    The Potential Trajectory of Carbapenem-Resistant Enterobacteriaceae, an Emerging Threat to Health-Care Facilities, and the Impact of the Centers for Disease Control and Prevention Toolkit.

    Carbapenem-resistant Enterobacteriaceae (CRE), a group of pathogens resistant to most antibiotics and associated with high mortality, are a rising emerging public health threat. Current approaches to infection control and prevention have not been adequate to prevent spread. An important but unproven approach is to have hospitals in a region coordinate surveillance and infection control measures. Using our Regional Healthcare Ecosystem Analyst (RHEA) simulation model and detailed Orange County, California, patient-level data on adult inpatient hospital and nursing home admissions (2011-2012), we simulated the spread of CRE throughout Orange County health-care facilities under 3 scenarios: no specific control measures, facility-level infection control efforts (uncoordinated control measures), and a coordinated regional effort. Aggressive uncoordinated and coordinated approaches were highly similar, averting 2,976 and 2,789 CRE transmission events, respectively (72.2% and 77.0% of transmission events), by year 5. With moderate control measures, coordinated regional control resulted in 21.3% more averted cases (n = 408) than did uncoordinated control at year 5. Our model suggests that without increased infection control approaches, CRE would become endemic in nearly all Orange County health-care facilities within 10 years. While implementing the interventions in the Centers for Disease Control and Prevention's CRE toolkit would not completely stop the spread of CRE, it would cut its spread substantially, by half

    Successful strategies for high participation in three regional healthcare surveys: an observational study

    <p>Abstract</p> <p>Background</p> <p>Regional healthcare facility surveys to quantitatively assess nosocomial infection rates are important for confirming standardized data collection and assessing health outcomes in the era of mandatory reporting. This is particularly important for the assessment of infection control policies and healthcare associated infection rates among hospitals. However, the success of such surveys depends upon high participation and representativeness of respondents.</p> <p>Methods</p> <p>This descriptive paper provides methodologies that may have contributed to high participation in a series of administrative, infection control, and microbiology laboratory surveys of all 31 hospitals in a large southern California county. We also report 85% (N = 72) countywide participation in an administrative survey among nursing homes in this same area.</p> <p>Results</p> <p>Using in-person recruitment, 48% of hospitals and nursing homes were recruited within one quarter, with 75% recruited within three quarters.</p> <p>Conclusions</p> <p>Potentially useful strategies for successful recruitment included in-person recruitment, partnership with the local public health department, assurance of anonymity when presenting survey results, and provision of staff labor for the completion of detailed survey tables on the rates of healthcare associated pathogens. Data collection assistance was provided for three-fourths of surveys. High compliance quantitative regional surveys require substantial recruitment time and study staff support for high participation.</p

    Meeting Forest Restoration Challenges: Using the Target Plant Concept

    Meeting forest restoration challenges relies on successful establishment of plant materials (e.g., seeds, cuttings, rooted cuttings, or seedlings, etc.; hereafter simply “seedlingsâ€). The Target Plant Concept (TPC) provides a flexible framework that nursery managers and their clients can use to improve the survival and growth of these seedlings. The key tenets of the TPC are that (1) more emphasis is placed on how seedlings perform on the outplanting site rather than on nursery performance, (2) a partnership exists between the nursery manager and the client to determine the target plant based on site characteristics, and (3) that information gleaned from post-planting monitoring is used to improve subsequent plant materials. Through the nursery manager–client partnership, answers to a matrix of interrelated questions define a target plant to meet the reforestation or forest restoration objectives. These questions focus on project objectives; site characteristics, limiting factors, and possible mitigation efforts; species and genetic criteria; stocktype; outplanting tools and techniques; and outplanting window. We provide examples from the southeastern United States, Hawai‛i, and Lebanon on how the TPC process has improved performance of seedlings deployed for reforestation and forest restoration

    Long-Term Care Facilities: Important Participants of the Acute Care Facility Social Network?

    Background: Acute care facilities are connected via patient sharing, forming a network. However, patient sharing extends beyond this immediate network to include sharing with long-term care facilities. The extent of long-term care facility patient sharing on the acute care facility network is unknown. The objective of this study was to characterize and determine the extent and pattern of patient transfers to, from, and between long-term care facilities on the network of acute care facilities in a large metropolitan county. Methods/Principal Findings: We applied social network constructs principles, measures, and frameworks to all 2007 annual adult and pediatric patient transfers among the healthcare facilities in Orange County, California, using data from surveys and several datasets. We evaluated general network and centrality measures as well as individual ego measures and further constructed sociograms. Our results show that over the course of a year, 66 of 72 long-term care facilities directly sent and 67 directly received patients from other long-term care facilities. Long-term care facilities added 1,524 ties between the acute care facilities when ties represented at least one patient transfer. Geodesic distance did not closely correlate with the geographic distance among facilities. Conclusions/Significance: This study demonstrates the extent to which long-term care facilities are connected to the acute care facility patient sharing network. Many long-term care facilities were connected by patient transfers and further added many connections to the acute care facility network. This suggests that policy-makers and health officials should account for patient sharing with and among long-term care facilities as well as those among acute care facilities when evaluating policies and interventions. © 2011 Lee et al

    Comparison of web-based information about cell-free DNA prenatal screening: implications for differences of sex development care

    ObjectiveCell-free DNA (cfDNA) prenatal screening is a commercially available noninvasive test that detects fetal genetic material in maternal blood. While expectant parents often use it for “gender” determination, there is little information about unintended consequences of testing, such as revelation of a difference of sex development (DSD). The study aimed to characterize currently available website information about cfDNA and compare the cfDNA-related content.MethodsA systematic search for websites with information about cfDNA was conducted using search terms generated by a natural language processing analysis of the results of an Amazon Mechanical Turk (MTurk) survey of 1,000 parents and then performing a “Google” search, using the terms. Commercial cfDNA testing companies (CC) websites were also identified by consulting a genetic counselor (AGW). Data were collected on about each website’s characteristics and information about cfDNA. Information about cfDNA was compared between websites. Data were analyzed using descriptive statistics, Fisher’s exact test or Kruskal-Wallis test were applied, as appropriate.ResultsSixty websites were identified. After eliminating duplicates, 11 commercial company (CC) websites were identified. Nineteen other websites were reviewed of which six overlapped with five CC websites. Most of the websites had non-professional authors (73.7%), such as laypersons and CC representatives. CC websites were significantly more likely than search term-identified websites to state that cfDNA can screen for trisomy 21 (p=0.002), trisomy 18 (p&lt;0.0001), trisomy 13 (p&lt;0.001), sex chromosome aneuploidies (p&lt;0.001), and microdeletions (p=0.002).ConclusionsThis study shows that most website currently available information for expectant parents about cfDNA prenatal screening is produced by non-professional organizations. There are significant differences between the information provided by CC and Google search websites, specifically about the number of conditions screened for by cfDNA. Improving availability and quality of information about cfDNA could improve counseling future expectant parents. Inclusion of information about the potential for detection of a DSD is needed

    Simulated Annealing-Based Hyperspectral Data Optimization for Fish Species Classification: Can the Number of Measured Wavelengths Be Reduced?

    Relative to standard red/green/blue (RGB) imaging systems, hyperspectral imaging systems offer superior capabilities but tend to be expensive and complex, requiring either a mechanically complex push-broom line scanning method, a tunable filter, or a large set of light emitting diodes (LEDs) to collect images in multiple wavelengths. This paper proposes a new methodology to support the design of a hypothesized system that uses three imaging modes—fluorescence, visible/near-infrared (VNIR) reflectance, and shortwave infrared (SWIR) reflectance—to capture narrow-band spectral data at only three to seven narrow wavelengths. Simulated annealing is applied to identify the optimal wavelengths for sparse spectral measurement with a cost function based on the accuracy provided by a weighted k-nearest neighbors (WKNN) classifier, a common and relatively robust machine learning classifier. Two separate classification approaches are presented, the first using a multi-layer perceptron (MLP) artificial neural network trained on sparse data from the three individual spectra and the second using a fusion of the data from all three spectra. The results are compared with those from four alternative classifiers based on common machine learning algorithms. To validate the proposed methodology, reflectance and fluorescence spectra in these three spectroscopic modes were collected from fish fillets and used to classify the fillets by species. Accuracies determined from the two classification approaches are compared with benchmark values derived by training the classifiers with the full resolution spectral data. The results of the single-layer classification study show accuracies ranging from ~68% for SWIR reflectance to ~90% for fluorescence with just seven wavelengths. The results of the fusion classification study show accuracies of about 95% with seven wavelengths and more than 90% even with just three wavelengths. Reducing the number of required wavelengths facilitates the creation of rapid and cost-effective spectral imaging systems that can be used for widespread analysis in food monitoring/food fraud, agricultural, and biomedical applications

    Statistical Characterization of the Chandra Source Catalog

    The first release of the Chandra Source Catalog (CSC) contains ~95,000 X-ray sources in a total area of ~0.75% of the entire sky, using data from ~3,900 separate ACIS observations of a multitude of different types of X-ray sources. In order to maximize the scientific benefit of such a large, heterogeneous data-set, careful characterization of the statistical properties of the catalog, i.e., completeness, sensitivity, false source rate, and accuracy of source properties, is required. Characterization efforts of other, large Chandra catalogs, such as the ChaMP Point Source Catalog (Kim et al. 2007) or the 2 Mega-second Deep Field Surveys (Alexander et al. 2003), while informative, cannot serve this purpose, since the CSC analysis procedures are significantly different and the range of allowable data is much less restrictive. We describe here the characterization process for the CSC. This process includes both a comparison of real CSC results with those of other, deeper Chandra catalogs of the same targets and extensive simulations of blank-sky and point source populations.Comment: To be published in the Astrophysical Journal Supplement Series (Fig. 52 replaced with a version which astro-ph can convert to PDF without issues.

    Reversing Blood Flows Act through klf2a to Ensure Normal Valvulogenesis in the Developing Heart

    Heart valve anomalies are some of the most common congenital heart defects, yet neither the genetic nor the epigenetic forces guiding heart valve development are well understood. When functioning normally, mature heart valves prevent intracardiac retrograde blood flow; before valves develop, there is considerable regurgitation, resulting in reversing (or oscillatory) flows between the atrium and ventricle. As reversing flows are particularly strong stimuli to endothelial cells in culture, an attractive hypothesis is that heart valves form as a developmental response to retrograde blood flows through the maturing heart. Here, we exploit the relationship between oscillatory flow and heart rate to manipulate the amount of retrograde flow in the atrioventricular (AV) canal before and during valvulogenesis, and find that this leads to arrested valve growth. Using this manipulation, we determined that klf2a is normally expressed in the valve precursors in response to reversing flows, and is dramatically reduced by treatments that decrease such flows. Experimentally knocking down the expression of this shear-responsive gene with morpholine antisense oligonucleotides (MOs) results in dysfunctional valves. Thus, klf2a expression appears to be necessary for normal valve formation. This, together with its dependence on intracardiac hemodynamic forces, makes klf2a expression an early and reliable indicator of proper valve development. Together, these results demonstrate a critical role for reversing flows during valvulogenesis and show how relatively subtle perturbations of normal hemodynamic patterns can lead to both major alterations in gene expression and severe valve dysgenesis
