    The impact of insulin resistance, dyslipidemia and high sensitivity C-reactivity protein on carotid intima-media thickness in metabolic syndrome

    Background: Carotid intima-media thickness (CIMT) is a strong predictor of cardiovascular events and associated with metabolic syndrome (MetS). The CIMT has been widely used as one of the parameters of atherosclerosis. The aim of the study was to evaluate the impact of insulin resistance, dyslipidemia and high sensitivity C-reactive protein on carotid intima-media thickness in metabolic syndrome patients of Western Maharashtra as very sparse data is available.Methods: It was a cross-sectional study of 400 adults (200 cases and 200 control), 18-50 years of age, both the sexes randomly selected from diabetes and obesity OPD at tertiary care hospital. Diagnosis of metabolic syndrome was done according to modified NCEP adult treatment panel III criteria. CIMT was measured by B mode ultrasound (Philips HT-11, Color Doppler), hs-CRP by ELISA method (Cal biotech). Insulin resistance by HOMA-IR (Homeostatic model assessment of insulin resistance). The predictors of CIMT with various variables were studied by multiple linear regression analysis.Results: We found significant increase in CIMT (0.7895±0.110, p<0.001) in MetS and a positive correlation of CIMT with age, waist to hip ratio, triglyceride levels and systolic blood pressure (p<0.001).Conclusions: Increased carotid intima-media thickness in metabolic syndrome may increase the risk of having a stroke and cardiovascular mortality. It was considered an early deterioration in the arterial intima and is a preclinical stage of atherosclerosis. Early diagnosis and prevention may help to reduce the risk of stroke and cardiovascular mortality.

This research intends to provide a detailed description of NaĂŻve Bayes and decision tree classifier that are applied in our research particularly in the prediction of Heart Disease. Some experiment has been conducted to compare the execution of predictive data mining technique on the same dataset, and the consequence reveals that Decision Tree outperforms over Bayesian classification

    Safety evaluation of directly observed treatment short course (DOTS) regimen in a tertiary care hospital, Pune

    Background: Directly observed treatment short course (DOTS) is a cornerstone of Revised National Tuberculosis Control Program of India. Adverse drug reactions (ADRs) induced by this therapy is common and it causes significant morbidity and mortality. Hence, the present study was undertaken to determine the incidence and pattern of ADRs and to assess causality and severity.Methods: We conducted prospective, observational study at DOTS center of tertiary care hospital, Pune. 150 pulmonary tuberculosis patients undergoing DOTS therapy were enrolled. They were monitored weekly in an intensive phase and monthly in the continuation phase. The suspected ADRs were recorded and assessed for causality and severity by standard algorithms.Results: Incidence of ADRs due to DOTS was 19.33% & total 35 ADRs had occurred in our study. Gastrointestinal intolerance, arthralgia & itching with or without rashes were most common ADRs (incidence rates: 12.67%, 2.67% and 2.67%, respectively). On evaluation of causality by Naranjo algorithm, majority of ADRs 91.43% were “possible.” As per WHO- Uppsala Monitoring Center scale, majority of ADRs 91.43% were “possible.” As per Modified Hartwig and Siegel scale, majority of ADRs were “moderate” (48.57%) but 8.57% were “severe.” Female gender was found to be a significant risk factor for developing ADRs (odds ratio: 3.08, 95% confidence interval: 1.33-7.12. 3.33%). ADRs & hepatotoxicity was major reason for defaulting from DOTS (60%).Conclusion: ADRs induced by DOTS are common and there is need of incorporating pharmacovigilance system for this vital public health program. Counseling of patients for timely prevention, detection, and management of ADRs will help in minimizing the further occurrence of ADRs

    Enhancement of reactivity and increased usage of low lime class -F-fly ash-possible avenues

    The low lime class-F fly ash available in the country shows high degree of variability in the quality, higher content of crystallites , lower glassy phase which accounts for lower of usage in cement and concrete . The time reactivity test used for assessing the pozzolanicity of fly ash did not always correlate with its observed reactivity in Blended cements . An alternative rapid alkali reactivity rest developed at the authors ' laboratory is illustrated in the paper. The paper also discusses the possibility of increasing the reactivity of fly ash and effect of the reactive fly ash on characteristics of PPC and concrete. The paper further discusses other avenues of fly ash utilisation, which could be categorised as low, medium and high value applications. One of such applications developed at the authors ' laboratory that merits special interest, is the Hydrogel process of clinkerisation , which has a potential for utilisation of 20-30% fly ash as a raw material in cement manufacture

    Good girls and boys: findings from a cross-sectional survey on adolescent rights, relationships, and sexuality in an urban informal settlement in India

    Around 20% of India’s population are adolescents aged 10–19 years. Our objective was to strengthen program interventions on gender equity, health, and participation by gauging adolescents’ levels of understanding and opinions. In a cross-sectional survey, we interviewed 2005 adolescents on their opinions on rights, friendship and sex, sexual refusal and coercion, and communication with family, using a two-stage probability proportional to size sample. Opinions on gender allocations were generally equitable, although females supported clothing proscriptions. Premarital sex, multiple partners, masturbation and non-heterosexual partnerships were frowned upon. Few respondents said that they felt pressure to be sexually active, 79% said that sexual coercion was a form of violence, but 14% of older adolescents said that it would be unreasonable to refuse sex. Our interviews described young people negotiating the terrain between perceived normative expectations and contemporary aspirations, showing limited manoeuvring within assumed gender roles in which family control was prominent

    An overview of Viscosity Solutions of Path-Dependent PDEs

    This paper provides an overview of the recently developed notion of viscosity solutions of path-dependent partial di erential equations. We start by a quick review of the Crandall- Ishii notion of viscosity solutions, so as to motivate the relevance of our de nition in the path-dependent case. We focus on the wellposedness theory of such equations. In partic- ular, we provide a simple presentation of the current existence and uniqueness arguments in the semilinear case. We also review the stability property of this notion of solutions, in- cluding the adaptation of the Barles-Souganidis monotonic scheme approximation method. Our results rely crucially on the theory of optimal stopping under nonlinear expectation. In the dominated case, we provide a self-contained presentation of all required results. The fully nonlinear case is more involved and is addressed in [12]

    Renormalization: a quasi-shuffle approach

    In recent years, the usual BPHZ algorithm for renormalization in perturbative quantum field theory has been interpreted, after dimensional regularization, as a Birkhoff decomposition of characters on the Hopf algebra of Feynman graphs, with values in a Rota-Baxter algebra of amplitudes. We associate in this paper to any such algebra a universal semi-group (different in nature from the Connes-Marcolli "cosmical Galois group"). Its action on the physical amplitudes associated to Feynman graphs produces the expected operations: Bogoliubov's preparation map, extraction of divergences, renormalization. In this process a key role is played by commutative and noncommutative quasi-shuffle bialgebras whose universal properties are instrumental in encoding the renormalization process
