    The effect of a cognitive or motor task on the gait parameters of those with diabetes, with and without neuropathy

    Aims: To compare the gait parameters of older people with diabetes (DM) and people with diabetes and diabetic peripheral neuropathy (DPN) and to investigate the effect of a secondary motor or cognitive task on their gait. Methods: Thirty subjects were recruited; 15 with DPN (mean age 69 Ā±3.0years) and 15 with diabetes and no neuropathy (70 Ā± 2.9years). The temporal and spatial parameters of gait were evaluated using the GAITRite walkway. Subjects undertook four walks under normal walking conditions (single task); four times while simultaneously undertaking an additional motor task, carrying a tray with cups of water (dual task); and four times whilst undertaking a cognitive dual task, counting backwards in sevens. This arithmetic task was also completed in sitting. Results: For all gait variables there was a statistically significant difference between the groups. Subjects with DPN walked slower and with smaller steps compared to those with diabetes. In general the secondary task had a significant and adverse effect on the gait parameters and this effect was greater for those with DPN in both absolute and relative terms. Both groups had poorer arithmetic ability when walking compared to sitting. Discussion: Patients with DPN have different gait parameters to diabetic patients without neuropathy. Problems with divided attention when walking were more evident in the DPN group and may increase their risk of falls

    Topical 5% benzoyl peroxide and 3% erythromycin gel: Experience from 191 patients with inflammatory acne

    Background. A dramatic increase in bacterial resistance as a result of the use of topical antibiotics in acne has been recorded over the past 20 years. Aim. In this study we investigated the efficacy and safety of the topical treatment with 5% benzoyl peroxide and 3% erythromycin gel in patients with inflammatory acne. Materials and Methods. One hundred and ninety one patients with inflammatory acne were evaluated. The patients included 54 males and 137 females with a mean average age being 22.3Ā±8.1 years. The topical gel was applied on the face, once daily, for 3 months.Results. The mean number of non-inflammatory and inflammatory decreased. These findings were statistically significant from baseline with a mean percentage reduction of the non-inflammatory and inflammatory lesions, corresponding to 42.2% and 57.5% respectively. Conclusion. Topical 5% benzoyl peroxide and erythromycin 3% as monotherapy is efficient in the treatment of inflammatory acne

    Wells' Syndrome Associated with Churg-Strauss Syndrome

    Churg-Strauss syndrome (CSS) is a multisystem granulomatous vasculitis that is characterized by peripheral eosinophilia and the infiltration of eosinophils into systemic organs. The skin lesions of CSS consist mainly of palpable purpura and nodules. Wells' syndrome (WS) is a rare inflammatory dermatosis that is associated with recurrent granulomatous dermatitis and eosinophilia. Since these two diseases are rare, any overlap between them is very unusual. Herein, we report a patient with CSS, who initially presented a skin eruption of erythematous urticarial-plaques, vesicles, and blisters. Upon biopsy, the histology of these plaques indicated eosinophilic infiltration and "flame figures" within the dermis, which was consistent with a diagnosis of WS. Although the association between WS and CSS that was observed in our patient may be purely coincidental, it could also suggest a common pathogenetic background of these two distinct diseases, as both share several many common features

    Toxic Epidermal Necrolysis with Ocular Involvement Following Vaccination for Hemorrhagic Fever with Renal Syndrome

    We report a case of toxic epidermal necrolysis with ocular involvement following vaccination for hemorrhagic fever with renal syndrome. A healthy 20-year-old male soldier presented with confluent purpuric and erythematous dusky red macules evolving to flaccid blister and epidermal detachment on the whole body with conjunctival injection. The patient had no antecedent medical or surgical conditions except for two doses of hemorrhagic fever with renal syndrome vaccination. With supportive care, skin lesions were improved. Ophthalmic examinations revealed conjunctival injection with epithelial defects in both eyes. Ocular complications were resolved after amniotic membrane transplantation. Toxic epidermal necrolysis may be considered as a possible complication of hemorrhagic fever with renal syndrome vaccination

    Objectives: Agents targeting the epidermal growth factor receptor (EGFR)-mediated signalling pathway are increasingly being used for the treatment of advanced lung, pancreatic, colorectal and head and neck cancers. Case presentation: Here, we report the first case of eruptive seborrhoeic keratosis following panitumumab treatment, an anti-EGFR monoclonal antibody, in a 73-year-old patient with stage 4 (IV) colorectal cancer with hepatic metastasis. Conclusion: While panitumumab is an emerging therapy for RAS wild-type metastatic colorectal cancer, physicians should consider panitumumab as a potential cause of eruptive seborrhoeic keratosis

    Introduction: Turicella otitidis, described as a new species over 20 years ago, has been isolated mainly from the external ear canal and middle ear fluid. Here, we report the first case of palmoplantar eczema related to T. otitidis. Case presentation: Here, we report the first case of palmoplantar eczema in a 74-year-old female related to T. otitidis.
