    An Integrated Management System for a Medical Center

    Recently, the use of technology in medicine and healthcare has increased, particularly during the pandemic period. This adaptation has shown to be fruitful in many scenarios. Implementing a computerized system to manage daily tasks like handling patient records, appointments and other administrative tasks can result in an efficient system. This will in turn result in a better experience for the patients as well as the staff members. This research paper documents the development process of creating an online web application for an existing manual medical center management system. The previous system made simple and frequent tasks like retrieving laboratory results or checking a doctor’s appointment a time-consuming hassle. Further, the requirement of a digitized system to track the medical staff was evident as many incorrect appointments and patient records were detected. This proposed system provides the benefits of appointment management, laboratory management, staff management, payment management. This solution makes it easier to check staff profiles and schedule appointments for patients, preventing making an appointment for a doctor on a day. Additionally, the laboratory division may post reports and this system manages all the patient payment details. The proposed system's functionalities will next be discussed, along with the testing methodologies and outcomes

    Tobacco Smoking Among School Children in Colombo District, Sri Lanka.

    Tobacco smoking is an important problem among schoolchildren. The authors studied the patterns of tobacco smoking among schoolchildren in Colombo, Sri Lanka, using a self-administered questionnaire. Multistaged stratified random sampling was used to select 6000 students. Response rate was 90.7% (5446), out of which 53.4% were males. Prevalence rates for males and females, respectively, were as follows: having smoked at least 1 complete cigarette: 27.0% and 13.3%, smoked more than 100 cigarettes: 2.3% and 0.3%, daily smoking: 1.8% and 0.2%. Mean age of starting to smoke was 14.16 years. The tobacco products most used were cigarettes (91.5%) and bidis (3.8%). In univariate analysis, male gender, parental smoking, studying non-science subjects, peer smoking, and participating in sports were significantly associated with smoking of at least 1 complete cigarette (P < .05). In multivariate analysis, the most significant correlates were having close friends (odds ratio = 3.29, confidence interval = 2.47-4.37) or parents who smoked (odds ratio = 1.86, confidence interval = 1.28-2.71).     Sharps injuries among medical students in the faculty of medicine, Colombo, Sri Lanka

    Introduction: Medical students undertake clinical procedures which carry a risk of sharps injuries exposing them to bloodborne infections. Objectives: To study the prevalence and correlates of sharps injuries among 4th-year medical students in the Faculty of Medicine, University of Colombo, Sri Lanka. Materials and Methods: The survey was conducted among 4th-year medical students to find out the incidence of injuries during high-risk procedures, associated factors and practice and perceptions regarding standard precautions. A self-administered questionnaire was administered to a batch of 197 4th-year medical students. Results: A total of 168 medical students responded. One or more injury was experienced by 95% (N = 159) of the students. The majority (89%) occurred during suturing; 23% during venipuncture and 14% while assisting in deliveries. Most of the incidents (49%) occurred during Obstetrics and Gynecology attachments. Recapping needles led to 8.6% of the injuries. Thirty-five percent of students believed they were inadequately protected. In this group, adequate protection was not available in 21% of the incidences and 24% thought protection was not needed. Following the injury, 47% completely ignored the event and only 5.7% followed the accepted post-exposure management. Only 34% of the students knew about post-exposure management at the time of the incident. Only 15% stated that their knowledge regarding prevention and management was adequate. The majority (97%) believed that curriculum should put more emphasis on improving the knowledge and practice regarding sharps injuries. Conclusions: The incidence of sharps injuries was high in this setting. Safer methods of suturing should be taught and practiced. The practice of standard precautions and post-injury management should be taught

    Changes in Cortical Directional Connectivity during Difficult Listening in Younger and Older Adults

    One way to investigate the mechanisms that underlie speech comprehension under difficult listening conditions is via cortical connectivity. The innovative Network Localized Granger Causality (NLGC) framework was applied to magnetoencephalography (MEG) data, obtained from older and younger subjects performing a speech listening task in noisy conditions, in delta and theta frequency bands. Directional connectivity between frontal, temporal, and parietal lobes was analyzed. Both aging- and condition-related changes were found, particularly in theta. In younger adults, as background noise increased, there was a transition from predominantly temporal-to-frontal (bottom-up) connections, to predominantly frontal-to-temporal (top-down). In contrast, older adults showed bidirectional information flow between frontal and temporal cortices even for speech in quiet, not changing substantially with increased noise. Additionally, younger listeners did not show changes in the nature of their cortical links for different listening conditions, whereas older listeners exhibited a switch from predominantly facilitative links to predominantly sharpening, when noise increased.https://doi.org/10.1101/2023.05.19.54150

    Attention Mobilization as a Modulator of Listening Effort: Evidence from Pupillometry

    Listening to speech in noise can require substantial mental effort, even among younger normal-hearing adults. The task-evoked pupil response (TEPR) has been shown to track the increased effort exerted to recognize words or sentences in increasing noise. However, few studies have examined the trajectory of listening effort across longer, more natural, stretches of speech, or the extent to which expectations about upcoming listening difficulty modulate the TEPR. Seventeen younger normal-hearing adults listened to 60-s-long audiobook passages, repeated three times in a row, at two different signal-to-noise ratios (SNRs) while pupil size was recorded. There was a significant interaction between SNR, repetition, and baseline pupil size on sustained listening effort. At lower baseline pupil sizes, potentially reflecting lower attention mobilization, TEPRs were more sustained in the harder SNR condition, particularly when attention mobilization remained low by the third presentation. At intermediate baseline pupil sizes, differences between conditions were largely absent, suggesting these listeners had optimally mobilized their attention for both SNRs. Lastly, at higher baseline pupil sizes, potentially reflecting over-mobilization of attention, the effect of SNR was initially reversed for the second and third presentations: participants initially appeared to disengage in the harder SNR condition, resulting in reduced TEPRs that recovered in the second half of the story. Together, these findings suggest that the unfolding of listening effort over time depends critically on the extent to which individuals have successfully mobilized their attention in anticipation of difficult listening conditions
