    Solitary myofibroma of the mandible in an adult with magnetic resonance imaging and positron emission tomography findings: a case report

    Myofibroma is a benign tumor composed of myoid spindle cells. The prevalence of myofibroma in the oral cavity is very low, with the mandible being the most common site. This report describes an adult case of myofibroma that arose on the mandible and includes magnetic resonance imaging (MRI) and positron emission tomography (PET) findings. On the MRI T1-weighted images, the tumor appeared with signal iso-intensity and was highly and heterogeneously enhanced with contrast material. On the T2-weighted images, it appeared with increased signal intensity. (18) F-fluorodeoxyglucose (FDG)-PET imaging showed abnormal strong accumulation of FDG in the left mandibular region. The tumor was removed by marginal resection of the left mandible under general anesthesia. Histopathological findings revealed that the tumor stroma contained abundant thin-walled vessels. The postoperative course was uneventful, and we found no evidence of recurrence at the postoperative 34-month follow-up

    A 36-year-old woman was underwent emergency laparotomy for acute intraabdominal hemorrhage, but bleeding points were not found. Abdominal pains continued after the laparotomy, and rupture of aneurysm on uterine artery was found in angiography. An transcatheter arterial embolization was done for the uterine artery, and the aneurysm was found to disappear in 4-day-after the angiography. Rupture of an aneurysm on uterine artery should be considered for the causes of acute intraabdominal hemorrhage

    Abstract — This paper studies an optimal planning of falling motions of a human-sized humanoid robot to reduce the damage of the robot. We developed a human-sized robot HRP-2FX which has a simplified humanoid robot shape with seven d.o.f. and can emulate motions in the sagittal plane of a humanoid robot. An optimal control is applied to generate the falling motion of HRP-2FX to minimize a performance index, and the optimality has been verified by the experiments on HRP-2FX. I

    Gamma rays from a reverse shock with turbulent magnetic fields in GRB 180720B

    Gamma-ray bursts (GRBs) are the most electromagnetically luminous cosmic explosions. They are powered by collimated streams of plasma (jets) ejected by a newborn stellar-mass black hole or neutron star at relativistic velocities (near the speed of light). Their short-lived (typically tens of seconds) prompt γ\gamma-ray emission from within the ejecta is followed by long-lived multi-wavelength afterglow emission from the ultra-relativistic forward shock. This shock is driven into the circumburst medium by the GRB ejecta that are in turn decelerated by a mildly-relativistic reverse shock. Forward shock emission was recently detected up to teraelectronvolt-energy γ\gamma-rays, and such very-high-energy emission was also predicted from the reverse shock. Here we report the detection of optical and gigaelectronvolt-energy γ\gamma-ray emission from GRB 180720B during the first few hundred seconds, which is explained by synchrotron and inverse-Compton emission from the reverse shock propagating into the ejecta, implying a low-magnetization ejecta. Our optical measurements show a clear transition from the reverse shock to the forward shock driven into the circumburst medium, accompanied by a 90-degree change in the mean polarization angle and fluctuations in the polarization degree and angle. This indicates turbulence with large-scale toroidal and radially-stretched magnetic field structures in the reverse and forward shocks, respectively, which tightly couple to the physics of relativistic shocks and GRB jets -- launching, composition, dissipation and particle acceleration.Comment: 5 pages, 4 figures (main) plus Methods and Supplementary Methods, accepted for publicatio

    本研究の目的は,看護系大学の2 年生を対象とし,看護理論の学習に沿って学生自身を事例とした看護過程の展開(STEP1)を取り入れた授業の評価をすることである.授業は,STEP1 の後,グループワークを活用した紙上事例での看護過程の展開(STEP2)を実施し,看護過程の基礎的能力を養うことを目指している.社会人基礎力,批判的思考態度,一般性セルフ・エフィカシーの測定尺度を使用し,STEP1 前に1 回目,後に2 回目,STEP2 後に3 回目の質問紙調査を行った.対象者には,自身のID を作成してもらい,回答にID を付記してもらった.分析対象者は,STEP1 前後で27 名,STEP2 前後で5 名であった. STEP1 前後では,社会人基礎力の「シンキング」とその要素である「創造性」が有意に上昇し,学生は,自身を事例とした看護過程展開の学習により多角的な視野で解決策を導き出す能力を活用していることが示唆された.その一方で, 批判的思考態度の「協同的態度」と自己効力感が有意に低下した.STEP2 前後では,社会人基礎力の「チームワーク」の要素である「発信力」に有意な上昇を認め,グループワークの効果が得られたと考える.departmental bulletin pape

    Detection of significant antiviral drug effects on COVID-19 with reasonable sample sizes in randomized controlled trials : a modeling study

    Background Development of an effective antiviral drug for Coronavirus Disease 2019 (COVID-19) is a global health priority. Although several candidate drugs have been identified through in vitro and in vivo models, consistent and compelling evidence from clinical studies is limited. The lack of evidence from clinical trials may stem in part from the imperfect design of the trials. We investigated how clinical trials for antivirals need to be designed, especially focusing on the sample size in randomized controlled trials. Methods and findings A modeling study was conducted to help understand the reasons behind inconsistent clinical trial findings and to design better clinical trials. We first analyzed longitudinal viral load data for Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) without antiviral treatment by use of a within-host virus dynamics model. The fitted viral load was categorized into 3 different groups by a clustering approach. Comparison of the estimated parameters showed that the 3 distinct groups were characterized by different virus decay rates (p-value < 0.001). The mean decay rates were 1.17 d−1 (95% CI: 1.06 to 1.27 d−1), 0.777 d−1 (0.716 to 0.838 d−1), and 0.450 d−1 (0.378 to 0.522 d−1) for the 3 groups, respectively. Such heterogeneity in virus dynamics could be a confounding variable if it is associated with treatment allocation in compassionate use programs (i.e., observational studies). Subsequently, we mimicked randomized controlled trials of antivirals by simulation. An antiviral effect causing a 95% to 99% reduction in viral replication was added to the model. To be realistic, we assumed that randomization and treatment are initiated with some time lag after symptom onset. Using the duration of virus shedding as an outcome, the sample size to detect a statistically significant mean difference between the treatment and placebo groups (1:1 allocation) was 13,603 and 11,670 (when the antiviral effect was 95% and 99%, respectively) per group if all patients are enrolled regardless of timing of randomization. The sample size was reduced to 584 and 458 (when the antiviral effect was 95% and 99%, respectively) if only patients who are treated within 1 day of symptom onset are enrolled. We confirmed the sample size was similarly reduced when using cumulative viral load in log scale as an outcome. We used a conventional virus dynamics model, which may not fully reflect the detailed mechanisms of viral dynamics of SARS-CoV-2. The model needs to be calibrated in terms of both parameter settings and model structure, which would yield more reliable sample size calculation. Conclusions In this study, we found that estimated association in observational studies can be biased due to large heterogeneity in viral dynamics among infected individuals, and statistically significant effect in randomized controlled trials may be difficult to be detected due to small sample size. The sample size can be dramatically reduced by recruiting patients immediately after developing symptoms. We believe this is the first study investigated the study design of clinical trials for antiviral treatment using the viral dynamics model

    Distribution, temporal change, and conservation status of tropical seagrass beds in Southeast Asia: 2000–2020

    Although Southeast Asia is a hotspot of global seagrass diversity, there are considerable information gaps in the distribution of seagrass beds. Broad-scale seagrass distribution has not been updated in the global seagrass database by UNEP-WCMC since 2000, although studies on seagrasses have been undertaken intensively in each region. Here we analyze the recent distribution of tropical seagrass beds, their temporal changes, causes of decline and conservation status in Southeast Asia (plus southern mainland China, Taiwan and Ryukyu Island of Japan) using data collected after 2000. Based on the 195 literature published since 2000, we identified 1,259 point data and 1,461 polygon data showing the distribution of seagrass beds. A large discrepancy was found in the seagrass bed distribution between our updated data and the UNEP-WCMC database, mostly due to inaccurate and low resolution location information in the latter. Temporal changes in seagrass bed area analyzed for 68 sites in nine countries/regions demonstrated that more than 60% of seagrass beds declined at an average rate of 10.9% year–1, whereas 20% of beds increased at an average rate of 8.1% year–1, leading to an overall average decline of 4.7% year–1. Various types of human-induced threats were reported as causes for the decline, including coastal development, fisheries/aquaculture, and natural factors such as typhoons and tsunamis. The percentage of seagrass beds covered with existing marine protected areas (MPAs) varied greatly among countries/regions, from less than 1% in Brunei Darussalam and Singapore to 100% in southern Japan. However, the degree of conservation regulation was not sufficient even in regions with higher MPA coverage. The percentage of seagrass beds within EBSAs (Ecologically and Biologically Significant Area determined by the Convention of Biological Diversity) was higher than that within MPAs because EBSAs cover a greater area than MPAs. Therefore, designating EBSAs as legally effective MPAs can greatly improve the conservation status of seagrass beds in Southeast Asia
