    Assessing the outcome of orthognathic surgery by three-dimensional soft tissue analysis

    Studies of orthognathic surgery often focus on pre-surgical versus post-surgical changes in facial shape. In contrast, this study provides an innovative comparison between post-surgical and control shape. Forty orthognathic surgery patients were included, who underwent three different types of surgical correction: Le Fort I maxillary advancement, bilateral sagittal split mandibular advancement, and bimaxillary advancement surgery. Control facial images were captured from volunteers from local communities in Glasgow, with patterns of age, sex, and ethnic background that matched those of the surgical patients. Facial models were fitted and Procrustes registration and principal components analysis used to allow quantitative analysis, including the comparison of group mean shape and mean asymmetry. The primary characteristic of the difference in shape was found to be residual mandibular prognathism in the group of female patients who underwent Le Fort I maxillary advancement. Individual cases were assessed against this type of shape difference, using a quantitative scale to aid clinical audit. Analysis of the combined surgical groups provided strong evidence that surgery reduces asymmetry in some parts of the face such as the upper lip region. No evidence was found that mean asymmetry in post-surgical patients is greater than that in controls

    RFID-enabled warehouse optimization: lessons from early adopters in the 3PL industry

    This paper presents the impact of RFID technology on the picking and shipping processes of one RFID-enabled warehouse in the 3PL industry. The findings from our study confirm initial results from many studies where RFID implementation has been shown to enable business process redesign, improve data quality, real-time data collection and synchronization and enhance system integration. In this study we show that the full potential of RFID technology is dependent upon the involvement of all supply chain members involved in implementation. Moreover, firms considering implementing RFID technology need to take into account their investment in complementary assets such as employee training and knowledge development. The implication of this is that business process modeling and simulation is critical to ensure that stakeholders involved in a RFID project fully understand the impacts of integrating RFID technology on business processes

    A Governance Perspective for System-of-Systems

    The operating landscape of 21st century systems is characteristically ambiguous, emergent, and uncertain. These characteristics affect the capacity and performance of engineered systems/enterprises. In response, there are increasing calls for multidisciplinary approaches capable of confronting increasingly ambiguous, emergent, and uncertain systems. System of Systems Engineering (SoSE) is an example of such an approach. A key aspect of SoSE is the coordination and the integration of systems to enable ‘system-of-systems’ capabilities greater than the sum of the capabilities of the constituent systems. However, there is a lack of qualitative studies exploring how coordination and integration are achieved. The objective of this research is to revisit SoSE utility as a potential multidisciplinary approach and to suggest ‘governance’ as the basis for enabling ‘system-of-systems’ coordination and integration. In this case, ‘governance’ is concerned with direction, oversight, and accountability of ‘system-of-systems.’ ‘Complex System Governance’ is a new and novel basis for improving ‘system-of-system’ performance through purposeful design, execution, and evolution of essential metasystem functions.

    Liver pathomorphology of Mus musculus C57BL6 on atherogenic diet

    Atherosclerosis is one of the leading causes of disability and death worldwide. Liver plays a huge role in pathogenesis of atherogenic dislipidemia, development and progression of atherosclerotic lesions. We studied the effect of atherogenic diet on liver morphology in animal model of diet-induced atherosclerosis in mice Mus musculus C57BI6. This strain has a natural ability to develop atherosclerosis, while some other mouse stains has not. After 14 weeks on atherogenic diet a severe hepathomegaly (9% of body mass) and lobular structure deformation was found. We also observed signs of micro- and macrovesicular steatosis, cell apoptosis, fibrosis and inflammatory leukocyte infiltration. So, liver not only plays an important role in dislipidemia, but it is also a target-organ in lipid metabolism imbalance

    Evaluation of a novel immunoassay to detect p-Tau Thr127 in the CSF to distinguish Alzheimer disease from other dementias

    OBJECTIVE: To investigate whether p-tau T217 assay in cerebrospinal fluid (CSF) can distinguish Alzheimer's disease from other dementias and healthy controls. METHODS: We developed and validated a novel Simoa immunoassay to detect p-tau T217 in CSF. There was a total of 190 participants from three cohorts with AD (n = 77) and other neurodegenerative diseases (n = 69) as well as healthy subjects (n = 44). RESULTS: The p-tau T217 assay (cut-off 242 pg/ml) identified AD subjects with accuracy of 90%, with 78% positive predictive value (PPV), 97% negative predictive value (NPV), 93% sensitivity, 88% specificity compared favorably with p-tau T181 ELISA (52 pg/ml) showing 78% accuracy, 58% PPV, 98% NPV, 71% specificity, 97% sensitivity. The assay distinguished AD patients from age-matched healthy subjects (cut-off 163 pg/ml, sensitivity 98%, specificity 93%) similarly to p-tau T181 ELISA (cut-off 60 pg/ml, 96% sensitivity and 86% specificity). In AD patients, we found a strong correlation between p-tau T217-tau and p-tau T181, t-tau and Aβ40 but not with Aβ42. CONCLUSIONS: This study demonstrates that p-tau T217 displayed better diagnostic accuracy than p-tau T181. The data suggests that the new p-tau T217 assay has a potential as an AD diagnostic test in the clinical evaluation. CLASSIFICATION OF EVIDENCE: This study provides Class III evidence that a CSF immunoassay for p-tau T217 distinguishes AD from other dementias and healthy controls

    Perinatal depression in the Spanish context: consensus report from the general council of psychology of Spain

    Background: Perinatal depression is a major public health problem, with an estimated prevalence of approximately 15% during the first postpartum year. Despite the high prevalence of postpartum depression, there is no consensus regarding assessment and treatment in the Spanish context. The General Council of Psychology of Spain convened a working group of experts in the field, including both academics and healthcare professionals, to review and propose recommendations based on evidence and best practices that could be applied in the Spanish context. Method: A literature search was completed in various databases (e.g., Medline, PsychInfo) including a combination of terms related to peripartum depression (PPD) assessment and diagnosis, prevention, treatment, and cost-effectiveness. A narrative synthesis of the literature has been conducted together with a critical overview of PPD with a special focus on the Spanish context. Results: In this consensus report, developing questions including prevalence and assessment tools, comparative effectiveness of preventive and treatment interventions, and cost-effectiveness of PPD management have been analyzed. Conclusions: The General Council of Psychology of Spain network advocates the establishment of compulsory screening protocols in all the autonomous regions. Also, it is necessary to promote the inclusion in maternal education of programs for the promotion of mental well-being and selective/indicated prevention carried out by a psychology professional with specialized training in the area. This consensus document also promotes the presence of a psychology professional with specialized training in the area. Antecedentes. La depresión perinatal es un importante problema de salud pública, con una prevalencia estimada de aproximadamente el 15% durante el primer año posparto. A pesar de la gran prevalencia de depresión posparto (DPP), no existe consenso sobre la evaluación y el tratamiento en el contexto español. El Consejo General de Psicología de España convocó a un grupo de trabajo de expertos en la materia, tanto académicos como profesionales sanitarios, para revisar y proponer recomendaciones basadas en evidencias y mejores prácticas que puedan aplicarse en el contexto español. Método. Se realizó una búsqueda bibliográfica en varias bases de datos (p. ej., Medline, PsychInfo) que incluía una combinación de términos relacionados con la evaluación y el diagnóstico de la DPP, prevención, tratamiento y coste-eficacia. Se ha realizado una síntesis narrativa de la literatura junto con una visión crítica de la DPP con un enfoque especial en el contexto español. Resultados. Las preguntas de desarrollo que incluyen prevalencia y herramientas de evaluación, la eficacia comparativa de las intervenciones preventivas y de tratamiento y la relación coste-efectividad de la gestión de la PPD se han analizado en este informe de consenso. Conclusiones. La red del Consejo General de Psicología de España aboga por el establecimiento de protocolos obligatorios de cribado en todas las Comunidades Autónomas. Asimismo, es necesario promover la inclusión en la educación materna de programas de promoción del bienestar mental y la prevención selectiva indicada llevadas a cabo por un profesional de la psicología con formación especializada en el área

    Integrative Analysis of Predictors for Unfavorable Course of Parkinson Disease According to Medical Care Assessment: Cohort Retrospective Study

    Background. Parkinson's disease is a neurodegenerative disorder that leads to severe disability, especially in its advanced stages. Medical care should be performed in accordance with modern clinical recommendations and standards. Assessment of the quality of medical care in the Russian Federation is carried out via examination of medical documentation by insurance experts regarding compliance with Russian standards and other regulatory documents.Objective. To study the predictors for unfavorable course of Parkinson's disease based on routine examinations of the quality of medical care.Methods. The cohort retrospective study included 7,264 examinations of the medical care quality of 1,754 patients with Parkinson's disease for the period from 2011 to 2018 in the Russian Federation. The study focused on two indicators: presence/absence of disease progression and determination/aggravation of a degree of disability or maintenance of the previous social and professional activity. The obtained data were used to develop diagnostic tables with prognostic factors of Parkinson's disease according to A. Wald. Statistical analysis of the results was carried out using Statistica 13 (StatSoft, USA). Results. Diagnostic tables have been developed to determine the prognostic factors affecting the progression of Parkinson's disease and, accordingly, decreasing the degree of social and/ or professional activity and to assess the diagnostic factors when the risk of the disease aggravation will be more than 95%.Conclusion. The greatest progression of Parkinson's disease (p < 0.05) was observed in the cases of poor collection of complaints, history and clinical evaluation of the patient's condition, inadequate prescription or absence of prescription of levodopa when indicated, as well as defects in the correction of additional disorders. The likelihood of social disadaptation in patients with Parkinson's disease (p < 0.05) was found to be greater in the cases of poor collection of complaints and history, neurological examination and clinical evaluation of the patient's condition as well as inadequate prescription or absence of prescription of levodopa when indicated

    NMDAR inhibition-independent antidepressant actions of ketamine metabolites

    Major depressive disorder afflicts ~16 percent of the world population at some point in their lives. Despite a number of available monoaminergic-based antidepressants, most patients require many weeks, if not months, to respond to these treatments, and many patients never attain sustained remission of their symptoms. The non-competitive glutamatergic N-methyl-D-aspartate receptor (NMDAR) antagonist, (R,S)-ketamine (ketamine), exerts rapid and sustained antidepressant effects following a single dose in depressed patients. Here we show that the metabolism of ketamine to (2S,6S;2R,6R)-hydroxynorketamine (HNK) is essential for its antidepressant effects, and that the (2R,6R)-HNK enantiomer exerts behavioural, electroencephalographic, electrophysiological and cellular antidepressant actions in vivo. Notably, we demonstrate that these antidepressant actions are NMDAR inhibition-independent but they involve early and sustained α-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA) receptor activation. We also establish that (2R,6R)-HNK lacks ketamine-related side-effects. Our results indicate a novel mechanism underlying ketamine’s unique antidepressant properties, which involves the required activity of a distinct metabolite and is independent of NMDAR inhibition. These findings have relevance for the development of next generation, rapid-acting antidepressants

    Selection of reference genes for gene expression studies in ultraviolet B-irradiated human skin fibroblasts using quantitative real-time PCR

    <p>Abstract</p> <p>Background</p> <p>Reference genes are frequently used to normalise mRNA levels between different samples. The expression level of these genes, however, may vary between tissues or cells and may change under certain circumstances. Cytoskeleton genes have served as multifunctional tools for experimental studies as reference genes. Our previous studies have demonstrated that the expression of vimentin, one cytoskeletal protein, was increased in ultraviolet B (UVB)-irradiated fibroblasts. Thus, we examined the expression of other cytoskeleton protein genes, <it>ACTB </it>(<it>actin, beta</it>), <it>TUBA1A </it>(<it>tubulin, alpha 1a</it>), and <it>TUBB1 </it>(<it>tubulin, beta 1</it>), in human dermal fibroblasts irradiated by UVB to determine which of these candidates were the most appropriate reference genes.</p> <p>Results</p> <p>Quantitative real-time PCR followed by analysis with the NormFinder and geNorm software programmes was performed. The initial screening of the expression patterns demonstrated that the expression of <it>VIM </it>was suppressed after UVB irradiation at doses ≥25 mJ/cm<sup>2 </sup>and that the expression of <it>TUBA1A </it>was significantly reduced by UVB doses ≥75 mJ/cm<sup>2 </sup>in cultured human dermal fibroblasts. The analysis of the experimental data revealed <it>ACTB </it>to be the most stably expressed gene, followed by <it>GAPDH </it>(<it>aglyceraldehyde-3-phosphate dehydrogenase</it>), under these experimental conditions. By contrast, <it>VIM </it>was found to be the least stable gene. The combination of <it>ACTB </it>and <it>TUBB1 </it>was revealed to be the gene pair that introduced the least systematic error into the data normalisation.</p> <p>Conclusion</p> <p>The data herein provide evidence that <it>ACTB </it>and <it>TUBB1 </it>are suitable reference genes in human skin fibroblasts irradiated by UVB, whereas <it>VIM </it>and <it>TUBA1A </it>are not and should therefore be excluded as reference genes in any gene expression studies involving UVB-irradiated human skin fibroblasts.</p
