    OBJECTIVE: The case of a schizoaffective patient suffering from a malignant catatonic syndrome following combined lithium-risperidone therapy is explored. METHOD: A case report and relevant deliberations regarding pathophysiology of the catatonic dilemma are discussed. CONCLUSIONS: There are two critical transitions in the development of a malignant catatonic syndrome. Dopaminergic system and psychopharmacological factors are supposed to play a key role. However, other neurotransmitter systems and the individual predisposition must be considered

    Scanning gate experiments: from strongly to weakly invasive probes

    An open resonator fabricated in a two-dimensional electron gas is used to explore the transition from strongly invasive scanning gate microscopy to the perturbative regime of weak tip-induced potentials. With the help of numerical simulations that faithfully reproduce the main experimental findings, we quantify the extent of the perturbative regime in which the tip-induced conductance change is unambiguously determined by properties of the unperturbed system. The correspondence between the experimental and numerical results is established by analyzing the characteristic length scale and the amplitude modulation of the conductance change. In the perturbative regime, the former is shown to assume a disorder-dependent maximum value, while the latter linearly increases with the strength of a weak tip potential.Comment: 11 pages, 7 figure

    Imaging signatures of the local density of states in an electronic cavity

    We use Scanning Gate Microscopy to study electron transport through an open, gate-defined resonator in a Ga(Al)As heterostructure. Raster-scanning the voltage-biased metallic tip above the resonator, we observe distinct conductance modulations as a function of the tip-position and voltage. Quantum mechanical simulations reproduce these conductance modulations and reveal their relation to the partial local density of states in the resonator. Our measurements illustrate the current frontier between possibilities and limitations in imaging the local density of states in buried electron systems using scanning gate microscopy

    Suture repair of umbilical hernia during caesarean section: a case-control study

    Purpose: The objective of this study was to investigate the additional burdens in terms of pain, prolongation of surgery and morbidity which is added to elective caesarean section if umbilical hernia suture repair is performed simultaneously. Secondly, patient's satisfaction and hernia recurrence rate were assessed. Methods: Consecutive women with symptomatic umbilical hernia undergoing internal or external suture repair during elective caesarean were included in this retrospective cohort-control study. Data on post-operative pain, duration of surgery and morbidity of a combined procedure were collected. These patients were matched 1:10 to women undergoing caesarean section only. Additionally, two subgroups were assessed separately: external and internal suture hernia repair. These subgroups were compared for patient's satisfaction, cosmesis, body image and recurrence rate. Results: Fourteen patients with a mean age of 37years were analysed. Internal suture repair (n=7) prolonged caesarean section by 20min (p=0.001) and external suture repair (n=7) by 34min (p<0.0001). Suture repair did not increase morphine use (0.38±0.2 vs. 0.4±02mg/kg body weight), had no procedure-related morbidity and prolonged hospitalization by 0.5days (p=0.01). At a median follow-up of 37 (5-125) months, two recurrences in each surgical technique, internal and external suture repair, occurred (28%). Body image and cosmesis score showed a higher level of functioning in internal suture repair (p=0.02; p=0.04). Discussion: Despite a high recurrence rate, internal suture repair of a symptomatic umbilical hernia during elective caesarean section should be offered to women if requested. No additional morbidity or scar is added to caesarean section. Internal repair is faster, and cosmetic results are better, additional skin or fascia dissection is avoided, and it seems to be as effective as an external approach.     Qubits made by advanced semiconductor manufacturing

    AbstractFull-scale quantum computers require the integration of millions of qubits, and the potential of using industrial semiconductor manufacturing to meet this need has driven the development of quantum computing in silicon quantum dots. However, fabrication has so far relied on electron-beam lithography and, with a few exceptions, conventional lift-off processes that suffer from low yield and poor uniformity. Here we report quantum dots that are hosted at a 28Si/28SiO2 interface and fabricated in a 300 mm semiconductor manufacturing facility using all-optical lithography and fully industrial processing. With this approach, we achieve nanoscale gate patterns with excellent yield. In the multi-electron regime, the quantum dots allow good tunnel barrier control—a crucial feature for fault-tolerant two-qubit gates. Single-spin qubit operation using magnetic resonance in the few-electron regime reveals relaxation times of over 1 s at 1 T and coherence times of over 3 ms.</jats:p

    Meta-analyses allow to combine results systematically and to obtain more precise quantitative results on the efficacy of a therapy. For the clinician, the comparison of two therapy modalities is particularly of interest. Several meta-analyses exist in the field of periodontal regenerative procedures. The problem is that the results are difficult to interpret for the clinician. It is only the clinical effect that can indicate the superiority of a certain treatment modality, e.g. remaining periodontal pockets. The aim of the present systematic review was the re-evaluation of studies of existing meta-analyses and to determine the probability of remaining periodontal pockets of more than 3 respectively 5 mm after active therapy. The probability of remaining periodontal pockets over 3 respectively 5 mm was significantly higher after periodontal flap procedure without regenerative procedures, compared to guided tissue regeneration (GTR) or the use of enamel matrix derivatives. Moreover, the probability of remaining pockets over 3 mm was with GTR on average 57% and with the use of enamel maxtrix derivatives 74%. Using the cut-off value of 5 mm this probability was reduced to 8 and 17%, respectively. A new clinical attachment that was less than 50% of the original level was to be expected in 29% (GTR) and 15% using enamel matrix derivatives. This statistical interpretation permits not only the clinician, but also the patient to compare the efficacy of the different treatment modalities using the probability of primary clinical effect outcomes

    Notes on a stick: use and acceptability of woman-held maternity notes

    OBJECTIVE: To provide expectant mothers with a USB stick containing their maternity notes, including ultrasound images, and to assess its use and acceptability versus conventional care with exclusively hospital-held notes. STUDY DESIGN: USB group: 200 women attending Zurich University Hospital Obstetrics Department for antenatal-to-postnatal care in 2006-2007. Controls: 200 women attending the Obstetrics Department for delivery only, after receiving conventional antenatal care elsewhere. Women were interviewed using an essentially identical postpartum questionnaire for each group, with minor wording differences. Main outcome measures: Overall satisfaction with pregnancy and delivery, feeling of safety, interest in the pregnancy, partner involvement, usefulness of USB stick in emergencies, impact on smoking behaviour, data confidentiality concerns, pregnancy and infant outcome. Stepwise multiple regression was used to identify determinants of overall impressions of pregnancy and delivery. RESULTS: Of the USB group, 98.5% wished to repeat the USB experience in a subsequent pregnancy; of the controls, 86.5% would have appreciated the experience, and 18.0% could think of situations in their pregnancy (vacation, emergencies) where the stick would have helped; 7.5% of the USB group shared their stick data with a doctor outside the Department, and 80.5% felt safer having the stick available. Along with preterm delivery and mode of delivery, the USB stick was a significant determinant of the overall positive impression of pregnancy. Primary caesarean section was (inexplicably) more frequent in the USB group. CONCLUSION: The questionnaire confirmed that issuing women with their maternity notes on a USB stick is a major advance in patient empowerment, satisfaction and safety

    Evaluation of gridding procedures for air temperature over Southern Africa

    Africa is considered to be highly vulnerable to climate change, yet the availability of observational data and derived products is limited. As one element of the SASSCAL initiative (Southern African Science Service Centre for Climate Change and Adaptive Land Management), a cooperation of Angola, Botswana, Namibia, Zambia, South Africa and Germany, networks of automatic weather stations have been installed or improved (http://www.sasscalweathernet.org). The increased availability of meteorological observations improves the quality of gridded products for the region. Here we compare interpolation methods for monthly minimum and maximum temperatures which were calculated from hourly measurements. Due to a lack of longterm records we focused on data ranging from September 2014 to August 2016. The best interpolation results have been achieved combining multiple linear regression (elevation, a continentality index and latitude as predictors) with three dimensional inverse distance weighted interpolation
