    Purpose: To examine the predictive features which doctors use when prescribing antibiotics in upper respiratory tract infections (URTIs). Method: This is a cross sectional, prospective study done in a teaching university primary care centre in Kuala Lumpur from June to August 2000. Twelve primary care practitioners participated in the study. Each practitioner was asked to record clinical data and prescriptions given to twenty consecutive patients with URTIs using a structured questionnaire for each patient

    Incretin-based therapies: new treatments for type 2 diabetes in the new millennium

    The advent of ‘incretin-based therapies’ – GLP-1 agonists and dipeptidyl-peptidase-4 inhibitors – which result in improvements in glycemic control comparable to those with existing oral hypoglycemic agents, and potentially improve cardiovascular and pancreatic β-cell function, represents a major therapeutic advance in the management of type 2 diabetes. Gastrointestinal adverse effects occur commonly with GLP-1 agonists, and rarely with DPP-4 inhibitors, but are dose-dependent and usually transient. The low risk of hypoglycemia, and beneficial or neutral effects on body weight, render GLP-1 agonists and DPP-4 inhibitors suitable alternatives to insulin secretagogues and insulin in overweight and elderly patients. Incretin-based therapies also improve quality of life in patients with type 2 diabetes, and may be cost-effective in the long term

    ME450 Capstone Design and Manufacturing Experience: Fall 2015Thin film deposition is a technology of applying a very thin film of material onto a substrate surface to be coated, or onto a previously deposited coating to form layers. As a modified Atomic Layer Deposition (ALD) method, Spatial ALD greatly improves the efficiency of the process by separating the half-reactions spatially instead of through the use of purge steps in convectional ALD. This paper proposes the use of heated motorized stage as a research prototype to further study the effect of deposition conditions such as gap size, gap alignment and substrate temperature on the SALD coating results. This strategy utilized three stepper motors and high precision non-contact gap sensors to control gap alignment and gap size between the substrate and the depositor.     Respiratory dysfunction in myotonic dystrophy type 1: A systematic review

    Myotonic dystrophy type 1 (DM1) is one of the most common muscular dystrophies in adults. This review summarises the current literature regarding the natural history of respiratory dysfunction in DM1, the role of central respiratory drive and peripheral respiratory muscle involvement and its significance in respiratory function, and investigates the relationship between genetics (CTG repeat length) and respiratory dysfunction. The review included all articles that reported spirometry on 10 or more myotonic dystrophy patients. The final review included 55 articles between 1964 and 2017. The major conclusions of this review were (1) confirmation of the current consensus that respiratory dysfunction, predominantly a restrictive ventilatory pattern, is common in myotonic dystrophy and is associated with alveolar hypoventilation, chronic hypercapnia, and sleep disturbance in the form of sleep apnoea and sleep related disordered breathing; (2) contrary to commonly held belief, there is no consensus in the literature regarding the relationship between CTG repeat length and severity of respiratory dysfunction and a relationship has not been established; (3) the natural history and time-course of respiratory functional decline is very poorly understood in the current literature; (4) there is a consensus that there is a significant involvement of central respiratory drive in this alveolar hypoventilation however the current literature does not identify the mechanism for this

    Simple model for decay of superdeformed nuclei

    Recent theoretical investigations of the decay mechanism out of a superdeformed nuclear band have yielded qualitatively different results, depending on the relative values of the relevant decay widths. We present a simple two-level model for the dynamics of the tunneling between the superdeformed and normal-deformed bands, which treats decay and tunneling processes on an equal footing. The previous theoretical results are shown to correspond to coherent and incoherent limits of the full tunneling dynamics. Our model accounts for experimental data in both the A~150 mass region, where the tunneling dynamics is coherent, and in the A~190 mass region, where the tunneling dynamics is incoherent.Comment: 4 page

    Spreading Width for Decay out of a Superdeformed Band

    The attenuation factor F responsible for the decay out of a superdeformed (SD) band is calculated with the help of a statistical model. This factor is given by 1/F = (1 + Gamma(down) / Gamma(S)). Here, Gamma(S) is the width for the collective E2 transition within the superdeformed band, and Gamma(down) is the spreading width which describes the mixing between a state in the SD band and the normally deformed (ND) states of equal spin. The attenuation factor F is independent of the statistical E1 decay widths Gamma(N) of the ND states provided that the Gamma(N) are much larger than both Gamma(down) and Gamma(S).     Functional cross-talk between phosphorylation and disease-causing mutations in the cardiac sodium channel Na(v)1.5

    The voltage-gated sodium channel Nav1.5 initiates the cardiac action potential. Alterations of its activation and inactivation properties due to mutations can cause severe, life-threatening arrhythmias. Yet despite intensive research efforts, many functional aspects of this cardiac channel remain poorly understood. For instance, Nav1.5 undergoes extensive posttranslational modification in vivo, but the functional significance of these modifications is largely unexplored, especially under pathological conditions. This is because most conventional approaches are unable to insert metabolically stable posttranslational modification mimics, thus preventing a precise elucidation of the contribution by these modifications to channel function. Here, we overcome this limitation by using protein semisynthesis of Nav1.5 in live cells and carry out complementary molecular dynamics simulations. We introduce metabolically stable phosphorylation mimics on both wild-type (WT) and two pathogenic long-QT mutant channel backgrounds and decipher functional and pharmacological effects with unique precision. We elucidate the mechanism by which phosphorylation of Y1495 impairs steady-state inactivation in WT Nav1.5. Surprisingly, we find that while the Q1476R patient mutation does not affect inactivation on its own, it enhances the impairment of steady-state inactivation caused by phosphorylation of Y1495 through enhanced unbinding of the inactivation particle. We also show that both phosphorylation and patient mutations can impact Nav1.5 sensitivity toward the clinically used antiarrhythmic drugs quinidine and ranolazine, but not flecainide. The data highlight that functional effects of Nav1.5 phosphorylation can be dramatically amplified by patient mutations. Our work is thus likely to have implications for the interpretation of mutational phenotypes and the design of future drug regimens.Chemical Immunolog

    The Fragment Mass Analyzer at the ATLAS facility has been used to unambiguously identify the mass number associated with different decay modes of the nobelium isotopes produced via 204Pb(48Ca,xn)(252-x)No reactions. Isotopically pure (>99.7%) 204Pb targets were used to reduce background from more favored reactions on heavier lead isotopes. Two spontaneous fission half-lives (t_1/2 = 3.7+1.1-0.8 us and 43+22-15 us) were deduced from a total of 158 fission events. Both decays originate from 250No rather than from neighboring isotopes as previously suggested. The longer activity most likely corresponds to a K-isomer in this nucleus. No conclusive evidence for an alpha branch was observed, resulting in upper limits of 2.1% for the shorter lifetime and 3.4% for the longer activity.Comment: RevTex4, 10 pages, 5 figures, submitted to PR
