    The role of reference intervals of laboratory tests in early phase clinical research in healthy volunteers

    This article discusses one of the most disputable issues in national clinical research practice in healthy volunteers. Regulatory requirements forbid to assess even minor deviations of laboratory parameters from reference intervals as not clinically significant. Authors made analysis of such requirements from positions of foreign and national guidelines of early phase clinical research and with regard to their own experience in the field. As an important part of this work, retrospective analysis of the laboratory test results data of healthy volunteers taken from several clinical trials was conducted. The aim of this analysis was to establish the rate of not clinically significant deviations from reference intervals. The results of the analysis revealed rather high rate of not clinically significant deviations of some laboratory parameters from reference intervals in volunteers with confirmed status “healthy”. Authors noted that developers of clinical trial protocols have no unified approach to generate list of necessary laboratory tests and methods of assessment of the results. Ethical and financial aspects of the problem under discussion are also touched in this article. In conclusion, authors suggest the scale of clinical significance assessment of deviations of basic laboratory parameters from reference intervals as one of the possible options of optimizing current regulatory requirements

    The article analyzes the prerequisites and reasons for the unification of BRICS countries. It considers the level of innovative development of the participating countries in the light of international ratings, and conducts a SWOT analysis of the strengths and weaknesses of BRICS countries in innovation. Today there are many works devoted also to the economic development of BRICS countries.     Simulated Solar Microwave Radiation Blocks the Formation of Biofilms

    The article presents the results of the experimental study that was devoted to determining the blocking influence of the solar microwave radiation on the process of biofilm formation in Gram-positive and Gram-negative microorganisms. The microwave generator that allows simulating microwave “splashes ” of the Sun in the frequency range (4.0- 4.3 GHz) with the controlled inten-sity of radiation (from 50 µW/sm2 to 500 µW/sm2) was used for conducting this research. It is found out that the simulated solar radiation of the microwave range blocks the formation of the extracellular matrix by the opportunistic microorganisms. The results of this study confirm the hypothesis of the evolutionary nature of the leading role of the microwave radiation of the Sun in the life processes of organisms. The technology of the exposure on the microorganisms that was used in the experiment opens up the real prospects for reducing the persistent potential of mi-croorganisms and improving the efficiency of the bacterial infections treatment

    Vitamin d status during winter months among children and adolescents with cystic fibrosis living in southern Russia

    The article presents the results of vitamin D tests in children with cystic fibrosis and in healthy children living in the South of Russia. The study showed the high prevalence of vitamin D deficiency and inadequate levels in patients with cystic fibrosis (86.7%). 25(OH) D level characterizing the vitamin D status decreases progressively in patients and healthy children. With regard to the above mentioned, it is significantly lower in patients with cystic fibrosis than in healthy children in all age periods. The correlation between the serum calcidiol level and the age of patients with cystic fibrosis was r = -0.44 (p = 0.015). Intake of prophylactic (500--1000 IU/day) and therapeutic (1500--3000 IU/day) doses of cholecalciferol results in a less significant increase in calcidiol levels in patients with cystic fibrosis compared with healthy children. A significantly lower 25(OH)D level was detected in patients with cystic fibrosis infected with Staph. aureus. The 25(OH)D levels are significantly lower in children with cystic fibrosis and hypocholesterolemia, than in patients with normal cholesterol levels. Risk factors for the development of severe hypovitaminosis D in children with cystic fibrosis are age, physical developmental delay, exocrine pancreatic insufficiency, presence of chronic Staph. Aureus bronchopulmonary infection. Given the conducted study, the dosage of cholecalciferol in patients with cystic fibrosis should be at least 2 times higher than that in healthy children
