    Handling Encrypted Email at the Gateway of a Network

    Protection for email is provided by client side encryption from being read or hacked. But the security risk in this protection is that it also protects virus from being sensed by the anti virus software. It also hides the hacker�s destructive payload. The encrypted message can�t be examined by any anti virus software rather filtered by content specific filtering engine. In case of providing perimeter defense for email virus, the network administrator faces the problem of disclosing the email or denies sending the email. If the email is allowed then network is not secure, if the message is disclosed then the message privacy is not maintained. Hence we suggest having an Email Virus Certification Authority which certifies all the emails sent from the message generating gateway or mail exchange server. This paper addresses the above problem and provides a theoretical solution for such security and privacy issue. Simple simulation is also implemented

    Objective: To identify frequency, type, severity and predictors of potential drug-drug interactions(pDDIs), potential drug-food interactions(pDFIs), potential drug-alcohol interactions(pDAIs) and potential drug-tobacco interactions(pDTIs) and most frequently interacting drug combination pairs in hospitalized patients from departments(depts) of General Medicine(GM), Orthopedic(Ortho), Gynecology(OBG), Pulmonology(Pulmo), General Surgery (GS), Psychiatry (Psych), Otolaryngology(ENT) and Dermatology (Derm) of study population. Methods: A Prospective Observational Study was conducted in eight major dept's of a tertiary care teaching hospital for a period of 6 mo. A sample size of 650 prescriptions reflecting admission no's for each department were used. Results: A total of 650 patients were included in the study. Among them, 282(43.4%) were males and 368(56.6%) were females. The mean age of the study population was 39.67±15.23. A total of 487 pDDIs, 734 pDFIs, 586 pDAIs and 159 pDTIs were found out of 650 hospitalized episodes. OBG showed the highest pDDIs and pDAIs. Highest pDFIs and pDTIs were seen in Pulmo. The majority of DDIs were minor, DFIs and DAIs were moderate and DTIs were of major in severity. Pharmacokinetic types of interactions were seen in the majority of the depts. Logistic regression analysis showed that Polypharmacy was associated with the occurrence of DIs. Most of the DIs repeated several times in particular depts and a list of these combinations was prepared. Conclusion: With the high occurrence of overall DIs and characteristic patterns of DIs combination pairs among different departments of the hospital, the presence of clinical pharmacists in hospitals can play a great role, especially in developing nations like India where their role in hospitalized settings is always controversial

    External validation of anti-Müllerian hormone based prediction of live birth in assisted conception

    <p>Background - Chronological age and oocyte yield are independent determinants of live birth in assisted conception. Anti-Müllerian hormone (AMH) is strongly associated with oocyte yield after controlled ovarian stimulation. We have previously assessed the ability of AMH and age to independently predict live birth in an Italian assisted conception cohort. Herein we report the external validation of the nomogram in 822 UK first in vitro fertilization (IVF) cycles.</p> <p>Methods - Retrospective cohort consisting of 822 patients undergoing their first IVF treatment cycle at Glasgow Centre for Reproductive Medicine. Analyses were restricted to women aged between 25 and 42 years of age. All women had an AMH measured prior to commencing their first IVF cycle. The performance of the model was assessed; discrimination by the area under the receiver operator curve (ROCAUC) and model calibration by the predicted probability versus observed probability.</p> <p>Results - Live births occurred in 29.4% of the cohort. The observed and predicted outcomes showed no evidence of miscalibration (p = 0.188). The ROCAUC was 0.64 (95% CI: 0.60, 0.68), suggesting moderate and similar discrimination to the original model. The ROCAUC for a continuous model of age and AMH was 0.65 (95% CI 0.61, 0.69), suggesting that the original categories of AMH were appropriate.</p> <p>Conclusions - We confirm by external validation that AMH and age are independent predictors of live birth. Although the confidence intervals for each category are wide, our results support the assessment of AMH in larger cohorts with detailed baseline phenotyping for live birth prediction.</p&gt

    Experimental and numerical study of stenotic flows

    Atherosclerosis has been leading cause of deaths in several countries. Recent technical advances have allowed the investigation of stenotic flows and in understanding the implications with increased severity. Such studies shall provide detailed understanding of flow across stenosis and its progression. In the present study, a large artery representing segment of aorta is considered as test specimen. Experimental study is carried out by generating the pulsatile flow through pulsatile duplicator. Flow across normal and various severities of stenosis such as 25%, 50% and 75% are studied. Numerical simulation using CFD is also carried out in similar normal and stenosed models. A novel concept of using pulse duplicator to generate pulsatile waves and investigate the different stenosed models has been adopted. Results obtained experimentally and numerically are compared and agree well with that of clinical observations. This study demonstrates significant variation of haemodynamic in post-stenotic region with increased stenosis. Increased pulse pressure, phase lag is observed with increased severity. It is also observed that stenosis greater than 75% is significant as flow complexity is induced with considerable disturbance even in early and latter part of pulse cycle. Such study shall be useful in understanding the flow changes in stenosis and enhance clinical observation

    Focal adhesion kinase regulates the activity of the osmosensitive transcription factor TonEBP/NFAT5 under hypertonic conditions

    Ton EBP/NFAT5 is a major regulator of the urinary concentrating process and is essential for the osmoadaptation of renal medullary cells. Focal adhesion kinase (FAK) is a mechanosensitive non-receptor protein tyrosine kinase expressed abundantly in the renal medulla. Since osmotic stress causes cell shrinkage, the present study investigated the contribution of FAK on TonEBP/NFAT5 activation. Osmotic stress induced time-dependent activation of FAK as evidenced by phosphorylation at Tyr-397, and furosemide reduces FAK Tyr-397 phosphorylation in the rat renal medulla. Both pharmacological inhibition of FAK and siRNA-mediated knockdown of FAK drastically reduced TonEBP/NFAT5 transcriptional activity and target gene expression in HEK293 cells. This effect was not mediated by impaired nuclear translocation or by reduced transactivating activity of TonEBP/NFAT5. However, TonEBP/NFAT5 abundance under hypertonic conditions was diminished by 50% by FAK inhibition or siRNA knockdown of FAK. FAK inhibition only marginally reduced transcription of the TonEBP/NFAT5 gene. Rather, TonEBP/NFAT5 mRNA stability was diminished significantly by FAK inhibition, which correlated with reduced reporter activity of the TonEBP/NFAT5 mRNA 3' untranslated region (3'-UTR). In conclusion, FAK is a major regulator of TonEBP/NFAT5 activity by increasing its abundance via stabilization of the mRNA. This in turn, depends on the presence of the TonEBP/NFAT5 3'-UTR

    Fluid-structure interaction study of stenotic flow in subject specific carotid bifurcation: a case study

    Advances in numerical simulation have allowed the investigation of complex interaction of blood flow through elastic arteries which can be useful in demonstrating the disease progression and haemodynamics of cardiovascular diseases such as atherosclerosis. In the present study, patient is diagnosed with an occluded right Internal Carotid Artery (ICA) with partial stenosis at root of right External Carotid Artery (ECA) and partial stenosis at the root of left ICA. 3D patient specific carotid bifurcation is generated based on CT scan data using MIMICS-14.0 and numerical analysis is performed using FSI in ANSYS-14.5. The blood flow is assumed to be incompressible, homogenous and Newtonian, while artery wall is assumed to behave linearly elastic. The two-way sequentially coupled transient FSI analysis is performed using FSI solver for three pulse cycles and haemodynamic parameters such as flow pattern, Wall Shear Stress (WSS), pressure contours and arterial wall deformation are studied at the bifurcation and critical zones. The variation in flow behavior is investigated throughout the pulse and simulation results obtained reveals that there is a considerable increase in the flow behavior in partially stenosed carotid unlike occluded carotid. The investigation also demonstrates disturbed flow pattern especially at the bifurcation and stenosed zone elevating the haemodynamic variation during peak systole and later part of pulse cycle. The obtained results agree well with the clinical observation and demonstrate the potential of patient specific numerical studies in prognosis of disease progression and plaque rupture

    S100A8/A9 regulates CD11b expression and neutrophil recruitment during chronic tuberculosis

    Neutrophil accumulation is associated with lung pathology during active tuberculosis (ATB). However, the molecular mechanism or mechanisms by which neutrophils accumulate in the lung and contribute to TB immunopathology are not fully delineated. Using the well-established mouse model of TB, our new data provide evidence that the alarmin S100A8/A9 mediates neutrophil accumulation during progression to chronic TB. Depletion of neutrophils or S100A8/A9 deficiency resulted in improved Mycobacterium tuberculosis (Mtb) control during chronic but not acute TB. Mechanistically, we demonstrate that, following Mtb infection, S100A8/A9 expression is required for upregulation of the integrin molecule CD11b specifically on neutrophils, mediating their accumulation during chronic TB disease. These findings are further substantiated by increased expression of S100A8 and S100A9 mRNA in whole blood in human TB progressors when compared with nonprogressors and rapidly decreased S100A8/A9 protein levels in the serum upon TB treatment. Furthermore, we demonstrate that S100A8/A9 serum levels along with chemokines are useful in distinguishing between ATB and asymptomatic Mtb-infected latent individuals. Thus, our results support targeting S100A8/A9 pathways as host-directed therapy for TB
