Randomized controlled trial of single incision versus conventional multiport laparoscopic cholecystectomy with long-term follow-up
Background: Within the last years, single-incision laparoscopic cholecystectomy (SLC) emerged as an alternative to multiport laparoscopic cholecystectomy (MLC). SLC has advantages in cosmetic results, and postoperative pain seems lower. Overall complications are comparable between SLC and MLC. However, long-term results of randomized trials are lacking, notably to answer questions about incisional hernia rates, long-term cosmetic impact and chronic pain.
Methods: A randomized trial of SLC versus MLC with a total of 193 patients between December 2009 and June 2011 was performed. The primary endpoint was postoperative pain on the first day after surgery. Secondary endpoints were conversion rate, operative time, intraoperative and postoperative morbidity, technical feasibility and hospital stay. A long-term follow-up after surgery was added.
Results: Ninety-eight patients (50.8%) underwent SLC, and 95 patients (49.2%) had MLC. Pain on the first postoperative day showed no difference between the operative procedures (SLC vs. MLC, 3.4 ± 1.8 vs. 3.7 ± 1.9, respectively; p = 0.317). No significant differences were observed in operating time or the overall rate of postoperative complications (4.1% vs. 3.2%; p = 0.731). SLC exhibited better cosmetic results in the short term. In the long term, after a mean of 70.4 months, there were no differences in incisional hernia rate, cosmetic results or pain at the incision between the two groups.
Conclusions: Taking into account a follow-up rate of 68%, the early postoperative advantages of SLC in relation to cosmetic appearance and pain did not persist in the long term. In the present trial, there was no difference in incisional hernia rates between SLC and MLC, but the sample size is too small for a final conclusion regarding hernia rates.

Strukturierte Versorgung von Patient*innen mit atraumatischen Bauchschmerzen in der Notaufnahme
Background: Patients with atraumatic abdominal pain are common in the emergency department and have a relatively high hospital mortality, with a very wide spectrum of different causes. Rapid, goal-directed diagnosis is essential in this context.
Methods: In a Delphi process with representatives of different disciplines, a diagnostic treatment pathway was designed, which is called the Abdominal Pain Unit (APU).
Results: The treatment pathway was designed as an extended event process chain. Crucial decision points were specified using standard operating procedures.
Discussion: The APU treatment pathway establishes a consistent treatment structure for patients with atraumatic abdominal pain. It has the potential to improve the quality of care and reduce intrahospital mortality over the long term
Manipulation of growth and metastasis of pancreatic cancer - experimental research
Das Pankreaskarzinom ist ein häufiges Malignom des Gastrointestinaltraktes und
trotz deutlicher Erkenntnisfortschritte hinsichtlich seiner Biologie noch
immer mit einer ausgesprochen schlechten Prognose behaftet. Nur durch
R0-Resektion lässt sich eine mittlere Überlebenszeit von 20-22 Monaten
erreichen, in der palliativen Situation ĂĽberleben die Patienten meist nur 8-10
Monate. FĂĽr Untersuchungen zu Wachstum und Metastasierung des
Pankreaskarzinoms sind zelluläre und tierexperimentelle Versuchsanordnungen
unverzichtbar. Dabei weisen karzinogen-induzierte Tiermodelle den Vorteil auf,
ĂĽber einen definierten Zeitraum multiple Ereignisse auf mehreren biologischen
Ebenen hervorzurufen und damit wie im Zeitraffer die Karzinogenese und
idealerweise auch die Metastasierung nachzubilden. Das
N-Nitrosobis-2-oxopropylamin-(BOP)-Modell des duktalen Adenokarzinoms im
Syrischen Hamster weist in vielerlei Hinsicht eine anerkannt gute
Vergleichbarkeit mit dem häufigsten Typ des Pankreaskarzinoms im Menschen, dem
duktalen Adenokarzinom, auf. Durch Modifikation der Ernährung gelang uns die
Steigerung der im Ausgangsmodell bei maximal 30-60% liegenden
Lebermetastasierungsrate auf konstant mindestens 90%. Dabei wurde der
Fettanteil der Nahrung auf mehr als 20% angehoben, wobei Linolsäure der
Hauptbestandteil war. Oxidativer Stress und die Lipidperoxidation sind
Mechanismen, ĂĽber die sowohl die Nitrosamin-induzierte Karzinogenese als auch
die Hochfetternährung ihre Wirkung entfalten, was durch Veränderungen in den
Aktivitäten der antioxidativen Schutzenzyme GSH-Px und SOD und der TBARS-
Konzentration als MaĂź fĂĽr die Lipidperoxidation nachgewiesen werden konnte.
Für die konjugierte Form der Linolsäure waren in der Literatur im Gegensatz
zur konventionellen Isoform eher wachstumshemmende Effekte auf Neoplasien
beschrieben. Wir untersuchten daher den Einfluss beider Isoformen auf die
Pankreaskarzinominzidenz und die intrapankreatische sowie intratumorale
Lipidperoxidation. Dabei ließ sich weder hinsichtlich der Primärtumore noch
der Lebermetastasen ein signifikanter Unterschied nachweisen. Bei Patienten
mit einem Pankreaskarzinom können in diagnostischer, kurativer und palliativer
Absicht operative Eingriffe erforderlich werden. Die dabei regelhaft
erfolgende Spülung des Bauchraumes könnte je nach verwendeter Substanz
Auswirkungen auf das Tumorwachstum und die Metastasierung haben. Wir
simulierten daher nach BOP-Induktion eines Pankreaskarzinoms im Hamster eine
diagnostische Laparoskopie, in deren Rahmen wir abschlieĂźend verschiedene
Spüllösungen anwandten. Neben dem Standard mit 0,9%iger NaCl-Lösung wurde
Taurolidin verwendet, das in tierexperimentellen Studien wachstumshemmende
Effekte auf verschiedene Tumore gezeigt hatte. Zudem setzten wir das
Somatostatinanalogon Octreotid ein, für das wir eine Beeinflussung von Größe
und Anzahl der Lebermetastasen bei längerer subkutaner Applikation im BOP-
Modell nachgewiesen hatten. In dieser Laparoskopie-Studie konnte nur
Taurolidin eine signifikante Verringerung der Inzidenz makroskopischer
Pankreaskarzinome mit begleitender Verringerung der Lipidperoxidation im
Pankreas erreichen, Octreotid blieb wirkungslos. Die Lebermetastasierung wurde
in diesem Versuchsaufbau von keiner der beiden Substanzen beeinflusst.
Octreotid bindet nur an zwei der fĂĽnf bekannten Somatostatinrezeptoren. Das
neue Somatostatinanalogon SOM230 (Pasireotid) bindet hingegen mit nahezu
identischer Affinität an vier Rezeptorsubtypen. Daher haben wir in der
nächsten Studie Octreotid und SOM230 in ihren Wirkungen auf Wachstum und
Metastasierung des BOP-induzierten Pankreaskarzinoms im Hamster verglichen,
wobei wir zur kontinuierlichen und kontrolliert-dosierten Abgabe beider SST-
Analoga subkutan implantierte osmotische Mikropumpen verwendeten. Beide
Substanzen fĂĽhrten zu einer signifikanten Erniedrigung der
Lebermetastasierungsrate, erhöhten die Aktivität der antioxidativen
Schutzenzyme in der Leber und verringerten dort das AusmaĂź der
Lipidperoxidation. Matrixmetalloproteinasen werden regelhaft von
Pankreaskarzinomen exprimiert, deren Wachstum in-vitro durch MMP-Inhibitoren
gehemmt werden konnte. Zudem ist nachgewiesen, dass bestimmte Fettsäuren –
auch Linolsäure – die MMP-Produktion in Zelllinien epithelialer Tumore
steigern kann. In der nächsten Studie untersuchten wir daher den Einfluss
eines neuen MMP-Inhibitors, RO 28-2653, auf die Metastasierung des BOP-
induzierten Pankreaskarzinoms. Zudem wurde in metastasenfreiem Lebergewebe und
in den Lebermetastasen die Konzentration der MMP-2 und der MMP-9 bestimmt.
Durch beide von uns verwendete RO 28-2653-Dosierungen wurde die
Lebermetastaseninzidenz signifikant gesenkt. Desweiteren kam es zu einer
signifikanten Erniedrigung der MMP-2-Konzentration in den Lebermetastasen,
während die MMP-2-Konzentration in metastasenfreiem Lebergewebe faktisch nicht
beeinflusst wurde. Das hier verwendete Tiermodell kombiniert zwei Faktoren,
die auch bei der Karzinogenese des humanen Pankreaskarzinoms eine bedeutende
Rolle spielen: Nitrosamine und n-6-Fettsäuren in hoher Dosierung. Oxidativer
Stress und die Lipidperoxidation sind sowohl an Tumorwachstum und
Metastasierung wie auch bei der pharmakologischen Beeinflussung des Modells
durch Taurolidin, Somatostatinanaloga und Matrixmetalloproteinase-Inhibitoren
entscheidend beteiligt. Die zukĂĽnftige klinische Forschung sollte einerseits
im Rahmen der bei Pankreaskarzinompatienten oft erforderlichen parenteralen
Ernährung auf n-6—Fettsäuren-reiche Lösungen verzichten und die Wirkungen von
n-3-Fettsäuren-angereicherten Nährlösungen weiter eruieren. Andererseits muss
die Wirkung von Taurolidin als intraoperativer Spüllösung bei Eingriffen an
Patienten mit einem Pankreaskarzinom weiter untersucht werden. SchlieĂźlich
sollte – nicht zuletzt angesichts der günstigen Nebenwirkungsprofile – in
einer klinischen Phase-2-Studie die Kombination aus Gemcitabine, SOM230 und RO
28-2653 in adjuvanter und palliativer Situation geprĂĽft werden.Pancreatic cancer is a common tumor of the gastrointestinal tract and is
associated with poor prognosis. The median survival following curative
resection is about 20-22 month, in the palliative situation patients are alive
only for 8-10 months. For studies concerning advanced stages of pancreatic
cancer, animal models are useful. The N-nitrosobis-2-oxopropylamine-induced
ductal adenocarcinoma of the pancreas in the Syrian hamster is a well-
established model, which we have modified to achieve an 90% incidence of liver
metastases. Therefore, the raw fat content of hamster diet has been elevated
up to more than 20%, with linoleic acid as the main component. Oxidative
stress as well as non-enzymatic lipid peroxidation have been detected as parts
of the underlying mechanisms, shown through activity changes of the
antioxidative enzymes gluthathion-peroxidase (GSH-Px) and superoxiddismutase
(SOD) and through enhanced concentrations of thiobarbituric acid-reactive
substances (TBARS) as lipid peroxidation marker. In the literature, tumor-
depressing effects of the conjugated isoform of linoleic acid are described.
Therefore we evaluated the influence of both isoforms - conjugated and
conventional - on tumor growth, liver metastasis and lipid peroxidation. There
was no significant difference concerning the parameters measured. Because the
substances used for abdominal lavage during operative procedures in patients
with pancreatic cancer could influence tumor growth, we simulated a
laparoscopy using different lavage substances in the animal model. Only
taurolidine, which has demonstrated tumor growth-inhibitory effects in
different tumor models, could inhibit the growth of the primary tumor. The
standard, sodium chlorid, as well as the somatostatin analogue, octreotide,
showed no influence on tumor growth. The rate of liver metastases was not
influenced at all. Octreotide is a selective somatostatin analogue, binding on
the receptor subtypes 2 and 4, whereas the new analogue, SOM230, binds on all
five receptor subtypes with nearly similar affinity. Therefore we compared
both somatostatin analogues in the animal model. Both substances diminished
the incidence of liver metastases significantly, elevated the activities of
the antioxidative enzymes in the liver and decreased the amount of lipid
peroxidation in the liver. Matrixmetalloproteinases have been shown to play a
pivotal role in the growth and metastasis of pancreatic cancer. Furhtermore,
fatty acids - for instance linoleic acid - can increase the production and
release of matrixmetalloproteinases. Therefore we examined the effect of the
novel matrixmetalloproteinase inhibitor, Ro 28-2653, on the liver metastasis
in the animal model. Both dosages used could decrease the incidence of liver
metastases as well as the concentration of the matrixmetalloproteinases 2 and
9 in the liver metastases. Future research have to clearify the role of the
substances evaluated in the animal model for patients with advanced stages of
pancreatic cancer
Entwicklung eines Designtools zur konzeptionellen Auslegung von druckaktuierten Zellen fĂĽr den Einsatz in formvariablen FlĂĽgelhinterkanten
Shape-variable structures for aviation are developed to increase the efficiency in the different flight phases. Those structures can adapt to the various environmental conditions. A concept to control these structures is based on the use of pressure-actuated cells. The aim of the current bachelor thesis is to develop a design tool that enables the conceptional design of pressure-acuated cells for the use in the trailing edge of a shape-variable wing. The main function of these actuators is the ability of deforming their structure. The first step within the bachelor thesis is the further development of cells. That is neccesarry for implantation of a concept regarding the burden of aerodynamic loadings. For a better understanding of these pressure-actuated cells the identification of the decisive design parameters follows. Additionally their impact of relevant cell deformations is defined. The tool enables the preliminary design of the descripted concept to prove the principal feasibility under previously defined marginal conditions. On the basis of the finite element method a numerical design tool is designed to shape conceptual optimized pressure-actuated cells. Finally the tool will be demonstrated with the aid of an application and its functionality will be assessed
Compression of Acetylated Beech (<i>Fagus sylvatica</i> L.) Laminated Veneer Lumber (LVL)
Acetylation with acetic anhydride is well known to improve the dimensional stability and durability of wood. Veneer is appealing for acetylation because of its thin thickness, which supports a complete and even impregnation of difficult-to-treat wood species, such as beech (Fagus sylvatica L.). Unlike resin-based veneer impregnation, acetylated veneer does not require any additional curing. As a result, veneer properties are already altered prior to bonding. The compression thickness reduction in acetylated beech veneer during the manufacturing of laminated veneer lumber (LVL) utilizing 1, 3, and 6 MPa at 150 °C for 30 min is investigated in this study. The results show that acetylated beech veneer is considerably less compressible than the references. Moreover, the density of acetylated LVL at low pressure (1 MPa) is similar to the one of references, even though the compressibility is much lower. This is due to the added acetyl groups after acetylation. The reduction in compressibility is most likely caused due to a decrease in moisture content (MC) and its accompanied mechanisms
Representative Sampling Implementation in Online VFA/TIC Monitoring for Anaerobic Digestion
This paper describes an automatic sampling system for anaerobic reactors that allows taking representative samples following the guidelines of Gy’s (1998) theory of sampling. Due to the high heterogeneity degree in a digester the sampling errors are larger than the analysis error, making representative sampling a prerequisite for successful process control. In our system, samples are automatically processed, generating a higher density of data and avoiding human error by sample manipulation. The combination of a representative sampling system with a commercial automate titration unit generates a robust online monitoring system for biogas plants. The system was successfully implemented in an operating biogas plant to control a feeding-on-demand biogas system
