Making a difference for young adult patients

This briefing presents key findings from a national study (the STEPP project) on health care transitions for young adults with significant, and potentially life-limiting, conditions. The STEPP project focussed particularly on practice at the 'adult end': the clinics and wards which care for young adults with significant health conditions following their transfer from paediatrics. The project involved interviews with young adults, parents and health professionals. We have used excerpts from the interviews with young adults and parents to illustrate, and bring to life, key findings. As well as providing a rare insight into the views and experiences of young adults themselves, it also reveals how small changes in practice, that do not cost anything, can make a real difference
