual property rights is a hot button among many governments around the world. Until now, nor legislation or its enforcement have managed to keep up with the most recent technologies facilitating piracy. Piracy rates may significantly affect both internal demand and international trade of cultural goods. This paper aims to empirically assess the effect of digital piracy on bilateral trade in cultural goods. We focus on trade in music and media. A trophic model of the benthopelagic fauna distributed in the Santa Maria di Leuca cold-water coral province (Mediterranean Sea)

The benthopelagic fauna distributed in and around the Santa Maria di Leuca (SML) cold-water coral (CWC) province (Northern Ionian Sea–Central Mediterranean Sea) was analyzed through the development of a mass-balanced ecosystem model. A total of nine balanced simulations were provided taking into account the biomass data collected during several experimental bottom trawl surveys carried out from 2005 to 2010 in a depth range between 101 and 541 m. A total of 100 species were included in the analysis corresponding to 15 cephalopods, 25 crustaceans, 7 chondrichthyes and 54 osteichthyes. Simulations were developed maintaining the species-level detail. The total system throughput estimated in the SML CWC province trophic web resulted mainly due to consumption and about 80% of it was maintained by species with trophic level between 2 and 4. Results highlighted important features in terms of the structure and functioning of the system, such as the high level of activity maintained by intermediate trophic levels and the important role of top-down control in the faunal assemblage due to both vertebrate and invertebrate species

Risk of appendiceal neoplasm after interval appendectomy for complicated appendicitis: A systematic review and meta-analysis

Background: Non-operative management is often the treatment of choice in cases of complicated appendicitis and routine interval appendectomy is not usually recommended. Actually, recent studies show an alarming number of appendiceal neoplasms following interval appendectomy. The aim of this study is to evaluate the prevalence of appendiceal neoplasms and their histological types after interval appendectomy for complicated appendicitis in adults. Methods: A comprehensive literature search of the PubMed, Scopus and Web of Science databases was conducted according to the PRISMA statement. Studies reporting appendiceal neoplasm rates after interval appendectomy and histopathological characteristics were included. The most recent World Health Organization (WHO) classification of malignant tumours was considered. A pooled prevalence analysis for both prevalence and pathology was performed. Results: A total of eight studies was included: seven retrospective series and one randomized controlled trial. The pooled prevalence of neoplasms after interval appendectomy was 11% (95% CI 7-15; I2 = 37.5%, p = 0.13). Appendiceal mucinous neoplasms occurred in 43% (95% CI 19-68), adenocarcinoma in 29% (95% CI 6-51), appendiceal neuroendocrine neoplasm in 21% (95% CI 6-36), globet cell carcinoma in 13% (95% CI -2-28), adenoma or serrated lesions in 20% (95% CI -0-41) of cases. Conclusion: The risk of appendiceal neoplasm in patients treated with interval appendectomy for complicated appendicitis is 11%; mucinous neoplasm is the most common histopathological type. Further studies should investigate this association in order to clarify the biological pathway and clinical implications

Multisystem autoimmune disease caused by increased STAT3 phosphorylation, and dysregulated gene expression

Signal transducer and activator of transcription (STAT) 3 is a member of the STAT family, and plays a major role in various immunological mechanisms.1 Mutations in STAT3 are associated with a broad spectrum of manifestations, including immunodeficiency, autoimmunity, and malignancy.2 In particular, heterozygous germline loss-of-function (LOF) mutations cause Hyper-IgE syndrome (HIES),3–5 while heterozygous germline gain-of-function (GOF) mutations have recently been associated to multi-organ autoimmune manifestations (i.e. type 1 diabetes, enteropathy, cytopenia, interstitial lung disease, hypothyroidism), lymphoproliferation, short stature, and recurrent infections (OMIM #615952).6–8 We report a 7-year-old boy who presented with early-onset severe enteropathy, and diffuse eczematous dermatitis since birth. During the first weeks of life, Hirschsprung disease was also suspected and surgically treated. Gastrointestinal and cutaneous manifestations were first ascribed to food allergy with quite a good response to amino acid-based formula. In the following months, the patient failed to thrive, and developed respiratory tract infections. At two years, the patient presented with progressive interstitial lung disease characterized by lymphocytic interstitial infiltration leading to pulmonary hypertension, tricuspid insufficiency, and right ventricular heart failure with hepatomegaly. Because of the increased risk of infections, he received intravenous (IV) immunoglobulin infusions (400 mg/kg), prophylaxis with cotrimoxazole and fluconazole. Methylprednisolone at 0.3 mg/kg/day was also given to treat autoimmune manifestations

Van Gennep in the operating theatre The importance of the placebo effect in surgery, the heavy symbolic investment in actions, instruments and the place of surgical intervention, the existence of a myth of common reference shared by the patient, by his entourage and by the surgeons themselves, opens the way to an interpretation of work in the operating theatre as a form of secular ritual whose structure conveys meanings underlying the purely technical. In particular the efficiency of the "surgical metaphor" seems to be based on strict adhesion to the ritual in the operating theatre and to the fundamental structure of the rite of passage as analyzed by Van Gennep.L'importance de l'effet placebo en chirurgie, le fort investissement symbolique des gestes, des instruments et du lieu d'intervention chirurgicale, l'existence d'un mythe de référence commun partagé par le patient, par son entourage et par les chirurgiens eux-mêmes, ouvrent la voie à une interprétation du travail au bloc chirurgical comme une forme de rituel séculaire dont la structure véhicule des significations qui sous-tendent celles purement techniques.
