    The impact of hearing impairment and hearing aid use on progression to mild cognitive impairment in cognitively healthy adults: An observational cohort study

    INTRODUCTION: We assessed the association of self‐reported hearing impairment and hearing aid use with cognitive decline and progression to mild cognitive impairment (MCI). METHODS: We used a large referral‐based cohort of 4358 participants obtained from the National Alzheimer's Coordinating Center. The standard covariate‐adjusted Cox proportional hazards model, the marginal structural Cox model with inverse probability weighting, standardized Kaplan‐Meier curves, and linear mixed‐effects models were applied to test the hypotheses. RESULTS: Hearing impairment was associated with increased risk of MCI (standardized hazard ratio [HR] 2.58, 95% confidence interval [CI: 1.73 to 3.84], P = .004) and an accelerated rate of cognitive decline (P < .001). Hearing aid users were less likely to develop MCI than hearing‐impaired individuals who did not use a hearing aid (HR 0.47, 95% CI [0.29 to 0.74], P = .001). No difference in risk of MCI was observed between individuals with normal hearing and hearing‐impaired adults using hearing aids (HR 0.86, 95% CI [0.56 to 1.34], P = .51). DISCUSSION: Use of hearing aids may help mitigate cognitive decline associated with hearing loss

    Prevalence of Chlamydia infection among women visiting a gynaecology outpatient department: evaluation of an in-house PCR assay for detection of Chlamydia trachomatis

    <p>Abstract</p> <p>Background</p> <p>Screening women for <it>Chlamydia trachomatis </it>infection in developing countries is highly desirable because of asymptomatic infection. The existing diagnostic methods in developing countries are not effective and their sensitivity fall below 45.0% which leads to further spread of infection. There is an urgent need for improved and cost effective diagnostic tests that will reduce the burden of sexually transmitted infections in the developing world.</p> <p>Methods</p> <p>Prevalence of <it>C. trachomatis </it>infection among women visiting gynaecology department of Hindu Rao hospital in Delhi, India was determined using Roche Amplicor Multi Well Plate kit (MWP) as well as using in-house PCR assay. We used 593 endocervical swabs for clinical evaluation of the in-house developed assay against Direct Fluorescence Assay (DFA; Group I n = 274) and Roche Amplicor MWP kit (Group II, n = 319 samples) and determined the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) of the in-house developed assay.</p> <p>Results</p> <p>We detected 23.0% positive cases and there was a higher representation of women aged 18-33 in this group. An in-house PCR assay was developed and evaluated by targeting unique sequence within the <it>gyrA </it>gene of <it>C. trachomatis</it>. Specificity of the reaction was confirmed by using genomic DNA of human and other STI related microorganisms as template. Assay is highly sensitive and can detect as low as 10 fg of <it>C. trachomatis </it>DNA. The resolved sensitivity of in-house PCR was 94.5% compared with 88.0% of DFA assay. The high specificity (98.4%) and sensitivity (97.1%) of the in-house assay against Roche kit and availability of test results within 3 hours allowed for immediate treatment and reduced the risk of potential onward transmission.</p> <p>Conclusions</p> <p>The in-house PCR method is cost effective (~ 20.0% of Roche assay) and hence could be a better alternative for routine diagnosis of genital infection by <it>C. trachomatis </it>to facilitate improved screening and treatment management.</p

    Environmental impacts of using desalinated water in concrete production in areas affected by freshwater scarcity

    Up to 500 litres of water may be consumed at the batching plant per cubic meter of ready mix concrete, if water for washing mixing trucks and equipment is included. Demand for concrete is growing almost everywhere, regardless of local availability of freshwater. The use of freshwater for concrete production exacerbates stress on natural water resources. In water-stressed coastal countries such as Israel, desalinated seawater (DSW) is often used in the production of concrete. However, the environmental impacts of this practice have not yet been assessed. In this study the effect of using DSW on the water and carbon footprints of concrete was investigated using life cycle assessment. Water footprint results highlight the benefits of using DSW rather than freshwater to produce concrete in Israel. In contrast, because desalination is an energy intensive process, using DSW increases the greenhouse gas intensity of concrete. Nevertheless, this increase (0.27 kg CO2e/m3 concrete) is small, if compared to the life cycle greenhouse gas emissions of concrete. Our results show that using untreated seawater in the mix (transported by truck from the coast) in place of DSW, would be beneficial in terms of water and carbon footprints if the batching plant were located less than 13 km from the withdrawal point. However, use of untreated seawater increases steel reinforcement corrosion, resulting in loss of structural integrity of the reinforced concrete composite. Sustainability of replacing steel with non-corrosive materials should be explored as a way to reduce both water and carbon footprints of concrete

    Systematic significance of stipe anatomy of Selaginella (Selaginellaceae) in Peninsular Malaysia

    An anatomical study was carried out on 14 taxa belonging to Selaginellaceae in an attempt to study their stipe anatomical characteristics and to provide anatomical data for the selected taxa in Selaginellaceae. Out of 29 taxa of Selaginellaceae recorded in Peninsular Malaysia, 14 taxa have been selected namely Selaginella alutacia, S. argentea, S. frondosa, S. intermedia var. intermedia, S. intermedia var. dolichocentrus, S. mayeri, S. morganii, S. ornata, S. plana, S. polita, S. roxburghii var. roxburghii, S. stipulata, S. wallichii and S. willdenowii. Method used in this study was sectioning using sliding microtome. Findings in this study have shown that Selaginellaceae species studied can be clustered into two groups based on the stipe stellar systems, which are monostelic and tristelic groups. There are some variations exist in the cross sections of the stipes of the same species due to the presence and absence of the leaf trace. Each species is proved to have distinct stipe anatomical characteristics that can be used to differentiate species in Selaginellaceae

    Responsive Nanogel Probe for Ratiometric Fluorescent Sensing of pH and Strain in Hydrogels

    In this study a new pH-responsive nanogel probe containing a complementary nonradiative resonance energy transfer (NRET) fluorophore pair is investigated and its ability to act as a versatile probe of network-related changes in three hydrogels demonstrated. Fluorescent sensing using NRET is a powerful method for studying relationships between Angstrom length-scale structure and macroscopic properties of soft matter. Unfortunately, inclusion of NRET fluorophores into such materials requires material-specific chemistry. Here, low concentrations of preformed nanogel probes were included into hydrogel hosts. Ratiometric photoluminescence (PL) data for the gels labeled with the nanogel probes enabled pH-triggered swelling and deswelling to be studied as well as Ca2+-triggered collapse and solute release. PL measurements during compression of a nanogel probe-labeled nanocomposite gel demonstrated mechanochromic behavior and strain sensing. The new nanogel probes have excellent potential for investigating the internal structures of gels and provide a versatile ratiometric fluorescent platform for studying pH and strain

    Cognitive and social functioning deficits after anti-N-methyl-D-aspartate receptor encephalitis: an exploratory case series

    Background: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a recently described life-threatening autoimmune disorder associated with a characteristic multi-stage neuropsychiatric syndrome. Although it is known that the majority of patients experience neuropsychological disturbance post-treatment, some aspects of the cognitive profile remain unclear. Methods: This study sought to investigate patterns of cognitive functioning in a sample of anti-NMDAR encephalitis patients. Seven (6F:1M; mean age, 26.4 years; range, 16-37 years) treated patients completed a comprehensive set of neurocognitive and social functioning measures. Performance was analyzed using normative data (where available), and comparison with matched controls (10F:4M; mean age, 25.8 years; range, 16-38 years). Results: Individual cognitive profiles ranged from within normal limits to extensive dysfunction. Relative to controls, the patient group's performance was affected in the domains of verbal/ visual memory, working memory, attention, processing speed, executive functioning, and social cognition. The patient group also reported significantly higher levels of anxiety compared to controls. Conclusions: These results add to the accumulating evidence that neurocognitive deficits, consistent with the distribution and functions of the NMDAR system can persist during recovery from anti-NMDAR encephalitis. This is the first study to provide evidence of performance decrements on measures of social cognition, including some involving theory of mind

    Optimasi micro frontend website untuk meningkatkan load times: teknik, tantangan, dan best practice

    In recent years, there has been an increasing popularity of the micro frontend architecture due to its implementation by large companies such as IKEA, Starbucks, and Amazon. Due to its &nbsp;characteristics that similar to microservices, this architecture started to be implemented by various companies to improve their developer experience. However, this architecture has some issues, one of which is the performance of page load time. The objective of this research is to find and determine the best practices for optimizing the page load time of micro frontend applications and to identify the challenge involved. The research is conducted by implementing optimization techniques such as code splitting, lazy loading, tree shaking, minification, and utility modules to micro frontend website. After that, the website is tested with a sample size of 200 which determined by using Lemeshow formula. The research is conducted in both local and server environments using the Google Chrome browser and used "fully loaded" metric. The research use a simple Enterprise Resource Planning (ERP) application consisting of five micro frontends built with React, Vue, and Angular frameworks. The experimental results show that implementing all of the optimization techniques on all micro frontends can improve the application's page load time performance by 31.79% in the local and 47.5% in the server environment.Dalam beberapa tahun terakhir, terjadi peningkatan popularitas dari arsitektur micro frontend dikarenakan mulai diimplementasikan oleh perusahaan besar seperti IKEA, Starbucks, dan Amazon. Karakteristiknya yang menyerupai microservice membuat arsitektur ini mulai banyak diterapkan untuk meningkatkan developer experience. Namun, arsitektur ini memiliki beberapa masalah, salah satunya adalah performa page load time yang rendah. Tujuan dari dilakukannya penelitian ini adalah untuk menentukan bagaimana best practice dalam mengoptimasi performa page load time dari aplikasi micro frontend. Penelitian dilakukan dengan mengimplementasikan teknik optimasi seperti code splitting, lazy loading, tree shaking, minification, dan utility module kepada setiap micro frontend yang dimiliki oleh suatu website, kemudian dilakukan pengujian sebanyak 200 kali yang didapatkan menggunakan formula Lemeshow di local dan server environment menggunakan browser Google Chrome dengan metrik fully loaded, yaitu ukuran waktu yang dibutuhkan suatu website untuk memuat seluruh resources yang digunakan oleh website tersebut. Penelitian dilakukan pada aplikasi Enterprise Resources Planning (ERP) yang terdiri dari lima micro frontend dengan framework React, Vue, dan Angular. Hasil eksperimen yang dilakukan menunjukkan bahwa mengimplementasikan setiap teknik optimasi pada seluruh micro frontend dapat meningkatkan performa page load time aplikasi sebesar 31,79% pada local dan 47,5% pada server environment

    Topical immunotherapy of severe alopecia areata with diphenylcyclopropenone (DPCP): experience in an Iranian population

    BACKGROUND: Highly variable results of topical diphenylcyclopropenone (DPCP) in the treatment of alopecia areata have been reported so far. The purposes of the present study were to evaluate the efficacy and tolerability of DPCP treatment in severe alopecia areata. METHODS: Twenty-eight patients (16 female and 12 male, 10–35 years old, mean age 25 years) with extensive alopecia areata were enrolled in an open-label clinical trial. After sensitization with 2% DPCP, progressively higher concentrations beginning at 0.001% were applied weekly for 6 months to one side of the scalp, after which, if terminal hair growth was noted, the entire scalp was then treated under the same weekly protocol. The maximum concentration of DPCP in acetone was 2%. RESULTS: Twenty-seven of 28 patients completed therapy. The overall response rate was 81.5% (22/27), complete remission (90%-100% terminal hair re-growth) was obtained 22.2% (6/27) and partial remission (10%-90% terminal hair re-growth) in 59.3% (16/27). In all patients an eczematous reaction consisting of erythema, itching, and scaling at the site of application were observed. During therapy, other side effects including, occipital lymphadenopathy 40.7% (11/27), severe eczema/blister formation 40.7% (11/27), hyperpigmentation 18.5% (5/27) were observed, but no hypopigmentation, vitiligo, contact urticaria, and erythema multiforme-like reaction were seen in the patients. Nineteen of 27 (70.4%) patients had at least one side effect, other than eczematous reaction. Notably, partial recurrence was observed in 50.9% (13/22) of these patients after 6 to 12 months of follow-up. During the follow-up time the maintenance DPCP immunotherapy was continued. CONCLUSION: Topical DPCP treatment for alopecia areata is an effective therapy with a slightly high relapse rate during bilateral maintenance treatment. According to the author's knowledge this is the first experience with DPCP in Iran

    Tumor Suppressor Protein p53 Recruits Human Sin3B/HDAC1 Complex for Down-Regulation of Its Target Promoters in Response to Genotoxic Stress

    Master regulator protein p53, popularly known as the “guardian of genome” is the hub for regulation of diverse cellular pathways. Depending on the cell type and severity of DNA damage, p53 protein mediates cell cycle arrest or apoptosis, besides activating DNA repair, which is apparently achieved by regulation of its target genes, as well as direct interaction with other proteins. p53 is known to repress target genes via multiple mechanisms one of which is via recruitment of chromatin remodelling Sin3/HDAC1/2 complex. Sin3 proteins (Sin3A and Sin3B) regulate gene expression at the chromatin-level by serving as an anchor onto which the core Sin3/HDAC complex is assembled. The Sin3/HDAC co-repressor complex can be recruited by a large number of DNA-binding transcription factors. Sin3A has been closely linked to p53 while Sin3B is considered to be a close associate of E2Fs. The theme of this study was to establish the role of Sin3B in p53-mediated gene repression. We demonstrate a direct protein-protein interaction between human p53 and Sin3B (hSin3B). Amino acids 1–399 of hSin3B protein are involved in its interaction with N-terminal region (amino acids 1–108) of p53. Genotoxic stress induced by Adriamycin treatment increases the levels of hSin3B that is recruited to the promoters of p53-target genes (HSPA8, MAD1 and CRYZ). More importantly recruitment of hSin3B and repression of the three p53-target promoters upon Adriamycin treatment were observed only in p53+/+ cell lines. Additionally an increased tri-methylation of the H3K9 residue at the promoters of HSPA8 and CRYZ was also observed following Adriamycin treatment. The present study highlights for the first time the essential role of Sin3B as an important associate of p53 in mediating the cellular responses to stress and in the transcriptional repression of genes encoding for heat shock proteins or proteins involved in regulation of cell cycle and apoptosis
