    Structure and stability of bosonic clouds: alkali atoms with negative scattering length

    We investigate the form and stability of a cloud of atoms confined in a harmonic trap when the scattering length is negative. We find that, besides the known low density metastable solution, a new branch of Bose condensate appears at higher density when non locality effects in the attractive part are taken into account. The transition between the two classes of solutions as a function of the number NN of atoms can be either sharp or smooth according to the strength and range of the attractive interaction. Use of tight traps is favorable for investigating the evolution of the system as the strength of the effective interaction increases with NN.Comment: 11 pages, Latex, 2 figures, to be published in Phys.     Optical excitations in a non-ideal Bose gas

    Optical excitations in a Bose gas are demonstrated to be very sensitive to many-body effects. At low temperature the momentum relaxation is provided by momentum exchange collisions, rather than by elastic collisions. A collective excitation mode forms, which in a Boltzmann gas is manifest in a collision shift and dramatic narrowing of spectral lines. In the BEC state, each spectral line splits into two components. The doubling of the optical excitations results from the physics analogous to that of the second sound. We present a theory of the line doubling, and calculate the oscillator strengths and linewidth.Comment: 5 pages, 3 eps figure

    Influence of nearly resonant light on the scattering length in low-temperature atomic gases

    We develop the idea of manipulating the scattering length aa in low-temperature atomic gases by using nearly resonant light. As found, if the incident light is close to resonance with one of the bound pp levels of electronically excited molecule, then virtual radiative transitions of a pair of interacting atoms to this level can significantly change the value and even reverse the sign of aa. The decay of the gas due to photon recoil, resulting from the scattering of light by single atoms, and due to photoassociation can be minimized by selecting the frequency detuning and the Rabi frequency. Our calculations show the feasibility of optical manipulations of trapped Bose condensates through a light-induced change in the mean field interaction between atoms, which is illustrated for 7^7Li.Comment: 12 pages, 1 Postscript figur

    A systematic review and meta-analysis of COVID-19 in kidney transplant recipients:Lessons to be learned

    Kidney transplant recipients (KTR) may be at increased risk of adverse COVID-19 outcomes, due to prevalent comorbidities and immunosuppressed status. Given the global differences in COVID-19 policies and treatments, a robust assessment of all evidence is necessary to evaluate the clinical course of COVID-19 in KTR. Studies on mortality and acute kidney injury (AKI) in KTR in the World Health Organization COVID-19 database were systematically reviewed. We selected studies published between March 2020 and January 18th 2021, including at least five KTR with COVID-19. Random-effects meta-analyses were performed to calculate overall proportions, including 95% confidence intervals (95% CI). Subgroup analyses were performed on time of submission, geographical region, sex, age, time after transplantation, comorbidities, and treatments. We included 74 studies with 5559 KTR with COVID-19 (64.0% males, mean age 58.2 years, mean 73 months after transplantation) in total. The risk of mortality, 23% (95% CI: 21%-27%), and AKI, 50% (95% CI: 44%-56%), is high among KTR with COVID-19, regardless of sex, age and comorbidities, underlining the call to accelerate vaccination programs for KTR. Given the suboptimal reporting across the identified studies, we urge researchers to consistently report anthropometrics, kidney function at baseline and discharge, (changes in) immunosuppressive therapy, AKI, and renal outcome among KTR

    Varus inclination of the proximal tibia or the distal femur does not influence high tibial osteotomy outcome

    We have analysed retrospectively the influence of different sources of knee deformity on failure of closing wedge high tibial valgus osteotomy (HTO). Preoperative frontal plane varus deformities of the lower extremity, distal femur and proximal tibia, and medial convergence of the knee joint line were assessed on a standard whole leg radiograph in 76 patients. Using the logistic regression model, the probability of survival for HTO was 77% (SD 4%) at 10-years follow-up. Varus deformity of the lower extremity ( 3 degrees ) were identified as preoperative risk factors for conversion to arthroplasty (P = 0.03 and P = 0.006). We found no evidence that varus inclination of the proximal tibia or distal femur influences long-term survival of HTO

    Collective dynamics of internal states in a Bose gas

    Theory for the Rabi and internal Josephson effects in an interacting Bose gas in the cold collision regime is presented. By using microscopic transport equation for the density matrix the problem is mapped onto a problem of precession of two coupled classical spins. In the absence of an external excitation field our results agree with the theory for the density induced frequency shifts in atomic clocks. In the presence of the external field, the internal Josephson effect takes place in a condensed Bose gas as well as in a non-condensed gas. The crossover from Rabi oscillations to the Josephson oscillations as a function of interaction strength is studied in detail.Comment: 18 pages, 2 figure

    Physiotherapy alone or in combination with corticosteroid injection for acute lateral epicondylitis in general practice: A protocol for a randomised, placebo-controlled study

    <p>Abstract</p> <p>Background</p> <p>Lateral epicondylitis is a painful condition responsible for loss of function and sick leave for long periods of time. In many countries, the treatment guidelines recommend a wait-and-see policy, reflecting that no conclusions on the best treatment can be drawn from the available research, published studies and meta-analyses.</p> <p>Methods/Design</p> <p>Randomized double blind controlled clinical trial in a primary care setting. While earlier trials have either compared corticosteroid injections to physical therapy or to naproxen orally, we will compare the clinical effect of physiotherapy alone or physiotherapy combined with corticosteroid injection in the initial treatment of acute tennis elbow. Patients seeing their general practitioner with lateral elbow pain of recent onset will be randomised to one of three interventions: 1: physiotherapy, corticosteroid injection and naproxen or 2: physiotherapy, placebo injection and naproxen or 3: wait and see treatment with naproxen alone. Treatment and assessments are done by two different doctors, and the contents of the injection is unknown to both the treating doctor and patient. The primary outcome measure is the patient's evaluation of improvement after 6, 12, 26 and 52 weeks. Secondary outcome measures are pain, function and severity of main complaint, pain-free grip strength, maximal grip strength, pressure-pain threshold, the patient's satisfaction with the treatment and duration of sick leave.</p> <p>Conclusion</p> <p>This article describes a randomized, double blind, controlled clinical trial with a one year follow up to investigate the effects of adding steroid injections to physiotherapy in acute lateral epicondylitis.</p> <p>Trial Registration</p> <p>ClinicalTrials.gov Identifier: NCT00826462</p

    Representation of Patients With Chronic Kidney Disease in Clinical Trials of Cardiovascular Disease Medications: A Systematic Review

    IMPORTANCE: Patients with chronic kidney disease (CKD) are at high risk for cardiovascular disease, but their systematic underrepresentation in cardiovascular randomized clinical trials (RCTs) limits the generation of appropriate evidence to guide cardiovascular risk management (CVRM). OBJECTIVE: To evaluate the underrepresentation of patients with CKD in cardiovascular RCTs, and to highlight evidence gaps in CVRM medications in this population. EVIDENCE REVIEW: A systematic search was conducted in ClinicalTrials.gov from February 2000 through October 2021 for RCTs with full-text publications. If no full-text publications were found in ClinicalTrials.gov, MEDLINE, Embase, and Google Scholar were also searched. Eligible RCTs were those evaluating the effectiveness of antiplatelets, anticoagulants, blood pressure-lowering drugs, glucose-lowering drugs, or cholesterol-lowering drugs in adults with cardiovascular disease or cardiovascular risk factors. Trials with a sample size of fewer than 100 patients were excluded. FINDINGS: In total, 1194 RCTs involving 2 207 677 participants (mean [SD] age, 63 [6] years; 1 343 970 males [64%]) were included. Since 2000, the percentage of cardiovascular RCTs excluding patients with CKD has increased from 66% to 79% (74% overall [884 RCTs]). In 864 RCTs (72%), more patients were excluded than anticipated on safety grounds (63% [306] of trials required no dose adjustment, and 79% [561] required dose adjustment). In total, 158 RCTs (13%) reported results for patients with CKD separately (eg, in subgroup analyses). Significant evidence gaps exist in most CVRM interventions for patients with CKD, particularly for those with CKD stages 4 to 5. Twenty-three RCTs (2%) reported results for patients with an estimated glomerular filtration rate less than 30 mL/min/1.73 m2, 15 RCTs (1%) reported for patients receiving dialysis, and 1 RCT (0.1%) reported for recipients of kidney transplant. CONCLUSIONS AND RELEVANCE: Results of this systematic review suggest that representation of patients with CKD in cardiovascular RCTs has not improved in the past 2 decades and that these RCTs excluded more patients with CKD than expected on safety grounds. Lack of reporting or underreporting of results for this patient population is associated with evidence gaps in the effectiveness of most CVRM medications in patients with all stages of CKD, particularly CKD stages 4 to 5

    A biology-based approach for quantitative structure-activity relationships (QSARs) in ecotoxicity.

    Quantitative structure-activity relationships (QSARs) for ecotoxicity can be used to fill data gaps and limit toxicity testing on animals. QSAR development may additionally reveal mechanistic information based on observed patterns in the data. However, the use of descriptive summary statistics for toxicity, such as the 4-day LC50 for fish, introduces bias and ignores valuable kinetic information in the data. Biology-based methods use all of the toxicity data in time to derive time-independent and unbiased parameter estimates. Such an approach offers whole new opportunities for mechanism-based QSAR development. In this paper, we apply the hazard model from DEBtox to analyse survival data for fathead minnows (Pimephales promelas). Different modes of action resulted in different patterns in the parameter estimates, and therefore, the toxicity data by themselves reveal insight into the actual mechanism of toxic action
