Treatment of iatrogenic iliac artery rupture during endovascular surgery
Recently a growing number of endovascular operations are associated with an increased number of iatrogenic
vascular injuries. The aim of the study is to analyse the treatment of iliac artery damage during angioplasty.
Method. In the period 2002–2009, 328 patients underwent endovascular surgery of iliac artery stenosis,
219 men and 109 women aged from 47 to 85 years. Qualified patients displayed IIB, III, and IV stage of
ischaemia by Fontaine. The collected information concerned details of the performed procedures, possible
complications and ways of solving problems. Results. In 132 cases, endovascular surgery consisted of extension
of the iliac artery using a stent, while in 196 no stent was applied. Iatrogenic injury to the iliac artery
during the angioplasty occurred in six patients. In four cases, there was damage to the common iliac artery,
and in the next two patients to the external iliac artery. In four cases, there was damage to the artery during
endovascular expansion, and in two cases after stent implantation. In four cases, the damage to the artery
was treated by an endovascular method, in two patients a stent was implanted, in one patient a stent-graft
was introduced, and in one case a spillage was sealed using a balloon. Two patients underwent classic surgery
due to symptoms of hypovolaemic shock caused by bleeding into the retroperitoneal space; in one case
arterial damage was sutured, and in the second patient an aortal-femoral bypass graft was implanted. Of
the patients supplied by an intravascular method, in one case, because of the very large retroperitoneal haematoma
and the compression symptoms associated with it, surgical drainage was necessary. In the remaining
cases, the postoperative course passed without complications. Among the patients who underwent classic
surgery there was one case of deep vein thrombosis. All patients achieved satisfactory haemodynamic and
clinical effects. There were no deaths. Conclusions. 1. Each instance of iatrogenic iliac artery injury during
endovascular surgery requires an individual approach. 2. An attempt to repair damage by an intravascular
method is the treatment of choice in haemodynamically stable patients. 3. In cases of large retroperitoneal
bleeding, patients require immediate conversion to classical surgery.
Acta Angiol 2011; 17, 2: 150–157Wstęp. Rosnąca ostatnio liczba operacji wewnątrznaczyniowych wiąże się również ze zwiększoną liczbą
jatrogennych uszkodzeń naczyń. Celem pracy jest analiza sposobu postępowania przy uszkodzeniach tętnic
biodrowych w czasie angioplastyki.
Materiał i metoda. W latach 2002–2009 operowano wewnątrznaczyniowo 328 chorych ze zwężeniami
tętnic biodrowych, w tym 219 mężczyzn i 109 kobiet w wieku 47–85 lat. Do operacji kwalifikowano chorych
w IIB, III i IV stadium niedokrwienia według klasyfikacji Fontaine’a. Zbierano informacje na temat szczegółów
przeprowadzonych zabiegów, ewentualnych powikłań i sposobów rozwiązywania problemów.
Wyniki. W 132 przypadkach wykonano poszerzenie wewnątrznaczyniowe tętnicy biodrowej ze stentem, zaś
w 196 bez stentu. Do jatrogennego urazu tętnic biodrowych w miejscu przeprowadzanej angioplastyki doszło u 6
chorych. W 4 przypadkach stwierdzono uszkodzenie tętnicy biodrowej wspólnej, w kolejnych 2 — tętnicy biodrowej
zewnętrznej. U 4 chorych do uszkodzenia tętnicy doszło podczas wewnątrznaczyniowego poszerzenia, zaś w 2
przypadkach po wszczepieniu stentu. U 4 chorych uszkodzenie tętnicy leczono metodą wewnątrznaczyniową,
u 2 osób implantowano stent, u 1 pacjenta — stentgraft, zaś w 1 przypadku uszczelniono wyciek za pomocą
balonu. Dwóch chorych ze względu na objawy wstrząsu hipowolemicznego spowodowanego krwawieniem do
przestrzeni zaotrzewnowej operowano klasycznie, w jednym przypadku zszyto uszkodzenie tętnicy, zaś u drugiego
chorego wszczepiono pomost aortalno-udowy. Spośród chorych zaopatrzonych metodą wewnątrznaczyniową
w jednym przypadku ze względu na bardzo dużego krwiaka zaotrzewnowego i objawy uciskowe z nim związane
konieczne było przeprowadzenie jego chirurgicznego drenażu. U pozostałych przebieg pooperacyjny upłynął
bez powikłań. Wśród operowanych klasycznie u 1 chorego stwierdzono zakrzepicę żył głębokich. U wszystkich
pacjentów uzyskano zadowalający efekt hemodynamiczny i kliniczny. Nie zanotowano zgonów.
Wnioski. 1. Każdy przypadek jatrogennego urazu tętnicy biodrowej podczas operacji wewnątrznaczyniowej
wymaga indywidualnego podejścia. 2. Próba wewnątrznaczyniowej naprawy uszkodzenia jest postępowaniem
z wyboru u chorych w stanie stabilnym hemodynamicznie. 3. W przypadku dużych krwawień do przestrzeni
zaotrzewnowej konieczne jest natychmiastowe przeprowadzenie operacji klasycznej.

Shear Wave Elastography May Add a New Dimension to Ultrasound Evaluation of Thyroid Nodules: Case Series with Comparative Evaluation
Although elastography can enhance the differential diagnosis of thyroid nodules, its diagnostic performance is not ideal at present. Further improvements in the technique and creation of robust diagnostic criteria are necessary. The purpose of this study was to compare the usefulness of strain elastography and a new generation of elasticity imaging called supersonic shear wave elastography (SSWE) in differential evaluation of thyroid nodules. Six thyroid nodules in 4 patients were studied. SSWE yielded 1 true-positive and 5 true-negative results. Strain elastography yielded 5 false-positive results and 1 false-negative result. A novel finding appreciated with SSWE, were punctate foci of increased stiffness corresponding to microcalcifications in 4 nodules, some not visible on B-mode ultrasound, as opposed to soft, colloid-inspissated areas visible on B-mode ultrasound in 2 nodules. This preliminary paper indicates that SSWE may outperform strain elastography in differentiation of thyroid nodules with regard to their stiffness. SSWE showed the possibility of differentiation of high echogenic foci into microcalcifications and inspissated colloid, adding a new dimension to thyroid elastography. Further multicenter large-scale studies of thyroid nodules evaluating different elastographic methods are warranted
The Utility of 3D TOF Magnetic Resonance Angiography of Renal Arteries for Diagnosing Renal Artery Stenosis in the Hypertensive Patiens
Wstęp Celem pracy była ocena przydatności angiografii metodą rezonansu magnetycznego (MRA)
w ocenie tętnic nerkowych u chorych z podejrzeniem zwężenia tętnicy nerkowej oraz jej porównanie z podwójnym badaniem dopplerowskim u chorych poddanych następnie rentgenowskiej arteriografii nerkowej.
Materiał i metody Badanie arteriografii rezonansu magnetycznego i badanie dopplerowskie wykonano u 50 osób (33 kobiety, 17 mężczyzn, średni wiek:
49 lat) z klinicznym podejrzeniem zwężenia tętnicy nerkowej. U wszystkich chorych wykonano dotętniczą angiografię subtrakcyjną w celu weryfikacji wyników badań wykonanych metodami nieinwazyjnymi. Badania angiografii rezonansu magnetycznego wykonano za pomocą systemu rezonansu magnetycznego MAGNETOM SP 63 (1,5T) Siemens z zastosowaniem cewki do całego ciała, stosując technikę 3D Time of Flight (3D TOF). Badania dopplerowskie wykonano na aparacie ATL 3000 z opcją podwójną.
Wyniki Wyniki badań wskazują, że czułość, swoistość i skuteczność diagnostyczna angiografii rezonansu magnetycznego wynosi 98,1; 88,6 i 93,9%, natomiast badania dopplerowskiego 85,7; 90,6 i 87,8%. Różnice między tymi dwiema metodami były znamienne statystycznie.
Wniosek Niniejsza praca wskazuje, że angiografia rezonansu magnetycznego ma wysoką czułość i swoistość w wykrywaniu zwężenia tętnic nerkowych
i jest porównywalna z wynikami prezentowanymi
w innych doniesieniach.Background The aim of the study was to prospectively evaluate the diagnostic value of MRA of the renal arteries compared with renal duplex scanning in patients with clinical suspicion of renovascular disease using conventional angiography as a standard reference.
Material and methods The study included 50 patients
(33 women, 17 men, mean age: 49 yrs) with clinical suspicion of renovascular disease in whom MRA and duplex doppler were performed. All subjects underwent conventional angiography to verify the results of non-invasive methods. MRA examinations were performed with the use of of magnetic resonance system 1.5 T MAGNETOM SP 63 (Siemens) with the application of the whole-body coil. For evaluation of renal arteries 3D TOF technique has been used. Doppler Duplex (ATL 3000 HDI) evaluation was done with the ATL 3000 HDI.
Results Our results indicate that sensitivity, specificity and accuracy for MRA was 98,1%, 88,6% and 93,9% respectively and for duplex doppler 87,7%, 90,6% and 87,8% respectively. The differences in sensitivity, specificity and accuracy between two methods were statistically significant.
Conclusion Our results indicate that MRA has high sensitivity and specificity for the detection of renal artery stenosis and are comparable with the data published so far. Our study demonstrate that this method has the potential usefulness for diagnosing patients suspected for renovascular disease
Integration of Sonoelastography Into the TIRADS Lexicon Could Influence the Classification
Aim: Numerous TIRADS (Thyroid Image Reporting and Data System) classifications have been developed, and various ultrasound (US) parameters are employed in different countries. The aim of our study was to introduce risk classification and management in a native population based on the Guidelines of Polish National Societies Diagnostics and Treatment of Thyroid Carcinoma but with the addition of sonoelastography.Materials and Methods: We examined prospectively 208 patients with 305 thyroid lesions employing B-mode ultrasound and sonoelastography (SE). Nodule composition, echogenicity, margins, shape, presence or absence of calcifications, thyroid capsule, nodule size were assessed using B-mode ultrasound. Moreover, sonoelastography results were presented using the Asteria scale.Results: In univariate analysis, the following US features were significantly associated with malignancy: >50% solid /solid component, marked hypoechogenicity, ill-defined margins, micro and macrocalcification, taller-than wide shape, no/partial halo pattern, infiltration of the capsule and an Asteria score of 4. Multivariate logistic regression analysis of B-mode features revealed that ill-defined margins (OR 10.77), markedly hypoechogenicity (OR 5.12), microcalcifications (OR 4.85), thyroid capsule infiltrations (OR 3.2), macrocalcifications (OR 3.01), and hard lesion in SE (OR 6.85) were associated with a higher Odds Ratio (OR) for malignancy. Multivariate logistic regression analysis revealed that combining two features increases the OR and the best combination was irregular margins and Asteria scale 4 (OR 20.21). Adding a third feature did not increase the OR.Conclusions: Sonoelastography increases the value risk of predicted malignancy, with consequent different approach to further clinical investigation and management. A solitary feature (Asteria 4) in a solid tumor can result in its categorization as TIRADS 4, but coexistence with high risk features allows it to be upgraded to TIRADS 5. The irregular margin was the strongest single feature which allowed for the assignment of a solid tumor into TIRADS 5 category. The highest accuracy was found by combining the features of age, margin, echogenicity (markedly hypoechoic), capsule infiltration, microcalcifications and sonoelastography (Asteria 3,4) of the tumors
Application of high-frequency ultrasonography in closing small blood vessels
One of the most common treatments performed in phlebological and aesthetic medicine clinics is closing small blood vessels in the lower extremities, so-called telangiectasias and reticular vessels. Currently, there are several methods that allow for closing the dilated vessels and obtaining desirable effects, both therapeutic and aesthetic. Unfortunately, despite applying various methods and instruments, the effects of treatments are frequently not satisfactory. The factor that largely contributes to decreasing the efficacy of such procedures is complicated anatomy of the venous system and the lack of a method to precisely specify the vessel’s course, its diameter, location in the skin etc. High-frequency ultrasonography is a method enabling accurate determination of the vessels’ course as well as the measurement of their basic parameters, such as diameter, depth in the skin and presence or absence of perfusion. Thanks to ultrasound imaging with the use of high-frequency transducers, an adequate treatment method and procedure parameters may be selected, which entails enhancing the efficacy of the procedure itself. Ultrasonography may be also used for monitoring the performed procedures.Jednym z najczęściej wykonywanych zabiegów w gabinetach flebologicznych lub medycyny estetycznej jest zamykanie drobnych naczyń krwionośnych na kończynach dolnych, tj. teleangiektazji i naczyń siatkowatych. Obecnie dysponujemy kilkoma metodami, które pozwalają na zamknięcie poszerzonych naczyń krwionośnych i uzyskanie pożądanego efektu zarówno terapeutycznego, jak i estetycznego. Niestety, mimo stosowania różnych metod i urządzeń bardzo często efekty prowadzonych zabiegów nie są zadowalające. Czynnikiem, który w dużym stopniu przyczynia się do obniżenia skuteczności zabiegów, jest skomplikowana anatomia układu żylnego i brak metody pozwalającej na dokładne określenie przebiegu naczynia, jego średnicy, położenia w skórze etc. Metodą umożliwiającą precyzyjne określenie przebiegu naczyń, a także pomiar podstawowych parametrów, takich jak średnica naczynia, głębokość położenia w skórze, brak lub obecność przepływu w naczyniu, jest ultrasonografia wysokich częstotliwości. Dzięki ultrasonograficznemu obrazowaniu naczyń za pomocą głowic wysokich częstotliwości można dobrać odpowiednią metodę leczenia, parametry zabiegowe, a tym samym zwiększyć jego skuteczność. Ultrasonografia służy również do monitorowania wykonywanych zabiegów
Ultrasound image of the skin, apparatus and imaging basics
Ultrasound imaging of the skin is becoming more and more popular. Skin ultrasound
examinations are used both in order to assess healthy skin and to evaluate pathological
lesions. They are mainly performed in dermatology as well as in broadly understood aesthetic
medicine and cosmetology. At present, skin imaging is enabled by high-frequency
equipment and high-quality conventional devices. The introduction of high-frequency
electronic transducers which are supported by conventional scanners may be a turning
point in skin ultrasound equipment. Irrespective of the ultrasound scanner, three layers
may be distinguished in the image of the healthy skin: epidermal echo, dermis and
subcutaneous tissue. High-frequency equipment allows for detailed imaging of the epidermal
echo, dermis and upper part of the subcutaneous tissue. It is also possible to
visualize the skin appendages (hair with follicles and nails) as well as slight vessels that
run in the dermis and upper subcutaneous tissue. Contrary to high-frequency equipment,
conventional scanners do not allow for a detailed assessment of the epidermal and dermal
echoes. Instead, they enable the visualization of the entire subcutaneous tissue. The following
parameters are used for the assessment of skin ultrasound images: thickness of
individual skin layers, caliber of blood vessels, echogenicity of the dermis or its individual
layers, echogenicity of the subcutaneous tissue as well as the presence or absence of flow
in slight venous vessels. Currently, the studies on the usage of sonoelastography for skin
assessment are in progress. Considering the dynamic development of skin imaging equipment
and its diagnostic possibilities, one might suspect that high-frequency examinations
will become more common and will be fundamental for the evaluation of both healthy
and pathologically altered skin. This paper is an introduction to a series of articles on
the clinical application of high-frequency ultrasound. The next articles will be published
in the subsequent issues.Ultrasonograficzne obrazowanie skóry staje się coraz bardziej popularne. Badania ultrasonograficzne
skóry znajdują zastosowanie zarówno w ocenie skóry zdrowej, jak i zmienionej
chorobowo i są wykorzystywane przede wszystkim na gruncie dermatologii oraz
szeroko rozumianej medycyny estetycznej i kosmetologii. Obecnie obrazowanie skóry
możliwe jest przy wykorzystaniu aparatów wysokich częstotliwości oraz wysokiej klasy
aparatów klasycznych. Przełomem w aparaturze do ultrasonograficznego badania skóry
może być wprowadzenie do użycia wysokoczęstotliwościowych głowic elektronicznych,
które współpracują z klasycznymi aparatami. Na obrazie skóry zdrowej bez względu
na typ zastosowanego ultrasonografu wyróżniamy trzy warstwy: echo naskórka, skórę
właściwą i tkankę podskórną. Aparaty wysokich częstotliwości umożliwiają szczegółowe
obrazowanie echa naskórka, skóry właściwej i górnej części tkanki podskórnej.
Możliwe jest również obrazowanie przydatków skóry (włosy wraz z mieszkami włosowymi,
paznokcie) oraz drobnych naczyń krwionośnych biegnących w skórze właściwej
i górnej części tkanki podskórnej. W ultrasonografach klasycznych, w porównaniu z aparatami
wysokich częstotliwości, nie ma możliwości szczegółowej oceny echa naskórka
i skóry właściwej. W zamian istnieje możliwość zobrazowania całej tkanki podskórnej.
Do oceny ultrasonograficznych obrazów skóry wykorzystuje się następujące parametry:
pomiary grubości poszczególnych warstw skóry, pomiar średnicy naczyń krwionośnych,
pomiar echogeniczności skóry właściwej lub jej poszczególnych warstw, pomiar echogeniczności
tkanki podskórnej, brak lub obecność przepływu w drobnych naczyniach
żylnych. Aktualnie trwają prace nad wykorzystaniem do oceny skóry sonoelastografii.
Biorąc pod uwagę dynamiczny rozwój aparatów do obrazowania skóry i ich możliwości
diagnostyczne, można przypuszczać, że badania wysokich częstotliwości upowszechnią
się i będą podstawą w ocenie skóry zarówno zdrowej, jak i zmienionej chorobowo.
