    A Fundamental Study on Evaluation of Public Transport Transfer Nodes by Data Envelop Analysis Approach Using Smart Card Data

    AbstractThis research proposes a method of evaluating transfer nodes based on smart card data with the objective of making a contribution to public transportation restructuring in regional cities. The study seeks to better comprehend the use of public transportation systems (trams and buses) in central Kochi City in Japan based on the transportation mode transfers recorded on user Smart Cards. Specifically, this study seeks to use the Data Envelop Analysis (DEA) model, which allows us to reference multiple indices, in order to evaluate the efficiency of user transfers between transportation systems while also considering transfer times and user age groups. The study results show that efficiency varied according to the time of day and user age groups, even at the same transfer nodes, and identified the need for more thorough understanding of the properties of each transfer point based on the efficiency values of multiple indices

    Lack of an association between marital status and survival in patients receiving stereotactic body radiotherapy for early-stage non-small-cell lung cancer

    Marital status has been proposed as a promising prognostic factor in many malignancies, including non-small-cell lung cancer (NSCLC). However, its prognostic value is still unclear for individual non-surgical treatments for stage I NSCLC. This study investigated the prognostic value of marital status in patients with early-stage NSCLC treated with stereotactic body radiotherapy (SBRT). Patients with early-stage NSCLC treated with SBRT between January 2003 and March 2014 at our institute were enrolled, and marital status at the time of SBRT was investigated. Propensity score matching (PSM) was applied to reduce potential selection bias between the married and unmarried groups. Two hundred and forty patients (median age 77 years; 152 married, 87 unmarried) were analyzed. The unmarried included higher proportions of the elderly, women, never smokers, and those with decreased pulmonary function compared to the married. PSM identified 53 matched pairs of married and unmarried patients, with no significant difference in patient background parameters. The 5-year overall survival (OS) was 52.8% and 46.9% in the married and unmarried groups, respectively (P = 0.26). There was no significant difference in NSCLC death or non-NSCLC death between the two groups (P = 0.88 and 0.30, respectively). There was no significant difference in OS between married and unmarried male patients (n = 85, 5-year OS, 52.6% vs. 46.0%; P = 0.42) and between married and unmarried female patients (n = 21, 54.5% vs. 50.0%; P = 0.44). In conclusion, marital status was not associated with OS in patients receiving SBRT for early-stage NSCLC

Advances in methodologies based on chromatin conformation capture (3C) have shed light on the genome-wide organization of chromatin in developmental processes. Here, we review recent discoveries regarding the regulation of three-dimensional (3D) chromatin conformation, including promoter–enhancer looping, and the dynamics of large chromatin domains such as topologically associated domains (TADs) and A/B compartments. We conclude with perspectives on how these conformational changes govern neural development and may go awry in disease states

    Abundance and diversity of predominant sulfate-reducing bacteria in the gut of pufferfish

    Sulfate-reducing bacteria (SRB) exist in anaerobic environments, such as marine sediments, and produce hydrogen sulfide, which is toxic to marine animals. However, little is known about the ecology of SRB in the gut of fish. In the present study, we used molecular techniques to analyze the predominant SRB community in the gut of pufferfish inhabiting coastal areas of Japan. The density of the dissimilatory sulfite reductase alpha gene, dsrA, derived from SRB and total count of bacteria in guts of pufferfish was 4.4í—106 - 1.8í—107 copies/g and 3.5í—108 - 6.3í—109 cells/g, respectively, in all specimens. Clones of dsrA associated with Desulfobulbus oligotrophicus, a dominant SRB species, were detected in all 12 libraries, accounting for 57.7-94.3% of clones in each library. These results strongly suggest that SRB are indigenous bacteria in the gut of pufferfish and that hydrogen sulfide produced by SRB may be a risk factor for fish health

    Dynamic effects of the Nuss procedure on the spine in asymmetric pectus excavatum

    ObjectiveThis study aimed to elucidate dynamic effects of the Nuss procedure on the spine in the treatment of patients with pectus excavatum with asymmetric thoraces.MethodsTwenty-five patients with pectus excavatum who underwent the Nuss procedure were categorized into 4 groups by preoperative morphology of the spine and thoracic asymmetry. In group 1 (n = 8), the right side of the thorax was concave and the spine bowed to the right. In group 2 (n = 4), the right side of the thorax was concave and the spine bowed to the left. In group 3 (n = 5), the left side of the thorax was concave and the spine bowed to the right. In group 4 (n = 8), the left side of the thorax was concave and the spine bowed to the left. With computed tomographic data, finite-element models were produced to simulate each patient's thorax. Thereafter, dynamic response patterns of the spine to the Nuss procedure were examined. Validity of these biomechanical findings was verified by referring to clinical outcomes.ResultsIn group 1 and group 4 models, deformed spines were straightened; in group 2 and group 3 models, spinal bowing increased. These biomechanical findings were compatible with clinical evaluations.ConclusionsPerformance of the Nuss procedure for asymmetric pectus excavatum exerts dynamic influence on the spine. Response patterns of the spine are predictable from morphologic relationships between the asymmetric patterns of the anterior thoracic wall and the spine

    Clinical evaluation of presepsin considering renal function

    Presepsin, a glycoprotein produced during bacterial phagocytosis, is used as a sepsis marker for bacterial infections. However, presepsin levels are affected by renal function, and the evaluation criteria according to kidney function or in chronic kidney diseases remain controversial. Furthermore, presepsin may be increased by sample stirring, but no studies have evaluated this effect.In this study, we excluded the effect of stirring by standardizing the blood collection conditions, analyzed the influence of kidney function on presepsin concentrations, and recalculated the reference range based on the findings. EDTA-whole blood from 47 healthy subjects and 85 patients with chronic kidney disease was collected to measure presepsin by PATHFAST. Presepsin was found to be significantly correlated with the levels of creatinine (r = 0.834), eGFRcreat (r = 0.837), cystatin-C (r = 0.845), and eGFRcys (r = 0.879). Furthermore, in patients with CKD, presepsin levels stratified by eGFRcys showed a significant increase in the CKD G2 patient group and with advancing glomerular filtration rate stage. The following values were obtained: Normal: 97.6 ± 27.4 pg/mL, CKD G1: 100.2 ± 27.6 pg/mL, CKD G2: 129.7 ± 40.7 pg/mL, CKD G3: 208.1 ± 70.2 pg/mL, CKD G4: 320.2 ± 170.1 pg/mL, CKD G5: 712.8 ± 336.3 pg/mL. The reference range, calculated by a nonparametric method using 67 cases of healthy volunteers and patients with chronic kidney disease G1, was found to be 59–153 pg/mL, which was notably lower than the standard reference range currently used. Presepsin concentrations were positively correlated with a few biomarkers of renal function, indicating the necessity to consider the effect of renal function in patients with renal impairment. Using the recalculated reference range considering kidney function may improve the accuracy of evaluating presepsin for diagnosis of sepsis compared to the standard reference currently in use

    Extending the Operating Distance of Inductive Proximity Sensor Using Magnetoplated Wire

    Inductive proximity sensors are noncontact sensing devices used to detect the approach of a target by an increase in coil resistance due to eddy current loss. Extending the operating distance of these sensors is demanded. In this paper, we propose the use of a magnetoplated wire (MPW) as a sensing coil. The MPW is a copper wire, whose circumference is plated with a magnetic thin film. We analyze the impedance of a proximity sensor using a copper wire (COW) and MPW coils by a finite element method. The use of the MPW results in a decrease in AC resistance due to the proximity effect, an increase in inductance, and the generation of a higher flux than when the COW is used. Therefore, it is possible to increase the quality factor Q of the MPW coil. As a result, the operating distances of the MPW and COW coils are 5.0 and 3.8 mm, respectively.     Development and validation of a prognostic model for non-lung cancer death in elderly patients treated with stereotactic body radiotherapy for non-small cell lung cancer

    This study sought to develop and validate a prognostic model for non-lung cancer death (NLCD) in elderly patients with non-small cell lung cancer (NSCLC) treated with stereotactic body radiotherapy (SBRT). Patients aged ≥65 diagnosed with NSCLC (Tis-4N0M0), tumor diameter ≤5 cm and SBRT between 1998 and 2015 were retrospectively registered from two independent institutions. One institution was used for model development (arm D, 353 patients) and the other for validation (arm V, 401 patients). To identify risk factors for NLCD, multiple regression analysis on age, sex, performance status (PS), body mass index (BMI), Charlson comorbidity index (CCI), tumor diameter, histology and T-stage was performed on arm D. A score calculated using the regression coefficient was assigned to each factor and three risk groups were defined based on total score. Scores of 1.0 (BMI ≤18.4), 1.5 (age ≥ 5), 1.5 (PS ≥2), 2.5 (CCI 1 or 2) and 3 (CCI ≥3) were assigned, and risk groups were designated as low (total ≤ 3), intermediate (3.5 or 4) and high (≥4.5). The cumulative incidences of NLCD at 5 years in the low, intermediate and high-risk groups were 6.8, 23 and 40% in arm D, and 23, 19 and 44% in arm V, respectively. The AUC index at 5 years was 0.705 (arm D) and 0.632 (arm V). The proposed scoring system showed usefulness in predicting a high risk of NLCD in elderly patients treated with SBRT for NSCLC

    OUT-OF-CORE CONE BEAM RECONSTRUCTION USING MULTIPLE GPUS

    ABSTRACT This paper presents a graphics processing unit (GPU) based method capable of accelerating cone-beam reconstruction of large volume data, which cannot be entirely stored in video memory. Our method accelerates the Feldkamp, Davis and Kress (FDK) algorithm in a multi-GPU environment. We present how the entire volume can be efficiently decomposed into small portions to reduce the usage of video memory on each graphics card. Experimental results are also presented to understand the reconstruction throughput on an nVIDIA Tesla S1070 server. It takes approximately three minutes to reconstruct a 2048 3 -voxel volume from 720 2048 2 -pixel projections. The effective bandwidth of video memory reaches 137 GB/s per GPU, demonstrating a higher utilization of texture caches

    Bilateral Approach for Thoracoscopic Esophagectomy in a Patient with Esophageal Cancer and Solitary Posterior Thoracic Para-aortic Lymph Node Metastasis

    We report a successful dissection of metastatic posterior thoracic para-aortic lymph node (No. 112aoP) via bilateral thoracoscopic surgery. With the anesthetized patient (a 73-year-old Japanese woman) in the prone position, two working ports were inserted for the left-side approach, and artificial pneumothorax was created. Thoracoscopic examination revealed a swollen LN posterior to the descending aorta. Fat and metastatic LNs posterior to the aorta were dissected from the aortic arch level to the diaphragm while preserving intercostal arteries. For the right-side approach, two working ports were inserted and a routine thoracoscopic esophagec-tomy was performed. Gastric conduit reconstruction was achieved laparoscopically. Operation time for the left thoracic procedure: 54 min; estimated blood loss: almost none. No recurrence was detected 24 months post-operatively. There are several surgical options for approaching No. 112aoP, including transhiatal, left thora-cotomy, and thoracoscopy. Although a wide dissection of the posterior thoracic para-aortic area has not been reported, it may be feasible and safe if the artery of Adamkiewicz and intercostal arteries are preserved. A min-imally invasive bilateral thoracoscopic approach for a thoracoscopic esophagectomy is safe and useful for esophageal cancer patients with solitary No. 112aoP metastasis
