Identification of bony abnormalities in diabetic foot
syndrome depends not only on the clinical data, but
also laboratory results and diagnostic imaging. Team
work and communication between radiologist,
diabetologist and other team members is crucial in
the process of reaching proper diagnosis. The
radiologist needs to be provided with all necessary
clinical data that can influence assessment of images
and the clinician needs to know, how to use the
potential of diagnostic imaging and how to interpret

w zespole stopy cukrzycowej wpływają zarówno
obraz kliniczny, jak i wyniki badań laboratoryjnych
oraz obrazowych. W celu pełnego wykorzystania
możliwości diagnostyki obrazowej istotne jest nie
tylko propagowanie wiedzy na temat specyfiki zmian
w zespole stopy cukrzycowej wśród radiologów, ale
również pogłębianie współpracy między diabetologami,
radiologami i innymi członkami zespołu opiekującego
się pacjentami. Dzięki efektywnej wymianie
doświadczeń do diagnosty powinny docierać
wszystkie informacje, które mogą być istotne w ocenie
badania obrazowego, a klinicysta ma szansę się
dowiedzieć, jak wykorzystywać możliwości diagnostyki
obrazowej i w jaki sposób interpretować wyniki.

Broaching sexual health topics during a doctor-patient encounter in ambulatory psychiatric care
WSTĘP: Trudności w sferze seksualnej są częstym problemem pacjentów psychiatrycznych. Zarówno psychopatologia niektórych zaburzeń i chorób psychicznych, jak i efekty niepożądane wielu leków stosowanych w psychiatrii mogą negatywnie wpływać na sferę seksualną pacjenta. Celem niniejszego badania było określenie, w jakim stopniu lekarze psychiatrzy podejmują z pacjentami rozmowę na temat sfery seksualnej oraz jakie są oczekiwania pacjentów w tym zakresie.MATERIAŁ I METODY: 615 ambulatoryjnych pacjentów psychiatrycznych oceniło swoją postawę do lekarza prowadzącego, umiejętność psychiatry do taktownego wprowadzenia tematów związanych ze sferą seksualną oraz swoje oczekiwania w zakresie takiej rozmowy. Badanie zostało przeprowadzone w 10 spośród 30 losowo wybranych poradni zdrowia psychicznego w północno-wschodniej Polsce.WYNIKI: Pacjenci ujawniali pozytywną postawę do leczących ich psychiatrów. Ponad połowa ankietowanych ujawniła, że psychiatra nie omawia z nimi sfery seksualnej. Jedna czwarta badanych chciałaby rozmawiać podczas wizyty u psychiatry o swoim życiu seksualnym, jedna czarta nie była pewna, czy chce ten temat podejmować. Gotowość pacjentów do podejmowania w rozmowie tematów seksualnych była istotnie związana z wprowadzaniem przez lekarza tego tematu do rozmowy.WNIOSKI: Psychiatra jako osoba dominująca w diadzie lekarz−pacjent ma wpływ na kształtowanie relacji z pacjentem i wprowadzanie do rozmowy tematów związanych ze sferą seksualną. Taktowne poruszanie przez psychiatrę tematów dotyczących funkcjonowania seksualnego pacjenta zwiększa jego gotowość do rozmowy na ten temat. Introduction: Difficulties in sexual functioning are a frequent problem for psychiatric patients. Both the psychopathology of some psychiatric diseases as well as the side effects of medication used in psychiatry may adversely affect the patient’s sexual functioning. The aim of this study was to investigate to what extent psychiatrists raise sexual matters with patients during their encounters in psychiatric ambulatory care and what the patients’ expectations in this area are.Material and methods: 615 psychiatric outpatients responded to the anonymous questionnaire regarding their attitudes towards their psychiatrists, their evaluation of the doctors’ ability to tactfully address sexual matters during the encounter and their expectations from such an interaction. The study was conducted in 10 out of 30 randomly chosen public mental health clinics in north-eastern Poland.Results: In general, patients expressed a positive attitude towards their therapists. Over half of the study group revealed that the psychiatrist did not discuss with them their sexual health concerns. A quarter of the patients surveyed would have liked to talk about their sex life during their encounter with the psychiatrist whereas a quarter were not certain if they wanted to address the subject. The patient’s readiness to discuss sexual health issues was significantly correlated with the initiation of this topic by the doctor.Conclusions: The psychiatrist as the dominant person in the doctor–patient dyad has a considerable influence on the shape of the patient’s relationship with the therapist and the initiation of sexual health topics during the doctor-patient encounter. The tactful introduction of the topic of the patient’s sexual functioning by the psychiatrist increases the patient’s willingness to discuss their sexual health concerns
Aging and Hypertension – Independent or Intertwined White Matter Impairing Factors? Insights From the Quantitative Diffusion Tensor Imaging
Aging disrupts white matter integrity, and so does continuous elevated blood pressure that accompanies hypertension (HTN). Yet, our understanding of the interrelationship between these factors is still limited. The study aimed at evaluating patterns of changes in diffusion parameters (as assessed by quantitative diffusion fiber tracking – qDTI) following both aging, and hypertension, as well as the nature of their linkage. 146 participants took part in the study: the control group (N = 61) and the patients with hypertension (N = 85), and were divided into three age subgroups (25–47, 48–56, 57–71 years). qDTI was used to calculate the values of fractional anisotropy, mean, radial and axial diffusivity in 20 main tracts of the brain. The effects of factors (aging and hypertension) on diffusion parameters of tracts were tested with a two-way ANOVA. In the right hemisphere there was no clear effect of the HTN, nor an interaction between the factors, though some age-related effects were observed. Contrary, in the left hemisphere both aging and hypertension contributed to the white matter decline, following a functional pattern. In the projection pathways and the fornix, HTN and aging played part independent of each other, whereas in association fibers and the corpus callosum if the hypertension effect was significant, an interaction was observed. HTN patients manifested faster decline of diffusion parameters but also reached a plateau earlier, with highest between-group differences noted in the middle-aged subgroup. Healthy and hypertensive participants have different brain aging patterns. The HTN is associated with acceleration of white matter integrity decline, observed mainly in association fibers of the left hemisphere

Dieser Beitrag befasst sich mit den Werken des polnischen Gegenwartsautors und Journalisten Mariusz Szczygieł (*1966). Szczygiełs tschechische Reportagen (Gottland [2006], Zrób sobie raj [2010] und in gewissem Maße Nie ma [2018]) werden aus der Perspektive der Gedächtnisforschung analysiert, wobei besonderes Augenmerk sich auf die Frage des Tabus im kollektiven Gedächtnis und das Thema der falschen Erinnerung (die Produkte des Geistes einzelner Personen oder Gruppen von Menschen, die ihnen die Gewissheit geben, dass ein bestimmtes Ereignis stattgefunden hat, obwohl es nie stattgefunden hat) richtet, die die Unglaubwürdigkeit von Erinnerungen und Reminiszenzen aufzeigen sollen. Die Autorin argumentiert, dass Szczygiełs tschechische Reportagen im Kontext von Fragen zu lesen sind, die in der polnischen Öffentlichkeit in den letzten Jahrzehnten lebhaft diskutiert wurden: das Dilemma, wie mit der kommunistischen Vergangenheit nach 1989 umzugehen ist (lustration; polnisch: lustracja); und wie mit dem Referenzpakt beim Schreiben und Lesen von literarischen Sachbüchern zu verfahren ist

Thrombosis of the persistent median artery as a cause of carpal tunnel syndrome – case study
Carpal tunnel syndrome is the most frequent neuropathy of the upper extremity, that
mainly occurs in manual workers and individuals, whose wrist is overloaded by performing
repetitive precise tasks. In the past it was common among of typists, seamstresses
and mechanics, but nowadays it is often caused by long hours of computer keyboard use.
The patient usually complains of pain, hypersensitivity and paresthesia of his hand and
fingers in the median nerve distribution. The symptoms often increase at night. In further
course of the disease atrophy of thenar muscles is observed. In the past the diagnosis was
usually confirmed in nerve conduction studies. Nowadays a magnetic resonance scan or
an ultrasound scan can be used to differentiate the cause of the symptoms. The carpal
tunnel syndrome is usually caused by compression of the median nerve passing under
the flexor retinaculum due to the presence of structures reducing carpal tunnel area,
such as an effusion in the flexor tendons sheaths (due to overload or in the course of
rheumatoid diseases), bony anomalies, muscle and tendon variants, ganglion cysts or
tumors. In some cases diseases of upper extremity vessels including abnormalities of the
persistent median artery may also result in carpal tunnel syndrome. We present a case
of symptomatic carpal tunnel syndrome caused by thrombosis of the persistent median
artery which was diagnosed in ultrasound examination. The ultrasound scan enabled
for differential diagnosis and resulted in an immediate referral to clinician, who recommended
instant commencement on anticoagulant treatment. The follow‑up
observation
revealed nearly complete remission of clinical symptoms and partial recanalization of
the persistent median artery.Zespół cieśni kanału nadgarstka jest najczęstszą neuropatią kończyny górnej, która występuje
u osób pracujących fizycznie, jak również długotrwale wykonujących precyzyjną
pracę ręką. Dotąd dotyczył on przede wszystkim maszynistek, szwaczek i mechaników,
jednak obecnie występuje także u osób pracujących długo przy komputerze. Pacjent
skarży się na ból, przeczulicę oraz drętwienie ręki i palców unerwionych przez nerw
pośrodkowy. Objawy nasilają się często w godzinach nocnych. Przy dłuższym czasie
trwania choroby do dolegliwości bólowych dołączają się zaniki mięśniowe mięśni kłębu
kciuka. W przeszłości diagnoza potwierdzana była zazwyczaj w badaniu czynnościowym
przewodnictwa nerwowego. Obecnie wykorzystuje się tomografię rezonansu magnetycznego,
a zwłaszcza badanie ultrasonograficzne. Badania te pozwalają na różnicowanie
przyczyn wystąpienia dolegliwości. Zespół cieśni kanału nadgarstka jest z reguły spowodowany
uciskiem na przebiegający pod troczkiem zginaczy nerw pośrodkowy przez
znajdujące się tam nieprawidłowe struktury, takie jak wysięk w pochewkach ścięgien
zginaczy w związku z chorobami reumatoidalnymi lub zmianami przeciążeniowymi, anomalie
kostne i ścięgnisto‑mięśniowe
gangliony lub zmiany o charakterze rozrostowym.
Także choroby naczyń, a szczególnie zmiany w przetrwałej tętnicy pośrodkowej, mogą
powodować pojawienie się dolegliwości typowych dla zespołu cieśni kanału nadgarstka.
W pracy przedstawiono przypadek zespołu cieśni nadgarstka spowodowanego zakrzepicą
przetrwałej tętnicy pośrodkowej, rozpoznaną w badaniu ultrasonograficznym. Badanie
ultrasonograficzne umożliwiło zróżnicowanie przyczyny dolegliwości i ukierunkowanie
dalszego toku postępowania. Pacjentka w trybie pilnym została skierowana do klinicysty,
w efekcie czego wdrożono skuteczne leczenie przeciwzakrzepowe i uzyskano znaczne
ustąpienie dolegliwości i częściową rekanalizację przetrwałej tętnicy pośrodkowej
Ultrasound-diagnosed bone and joint destruction as a typical image in advanced Charcots arthropathy – case report
The paper presents a case of Charcot foot in a patient with long standing type 2 diabetes
and complicated by peripheral neuropathy. It was initially diagnosed by an ultrasound
examination and subsequently confirmed by an X-ray and an magnetic resonance
imaging. Diabetic neuropathy is nowadays the most frequent cause of Charcot arthropathy,
although it can be also a result of other diseases of the nervous system. In the acute
phase the patient usually presents with edema, redness and increased temperature of the
foot, which can suggest many other diagnoses including bacterial infection, gout, venous
thrombosis or trauma. Because of its non specific clinical presentation and unsufficient
awareness of the specificity of the diabetic foot syndrome among health professionals and
the patients the diagnosis of this process is in many cases delayed. In the acute phase
appropriate treatment needs to be initiated (mainly off loading and immobilization of the
foot in a total contact cast), otherwise a rapidly progressing destruction of the bones and
joints will usually begin, leading to fractures, dislocations and a severe foot deformity.
Increased awareness among doctors taking care of the diabetic patients and appropriate
use of the imaging methods can definitely improve efficacy of the diagnostic process and
help to optimize the treatment of Charcot arthropathy. The standard approach usually
includes use of radiography, magnetic resonance imaging and scintigraphy. In some cases
a sonographer may be the first one to notice typical signs of bony destruction in a patient
with Charcot arthropathy and suggest immediate further imaging in order to confirm the
diagnosis and to minimize the risk of mutilating complications
