    Spatial determinants of local government action on climate change: an analysis of local authorities in England

    The engagement of UK local authorities is vital if national government is to meet its climate change commitments. However, with no mandatory targets at local government level, other drivers must explain engagement. Using a Geographic Information System, this study compares the spatial distribution of action on climate change based on past actions and stated intentions to a suite of relevant independent variables. The Action Index created is among the first to quantify climate change engagement beyond a simple binary measure and provides a useful comparative study to recent work in the US. The Index enables investigation of both mitigation and adaptation, which show different trends in relation to some variables. The study shows that action is strongest where the voting habits of the local population suggest environmental concern and where neighbouring local authorities are also engaging in action on climate change. Physical vulnerability to the effects of climate change is a motivator for action only where the dangers are obvious. Action is less likely where other resource intensive issues such as crime and housing exist within a local authority area

    Using Vignettes in Qualitative Research to Explore Barriers and Facilitating Factors to the Uptake of Prevention of Mother-to-Child Transmission Services in Rural Tanzania: A Critical Analysis.

    Vignettes are short stories about a hypothetical person, traditionally used within research (quantitative or qualitative) on sensitive topics in the developed world. Studies using vignettes in the developing world are emerging, but with no critical examination of their usefulness in such settings. We describe the development and application of vignettes to a qualitative investigation of barriers to uptake of prevention of mother-to-child transmission (PMTCT) HIV services in rural Tanzania in 2012, and critique the successes and challenges of using the technique in this setting. Participatory Learning and Action (PLA) group activities (3 male; 3 female groups from Kisesa, north-west Tanzania) were used to develop a vignette representing realistic experiences of an HIV-infected pregnant woman in the community. The vignette was discussed during in-depth interviews with 16 HIV-positive women, 3 partners/relatives, and 5 HIV-negative women who had given birth recently. A critical analysis was applied to assess the development, implementation and usefulness of the vignette. The majority of in-depth interviewees understood the concept of the vignette and felt the story was realistic, although the story or questions needed repeating in some cases. In-depth interviewers generally applied the vignette as intended, though occasionally were unsure whether to steer the conversation back to the vignette character when participants segued into personal experiences. Interviewees were occasionally confused by questions and responded with what the character should do rather than would do; also confusing fieldworkers and presenting difficulties for researchers in interpretation. Use of the vignette achieved the main objectives, putting most participants at ease and generating data on barriers to PMTCT service uptake. Participants' responses to the vignette often reflected their own experience (revealed later in the interviews). Participatory group research is an effective method for developing vignettes. A vignette was incorporated into qualitative interview discussion guides and used successfully in rural Africa to draw out barriers to PMTCT service use; vignettes may also be valuable in HIV, health service use and drug adherence research in this setting. Application of this technique can prove challenging for fieldworkers, so thorough training should be provided prior to its use

    Determinants of successful clinical networks : The conceptual framework and study protocol

    Background Clinical networks are increasingly being viewed as an important strategy for increasing evidence-based practice and improving models of care, but success is variable and characteristics of networks with high impact are uncertain. This study takes advantage of the variability in the functioning and outcomes of networks supported by the Australian New South Wales (NSW) Agency for Clinical Innovation's non-mandatory model of clinical networks to investigate the factors that contribute to the success of clinical networks. Methods/Design The objective of this retrospective study is to examine the association between external support, organisational and program factors, and indicators of success among 19 clinical networks over a three-year period (2006-2008). The outcomes (health impact, system impact, programs implemented, engagement, user perception, and financial leverage) and explanatory factors will be collected using a web-based survey, interviews, and record review. An independent expert panel will provide judgements about the impact or extent of each network's initiatives on health and system impacts. The ratings of the expert panel will be the outcome used in multivariable analyses. Following the rating of network success, a qualitative study will be conducted to provide a more in-depth examination of the most successful networks. Discussion This is the first study to combine quantitative and qualitative methods to examine the factors that contribute to the success of clinical networks and, more generally, is the largest study of clinical networks undertaken. The adaptation of expert panel methods to rate the impacts of networks is the methodological innovation of this study. The proposed project will identify the conditions that should be established or encouraged by agencies developing clinical networks and will be of immediate use in forming strategies and programs to maximise the effectiveness of such networks

    Emotional problems in preadolescents in Norway: the role of gender, ethnic minority status, and home- and school-related hassles

    <p>Abstract</p> <p>Background</p> <p>"The gender gap" refers to a lifelong higher rate of emotional problems in girls, as compared to boys, that appears during adolescence. The gender gap is a well-replicated finding among older adolescents and is assumed to be a cross-cultural phenomenon. However, these cross-cultural studies have not investigated the gender gap in ethnic minorities but sampled ethnic majority adolescents in different countries. Some studies that investigated the gender gap across ethnic groups indirectly (by presenting emotional problem scores stratified by gender and ethnic group) indicate that the gender gap is less prominent or even absent among minorities. The aims of this study were to assess whether the gender gap is found in both majority and minority preadolescents, and to investigate whether a possible (gender and ethnic) group difference can be accounted for by differences in home or school hassles.</p> <p>Methods</p> <p>Participants were 902 preadolescent students (aged 10 to 12) from two cities in Norway. We collected self-report measures of emotional problems and home and school hassles. Using mediated moderation analysis we tested whether the interaction effect between gender and ethnic minority background on emotional problems was mediated by home or school hassles.</p> <p>Results</p> <p>The gender gap in emotional problems was restricted to ethnic majority preadolescents. School hassles but not home hassles accounted in part for this effect.</p> <p>Conclusions</p> <p>The absence of the gender gap among minority as opposed to majority preadolescents may indicate that social circumstances may postpone or hamper the emergence and magnitude of the gender gap in ethnic minority preadolescents. In this study, school hassles partly accounted for the combined gender and ethnic group differences on emotional problems. This indicates that school hassles may play a role in the higher levels of emotional problems in preadolescent minority boys and consequently the absence of a gender gap found in our minority sample.</p

    Analysis of human mandibular mechanics based on screw theory and in vivo data.

    In this paper the mechanics of human mandibular function is described in terms of the associated screws. The two distinct, yet related features of jaw mechanics, involving the motion itself as well as the forces, are both functions of the anatomical constraints, namely the contact areas that exist within the temporomandibular joint, and the forces of the muscles and tendons that allow motion to occur. The relationships that exist between these two aspects of jaw-motion are identified in this paper showing that muscle forces can be uniquely represented in terms of the action screw. This new approach to analyzing the mechanics of jaw-motion also incorporates the previously studied motion screw or helical axis. A consistent dynamic model is formulated where the action screw is used to represent the action of the closing muscle forces while the moment arms of the muscle forces are determined about the motion screw representing mandibular kinematics. The action screw formulation is verified using in vivo motion data and MR image information for a single asymptomatic subject. The results confirm the feasibility of the method and its application in dental research. A general increase in the mechanical advantage of most muscles, in the distance between action and motion screws as well as in the expended energy towards the end of the jaw-closing phase was observed. Asymmetries in the distribution of muscle force magnitudes appeared to influence the resultant force and moment of the action screw but had little effect on its spatial location.
