    Three-dimensional finite element analysis of anterior two-unit cantilever resin-bonded fixed dental prostheses

    The aim of this study was to evaluate the influence of different framework materials on biomechanical behaviour of anterior two-unit cantilever resin-bonded fixed dental prostheses (RBFDPs). A three-dimensional finite element model of a two-unit cantilever RBFDP replacing amaxillary lateral incisorwas created. Five frameworkmaterialswere evaluated: direct fibre-reinforced composite (FRC-Z250), indirect fibre-reinforced composite (FRC-ES), gold alloy (M), glass ceramic (GC), and zirconia (ZI). Finite element analysis was performed and stress distribution was evaluated. A similar stress pattern, with stress concentrations in the connector area, was observed in RBFDPs for all materials.Maximal principal stress showed a decreasing order: ZI >M>GC> FRC-ES > FRCZ250. The maximum displacement of RBFDPs was higher for FRC-Z250 and FRC-ES than for M, GC, and ZI. FE analysis depicted differences in location of the maximum stress at the luting cement interface between materials. For FRC-Z250 and FRC-ES, the maximum stress was located in the upper part of the proximal area of the retainer, whereas, for M, GC, and ZI, the maximum stress was located at the cervical outline of the retainer. The present study revealed differences in biomechanical behaviour between all RBFDPs.The general observation was that a RBFDP made of FRC provided a more favourable stress distribution

    Haemoadsorption reduces the inflammatory response and improves blood flow during ex vivo renal perfusion in an experimental model.

    BACKGROUND: Ex-vivo normothermic perfusion strategies are a promising new instrument in organ transplantation. The perfusion conditions are designed to be protective however the artificial environment can induce a local inflammatory response. The aim of this study was to determine the effect of incorporating a Cytosorb adsorber into an isolated kidney perfusion system. METHODS: Porcine kidneys were subjected to 22 h of cold ischaemia then reperfused for 6 h on an ex vivo reperfusion circuit. Pairs of kidneys were randomised to either control (n = 5) or reperfusion with a Cytosorb adsorber (n = 5) integrated into the circuit. Tissue, blood and urine samples were taken for the measurement of inflammation and renal function. RESULTS: Baseline levels of cytokines (IL-6, TNFα, IL-8, IL-10, IL-1β, IL-1α) were similar between groups. Levels of IL-6 and IL-8 in the perfusate significantly increased during reperfusion in the control group but not in the Cytosorb group (P = 0.023, 0.049). Levels of the other cytokines were numerically lower in the Cytosorb group; however, this did not reach statistical significance. The mean renal blood flow (RBF) was significantly higher in the Cytosorb group (162 ± 53 vs. 120 ± 35 mL/min/100 g; P = 0.022). Perfusate levels of prostaglandin E2 were significantly lower in the Cytosorb group (642 ± 762 vs. 3258 ± 980 pg/mL; P = 0.0001). Levels of prostacyclin were significantly lower in the Cytosorb group at 1, 3 and 6 h of reperfusion (P = 0.008, 0.003, 0.0002). Levels of thromboxane were also significantly lower in the Cytosorb group throughout reperfusion (P = 0.005). Haemoadsorption had no effect on creatinine clearance (P = 0.109). CONCLUSION: Haemoadsorption can reduce the inflammatory response and improve renal blood flow during perfusion. Nonetheless, in this model haemoadsorption had no influence on renal function and this may relate to the broad-spectrum action of the Cytosorb adsorber that also removes potentially important anti-inflammatory mediators

    Clinical wear of approximal glass ionomer restorations protected with a nanofilled self-adhesive light-cured protective coating

    High viscous glass ionomer cement (GIC) has gained popularity as a restorative material; however, high wear is pointed as one of the major drawbacks of this material. Protective surface coatings were developed to protect GIC from water contamination with the additional advantage of occluding any surface cracks or porosities commonly found in this material, possibly resulting in an increased wear resistance of the restorations. Objective: The objective of this study was to investigate the clinical wear of GIC approximal restorations in primary molars protected either with a nanofilled self-adhesive light-cured protective coating (NPC) or with petroleum jelly. Material and Methods: Approximal caries lesions in primary molars from 32 schoolchildren previously enrolled in another clinical trial were included in this investigation. GIC restorations were performed according to the Atraumatic Restorative Treatment approach and protected with either petroleum jelly or a NPC. Impressions of the restored hemiarch were done after 1 day and 6, 12, 24 and 36 months. The impressions were scanned in a 3-D appliance and the obtained images were superimposed using an appropriate computer software. Two-way ANOVA for repeated measures and Tukey's post-hoc test were used to analyze the wear of restorations (α=5%). Results: A significant difference was found between the two groups, with a wear protection offered by the application of a NPC. Conclusion: These results suggest that the application of a NPC has a protective effect on the clinical wear of approximal GIC restorations in primary teeth

    Ship incident risk in the areas of Tubbataha and Banc d’Arguin: A case for designation as Particular Sensitive Sea Area?

    Since the early 1990's, the International Maritime Organization (IMO) has designated fourteen sea areas as Particular Sensitive Sea Areas (PSSA) that enjoy special protection because of their various important attributes and vulnerability to potential harm by increasing shipping activities. The United Nations Educational, Scientific and Cultural Organization (UNESCO) has identified two possible sites for possible designation as PSSA under IMO: the Banc d'Arguin National Park (Mauritania) and the Tubbataha Reef National Park (Philippines). This article presents an integrated framework for the estimation of total risk exposure due to shipping activities and various risk measures for ships trading in the areas of interest. Using a unique and comprehensive combination of data, we test whether ship specific risk increased over time. The results confirm an increase in the considered risk measures of ships trading through or nearby West Africa (Banc d’Arguin) and South-East Asia (Tubbataha) in general and also close to both regions and therefore support the recommendation for an increased level of protection

    Effect of hydrothermal aging on the microhardness of high- and low-viscosity conventional and additively manufactured polymers.

    STATEMENT OF PROBLEM Studies on the microhardness of novel additively manufactured polymers compared with well-established low- and high-viscosity composite resins with regard to chemical composition are lacking. PURPOSE The purpose of this in vitro study was to evaluate the effect of hydrothermal aging on the microhardness of various conventional and additively manufactured polymers. MATERIAL AND METHODS Cylindrically shaped specimens (N=240, n=10 per group) (Ø10×2 mm) were either additively manufactured (6 groups) or conventionally (6 groups) manufactured by using 3D (Optiprint Temp [OP; Dentona]; C&B MFH [ND; NextDent]; Saremco print CROWNTEC [SA; Saremco Dental AG]; Temp Print [TP; GC]; 3DELTA ETEMP [DM; Deltamed]; MED690 [ST; Stratasys, Ltd]) or conventional low (Gradia Direct [GR; GC]; Clearfil Majesty [CM; Kuraray Noritake]; Tetric EvoCeram [TE; Ivoclar Vivadent AG]) and high (Gradia Direct Flo [GR-F; GC]; Clearfil Majesty Flow [CM-F; Kuraray Noritake]; Tetric EvoFlow [TE-F; Ivoclar Vivadent AG]) viscous materials. All specimens were randomly allotted to 2 different aging methods (no-aging [dry] or aging by thermocycling [TC], ×6000, 5 °C-55 °C) and Vickers hardness (VH) tested (ZHV30; Zwick). Three indentations were made on each specimen (0.98-N load, duration 15 seconds). The calculated average microhardness value of each specimen was statistically analyzed by using 2-way ANOVA and Tukey post hoc tests (α=.05). Two-parameter Weibull distribution was calculated to predict the reliability of material type and aging method on VH. RESULTS The mean ±standard deviation VH ranged between 17 ±0.5 VHN and 68 ±0.5 VHN in the following ascending order: group STa.05). The mean ±standard deviation of HV for aged (37 ±1 VHN) and nonaged (35 ±1 VHN) specimens were statistically similar (P>.05). The Weibull distribution values presented the highest shape for the aged group SA (37.81). CONCLUSIONS The choice of the material had a significant effect and resulted in lower hardness for the 3-dimensionally printed materials than for the conventional composite resins. Under fatigue conditions, the choice of the material showed no significant difference when the Vickers microhardness was evaluated
