    Tracing Amazon River water into the Caribbean Sea

    Monthly Amazon River discharge is correlated to historical monthly sea surface salinity (SSS) in the western tropical Atlantic Ocean and the Caribbean Sea. At Barbados a very high inverse correlation (R2 = 0.92) exists if the discharge is lagged by two months, which corresponds to the travel time from the Amazon mouth to Barbados. Due to its proximity to Barbados, a small amount of the correlation can also be attributed to the Orinoco River. Between Barbados and the central Caribbean Sea (75W) the peak correlation of Amazon River discharge and SSS occurs at progressively longer lag times, representing the longer travel time. The correlation is highest at Barbados and diminishes with distance downstream. Downstream of the central Caribbean Sea (75W) no correlation is evident, as the Amazon water becomes too diluted to be clearly defined by available data. The results of the analysis are used to calculate surface current speeds of 0.34 ± 0.09 m/s from the Amazon mouth to Barbados and 0.10 ± 0.02 m/s from Barbados to the central Caribbean Sea (75W). Downstream of Barbados a subsurface maximum in correlation develops. The correlation is eroded more strongly at shallower depths due to more intense surface processes (e.g., evaporation and precipitation)

    Plaque-Type Psoriasis Treated with Calcipotriene Plus Betamethasone Dipropionate Aerosol Foam: A Prospective Study on Clinical and Dermoscopic Predictor Factors in Response Achievement and Retention

    Introduction: Little information on possible local factors that can influence the achievement and retention of response of plaque-type psoriasis to calcipotriene plus betamethasone dipropionate aerosol foam is available. The aim of this study was to assess possible correlations between baseline clinical/dermoscopic features of psoriatic plaques and therapeutic response, and between residual dermoscopic findings in clinically improved/healed lesions and post-treatment relapse. Methods: For each target lesion, we calculated the local psoriasis severity index and assessed dermoscopic findings at baseline and at the end of a 4-week treatment, correlating the therapeutic outcome with the initial clinical and dermoscopic features. The lesions were also followed for a 4-week post-treatment period, and possible associations between relapse and (1) baseline clinical/dermoscopic features and (2) dermoscopic findings detected at the end of the treatment were assessed. Results: A total of 105 lesions from 35 patients were included in the analysis. After 4\ua0weeks of therapy, 13 lesions showed no/limited improvement, while partial and optimal response were observed in 51 and 41 plaques, respectively. Poor outcomes were correlated with both legs localization\ua0and degree of lesion infiltration at baseline. Similarly, presence of globular vessels at baseline dermoscopy was more commonly associated with no/limited response and lesions on the legs, particularly those showing resistance to treatment. A correlation was also found between dotted vessels on the baseline dermoscopic examination and good outcomes (partial/optimal response). After a 4-week post-treatment follow-up, 58.7% of the lesions achieving improvement at the end of the therapy showed relapse, with a correlation between recurrence and vessel persistence\ua0on dermoscopy at the end of the therapy. Conclusion: Clinical response of plaque-type psoriasis to calcipotriol/betamethasone dipropionate spray foam may be adversely affected by the degree of infiltration of lesions at baseline and by legs\ua0localization, and the presence of globular vessels at the baseline dermoscopic assessment is related to poor outcomes. A significant post-treatment relapse rate was observed, and persistence of vascular changes on dermoscopy seems to play a role in promoting disease recurrence

    Association between benzodiazepine prescriptions and potential risk factors of adverse drug reactions among elderly and very elderly: Findings from Friuli Venezia Giulia region, Italy

    4noBackground: benzodiazepines (BDZs) highly increase the risk of adverse drug reactions (ADRs), especially in the elderly with certain medical conditions. Methods: point-prevalence study in December 2017; 2,456 patients ≥65 years were included from 14 regional public and private hospitals, 24 long-term chronic care facilities (LTCCFs) and 20 nursing homes (NHs). Data regarded BDZ prescriptions and comorbidities, co-prescriptions, or concurrent diseases, which could increase the risk of ADRs in BDZ users. Univariate and multivariate logistic regression analyses were used to assess associations between ADRs in BDZ users vs. non-users, as well as within users. Results: 671 patients were prescribed BDZ, whose half were in NHs. Being prescribed 10 or more drugs was twice more common among BDZ users compared to non-users. BDZs were prescribed for long-term to 80% of patients in NHs, while proportions were halved in hospitals and LTCCFs. In the analyses within BDZ users, patients in NHs and LTCCFs were at higher risk of dementia and concurrent use of opioid analgesic and antipsychotics compared to hospitalized patients. Conclusions: the use of BDZs in older patients is common in FVG. A great extent of comorbidities and concurrent medications at higher risk of ADRs was found.     A Probiotic-Based Sanitation System for the Reduction of Healthcare Associated Infections and Antimicrobial Resistances: A Budget Impact Analysis

    Healthcare Associated Infections (HAIs) and antibiotic resistance have high social and economic burdens. Healthcare environments play an important role in the transmission of HAIs. The Probiotic Cleaning Hygiene System (PCHS) showed to decrease hospital surface pathogens up to 90% vs. conventional chemical cleaning (CCC). This study compares PCHS to CCC as to reduction of HAIs and their severity, related antibiotic resistances, and costs. Incidence rates of HAIs/antibiotic resistances were estimated from a multicenter pre-post (6 months CCC + 6 months PCHS) intervention study after applying propensity score matching technique. A budget impact analysis compared the current scenario of use of CCC with future scenarios considering increasing utilization of PCHS, from 5% to 50% in the next five years, from the hospital perspective in Italy. The cumulative incidence of HAI was 4.6% and 2.4% (p <0.0001) for CCC (N=4,160) and PCHS (N=4,160) (OR = 0.47, CI 95% 0.37-0.60), with severe HAIs of 1.57% vs 1% and antibiotic resistances of 1.13% vs 0.53%, respectively. Increased use of PCHS over CCC in Italian internal medicine/geriatrics and neurology departments in the next 5 years is expected to avert at least about 31,000 HAIs and 8,500 antibiotic resistances, and save at least 14 million Euros, of which 11.6 for the treatment of resistant HAIs. Innovative, environmentally sustainable sanitation systems, like PCHS, might substantially reduce antibiotic resistance and increase protection of health worldwide

    Effects of Social Identity, Self-Esteem, Schooling, and Age on Hope [Efectos de la Identidad Social, la autoestima, la Escolaridad y la Edad sobre la esperanza]

    AbstractThe primary objective of this study was to analyze the direct explanatory power of social identity, schooling, and age on self-esteem, as well as the indirect effects of those variables (through the mediating variable self-esteem) on hope. A non-probabilistic sample composed of 657 persons from southern Nuevo Leon was recruited (mean age = 39.75 years; SD = 16.96). Structural equation modeling was used to analyze the effects, both direct and indirect, of the independent variables upon the dependent variables. With regard to hope, the model yields an explained variance of 37% (d ≥ 26% = large effect size) and shows well goodness-of-fit indices: χ2 / df = 2.618, GFI = .997, AFGI = .978, CFI = .995, NNFI = .995, NFI = .992, RMSEA = .048 (IC90%, .001, .100), and SRMR = .017. It is concluded that social identity, together with some contextual variables of a personal nature (for instance, age and schooling) is probably very important to explain the levels of self-esteem and hopeResumenEl objetivo primario de este estudio fue analizar el poder explicativo de las variables identidad social, escolaridad y edad, directamente sobre la autoestima y los efectos indirectos de las tres primeras, a través de la autoestima, sobre la esperanza. Se usó modelamiento de ecuaciones estructurales para analizar los efectos, tanto directos como indirectos, de las variables independientes sobre las dependientes.     HMGA1 Modulates Gene Transcription Sustaining a Tumor Signalling Pathway Acting on the Epigenetic Status of Triple-Negative Breast Cancer Cells

    Chromatin accessibility plays a critical factor in regulating gene expression in cancer cells. Several factors, including the High Mobility Group A (HMGA) family members, are known to participate directly in chromatin relaxation and transcriptional activation. The HMGA1 oncogene encodes an architectural chromatin transcription factor that alters DNA structure and interacts with transcription factors favouring their landing onto transcription regulatory sequences. Here, we provide evidence of an additional mechanism exploited by HMGA1 to modulate transcription. We demonstrate that, in a triple-negative breast cancer cellular model, HMGA1 sustains the action of epigenetic modifiers and in particular it positively influences both histone H3S10 phosphorylation by ribosomal protein S6 kinase alpha-3 (RSK2) and histone H2BK5 acetylation by CREB-binding protein (CBP). HMGA1, RSK2, and CBP control the expression of a set of genes involved in tumor progression and epithelial to mesenchymal transition. These results suggest that HMGA1 has an effect on the epigenetic status of cancer cells and that it could be exploited as a responsiveness predictor for epigenetic therapies in triple-negative breast cancers

    Comparison of commercial kits to measure cytokine responses to Plasmodium falciparum by multiplex microsphere suspension array technology

    <p>Abstract</p> <p>Background</p> <p>Multiplex cytokine profiling systems are useful tools for investigating correlates of protective immunity. Several Luminex and flow cytometry methods are commercially available but there is limited information on the relative performance of different kits. A series of comparison experiments were carried out to determine the most appropriate method for our subsequent studies.</p> <p>Methods</p> <p>Two Luminex methods were compared, the Bio-Rad human 17-plex panel and the Invitrogen (formerly BioSource) human cytokine 10-plex kit, and two flow cytometry methods, the Becton Dickinson Human Th1/Th2 Cytokine Kit (CBA) and the Bender MedSystems Human Th1/Th2 11plex FlowCytomix Multiplex Kit. All kits were tested for the measurement of cytokines in supernatants collected from human leukocytes stimulated with viable <it>Plasmodium falciparum </it>infected red blood cells (iRBC) or <it>P. falciparum </it>schizont lysates.</p> <p>Results</p> <p>Data indicated that the kits differed in sensitivity and reproducibility depending on the cytokine, and detected different quantities of some cytokines. The Bio-Rad 17-plex kit was able to detect more positive responses than the Invitrogen 10-plex kit. However, only when detecting IL-1, IL-6 or TNF did the two Luminex based methods correlate with one another. In this study, the flow cytometry based techniques were less variable and correlated better with one another. The two flow cytometry based kits showed significant correlation when detecting IFN-γ, IL-2, TNF, IL-10 and IL-6, but overall the BD kit detected more positive responses than the Bender MedSystems kit.</p> <p>Conclusions</p> <p>The microsphere suspension array technologies tested differed in reproducibility and the absolute quantity of cytokine detected. Sample volume, the number of cytokines measured, and the time and cost of the assays also differed. These data provide an accurate assessment of the four techniques, which will allow individual researchers to select the tool most suited for their study population.</p

    Oceanographic Observations in Chilean Coastal Waters Between Valdivia and Concepcion

    The physical oceanography of the biologically productive coastal waters of central Chile (36 degrees to 40 degrees S) is relatively unknown. In December 1998 we made a short exploratory cruise between Valdivia (40 degrees S) and Concepcion (37.8 degrees S) taking temperature, salinity, oxygen, and current velocity profiles. Coincident sea surface temperature and color measurements were obtained by satellite. The results showed an area dominated by wind-induced coastal upwelling, river runoff, intrusion of offshore eddies, mixing, and heating. Upwelling centers were found over the shelf at three locations: inshore of Mocha Island, off Valdivia, and off Lavapie Point. At these centers, equatorial subsurface water (ESSW) intrudes into the coastal waters, sometimes affecting the surface waters. Since ESSW has characteristically low-oxygen and high-salinity values, it is easily detected. Off Valdivia, runoff imparts stratification, while farther north, solar heating and reduced mixing may facilitate stratification. In some areas, even strong winds would not destroy the stratification imparted by the advection of buoyancy that occurs during the upwelling process. Strong equatorward currents (\u3e1 m s(-1)) in the form of an upwelling jet were found off Lavapie Point. This is also the location of an intruding anticyclone. Elsewhere, currents were mainly northward but highly variable because of intrusions from offshore eddies. The sea surface temperature and ocean color images show a complex field of onshore and offshore intrusions combined with the effects of mixing on chlorophyll concentrations. The residence time of upwelled water on the shelf is estimated to be less than 1 week
