    The influence of a song on school children’s knowledge growth for sustainable malaria prevention

    Music as a potential prevention strategy can play a pivotal role in developing behavioural changes and creating awareness about malaria and malaria prevention. The study was conducted in a moderate-risk malaria district in sub-Saharan Africa. A comparative analysis and pre-and post-intervention evaluation of three teaching intervention methods to teach and educate young children about malaria, e.g., song intervention, drama intervention, and song-and-drama combination, was evaluated. Data were collected by means of pre-and post-intervention interviews with Grade 3 primary school children and teachers, as well as a post-intervention questionnaire with teachers, parents, guardians, and caregivers. The purpose of the study was to investigate knowledge gains on malaria in relation to malaria awareness interventions. The results revealed that the songonly intervention was the most effective learning intervention strategy in this population and that behavioural changes and knowledge growth occurred regarding malaria and malaria prevention in this population.     Could a culturally and age-appropriate song contribute towards malaria prevention in primary school learners?

    BACKGROUND : Although it is a preventable disease, malaria continues to present one of the major global health challenges. The disease is especially deadly to children under the age of 5 years. This is partly due to the fact that they have yet to build up an immune system to help protect them against malaria. As a prevention strategy, music is one way of communicating knowledge to young children and could therefore potentially be used to change children’s behaviour in order to avoid getting sick from malaria. This exploratory study aimed to compare intervention strategies designed to educate and improve knowledge growth about malaria and its prevention in Grade 3 learners in a high-risk malaria region in Vhembe District, Limpopo Province, South Africa. METHODS : Various intervention strategies were employed and evaluated to determine the most effective intervention method. The study was split into two Phases. Phase 1 used a culturally and age-appropriate song as an intervention, along with a drama piece, a song and drama piece combination, and a control group. The most effective intervention strategy determined during Phase 1 was then subjected to Phase 2, against a control group to validate its efficacy. Questionnaires were used during pre-intervention and post-intervention interviews to evaluate the knowledge growth, if any, of the learners of selected primary schools in two different areas in Vhembe District. Pre-intervention interviews were followed by a specific intervention, based on the respective study groups. After 6 weeks, the post-intervention interviews were done to determine how much learners managed to learn and retain from the intervention received. RESULTS : The study found that the group that had only the song as intervention was the most effective learning intervention method in both Phases 1 and 2. Both phases showed that there was statistical significance in almost all of the nine questions asked on the questionnaires.     Mosquito‐repellent controlled‐release formulations for fighting infectious diseases

    Malaria is a principal cause of illness and death in countries where the disease is endemic. Personal protection against mosquitoes using repellents could be a useful method that can reduce and/or prevent transmission of mosquitoborne diseases. The available repellent products, such as creams, roll-ons, and sprays for personal protection against mosquitoes, lack adequate long-term efficacy. In most cases, they need to be re-applied or replaced frequently. The encapsulation and release of the repellents from several matrices has risen as an alternative process for the development of invention of repellent based systems. The present work reviews various studies about the development and use of repellent controlled-release formulations such as polymer microcapsules, polymer microporous formulations, polymer micelles, nanoemulsions, solid-lipid nanoparticles, liposomes and cyclodextrins as new tools for mosquitoborne malaria control in the outdoor environment. Furthermore, investigation on the mathematical modelling used for the release rate of repellents is discussed in depth by exploring the Higuchi, Korsmeyer-Peppas, Weibull models, as well as the recently developed Mapossa model. Therefore, the studies searched suggest that the final repellents based-product should not only be effective against mosquito vectors of malaria parasites, but also reduce the biting frequency of other mosquitoes transmitting diseases, such as dengue fever, chikungunya, yellow fever and Zika virus.     The use of the urogenital papillae of male feral African sharptooth catfish (Clarias gariepinus) as indicator of exposure to estrogenic chemicals in two polluted dams in an urban nature reserve, Gauteng, South Africa

    The African sharptooth catfish (Clarias gariepinus) is indigenous to South Africa where it is commonly found in calm waters such as lakes, streams, rivers, swamps and floodplains. The males of this gonochoristic species can be readily distinguished from females via a distinct elongated urogenital papilla with a pointed tip, located just behind the anus. This structure is an androgen controlled secondary sexual characteristic and could potentially be affected by chemicals displaying endocrine disrupting properties during development. Results of previous studies on effects on papillae of different fish species indicated that these structures could be useful as bio-indicators of exposure due to alterations in the structures. This study investigated the occurrence of abnormal morphology in urogenital papillae compared to the macroscopic and microscopic structure of the gonads of presumed male or intersex catfish sampled from the Rietvlei and Marais dams in an urban nature reserve, due to exposure to endocrine disrupting chemicals (EDCs). Macroscopic observation of the UGP, gonad examination after laparotomy and histology were done. Gonadosomatic index (GSI) and Urogenital papilla length index (UGPLI) were determined for all fish collected. Mesenteric fat analyzed for selected alkylphenols and organochlorines showed high levels of especially nonylphenol indicating long term exposure to these chemicals due to bio-accumulation. The GSI indicated that fish with intersex gonads were probably feminized males.     Acceptability and effectiveness of a monofilament, polyethylene insecticide-treated wall lining for malaria control after six months in dwellings in Vhembe District, Limpopo Province, South Africa

    BACKGROUND : South Africa uses indoor residual spraying (IRS) for vector control in its malaria control programme (MCP). Insecticide-treated wall linings (ITWLs) offer possible advantages over IRS and long-lasting, insecticide-treated nets (LLINs). This study assessed the user acceptability and perceived effectiveness, and the durability, including efficacy through bioassays, of a newly developed, monofilament polyethylene ITWL. METHODS : Four ITWL formulations/treatments, two incorporated with deltamethrin and two with alpha-cypermethrin in concentrations ranging from 0.29 to 0.85 wt%, and untreated linings were randomly installed on the inner walls of traditional mud huts (n = 20) and modern brick houses (n = 20) in a community village in Vhembe District, Limpopo Province. The linings were exposed to conditions within these dwellings over 6 months. Data were collected monthly through questionnaires and entomological residual efficacy analysis of ITWL, as part of durability testing, was done bimonthly using WHO prescribed bioassays. RESULTS : Monofilament polyethylene ITWLs were successfully installed in traditional sleeping huts and in bedrooms of modern type brick houses. ITWL remained intact throughout the entire 6 months of the study. Participants did not express any dissatisfaction towards the linings although two participants indicated the product should be fitted at a lower level for better results. User perceived effectiveness was very high with participants reporting observed mortality of mosquitoes and other nuisance insects. This perception coincided with results obtained through residual efficacy bioassays where a 100 % knockdown and mortality of mosquitoes was recorded throughout the trial period. Acceptability regarding appearance, including colour, position and attachment method, was also satisfactory with some participants citing the lining as decorative. All participants opted to keep ITWL and residual long-term efficacy will be determined annually for a further 3 years. CONCLUSIONS : The newly developed ITWLs are highly accepted amongst participants in an unsprayed section of a village in a malaria-endemic area. The perceived effectiveness that coincides with results obtained through bioassays and acceptance of the overall appearance of ITWL will be evaluated over a longer term to determine sustainability.     COVID-19 and the malaria elimination agenda in Africa : re-shifting the focus

    The global Coronavirus disease 2019 (COVID-19) pandemic has resulted in public health, political, scientific and private sector response at an unprecedented scale. However, this shift in focus has caused widespread disruption to global health services and has the potential to reverse gains made in efforts to control malaria. If health systems are not able to maintain malaria control interventions while managing the response to the COVID-19 pandemic, malaria cases will increase, thereby placing even more strain on already overtaxed systems. Using a Narrative Review Approach, this commentary explores the impact of COVID-19 on progress made with malaria control and prevention strategies in Africa; and discusses possible mitigation steps to aid community resilience building, through proactive planning and implementation of integrated, inclusive and sustainable strategies to re-shift the focus to attain the malaria elimination goals. We propose strengthening community partnerships, where academia and communities should collaborate and these knowledge-sharing strategies be implemented in order for awareness and interventions to become more networked, inclusive, resilient and effective.     Household living conditions and individual behaviours associated with malaria risk : a community-based survey in the Limpopo River Valley, 2020, South Africa

    AVAILABILITY OF DATA AND MATERIALS : The datasets generated during and/or analyzed during the current study are available from the corresponding author on reasonable request.BACKGROUND : Over the past decade, implementation of multiple malaria control strategies in most countries has largely contributed to advance the global malaria elimination agenda. Nevertheless, in some regions, seasonal epidemics may adversely affect the health of local populations. In South Africa, Plasmodium falciparum malaria is still present, with the Vhembe District experiencing an incidence rate of 3.79 cases/1000 person-years in 2018, particularly in the Limpopo River Valley, bordering Zimbabwe. To elucidate the complexity of the mechanisms involved in local regular malaria outbreaks, a community-based survey was implemented in 2020 that focused on the relationship between housing conditions and malaria risky behaviours. METHODS : The community-based cross-sectional survey was conducted among the population of three study sites in the Vhembe District, which were selected based on malaria incidence rate, social and health characteristics of inhabitants. The household survey used a random sampling strategy, where data were collected through face-to-face questionnaires and field notes; to described the housing conditions (housing questionnaire), and focus on individual behaviours of household members. Statistical analyses were performed combining hierarchical classifications and logistic regressions. RESULTS : In this study, 398 households were described, covering a population of 1681 inhabitants of all ages, and 439 adults who participated in community-based survey. The analysis of situations at risk of malaria showed that the influence of contextual factors, particularly those defined by the type of habitat, was significant. Housing conditions and poor living environments were factors of malaria exposure and history, regardless of site of investigation, individual preventive behaviours and personal characteristics of inhabitants. Multivariate models showed that, considering all personal characteristics or behaviours of inhabitants, housing conditions such as overcrowding pressures were significantly associated with individual malaria risk. CONCLUSIONS : The results showed the overwhelming weight of social and contextual factors on risk situations. Considering the Fundamental Causes Theory, malaria control policies based on health behaviour prevention, should reinforce access to care or promoting health education actions.     Geo-epidemiology of malaria incidence in the Vhembe District to guide targeted elimination strategies, South-Africa, 2015-2018 : a local resurgence

    DATA AVAILABILITY : The datasets analysed during the current study are available from the corresponding author on reasonable request.In South Africa, the population at risk of malaria is 10% (around six million inhabitants) and concern only three provinces of which Limpopo Province is the most affected, particularly in Vhembe District. As the elimination approaches, a finer scale analysis is needed to accelerate the results. Therefore, in the process of refining local malaria control and elimination strategies, the aim of this study was to identify and describe malaria incidence patterns at the locality scale in the Vhembe District, Limpopo Province, South Africa. The study area comprised 474 localities in Vhembe District for which smoothed malaria incidence curve were fitted with functional data method based on their weekly observed malaria incidence from July 2015 to June 2018. Then, hierarchical clustering algorithm was carried out considering different distances to classify the 474 smoothed malaria incidence curves. Thereafter, validity indices were used to determine the number of malaria incidence patterns. The cumulative malaria incidence of the study area was 4.1 cases/1000 person-years. Four distinct patterns of malaria incidence were identified: high, intermediate, low and very low with varying characteristics. Malaria incidence increased across transmission seasons and patterns. The localities in the two highest incidence patterns were mainly located around farms, and along the rivers. Some unusual malaria phenomena in Vhembe District were also highlighted as resurgence. Four distinct malaria incidence patterns were found in Vhembe District with varying characteristics. Findings show also unusual malaria phenomena in Vhembe District that hinder malaria elimination in South Africa.     Malaria vectors and vector surveillance in Limpopo Province (South Africa) : 1927 to 2018

    Despite the annual implementation of a robust and extensive indoor residual spraying programme against malaria vectors in Limpopo Province (South Africa), significant transmission continues and is a serious impediment to South Africa’s malaria elimination objectives. In order to gain a better understanding regarding possible causes of this residual malaria, we conducted a literature review of the historical species composition and abundance of malaria vector mosquitoes in the Limpopo River Valley region of the Vhembe District, northern Limpopo Province, the region with the highest remaining annual malaria cases in South Africa. In addition, mosquito surveys were carried out in the same region between October 2017 and October 2018. A total of 2225 adult mosquitoes were collected using CO2-baited tent and light traps, human landing catches and cow-baited traps. Of the 1443 Anopheles collected, 516 were members of the An. gambiae complex and 511 An. funestus group. In the malaria endemic rural areas outside the Kruger National Park, one specimen each of An. gambiae s.s. and An. funestus and only three of An. arabiensis were collected. The latter species was abundant at a remote hot spring in the neighboring Kruger National Park. Eighteen other species of Anopheles were collected. Our survey results support the historical findings that An. arabiensis, the species widely held to be the prime malaria vector in South Africa, is a rare species in the malaria endemic Limpopo River Valley.     Effects of zinc, copper and cadmium on Oreochromis mossambicus free-embryos and randomly selected mosquito larvae as biological indicators during acute toxicity testing

    M.Sc.Aquatic toxicology is the qualitative and quantitative study of toxic effects of pollutants on aquatic organisms. The main goal in toxicity testing is to predict, in combination with other environmental factors, with known accuracy, a concentration of a specific toxicant that will not harm a system and to make this prediction in a responsible and cost effective manner. There are a variety of unique toxicity tests, with fish being one of the most popular organisms to work with, due to being the best-understood organism in the aquatic environment and its commercial importance. Zinc, copper and cadmium are three biologically important heavy metals that are commonly used in various industries. Low concentrations zinc and copper are essential micronutrients for both plants and animals, but in higher concentrations they become toxic to the environment and its biota. Cadmium has a chemical structure similar to that of zinc and is often found in association with it, but it is a very toxic substance. The effects of zinc, copper and cadmium on the free-embryo life stage (yolk sac phase) of Oreochromis mossambicus were examined, for evaluation as a possible ¡§early life stage¡¨ fish lethality assessment. ¡§Fish early life stage tests¡¨ are considered to be relatively quick, comparable and inexpensive screening tools for testing effluents and chemicals. The yolk-sac stage is considered the most sensitive life stage in fish. O. mossambicus free-embryos feed endogenously and are indigenous to southern Africa and are therefore a good choice for lethality testing. The effects of the same metals on randomly selected Culicidae (mosquito) larvae were also tested. The reason for randomly selected larvae was to determine the possibility of doing toxicity testing without a species-specific culture. Mosquitoes are very common and well known due to being vectors of various human diseases. Recommendations towards future studies, to determine the usefulness of both O. mossambicus embryos and Culicidae larvae as biological indicator organisms, were also looked at.Prof. G.J. Stey
