De novo SCN1A géndeletio terápiarezisztens Dravet-szindrómában
A Dravet-szindróma vagy régebbi nevén súlyos csecsemőkori myoclonusos epilepszia igen ritka formája az epilepsziának. Az autoszomális domináns öröklésmenetet mutató kórkép leggyakoribb oka a feszültségfüggő nátriumcsatorna alfa-1 alegységet kódoló SCN1A gén mutációja. Az esetek többségének hátterében de novo pontmutáció áll, azonban néhány betegben a gén kópiaszám-változása figyelhető meg. Az SCN1A gén által kódolt fehérje a neuronalis ingerület kiváltásában és vezetésében játszik szerepet. A génmutációk többsége következtében kialakuló funkcióvesztés a neuronalis hálózat fokozott ingerelhetőségéhez vezet, ami az epilepsziás görcsök kialakulását eredményezi. A szerzők intézetében 2013 óta elérhető az SCN1A gén kópiaszám-változásának (deletio/duplikáció) vizsgálata is a gén teljes szekvenálása mellett. Jelen közleményben egy 7 éves beteg esetét mutatják be, aki 2 éves betegút után került intézetükbe. Súlyos epilepsziás encephalopathia hátterében azonosított MECP2-gén-mutáció fiúbetegben = MECP2 mutation in a male patient identified in the background of severe epileptic encephalopathy
Absztrakt:
A szerzők egy negatív családi anamnézisű, súlyos, neonatalis kezdetű epilepsziás
encephalopathiában szenvedő, jelenleg kétéves fiúgyermek esetét mutatják be. A
terápiarezisztens epilepszia és az igen súlyos fejlődésbeli elmaradás
etiológiáját kiterjedt klinikai vizsgálatokkal sem sikerült tisztázni. Genetikai
okot feltételezve, külföldi genetikai laboratóriumban 128 gént tartalmazó
újgenerációs szekvenálási (NGS-) panelvizsgálatot indikáltak epilepsziás
encephalopathiát okozó betegségek irányában. A vizsgálat egy eddig ismeretlen
hemizigóta misszensz mutációt igazolt a MECP2-génben. A szerzők
az esetbemutatás kapcsán áttekintik a lányokban klasszikusan Rett-szindrómát
okozó, a MECP2-gén mutációi által előidézett idegfejlődési
rendellenességek spektrumát fiúkban. Más, X-hez kötött domináns öröklődésű
betegségekhez hasonlóan sokáig úgy gondolták, hogy a
MECP2-gén-mutációt hordozó fiúmagzatok életképtelenek,
napjainkra azonban ez a nézet megváltozott. A szerzők úgy tudják, hogy betegük
az első magyar fiúgyermek, akinél a MECP2-gén mutációja

Abstract:
Here we report on a severe, neonatal onset epileptic encephalopathy manifested in
a currently 2-year-old boy with no family history of neurological disease.
Extensive clinical investigations were unable to clarify the etiology of the
infant’s condition characterized by drug-resistant seizures and markedly delayed
developmental skills. As in this class of disorders a genetic cause might be
identified, a next-generation sequencing (NGS) epilepsy panel examination
consisting of 128 genes was initiated for a correct diagnosis. The genetic
analysis identified a previously undescribed hemizygous missense mutation in the
MECP2 gene. Similarly to other, X-linked dominant
disorders, Rett syndrome was originally hypothesized to be lethal in males. This
theory, however, has been revised. The aim of this report is to review the wide
spectrum of neurodevelopmental diseases observed in male patients carrying
mutations in the MECP2 gene classically associated with Rett
syndrome in girls. To the author’s knowledge, this is the first report in
Hungary to document MECP2 mutation of a male patient diagnosed

A nagyobb méretű géndeletiók jelentősége a sclerosis tuberosa diagnosztikájában: az első magyar esetek bemutatása | Importance of gross deletions in the diagnosis of tuberous sclerosis complex: the first Hungarian cases

A sclerosis tuberosa nagyfokú fenotípusos heterogenitást mutató ritka betegség,
jellemző tünete a multiplex hamartomák megjelenése különböző szervekben.
Autoszomális domináns öröklődésmenetet mutat, hátterében két génnek, a
TSC1-nek, illetve a TSC2-nek a mutációi
állnak. Jelen közleményünkben két fiú, illetve két középkorú nőbeteg esetét
mutatjuk be, akiknél a TSC1/TSC2 génben
Sanger-szekvenálással kóroki eltérést nem tudtunk igazolni, azonban a multiplex
ligatiofüggő próba amplifikációjával különböző méretű deletiókat igazoltunk a
TSC2 különböző régióiban. Valamennyi beteg hordozza a
betegség jellegzetes vezető klinikai tüneteit, azonban az egyéni fenotípusos
variabilitásuk rendkívüli módon különbözik. Közleményünkkel a nagyobb méretű
géndeletiók relatív gyakori előfordulására hívjuk fel a figyelmet. Orv Hetil.

Abstract:
Tuberous sclerosis complex is a rare disease with high phenotypic heterogeneity,
characterized by the appearance of multiplex hamartomas in the different organs.
The disease is inherited by autosomal dominant manner, due to the mutations of
two genes: the TSC1 or the TSC2. In this
publication we present the cases of two young male and two middle-aged female
patients, where pathogenetic differences of TSC1/TSC2 could not
be verified by Sanger sequencing. However, multiplex ligation-dependent probe
amplification confirmed different sizes of deletions in different regions of the
TSC2 gene. All patients carry the typical clinical signs of
the disease. However, the individual phenotypic variability is very different.
With this manuscript, we would like to draw attention to the relative frequent

Deletion of 4q28.3-31.23 in the background of multiple malformations with pulmonary hypertension
The 4q deletion syndrome shows a broad spectrum of clinical manifestations consisting of key features comprising
growth failure, developmental delay, craniofacial dysmorphism, digital anomalies, and cardiac and skeletal defects.
We have identified a de novo interstitial distal deletion in a 9 month-old girl with growth failure, developmental
delay, ventricular septum defect in the subaortic region, patent foramen ovale and patent ductus arteriosus, vascular
malformation of the lung, dysgenesis of the corpus callosum and craniofacial dysmorphism using array-comparative
genomic hybridization. This de novo deletion is located at 4q28.3-31.23 (136,127,048 - 150,690,325), its size is 14.56 Mb,and contains 8 relevant genes (PCDH18, SETD7, ELMOD2, IL15, GAB1, HHIP, SMAD1, NR3C2) with possible contributions
to the phenotype. Among other functions, a role in lung morphogenesis and tubulogenesis can be attributed to the
deleted genes in our patient, which may explain the unique feature of vascular malformation of the lung leading to
pulmonary hypertension. With the detailed molecular characterization of our case with 4q- syndrome we hope to
contribute to the elucidation of the genetic spectrum of this disorder
Generalizált epilepszia hátterében azonosított ioncsatorna-génmutáció ritka formája = A rare form of ion channel gene mutation identified as underlying cause of generalized epilepsy

A molekuláris genetikai technológiák fejlődése következtében egyre több, korábban
idiopátiásnak tartott betegség hátterében ismerjük meg a genetikai eltérést. A
generalizált epilepsziában szenvedő, a betegség lefolyása során
epilepsziaszindrómát váltó, jó intellektusú, kiterjedt hipopigmentált folttal
rendelkező fiúgyermek célzott genetikai vizsgálata neurocutan szindróma irányába
nem hozott eredményt. Teljesexom-szekvenálás során egy
kálium-klorid-kotranszporter génjének heterozigóta misszensz mutációja
igazolódott, ami a fenotípussal összevetve, az irodalomban az idiopátiás
generalizált epilepszia 14-es típusaként ismert epilepsziaszindróma diagnózisát

Abstract:
The advances in molecular genetic methods has lead to the discovery of the
genetic alterations that underlie the etiology of most diseases previously held
to be idiopathic. Targeted genetic examination of a pediatric male patient
showing a normal intellect, an extended area of skin hypopigmentation, and
suffering from generalized epilepsy displaying a switch in epilepsy syndrome
during the course of the disease towards a neurocutaneous syndrome was
unsuccessful. Whole-exome sequencing identified a heterozygous missense mutation
in a potassium chloride cotransporter gene, which together with the phenotype
underscores the diagnosis of an epilepsy syndrome known in the literature as

Génvizsgálat Treacher Collins-szindrómában
A Treacher Collins-szindróma a mandibulofacialis dysostosisok csoportjába tartozó kórkép. Főbb jellegzetességei a maxillaris és mandibularis dysostosis, az antimongoloid szemrések, az alsó szemhéj colobomája, illetve a vezetéses halláscsökkenés. A szindrómával járó tünetek egyénenként és családon belül is nagyon eltérőek lehetnek; legenyhébb formái csaknem észrevehetetlenek, míg a súlyosabb esetekben az életet veszélyeztető légúti szövődmények léphetnek fel. Hátterében az esetek döntő többségében a TCOF1-gén eltérései játszanak szerepet, mely eltérések autoszomális domináns módon öröklődnek. Esetbemutatásunk célja, hogy felhívjuk a figyelmet a genetikai vizsgálat elvégzésének fontosságára olyan, klinikailag jól felismerhető tünetegyüttes, mint a Treacher Collins-szindróma esetén. Bár a betegség a klinikai kép alapján diagnosztizálható, az ismétlődés kockázatát csak úgy tudjuk pontosan meghatározni, ha ismerjük a családtagok genotípusát. A bemutatott család több tagjánál kimutatható volt a TCOF1-gén mutációja, annak ellenére, hogy klinikai tünetük nem volt. A jelenség magyarázata az inkomplett penetrancia, azaz a hibás gén fenotípusosan nem kerül kifejeződésre
Definition and clinical variability of SHANK3-related Phelan-McDermid syndrome
Phelan-McDermid syndrome (PMS) is an infrequently described syndrome that presents with a disturbed development, neurological and psychiatric characteristics, and sometimes other comorbidities. As part of the development of European medical guidelines we studied the definition, phenotype, genotype-phenotype characteristics, and natural history of the syndrome. The number of confirmed diagnoses of PMS in different European countries was also assessed and it could be concluded that PMS is underdiagnosed. The incidence of PMS in European countries is estimated to be at least 1 in 30,000. Next generation sequencing, including analysis of copy number variations, as first tier in diagnostics of individuals with intellectual disability will likely yield a larger number of individuals with PMS than presently known. A definition of PMS by its phenotype is at the present not possible, and therefore PMS-SHANK3 related is defined by the presence of SHANK3 haploinsufficiency, either by a deletion involving region 22q13.2–33 or a pathogenic/likely pathogenic variant in SHANK3. In summarizing the phenotype, we subdivided it into that of individuals with a 22q13 deletion and that of those with a pathogenic/likely pathogenic SHANK3 variant. The phenotype of individuals with PMS is variable, depending in part on the deletion size or whether only a variant of SHANK3 is present. The core phenotype in the domains development, neurology, and senses are similar in those with deletions and SHANK3 variants, but individuals with a SHANK3 variant more often are reported to have behavioural disorders and less often urogenital malformations and lymphedema. The behavioural disorders may, however, be a less outstanding feature in individuals with deletions accompanied by more severe intellectual disability. Data available on the natural history are limited. Results of clinical trials using IGF-1, intranasal insulin, and oxytocin are available, other trials are in progress. The present guidelines for PMS aim at offering tools to caregivers and families to provide optimal care to individuals with PMS.</p
