De economische en maatschappelijke gevolgen van bijterij zullen de komende tijd waarschijnlijk alleen maar toenemen. Varkenshouders en bedrijfsadviseurs kunnen daar nu al op inspelen in een sectorbrede aanpak die gericht is op het terugdringen van bijterij en het geleidelijk aan ‘toegroeien’ naar intacte oorpunten en krulstaarten. Zodra bijterij optreedt is een snelle en effectieve behandeling belangrijk om escalatie van de problemen te voorkomen en negatieve effecten op productie en dierenwelzijn te minimaliseren. Dit is het 6e artikel uit een reeks artikelen over de oorzaken, effecten en preventie en behandeling van staartbijten onder varkens en hoe varkenshouders hier tegenaan kijken.     Between hope and fear: patient’s expectations prior to pelvic organ prolapse surgery

    INTRODUCTION AND HYPOTHESIS: The aim of our study was to analyse the patient's expectations (fears and goals (hopes)) in women who are scheduled for pelvic organ prolapse (POP) surgery. METHODS: All consecutive women awaiting surgery for POP in a tertiary urogynaecological centre were included. A short questionnaire with two open questions on goals and fears with regard to the operation was used. RESULTS: Ninety-six out of 111 distributed questionnaires (86%) were analysed. Goals and fears were categorized into five groups. De novo symptoms (63%), POP recurrence (34%) and surgical complications (29%) were the most important fears. Symptom release (96%), improved lifestyle (physical capabilities; 30%) and improved sexual life (18%) were important goals. CONCLUSIONS: A wide variety of expectations both positive and negative can be found in women before POP surgery and should be an integral part of preoperative counselling. Achieving the individual goals as based on expectations, positive (goals) and negative (fears), should be part of the POP surgery evaluation

    Total laparoscopic hysterectomy versus total abdominal hysterectomy with bilateral salpingo-oophorectomy for endometrial carcinoma: a randomised controlled trial with 5-year follow-up

    This report is on recovery and long-term outcomes in a small-scale randomised controlled trial (RCT) after total laparoscopic hysterectomy versus total abdominal hysterectomy in (potential) endometrial carcinoma patients. An RCT was performed among women with atypical endometrial hyperplasia and endometrial carcinoma scheduled for hysterectomy in a teaching hospital in The Netherlands. Women were randomised to total laparoscopic hysterectomy versus total abdominal hysterectomy both with bilateral salpingo-oophorectomy and were followed until 5 years after the intervention. Patients completed the RAND 36-Item Short Form Health Survey (RAND-36), Quality of Recovery-40 (QoR-40) and Recovery Index-10 (RI-10) until 12 weeks after surgery. Main outcome measure was quality of life and recovery in the first 12 weeks after surgery. A linear mixed model was used for statistical analysis while accounting for baseline values where applicable. Seventeen women were included, of whom 11 allocated to the laparoscopic arm and 6 to the abdominal arm. Laparoscopic hysterectomy performed better on all scales and subscales used in the study. A statistically significant treatment effect, favouring laparoscopic hysterectomy, was found in the total RAND-36 (difference between groups 142 units, 95% confidence interval 46; 236). Clinical follow-up was completed after median 60 months, but this study was too small for conclusions regarding the safety and survival. Laparoscopic hysterectomy results in better postoperative quality of life in the first 12 weeks after surgery when compared with abdominal hysterectomy

    Does 4D transperineal ultrasound have additional value over 2D transperineal ultrasound for diagnosing posterior pelvic floor disorders in women with obstructed defecation syndrome?

    Objective To establish the diagnostic test accuracy of two‐dimensional (2D) and four‐dimensional (4D) transperineal ultrasound (TPUS) for diagnosis of posterior pelvic floor disorders in women with obstructed defecation syndrome (ODS), in order to assess if 4D ultrasound imaging provides additional value. Methods This was a prospective cohort study of 121 consecutive women with ODS. Symptoms of ODS and pelvic organ prolapse on clinical examination were assessed using validated methods. All women underwent both 2D‐ and 4D‐TPUS. Imaging analysis was performed by two blinded observers. Posterior pelvic floor disorders were dichotomized into presence or absence, according to predefined cut‐off values. In the absence of a reference standard, a composite reference standard was created from a combination of results of evacuation proctography, magnetic resonance imaging and endovaginal ultrasound. Primary outcome measures were diagnostic test characteristics of 2D‐ and 4D‐TPUS for rectocele, enterocele, intussusception and anismus. Secondary outcome measures were interobserver agreement, agreement between the two imaging techniques, and association of severity of ODS symptoms and degree of posterior vaginal wall prolapse with conditions observed on imaging. Results For diagnosis of all four posterior pelvic floor disorders, there was no difference in sensitivity or specificity between 2D‐ and 4D‐TPUS (P = 0.131–1.000). Good agreement between 2D‐ and 4D‐TPUS was found for diagnosis of rectocele (Îș = 0.675) and moderate agreement for diagnoses of enterocele, intussusception and anismus (Îș = 0.465–0.545). There was no difference in rectocele depth measurements between the techniques (19.9 mm for 2D vs 19.0 mm for 4D, P = 0.802). Interobserver agreement was comparable for both techniques, although 2D‐TPUS had excellent interobserver agreement for diagnosis of enterocele and rectocele depth measurements, while this was only moderate and good, respectively, for 4D‐TPUS. Diagnoses of rectocele and enterocele on both 2D‐ and 4D‐TPUS were significantly associated with degree of posterior vaginal wall prolapse on clinical examination (odds ratio (OR) = 1.89–2.72). The conditions observed using either imaging technique were not associated with severity of ODS symptoms (OR = 0.82–1.13). Conclusions There is no evidence of superiority of 4D ultrasound acquisition to dynamic 2D ultrasound acquisition for the diagnosis of posterior pelvic floor disorders. 2D‐ and 4D‐TPUS could be used interchangeably to screen women with symptoms of ODS

    In deze studie is middels een steekproef in Noord-Brabant en Gelderland in de praktijk op 48 varkensbedrijven het geurverwijderingsrendement van een luchtwasser bepaald. De steekproef omvatte zowel chemische, combi- als biologische wassers. Daarnaast zijn indicatieve metingen van de ammoniakverwijdering uitgevoerd en is een aantal procesparameters bepaald. Uit de studie blijkt dat de prestatieniveaus van de onderzochte chemische en biologische wassers dicht in de buurt van de Rgv- en Rav-reductieniveaus liggen, maar dat het prestatieniveau van de combi-wassers aanzienlijk lager is dan het Rgv- en Rav-reductieniveau
