    Preparation of zeolite filled glassy polymer membranes

    The incorporation of zeolite particles in the micrometer range into polymeric matrices was investigated as a way to improve the gas separation properties of the polymer materials used in the form of membranes. The adhesion between the polymer phase and the external surface of the particles appeared to be a major problem in the preparation of such membranes when the polymer is in the glassy state at room temperature. Various methods were investigated to improve the internal membrane structure, that is, surface modification of the zeolite external surface, preparation above the glass-transition temperature, and heat treatment. Improved structures were obtained as observed by scanning electron microscopy, but the influence on the gas separation properties was not in agreement with the observed structural improvements

    We show that if f is a probability density on Rn wrt Lebesgue measure (or any absolutely continuous measure) and 0 ≤ f ≤ 1, then there is another density g with only the values 0 and 1 and with the same (n−1)-dimensional marginals in any finite number of directions. This sharpens, unifies and extends the results of Lorentz and of Kellerer. Given a pair of independent random variables 0 ≤ X, Y ≤ 1, we further study functions 0 ≤ ϕ ≤ 1 such that Z = ϕ (X,Y) satisfies E(Z|X) = X and E(Z|Y) = Y.     A hybrid process combining ion exchange resin and bipolar membrane electrodialysis for reverse osmosis remineralization

    A new reverse osmosis (RO) permeate remineralization process combining ion exchange resin and bipolar membrane electrodialysis (BMED) was developed. Its feasibility for hardness ions recovery and RO permeate remineralization was investigated. The effect of several operation conditions on the efficiency of the combined remineralization process was studied. Highly efficient cation exchange resin loading was achieved at a low flow rate and low feed solution concentration. The recovered calcium purity and yield considerably improved under gradient elution methods in comparison with commonly applied conventional isocratic elution methods using the same eluent quantity. The purity of the produced acid and base using BMED dropped noticeably with increasing feed NaCl concentration, presumably related to decreased permselectivity of the ion-exchange membranes. The drop in the purity of the calcium recovered when eluting the cation exchange resin with BMED-produced HCl in comparison with commercially available acid at 50 % yield was shown not to affect the remineralization process, where a dilution factor could be applied. This study confirmed the technical feasibility of the developed process for RO permeate remineralization. However, its application can be limited by the water source characteristics, the energy-intensive bipolar membrane process, and applied operational conditions, where more investigation is still needed.</p

    Improving home haemodialysis: Stability evaluation of routine clinical chemistry analytes in blood samples of haemodialysis patients

    Introduction: A growing number of dialysis patients is treated with home haemodialysis. Our current pre-analytical protocols require patients to centrifuge the blood sample and transfer the plasma into a new tube at home. This procedure is prone to errors and precludes accurate bicarbonate measurement, required for determining dialysate bicarbonate concentration and maintaining acid-base status. We therefore evaluated whether cooled overnight storage of gel separated plasma is an acceptable alternative. Materials and methods: Venous blood of 34 haemodialysis patients was collected in 2 lithium heparin blood collection tubes with gel separator (LH PSTTM II, REF 367374; Becton Dickinson, New Jersey, USA). One tube was analysed directly for measurement of bicarbonate, potassium, calcium, phosphate, glucose, urea, lactate, aspartate aminotransferase (AST), and lactate dehydrogenase (LD); whereas the other was centrifuged and stored unopened at 4 °C and analysed 24 h later. To measure analyte stability after 24 h of storage, the mean difference was calculated and compared to the total allowable error (TEa) which was used as acceptance limit. Results: Potassium (Z = - 4.28, P < 0.001), phosphate (Z = - 3.26, P = 0.001), lactate (Z = - 5.11, P < 0.001) and AST (Z = - 2.71, P = 0.007) concentrations were higher, whereas glucose (Z = 4.00, P < 0.001) and LD (Z = 3.13, P = 0.002) showed a reduction. All mean differences were smaller than the TEa and thus not clinically relevant. Bicarbonate (Z = 0.69, P = 0.491), calcium (Z = - 0.23, P = 0.815) and urea (Z = 0.81, P =0.415) concentrations were stable. Conclusions: Our less complex, user-friendly pre-analytical procedure resulted in at least 24 h stability of analytes relevant for monitoring haemodialysis, including bicarbonate.     By studying the two-time overlap correlation function, we give a comprehensive analysis of the phase diagram of the Random Hopping Dynamics of the Random Energy Model (REM) on time-scales that are exponential in the volume.     The impact of loco-regional recurrences on metastatic progression in early-stage breast cancer: a multistate model

    To study whether the effects of prognostic factors associated with the occurrence of distant metastases (DM) at primary diagnosis change after the incidence of loco-regional recurrences (LRR) among women treated for invasive stage I or II breast cancer. The study population consisted of 3,601 women, enrolled in EORTC trials 10801, 10854, or 10902 treated for early-stage breast cancer. Data were analysed in a multivariate, multistate model by using multivariate Cox regression models, including a state-dependent covariate. The presence of a LRR in itself is a significant prognostic risk factor (HR: 3.64; 95%-CI: 2.02-6.5) for the occurrence of DM. Main prognostic risk factors for a DM are young age at diagnosis (</=40: HR: 1.79; 95%-CI: 1.28-2.51), larger tumour size (HR: 1.58; 95%-CI: 1.35-1.84) and node positivity (HR: 2.00; 95%-CI: 1.74-2.30). Adjuvant chemotherapy is protective for a DM (HR: 0.66; 95%-CI: 0.55-0.80). After the occurrence of a LRR the latter protective effect has disappeared (P = 0.009). The presence of LRR in itself is a significant risk factor for DM. For patients who are at risk of developing LRR, effective local control should be the main target of therapy

    Deconstructing the financialization of healthcare

    Financialization is promoted by alliances of multilateral 'development' organisations, national governments, and owners and institutions of private capital. In the healthcare sector, the leveraging of private sources of finance is widely argued as necessary to achieve the Sustainable Development Goal 3 target of universal health coverage. Employing social science perspectives on financialization, we contend that this is a new phase of capital formation. We trace the antecedents, institutions, instruments and ideas that facilitated the penetration of private capital in this sector, and the emergence of new asset classes that distinguish it. We argue that this deepening of financialization represents a fundamental shift in the organizing principles for healthcare systems, with negative implications for health and equality
