    Prescribing pattern of antibiotics in pediatric department of a tertiary care teaching hospital

    Background: Prescription is an order from doctor for medicine. Rational use of medicines requires that "patients receive medications appropriate to their clinical needs, in doses that meet their own individual requirements, for an adequate period of time, and at the lowest cost to them and their community". Irrational use of medicines is a major problem worldwide. This leads to serious morbidity and mortality also leads to reduction in the quality of treatment due to antibiotic resistance. Evaluation of prescribing pattern will help in minimizing adverse drug reactions, resistance among children. Also help to know the attitude of the physicians towards prescribing. Aim of the present study was to evaluate the prescription pattern of antibiotics in paediatric inpatients of Hassan institute of Medical Sciences.Methods: A prospective study, conducted among 110 patients below the age of 18 years and being treated with antibiotics were included in our study. The results were analyzed using descriptive statistics.Results: Out of 110 patients, female (58) and male (52) are enrolled in the study from inpatient paediatrics department, majority of patients belonged to age group of 0-5 years (74%), respiratory tract infections 29 (35%) , gastrointestinal infections 26 (22%) and central nervous system in 9 (11%). Out of 227 antimicrobial agent, about 83.48% were cephalosporins, followed by ciprofloxicin (33.94%), amoxicillin (32.11%), and amikacin (6.42%).Conclusions: Cephalosporins (ceftriaxone) were most commonly used antibiotic, which covers gram positive, gram negative and anaerobic organism

    Fabrication, Mechanical and Wear Properties of Aluminum (Al6061)-Silicon Carbide-Graphite Hybrid Metal Matrix Composites

    In recent times, the use of aluminum alloy-based Hybrid Metal Matrix Composites (HMMCs) is being increased in aerospace and automotive applications. HMMCs compensate for the low desirable properties of each filler used. However, the mechanical properties of HMMCs are not well understood. In particular, microstructural investigations and wear optimization studies of HMMCs are not clear. Therefore, further studies are required. The present study is aimed at fabricating and mechanical and wear characterizing and microstructure investigating of Silicon Carbide (SiC) and Graphite (Gr) added in Aluminum (Al) alloy Al6061 HMMCs. The addition of SiC particles was in the range from 0 to 9 weight percentage (wt.%) in steps of 3, along with the addition of 1 wt.% Gr in powder form. The presence of alloying elements in the Al6061 alloy was identified using the Energy Dispersive X-Ray Analysis (EDX). The dispersion of SiC and Gr particles in the alloy was investigated using metallurgical microscope and Scanning Electron Microscopy (SEM). The gain in strength can be attributed to the growth in dislocation density. The nature of fracture was quasi-cleavage. The microstructure examination reveals the uniform dispersion of the reinforcement. Density, hardness, and Ultimate Tensile Strength values observed to be increased with increased contents of SiC reinforcement. Besides, wear studies were performed in dry sliding conditions. Optimization studies were performed to investigate the effect of parameters that affecting the wear. The sliding wear resistance was noticed to be improved concerning higher amounts of reinforcement leading to a decrease in delamination and adhesive wear. The predicted values for the wear rate have also been compared with the experimental results and good correlation is obtained

    IN VITRO ANTIPLASMODIAL ACTIVITY OF NATIVE INDIAN SEAWEED SARGASSUM SP.

    ABSTRACTObjectives: To investigate the antiplasmodial activity of three different solvent extracts of Sargassum tenerrimum against Plasmodium falciparum.Methods: The seaweed species of S. tenerrimum were collected from Rameshwaram, Southeast coast of India. The collected samples were dried andextracted with three different polaritic (hexane, acetone, and ethylacetate) solvents and tested against P. falciparum parasite strain.Results: Acetone extract exhibited better activity than the other two extracts. The inhibitory concentration values of acetone S. tenerrimum werefound to be 27.82 and 18.14 µg/ml at 24-48 hrs, respectively. S. tenerrimum crude extracts were subjected for the phytochemical analysis, and itshowed the presence of steroids, alkaloids, flavonoids, tannins, glycosides, amino acids, and phenol compounds. The gas chromatography-massspectroscopy result reveals that the presence of 10 major and minor compounds in the S. tenerrimum extract. In that, cyclotrisiloxane hexamethylcompounds might be responsible for the effective parasite suppression.50Conclusion: It can be concluded from the present study that the acetone extract of S. tenerrimum has strong antiplasmodial activity.     INFLUENCE OF FIBRE VOLUME REINFORCEMENT IN DRILLING GFRP LAMINATES

    This paper presents an investigation on the influence of fiber volume reinforcement on various aspects of machining. Drilling experiments were conducted to study the tool wear, surface finish, delamination factor and hole quality on GFRP composites. The work reports the variation of tool wear, surface roughness, hole quality, chip characteristics, delamination factor with the variation of fibre volume reinforcement. Results revealed that the increase in fiber percentage increased the tool wear, delamination factor, surface roughness value and decreased hole quality. Minimum surface roughness, tool wear and better hole quality was obtained for 30% fibre content composites. 70% Fibre content composites produced hazardous surface roughness. Pull out of fibres and fibril formation are significant in decreasing the hole quality and increased surface roughness. Increased tool – fibre interaction and thermal softening of the tool causes increased tool wear. In higher fibre content composites, extensive plasticity was absent consequently brittle ceramic fibres were fractured easily. Hence small segment type chips were obtained. The fibre pull out and fibrils present near the hole exit forms the remainder of the laminate causes increased damage zone near the hole exit. Hence high delamination factor was obtained

    Association of TGFβ1, TNFα, CCR2 and CCR5 gene polymorphisms in type-2 diabetes and renal insufficiency among Asian Indians

    BACKGROUND: Cytokines play an important role in the development of diabetic chronic renal insufficiency (CRI). Transforming growth factor β1 (TGF β1) induces renal hypertrophy and fibrosis, and cytokines like tumor necrosis factor-alpha (TNFα), chemoattractant protein-1 (MCP-1), and regulated upon activation and normal T cell expressed and secreted (RANTES) mediate macrophage infiltration into kidney. Over expression of these chemokines leads to glomerulosclerosis and interstitial fibrosis. The effect of MCP-1 and RANTES on kidney is conferred by their receptors i.e., chemokine receptor (CCR)-2 and CCR-5 respectively. We tested association of nine single nucleotide polymorphisms (SNPs) from TGFβ1, TNFα, CCR2 and CCR5 genes among individuals with type-2 diabetes with and without renal insufficiency. METHODS: Type-2 diabetes subjects with chronic renal insufficiency (serum creatinine ≥ 3.0 mg/dl) constituted the cases, and matched individuals with diabetes of duration ≥ 10 years and normoalbuminuria were evaluated as controls from four centres in India. Allelic and genotypic contributions of nine SNPs from TGFβ1, TNFα, CCR2 and CCR5 genes to diabetic CRI were tested by computing odds ratio (OR) and 95% confidence intervals (CI). Sub-analysis of CRI cases diabetic retinopathy status as dependent variable and SNP genotypes as independent variable in a univariate logistic regression was also performed. RESULTS: SNPs Tyr81His and Thr263Ile in TGF β1 gene were monomorphic, and Arg25Pro in TGF β1 gene and Δ32 polymorphism in CCR5 gene were minor variants (minor allele frequency <0.05) and therefore were not considered for case-control analysis. A significant allelic association of 59029G>A SNP of CCR5 gene has been observed and the allele 59029A seems to confer predisposition to development of diabetic CRI (OR 1.39; CI 1.04–1.84). In CRI subjects a compound group of genotypes "GA and AA" of SNP G>A -800 was found to confer predisposition for proliferative retinopathy (OR 3.03; CI 1.08–8.50, p = 0.035). CONCLUSION: Of the various cytokine gene polymorphisms tested, allele 59029A of CCR5 gene is significantly associated with diabetic renal insufficiency among Asian Indians. Result obtained for 59029G>A SNP of CCR5 gene is in conformity with reports from a Japanese population but due to sub-optimal power of the sample, replication in larger sample set is warranted

    Development and evaluation of introgression lines with yield enhancing genes of the Indian mega-variety of rice, MTU1010

    MTU 1010 is an early maturing and high-yielding mega rice variety widely grown in an area of 3 Mha. It is characterised by limited grain number and panicle branching. To improve the grain number in MTU 1010, an IRRI breeding line, IR121055-2-10-5 was utilized as donor to transfer yield-enhancing genes Gn1a and OsSPL14 (associated with increased grain number and better panicle branching, respectively) into MTU1010 by Marker-Assisted Backcross Breeding (MABB). At each backcross generation, foreground selection was carried out with Gn1a and OsSPL14- specific molecular markers, whilst background selection was done with a set of SSR markers polymorphic between the IR121055-2-10-5 and MTU1010. With the use of a gene-specific marker, homozygous BC2 F2 plants carrying the yield-enhancing gene were identified and advanced through pedigree-method of selection till BC2 F6 and best performing ten lines were selected and evaluated in replicated station trials for yield contributing traits, where grain number and brancing per panicle exhibited high significant and positive correlation with single plant yield. Three promising lines namely RP6353-5-8-13-24, RP6353-26-13-39-5 and RP6353-32-12-8-16 with higher grain number and yield than MTU1010 were identified and nominated for evaluation in Initial Varietal Trial-Aerobic (IVT-Aerobic) of All India Crop Improvement Programme on Rice (AICRP), of which RP6353-26-13-39-5 (IET28674), was promoted for further testing

    Global, regional, and national burden of chronic kidney disease, 1990–2017 : a systematic analysis for the Global Burden of Disease Study 2017

    Background Health system planning requires careful assessment of chronic kidney disease (CKD) epidemiology, but data for morbidity and mortality of this disease are scarce or non-existent in many countries. We estimated the global, regional, and national burden of CKD, as well as the burden of cardiovascular disease and gout attributable to impaired kidney function, for the Global Burden of Diseases, Injuries, and Risk Factors Study 2017. We use the term CKD to refer to the morbidity and mortality that can be directly attributed to all stages of CKD, and we use the term impaired kidney function to refer to the additional risk of CKD from cardiovascular disease and gout. Methods The main data sources we used were published literature, vital registration systems, end-stage kidney disease registries, and household surveys. Estimates of CKD burden were produced using a Cause of Death Ensemble model and a Bayesian meta-regression analytical tool, and included incidence, prevalence, years lived with disability, mortality, years of life lost, and disability-adjusted life-years (DALYs). A comparative risk assessment approach was used to estimate the proportion of cardiovascular diseases and gout burden attributable to impaired kidney function. Findings Globally, in 2017, 1·2 million (95% uncertainty interval [UI] 1·2 to 1·3) people died from CKD. The global all-age mortality rate from CKD increased 41·5% (95% UI 35·2 to 46·5) between 1990 and 2017, although there was no significant change in the age-standardised mortality rate (2·8%, −1·5 to 6·3). In 2017, 697·5 million (95% UI 649·2 to 752·0) cases of all-stage CKD were recorded, for a global prevalence of 9·1% (8·5 to 9·8). The global all-age prevalence of CKD increased 29·3% (95% UI 26·4 to 32·6) since 1990, whereas the age-standardised prevalence remained stable (1·2%, −1·1 to 3·5). CKD resulted in 35·8 million (95% UI 33·7 to 38·0) DALYs in 2017, with diabetic nephropathy accounting for almost a third of DALYs. Most of the burden of CKD was concentrated in the three lowest quintiles of Socio-demographic Index (SDI). In several regions, particularly Oceania, sub-Saharan Africa, and Latin America, the burden of CKD was much higher than expected for the level of development, whereas the disease burden in western, eastern, and central sub-Saharan Africa, east Asia, south Asia, central and eastern Europe, Australasia, and western Europe was lower than expected. 1·4 million (95% UI 1·2 to 1·6) cardiovascular disease-related deaths and 25·3 million (22·2 to 28·9) cardiovascular disease DALYs were attributable to impaired kidney function. Interpretation Kidney disease has a major effect on global health, both as a direct cause of global morbidity and mortality and as an important risk factor for cardiovascular disease. CKD is largely preventable and treatable and deserves greater attention in global health policy decision making, particularly in locations with low and middle SDI

    Association analysis of ADPRT1, AKR1B1, RAGE, GFPT2 and PAI-1 gene polymorphisms with chronic renal insufficiency among Asian Indians with type-2 diabetes

    <p>Abstract</p> <p>Background</p> <p>To determine association of nine single nucleotide polymorphisms (SNPs) in ADP ribosyltransferase-1 (ADPRT1), aldo-keto reductase family 1 member B1 (AKR1B1), receptor for advanced glycation end-products (RAGE), glutamine:fructose-6-phosphate amidotransferase-2 (GFPT2), and plasminogen activator inhibitor-1 (PAI-1) genes with chronic renal insufficiency (CRI) among Asian Indians with type 2 diabetes; and to identify epistatic interactionss between genes from the present study and those from renin-angiotensin-aldosterone system (RAAS), and chemokine-cytokine, dopaminergic and oxidative stress pathways (previously investigated using the same sample set).</p> <p>Methods</p> <p>Type 2 diabetes subjects with CRI (serum creatinine ≥3.0 mg/dl) constituted the cases (n = 196), and ethnicity and age matched individuals with diabetes for a duration of ≥ 10 years, normal renal functions and normoalbuminuria recruited as controls (n = 225). Allelic and genotypic constitution of 10 polymorphisms (SNPs) from five genes namely- <it>ADPRT1</it>, <it>AKR1B1, RAGE, GFPT2 </it>and <it>PAI-1 </it>with diabetic CRI was investigated. The genetic associations were evaluated by computation of odds ratio and 95% confidence interval. Multiple logistic regression analysis was carried out to correlate various clinical parameters with genotypes, and to study epistatic interactions between SNPs in different genes.</p> <p>Results</p> <p>Single nucleotide polymorphisms -429 T>C in <it>RAGE </it>and rs7725 C>T SNP in 3' UTR in <it>GFPT2 </it>gene showed a trend towards association with diabetic CRI. Investigation using miRBase statistical tool revealed that rs7725 in <it>GFPT2 </it>was a perfect target for predicted miRNA (hsa miR-378) suggesting the presence of the variant 'T' allele may result in an upregulation of GFPT2 contributing to diabetic renal complication. Epistatic interaction between SNPs in transforming growth factor <it>TGF-β1 </it>(investigated using the same sample set and reported elsewhere) and <it>GFPT2 </it>genotype was observed.</p> <p>Conclusions</p> <p>Association of SNPs in <it>RAGE </it>and <it>GFPT2 </it>suggest that the genes involved in modulation of oxidative pathway could be major contributor to diabetic chronic renal insufficiency. In addition, GFPT2 mediated overproduction of TGF-β1 leading to endothelial expansion and thereby CRI seems likely, suggested by our observation of a significant interaction between GFPT2 with TGF-β1 genes. Further, identification of predicted miRNA targets spanning the associated SNP in <it>GFPT2 </it>implicates the rs7725 SNP in transcriptional regulation of the gene, and suggests <it>GFPT2 </it>could be a relevant target for pharmacological intervention. Larger replication studies are needed to confirm these observations.</p
