    Adrenal Myelolipoma, Cholelithiasis and Calcified Spleen: Retrospective Diagnosis of Sickle Cell Anemia Using a Novel Triad of Abdominal Imaging Findings

    Sickle cell anemia is an inherited abnormality of the globin chain with very high prevalence in the Indian subcontinent. A significant proportion of these patients present late in life and are at a risk of complications like acute chest syndrome and painful episodes till a definitive diagnosis is reached and appropriate treatment is started . We report a novel triad of abdominal imaging findings which is not reported in literature until now and which may suggest a diagnosis of sickle cell anemia in retrospect. Patients with this triad of abdominal findings should be suspected to have an underlying hemoglobinopathy and should be referred for further hematological workup. Although in our case the patient was diagnosed to have sickle cell anemia depending on the abnormal morphology of red cells and hemoglobin electrophoresis, it should be remembered that this triad of findings may be seen in other hemoglobinopathies which induce a state of chronic anemia.     Abdominoscrotal Haematocele in an Adult and Its Successful Treatment

    Abdominoscrotal hydrocele (ASH) is a variant of hydrocele that rarely occurs in adults. ASH has two sacs, one in the scrotum and one in the abdomen connected through the inguinal canal. Abdominoscrotal haematocele is a rare complication of ASH. We report a 57-year-old male patient who presented to a tertiary care hospital in Puducherry, India, in 2019 with complaints of swelling in the scrotum for 15 years and abdominal pain for two months. Both the swellings were soft and cross fluctuation was present. Imaging confirmed the diagnosis of ASH. During a diagnostic laparoscopy, the abdominal sac was decompressed and found to have thick brownish fluid suggestive of haematocele. It was demonstrated that both sacs were connected. Due to difficulty in the dissection of the sac, the procedure was converted to an open procedure. Both the sacs were excised and Lytle’s repair was done for the dilated internal ring. The patient recovered and no recurrence of any swelling in the abdomen or scrotum was found at the end of a one-year follow-up.     A ventriculoperitoneal shunt (VP shunt) is a fairly common and useful procedure to reduce intracranial pressure in patients with hydrocephalus. Complications associated with VP shunts are usually related to either shunt obstruction or infection. A pseudocyst formation owing to leakage of CSF into the breast tissue as a complication of VP shunting is a rare entity, with few cases in the literature. Nonetheless, it is an important cause of cystic breast lump and should be kept in mind in a patient with an indwelling VP shunt. We describe the case of a 16-year-old girl patient with an indwelling VP shunt who presented with a gradually increasing breast lump, and was diagnosed to have a CSF pseudocyst based on characteristic imaging findings

    Solar Desalination via Solar still is the best method of obtaining 2-3 liter of potable water in a typical summer day from brackish water. Solar still in its state of art is ‘Rain Machine’ or ‘Hydrological Cycle in trapezoidal basin.’ In analysis of solar still, it has been proved and well accepted that, rate of condensation is strong function of temperature difference between inner glass cover (condensing cover) and basin water temperature. In architecture and working of Masonic solar still, it is found that, temperature difference is not conducive for condensation in summer days and at peak Sun shines hours. Literature review and opinion of expertise suggest advent of internal condenser in solar still. Among different configurations box type copper metal make internal condenser is fabricated and fixed at back wall of still. Water is fed to box from overhead tank so as to get natural siphoning and cooling effect inside the still
