Like the condition of security and governance, health is also a major concern for Pakistan and there is a large population on an already insufficient medical treatment structure to cater for. Amongst various other diseases prevalent in Pakistan Poliomyelitis is one of the key concerns of the country with respect to medical situation. This research investigates the reasons behind the rejection of polio drops in Balochistan; weaknesses and efficiency of the polio eradication campaign and effects of cross-border movement and its governance between Afghanistan and Pakistan on the efficacy of the campaigns. The findings suggest that illiteracy, lack of motivation, religious interpretations, governance and security are the major challenges to the polio campaigns in the context of Balochistan. Objective: To determine the effect of Valproate as compared to Phenytoin in patients with traumatic brain injury to prevent early seizures.Patients and Methods: This randomized controlled trial (RCT) was conducted at Pakistan Institute of Medical sciences (PIMS) Islamabad from March to September 2015. A total of 100 cases of traumatic brain injury were enrolled in this study; which were randomly divided into two groups A and B. Gender distribution was similar in both A & B groups, with 36 (72%) males and 14 (28%) females. Consecutive (non-probability) sampling technique was used. Results: Group A patients received Valproate as anti-epileptic agent, while in group B, Phenytoin was given. In group A there were 45 (90%) patients with no seizures, 1 (2%) with simple partial, 1(2%) with complex partial and 3 (6%) with generalized tonic clonic seizures. In group B there were 40 (80%) patients with no seizures, 1 (2%) with simple partial, 5(10%) with complex partial and 4 (8%) with generalized tonic colonic seizures. In group A seizures were low (10 %) as compare to group B (20%), with statistically insignificant difference (p= 0.161).Conclusion: In this study, Valproate was found to be effective as a prophylactic anti-epileptic agent as compared to phenytoin. New drugs need to be studied on larger scale to find more effective and safe drug for Prophylactic use in post-traumatic seizures

Introduction: Neural tube defects are one of the most common congenital birth defects resulting in a significant number of physical, mental and social disabilities in the earlier years of life. Amongst the central nervous system anomalies which are reported in 31% of newborns with anomalies, myelomeningocele (MMC) is the most common of the central nervous system defects with upto 71% occurrence. Objective: To determine frequency of post-operative complications in the repair of myelomeningocele.Material and Method: This study was a Descriptive case series conducted at Pakistan Institute of Medical sciences (PIMS) Islamabad from March to September 2016. Consecutive (non-probability) sampling technique was used.Total of 156 patients were included in the study who presented in neurosurgical department of PIMS, Islamabad suffering from meningomyelocele. Mean Age of presentation of patients was 58.58 days ± 26.01 Std. deviation. 88 patients (56.4%) were males and 68 (43.6%) were females. 79 patients had size of defect < 5 cm, 68 patients had size of defect ranging from 5-10 cm and 9 patients had defect size > 10 cm in size. 21 patients (13.5%) wound showed signs of surgical site infection while 135 patients (86.5%) wounds healed satisfactorily. 37 patients (23.7%) had CSF leakage while 119 patients (76.3%) recovered completely. Post-operative pyrexia was noted in 141 patients (90.4%) while 15 patients (9.6%) remained afebrile. Post-operative Hydrocephalus was seen in 35 patients (22.4%) as compared to 121 patients (77.6%) with no hydrocephalus post operatively. Conclusion: The study concluded that early diagnosis and prompt management is key to recovery of child. Early operation without subsequent complications is the backbone of treatment

Rainfall forecast using SARIMA model along the coastal areas of Sindh Province

    Get PDF
Rainfall forecasting is critical for economic activities such as agriculture, watershed management, and flood control. It requires mathematical modelling and simulation. This paper investigates the time series analysis and forecasting of the monthly rainfall for the Sindh coastline, Pakistan. The seasonal autoregressive integrated moving average (SARIMA) model was used for the last three decades (1991-2020) and forecasting was done for the next two years. The model is based on the Box Jenkins methodology. The decomposition of time series plots into trend, seasonaland random components showed a seasonal effect. The Augmented Dickey–Fuller (ADF) and Mann–Kendall (MK) tests showed the inherent stationarity of the rainfall data. The best SARIMA models for monthly rainfall were SARIMA (1,0,1)(3,1,1)12 and SARIMA (1,0,1)(1,1,1)12 with Akaike information criterion corrected (AICC) values of 1507 and 1387, respectively. The model predictions indicate that, in the years 2021/22, July will likely have the most rainfall, followed by August and June. The diagnostic statistical test values directed that the adequacy of the models is consistent for projected monthly rainfall forecasts
