    Insights into the piezoceramic electromechanical impedance response for monitoring cement mortars during water saturation curing

    Lead Zirconate Titanate (PZT) based electromechanical impedance (EMI) sensors were used to monitor the mechanical properties development of different water to cement ratios (w/c) cementitious mortar mixes, during the first 28 days of curing under water. Through using the analytical procedure proposed in this study to analyse the EMI data, the different mixes mechanical properties development through the curing period were detected, and the EMI response was able to provide a more detailed interpretation regarding the difference between the surface and the bulk material mechanical properties development. Both the peaks from the impedance signature (Z) and the first difference of the impedance signature (dZ) showed shifts to higher frequency ranges as the age of the samples increased, indicating an increase in the material stiffness. Furthermore, the compressive and the flexural stresses showed an R2 > 0.8 and > 0.9, respectively in relation to the frequency shifts. The relationship between the PZT-EMI response through the curing period and the sample’s mechanical properties was shown to be frequency-dependent; hence a numerical analysis using ANSYS Workbench 18.1 was undertaken to understand this frequency-dependence phenomenon. From the numerical model, the impedance signature response at higher frequency ranges was shown to be dominated by the response from the surface of the hosting material, whereas the response from the specimen's interior dominated the lower frequencies EMI response. The analytical approach proposed in this study is expected to assist in differentiating between internal cementitious materials processes, such as internal curing, and those originating at the surface, such as aggressive chemical agents’ penetratio

    Noninvasive Sphenopalatine Ganglion Block for Acute Headache in the Emergency Department: A Randomized Placebo-Controlled Trial

    Study objective We seek to test the efficacy of noninvasive sphenopalatine ganglion block for the treatment of acute anterior headache in the emergency department (ED) using a novel noninvasive delivery device. Methods We conducted a randomized, double-blind, placebo-controlled trial evaluating bupivacaine anesthesia of the sphenopalatine ganglion for acute anterior or global-based headache. This study was completed in 2 large academic EDs. Bupivacaine or normal saline solution was delivered intranasally (0.3 mL per side) with the Tx360 device. Pain and nausea were measured at 0, 5, and 15 minutes by a 100-mm visual analog scale. The primary endpoint was a 50% reduction in pain at 15 minutes. Telephone follow-up assessed 24-hour pain and nausea through a 0- to 10-point verbal scale and adverse effects. Results The median reported baseline pain in the bupivacaine group was 80 mm (IQR 66 mm - 93 mm) and 78.5 mm (IQR 64 mm to 91.75 mm) in the normal saline solution group. A 50% reduction in pain was achieved in 48.8% of the bupivacaine group (20/41 patients) versus 41.3% in the normal saline solution group (19/46 patients), for an absolute risk difference of 7.5% (95% confidence interval [CI] –13% to 27.1%). As a secondary outcome, at 24 hours, more patients in the bupivacaine group were headache free (24.7% difference; 95% CI 2.6% to 43.6%) and more were nausea free (16.9% difference; 95% CI 0.8% to 32.5%). Conclusion For patients with acute anterior headache, sphenopalatine ganglion block with the Tx360 device with bupivacaine did not result in a significant increase in the proportion of patients achieving a greater than or equal to 50% reduction in headache severity at 15 minutes compared with saline solution applied in the same manner

    The outcomes of place kicks can have a large impact on Rugby Union match results. This study investigated the ball orientations used in place kicking and their potential implications for success. All 416 place kicks from the 2019 Rugby World Cup were grouped into one of three ball orientation categories, and predicted odds of success were calculated for each category using a binomial logistic regression which accounted for situational factors known to affect performance outcome. Kicks taken using a slanted orientation (n = 152) had the greatest odds of success (90.0%) when taken from the mean tournament distance (29.7 m), compared to a near vertical (n = 116) orientation (84.4%), and near horizontal (n = 148) orientation (86.8%).     Reducing Operating Room Costs through Real Time Cost Information Feedback: A Pilot Study

    Purpose: To create a protocol for providing real-time operating room (OR) cost feedback to surgeons. We hypothesize that this protocol will reduce costs in a responsible way without sacrificing quality of care. Methods: All OR costs were obtained and recorded for robot-assisted partial nephrectomy and laparoscopic donor nephrectomy. Before the beginning of this project, costs pertaining to the 20 most recent cases were analyzed. Items were identified from previous cases as modifiable for replacement or omission. Timely feedback of total OR costs and cost of each item used was provided to the surgeon after each case, and costs were analyzed. Results: A cost analysis of the robot-assisted partial nephrectomy before the washout period indicates expenditures of 5243.04percase.Tenrecommendedmodifiableitemswerefoundtohaveanaveragepercasecostof5243.04 per case. Ten recommended modifiable items were found to have an average per case cost of 1229.33 representing 23.4% of the total cost. A postwashout period cost analysis found the total OR cost decreased by 899.67(17.2899.67 (17.2%) because of changes directly related to the modifiable items. Therefore, 73.2% of the possible identified savings was realized. The same stepwise approach was applied to laparoscopic donor nephrectomies. The average total cost per case before the washout period was 3530.05 with 457.54attributedtomodifiableitems.Afterthewashoutperiod,modifiableitemscostswerereducedby457.54 attributed to modifiable items. After the washout period, modifiable items costs were reduced by 289.73 (8.0%). No complications occurred in the donor nephrectomy cases while one postoperative complication occurred in the partial nephrectomy group. Conclusion: Providing surgeons with feedback related to OR costs may lead to a change in surgeon behavior and decreased overall costs. Further studies are needed to show equivalence in patient outcomes

    MRI radiomic features are independently associated with overall survival in soft tissue sarcoma

    Purpose: Soft tissue sarcomas (STS) represent a heterogeneous group of diseases, and selection of individualized treatments remains a challenge. The goal of this study was to determine whether radiomic features extracted from magnetic resonance (MR) images are independently associated with overall survival (OS) in STS. Methods and Materials: This study analyzed 2 independent cohorts of adult patients with stage II-III STS treated at center 1 (N = 165) and center 2 (N = 61). Thirty radiomic features were extracted from pretreatment T1-weighted contrast-enhanced MR images. Prognostic models for OS were derived on the center 1 cohort and validated on the center 2 cohort. Clinical-only (C), radiomics-only (R), and clinical and radiomics (C+R) penalized Cox models were constructed. Model performance was assessed using Harrell\u27s concordance index. Results: In the R model, tumor volume (hazard ratio [HR], 1.5) and 4 texture features (HR, 1.1-1.5) were selected. In the C+R model, both age (HR, 1.4) and grade (HR, 1.7) were selected along with 5 radiomic features. The adjusted c-indices of the 3 models ranged from 0.68 (C) to 0.74 (C+R) in the derivation cohort and 0.68 (R) to 0.78 (C+R) in the validation cohort. The radiomic features were independently associated with OS in the validation cohort after accounting for age and grade (HR, 2.4; Conclusions: This study found that radiomic features extracted from MR images are independently associated with OS when accounting for age and tumor grade. The overall predictive performance of 3-year OS using a model based on clinical and radiomic features was replicated in an independent cohort. Optimal models using clinical and radiomic features could improve personalized selection of therapy in patients with STS

    Galaxy Zoo: Reproducing Galaxy Morphologies Via Machine Learning

    We present morphological classifications obtained using machine learning for objects in SDSS DR6 that have been classified by Galaxy Zoo into three classes, namely early types, spirals and point sources/artifacts. An artificial neural network is trained on a subset of objects classified by the human eye and we test whether the machine learning algorithm can reproduce the human classifications for the rest of the sample. We find that the success of the neural network in matching the human classifications depends crucially on the set of input parameters chosen for the machine-learning algorithm. The colours and parameters associated with profile-fitting are reasonable in separating the objects into three classes. However, these results are considerably improved when adding adaptive shape parameters as well as concentration and texture. The adaptive moments, concentration and texture parameters alone cannot distinguish between early type galaxies and the point sources/artifacts. Using a set of twelve parameters, the neural network is able to reproduce the human classifications to better than 90% for all three morphological classes. We find that using a training set that is incomplete in magnitude does not degrade our results given our particular choice of the input parameters to the network. We conclude that it is promising to use machine- learning algorithms to perform morphological classification for the next generation of wide-field imaging surveys and that the Galaxy Zoo catalogue provides an invaluable training set for such purposes.Comment: 13 Pages, 5 figures, 10 tables.
