In vivo Darstellung von Immunzellen mittels Eisenoxid-Nanopartikeln und MRT in einem Modell der Multiplen Sklerose
In Rahmen dieser wissenschaftlichen Arbeit wurde der Nutzen von USPION als KM in der MRTBildgebung
für die Darstellung und Quantifizierung von Entzündungsaktivität in einem Maus-Modell
der MS untersucht.
MS ist eine schwerwiegende autoimmune ZNS-Erkrankung, die mit Demyelinisierung und
Neurodegeneration einhergeht. Ausgelöst durch eine fehlgeleitete, vermutlich T-Zell-vermittelte
Immunantwort gegenüber den Myelinscheiden der neuronalen Axone, kommt es zur Entzündung und
Demyelinisierung der Neurone. Die weitverbreitete, bisher als Gold-Standard geltende Methode der
GD-unterstützen MRT-Bildgebung beschränkt sich auf die Darstellung einer BHS-Störung als Indikator
für MS Läsionen und weist hierdurch bedingt Einschränkungen hinsichtlich ihrer Sensitivität und
Spezifität auf. Relevante Aspekte der Erkrankung wie Neuroinflammation und zelluläre Mediatoren
werden nicht erfasst. SPION sind Nanopartikel mit superparamagnetischen Fähigkeiten, die als
neuartiges KM für MRT-Untersuchungen verwendet werden. Sie reichern sich in Entzündungszellen
an und eignen sich somit für die Zellverfolgung und zur Entzündungsbildgebung. Im Rahmen dieser
wissenschaftlichen Arbeit konnte gezeigt werden, dass CLIO, ein in experimentellen Studien
etabliertes SPION, sich nachweisbar in entzündlichen MS-Läsionen anreichert. Durchflusszytometrie
und histochemische Untersuchungen bestätigten, dass die Aufnahme von CLIO im Wesentlichen
durch Zellen der angeborenen Immunität, also Makrophagen und Mikroglia, erfolgt. B- und T-Zellen
phagozytieren CLIO nicht in relevantem Maße. Mechanistisch konnte gezeigt werden, dass myeloide
Zellen CLIO direkt aus der Blutstrombahn aufnehmen, während Mikroglia freie CLIO phagozytieren,
die im Rahmen einer BHS-Störung ins ZNS gelangen. Die Aufnahme geschieht in der akuten Phase
der Erkrankung. In chronischen Läsionen ließ sich keine signifikante Akkumulation von CLIO mehr
nachweisen. Die mit CLIO angereicherten MS Läsionen ließen sich mittels MRT-Untersuchung
darstellen. Dabei korrelierte das gemessene MRT-Signal mit der klinischen Krankheitsaktivität der MS.
In einem Vergleich mit dem etablierten GB-KM ließ sich eine Überlegenheit von CLIO hinsichtlich der
Korrelation mit dem klinischen Krankheitsbild belegen. Um auch die klinische Anwendbarkeit der
neuen Methode zu prüfen, führten wir einen Versuch mit dem bereits für die klinischen Anwendung
zugelassenem SPION, Ferumoxytol durch. Hier ließen sich die Ergebnisse reproduzieren.
Zusammenfassend konnte in dieser Arbeit eine Methode der nicht invasiven Entzündungsbildgebung
mit USPION etabliert werden. Sie liefert wichtige Informationen hinsichtlich der Entzündungsaktivität
und bietet die Möglichkeit der nicht invasiven Zellverfolgung. Ergänzend zur etablierten GB-KM
basierten Bildgebung ist somit eine umfassendere Charakterisierung der MS möglich, welche Vorteile
für die Diagnosestellung, Wahl der Therapie und das Therapiemonitoring mit sich bringt. Neben der
MS gibt es weitere mögliche Anwendungsgebiete, wie die Entzündungsbildgebung im Rahmen
anderer Autoimmunerkrankungen sowie die Entwicklung und Überwachung von Immuntherapien auf
dem Gebiet der Onkologie
SGLT2 inhibitor therapy and pulmonary artery pressure in patients with chronic heart failure—further evidence for improved hemodynamics by continuous pressure monitoring
Background!#!Right ventricular pacing (RVP) may cause electrical and mechanical desynchrony leading to impaired left ventricular ejection fraction (LVEF). We investigated the outcomes of RVP with His bundle pacing (HBP) and left bundle branch pacing (LBBP) for patients requiring a de novo permanent pacemaker (PPM) for bradyarrhythmia.!##!Methods and results!#!Systematic review of randomized clinical trials and observational studies comparing HBP or LBP with RVP for de novo PPM implantation between 01 January 2013 and 17 November 2020 was performed. Random and fixed effects meta-analyses of the effect of pacing technology on outcomes were performed. Study outcomes included all-cause mortality, heart failure hospitalization (HFH), LVEF, QRS duration, lead revision, atrial fibrillation, procedure parameters, and pacing metrics. Overall, 9 studies were included (6 observational, 3 randomised). HBP compared with RVP was associated with decreased HFH (risk ratio [RR] 0.68, 95% confidence interval [CI] 0.49-0.94), preservation of LVEF (mean difference [MD] 0.81, 95% CI - 1.23 to 2.85 vs. - 5.72, 95% CI - 7.64 to -3.79), increased procedure duration (MD 15.17 min, 95% CI 11.30-19.04), and increased lead revisions (RR 5.83, 95% CI 2.17-15.70, p = 0.0005). LBBP compared with RVP was associated with shorter paced QRS durations (MD 5.6 ms, 95% CI - 6.4 to 17.6) vs. (51.0 ms, 95% CI 39.2-62.9) and increased procedure duration (MD 37.78 min, 95% CI 20.04-55.51).!##!Conclusion!#!Of the limited studies published, this meta-analysis found that HBP and LBBP were superior to RVP in maintaining physiological ventricular activation as an initial pacing strategy
Full spectrum of clonal haematopoiesis-driver mutations in chronic heart failure and their associations with mortality
Aims: Somatic mutations in haematopoietic stem cells can lead to the clonal expansion of mutated blood cells, known as clonal haematopoiesis (CH). Mutations in the most prevalent driver genes DNMT3A and TET2 with a variant allele frequency (VAF) ≥ 2% have been associated with atherosclerosis and chronic heart failure of ischemic origin (CHF). However, the effects of mutations in other driver genes for CH with low VAF (<2%) on CHF are still unknown.
Methods and results: Therefore, we analysed mononuclear bone marrow and blood cells from 399 CHF patients by deep error-corrected targeted sequencing of 56 genes and associated mutations with the long-term mortality in these patients (3.95 years median follow-up). We detected 1113 mutations with a VAF ≥ 0.5% in 347 of 399 patients, and only 13% had no detectable CH. Despite a high prevalence of mutations in the most frequently mutated genes DNMT3A (165 patients) and TET2 (107 patients), mutations in CBL, CEBPA, EZH2, GNB1, PHF6, SMC1A, and SRSF2 were associated with increased death compared with the average death rate of all patients. To avoid confounding effects, we excluded patients with DNMT3A-related, TET2-related, and other clonal haematopoiesis of indeterminate potential (CHIP)-related mutations with a VAF ≥ 2% for further analyses. Kaplan–Meier survival analyses revealed a significantly higher mortality in patients with mutations in either of the seven genes (53 patients), combined as the CH-risk gene set for CHF. Baseline patient characteristics showed no significant differences in any parameter including patient age, confounding diseases, severity of CHF, or blood cell parameters except for a reduced number of platelets in patients with mutations in the risk gene set in comparison with patients without. However, carrying a mutation in any of the risk genes remained significant after multivariate cox regression analysis (hazard ratio, 3.1; 95% confidence interval, 1.8–5.4; P < 0.001), whereas platelet numbers did not.
Conclusions: Somatic mutations with low VAF in a distinct set of genes, namely, in CBL, CEBPA, EZH2, GNB1, PHF6, SMC1A, and SRSF2, are significantly associated with mortality in CHF, independently of the most prevalent CHIP-mutations in DNMT3A and TET2. Mutations in these genes are prevalent in young CHF patients and comprise an independent risk factor for the outcome of CHF, potentially providing a novel tool for risk assessment in CHF
DNMT3A clonal hematopoiesis-driver mutations induce cardiac fibrosis by paracrine activation of fibroblasts
Hematopoietic mutations in epigenetic regulators like DNA methyltransferase 3 alpha (DNMT3A), play a pivotal role in driving clonal hematopoiesis of indeterminate potential (CHIP), and are associated with unfavorable outcomes in patients suffering from heart failure (HF). However, the precise interactions between CHIP-mutated cells and other cardiac cell types remain unknown. Here, we identify fibroblasts as potential partners in interactions with CHIP-mutated monocytes. We used combined transcriptomic data derived from peripheral blood mononuclear cells of HF patients, both with and without CHIP, and cardiac tissue. We demonstrate that inactivation of DNMT3A in macrophages intensifies interactions with cardiac fibroblasts and increases cardiac fibrosis. DNMT3A inactivation amplifies the release of heparin-binding epidermal growth factor-like growth factor, thereby facilitating activation of cardiac fibroblasts. These findings identify a potential pathway of DNMT3A CHIP-driver mutations to the initiation and progression of HF and may also provide a compelling basis for the development of innovative anti-fibrotic strategies
