    Characterization of the Agrobacterium tumefaciens VirB2 pilin of the VirB/D4 Type IV Secretion System

    The Agrobacterium tumefaciens VirB/D4 type IV secretion system (T4SS) delivers oncogenic T-DNA and effector proteins to susceptible plant cells. This leads to the formation of tumors termed Crown Galls. The VirB/D4 T4SS is comprised of 12 subunits (VirB1 to VirB11 and VirD4), which assemble to form two structures, a secretion channel spanning the cell envelope and a T-pilus extending from the cell surface. In A. tumefaciens, the VirB2 pilin subunit is required for assembly of the secretion channel and is the main subunit of the T-pilus. The focus of this thesis is to define key reactions associated with the T4SS biogenesis pathway involving the VirB2 pilin. Topology studies demonstrated that VirB2 integrates into the inner membrane with two transmembrane regions, a small cytoplasmic loop, and a long periplasmic loop comprised of covalently linked N and C termini. VirB2 was shown by the substituted cysteine accessibility method (SCAM) to adopt distinct structural states when integrated into the inner membrane and when assembled as a component of the secretion channel and the T-pilus. The VirB4 and VirB11 ATPases were shown by SCAM to modulate the structural state of membrane-integrated VirB2 pilin, and evidence was also obtained that VirB4 mediates extraction of pilin from the membrane. A model that VirB4 functions as a pilin dislocase by an energy-dependent mechanism was further supported by coimmunoprecipitation and osmotic shock studies. Mutational studies identified two regions of VirB10, an N-terminal transmembrane domain and an outer membrane-associated domain termed the antennae projection, that contribute selectively to T-pilus biogenesis. Lastly, characterization of a VirB10 mutant that confers a ‘leaky’ channel phenotype further highlighted the role of VirB10 in gating substrate translocation across the outer membrane as well as T-pilus biogenesis. Results of my studies support a working model in which the VirB4 ATPase catalyzes dislocation of membrane-integrated pilin, and distinct domains of VirB10 coordinate pilin incorporation into the secretion channel and the extracellular T-pilus

    A survey of University Students' Perceptions of Learning Management Systems in a Low-Resource Setting using a Technology Acceptance Model

    Learning management systems have been widelyadvocated for the support of distance learning. Inlow-resource settings, the uptake of these systemsby students has been mixed. This study aimed to dentify, through the use of the Technology AcceptanceModel, the individual, organizational, and technological factors that could be influencing the use of learning management systems. A simple quantitative descriptive surveywas conducted of nursing and health science students at a university in South Africa as part of their first exposure to a learning management system. A total of 274 respondents (56.7%) completed the survey questionnaire,made up of 213 nursing respondents (87.7%) and 61 health sciences respondents (25%).Overall, the respondents found the learning management system easy to use and useful for learning. Therewere significant differences between the two groups of respondents, with the respondents from health sciences being both younger andmore computer literate. The nursing respondents, who received more support and orientations, reported finding the learning management system more useful.     The purpose of this study was to understand how abiotic factors change along Carp Lake River and how these factors influence species abundance and distribution. Temperature, velocity, pH, conductivity, depth, nutrient levels, dissolved oxygen concentration, slope, and substrate were measured at four sites along the stream. The species of fish sound at each site were also recorded. We found that groundwater inputs at the third site had a significant impact on the abiotic factors observed. We explored the relationships between the observed trends in abiotic factors and the variation in species diversity over the course of the stream. This study concludes that differences in velocity, substrate, and depth influence the distribution of fish species along Carp Lake River.     Marine Reserves and Reproductive Biomass: A Case Study of a Heavily Targeted Reef Fish

    Recruitment overfishing (the reduction of a spawning stock past a point at which the stock can no longer replenish itself) is a common problem which can lead to a rapid and irreversible fishery collapse. Averting this disaster requires maintaining a sufficient spawning population to buffer stochastic fluctuations in recruitment of heavily harvested stocks. Optimal strategies for managing spawner biomass are well developed for temperate systems, yet remain uncertain for tropical fisheries, where the danger of collapse from recruitment overfishing looms largest. In this study, we explored empirically and through modeling, the role of marine reserves in maximizing spawner biomass of a heavily exploited reef fish, Lethrinus harak around Guam, Micronesia. On average, spawner biomass was 16 times higher inside the reserves compared with adjacent fished sites. Adult density and habitat-specific mean fish size were also significantly greater. We used these data in an age-structured population model to explore the effect of several management scenarios on L. harak demography. Under minimum-size limits, unlimited extraction and all rotational-closure scenarios, the model predicts that preferential mortality of larger and older fish prompt dramatic declines in spawner biomass and the proportion of male fish, as well as considerable declines in total abundance. For rotational closures this occurred because of the mismatch between the scales of recovery and extraction. Our results highlight how alternative management scenarios fall short in comparison to marine reserves in preserving reproductively viable fish populations on coral reefs

    Physician assessments of medication adherence and decisions to intensify medications for patients with uncontrolled blood pressure: still no better than a coin toss

    Abstract Background Many patients have uncontrolled blood pressure (BP) because they are not taking medications as prescribed. Providers may have difficulty accurately assessing adherence. Providers need to assess medication adherence to decide whether to address uncontrolled BP by improving adherence to the current prescribed regimen or by intensifying the BP treatment regimen by increasing doses or adding more medications. Methods We examined how provider assessments of adherence with antihypertensive medications compared with refill records, and how providers’ assessments were associated with decisions to intensify medications for uncontrolled BP. We studied a cross-sectional cohort of 1169 veterans with diabetes presenting with BP ≥140/90 to 92 primary care providers at 9 Veterans Affairs (VA) facilities from February 2005 to March 2006. Using VA pharmacy records, we utilized a continuous multiple-interval measure of medication gaps (CMG) to assess the proportion of time in prior year that patient did not possess the prescribed medications; CMG ≥20% is considered clinically significant non-adherence. Providers answered post-visit Likert-scale questions regarding their assessment of patient adherence to BP medications. The BP regimen was considered intensified if medication was added or increased without stopping or decreasing another medication. Results 1064 patients were receiving antihypertensive medication regularly from the VA; the mean CMG was 11.3%. Adherence assessments by providers correlated poorly with refill history. 211 (20%) patients did not have BP medication available for ≥ 20% of days; providers characterized 79 (37%) of these 211 patients as having significant non-adherence, and intensified medications for 97 (46%). Providers intensified BP medications for 451 (42%) patients, similarly whether assessed by provider as having significant non-adherence (44%) or not (43%). Conclusions Providers recognized non-adherence for less than half of patients whose pharmacy records indicated significant refill gaps, and often intensified BP medications even when suspected serious non-adherence. Making an objective measure of adherence such as the CMG available during visits may help providers recognize non-adherence to inform prescribing decisions.http://deepblue.lib.umich.edu/bitstream/2027.42/112850/1/12913_2012_Article_2450.pd

    Is treadmill walking with virtual reality an acceptable and plausible training modality for stroke survivors?

    Introduction and Objectives: For many stroke survivors recovering independent walking is an important objective, however, although most survivors recover some walking ability it is often insufficient to meet the challenges of community walking[1]. Treadmill walking, through forced use of the paretic side, has some evidence as an intervention but lacks transfer to community walking[2]. With the rationale of providing a more realistic, variable walking experience, the addition of virtual reality (VR) to treadmills is a recent, promising, development. This aim of this study, therefore, was to investigate the credibility and acceptability of treadmill training with VR among stroke survivors. Methods: This was a feasibility study testing the validity of treadmill training with and without VR through comparison with overground walking. User acceptability was assessed through independent interviews. Six ambulant stroke survivors (aged 56.5±10.6years, 116±93.3months post stroke) were recruited from local stroke clubs. They attended an introductory session for baseline measurements (over ground gait biomechanics, functional mobility and cognition) and familiarisation with the treadmill VR system (MOTEKMedical, Netherlands) including wearing a harness. They then participated in two training sessions, one week apart, each session included treadmill walking without VR (TW) and treadmill walking with VR (TWVR) e.g. road and forest paths visualisations. The final session included a second measurement of gait biomechanics which provided the data for statistical comparison. Each individual was independently interviewed before and after their participation. Results: In general minor, non-significant, differences were observed for TM, TMVR and overground walking. There was, however, a consistent, and statistically significant (p= 0.032) finding of greater hip flexion for TWVR, see table for details. Universally, participants found the experience acceptable and enjoyable, although “challenging”. Conclusion: This feasibility study found treadmill walking (with and without VR) to be similar enough to overground walking to justify it as a training modality for chronic stroke survivors who had already attained some independence in walking. One possible difference, greater hip flexion during treadmill walking, may be a product of harness wearing, and/or relate to the setup of the visualisations. Participants found the experience of treadmill walking acceptable. Although they preferred walking with the visualisations there were few biomechanical differences to simple treadmill walking. Further exploration of individual variability, however, should be undertaken as this may be a factor in ranslating gains from treadmill training to community walking. In conclusion, similarities between overground and treadmill walking (with and without VR) support its use as a training modality in stroke rehabilitation. No great advantage was evident from the use of visualisation other than participant’s preference, however these findings require further testing
