How Much Margin Is Left for Degrading Agricultural Soils? The Coming Soil Crises
Agricultural soils are in peril. Multiple lines of observational and empirical evidence suggest that we are losing the world’s fertile soils at an alarming rate, worsening the on-going global food crisis. It is increasingly clear that the risk of soil crises driven by erratic precipitation, warming air, and farming mismanagement is coming sooner rather than later. At this critical time, society cannot avoid looking for ways to curb soil crises. We argue that now is the right time for science-based mitigation strategies and new insights to protect soils. We offer four research priority areas that society needs to address. Arresting and reversing the ongoing soil degradation are tantamount to safeguarding humanity and the environment. To the extent that we continue to treat soil crises as a problem for farmers only—not as a global challenge—we only escalate the scale to which the problem will grow in time and complexity
Vancomycin sensitivity of Staphylococcus aureus isolates from hospital patients in Karachi, Pakistan
Methicillin-resistant Staphylococcus aureus (S. aureus) (MRSA), resistant to all antibiotics including Vancomycin, has been reported in Japan, USA, Canada and Brazil. Hence, the main objective of this study was to evaluate the possible presence of Vancomycin resistant or intermediate S.aureus in Karachi. A total of 850 clinical isolates were collected from two civil hospitals in the city between February 2006 and January 2007. They were identified using standard bacteriological methods.Sensitivity to recommended antibiotics was determined by disc diffusion, agar dilution, and E-test quantitative minimum inhibitory concentration (MIC). Susceptibility to natural or semi-natural products was determined by the agar dilution method. Out of 850 isolates, 250 were MRSA, of which 22% were resistant to 4 µg/ml Vancomycin, 24% to 8 µg/ml, 15.2% to 16 µg/ml, 10% to 20 µg/ml, and 13.2% to 30 µg/ml; the remaining 15.6% were sensitive to all used concentrations. Although we did not detect any Vancomycin-resistant S. aureus (VRSA), we found that 13% of the strains were intermediates (VISA), i.e. resistant to 30 µg/ml of Vancomycin. Because of the continuously increasing prevalence of VISA, it is imperative to minimize the use of Vancomycin. Indeed, the drug should only be prescribed for the treatment of documented, culture-proven infections with MRSA that are not susceptible to routine or alternative agents. This should help avoid the consequences of the development of Vancomycin resistant S. aureus (VRSA) in our environment
Towards Estimation of Emotions From Eye Pupillometry With Low-Cost Devices
Emotional care is important for some patients and their caregivers. Within a clinical or home care situation, technology can be employed to remotely monitor the emotional response of such people. This paper considers pupillometry as a non-invasive way of classifying an individual's emotions. Standardized audio signals were used to emotionally stimulate the test subjects. Eye pupil images of up to 32 subjects of different genders were captured as video images by low-cost, infrared, Raspberry Pi board cameras. By processing of the images, a dataset of pupil diameters according to gender and age characteristics was established. Appropriate statistical tests for inference of the emotional state were applied to that dataset to establish the subjects' emotional states in response to the audio stimuli. Results showed agreement between the test subjects' opinions of their emotional state and the classification of emotions according to the range of pupil diameters found using the described method
Representation of people of South Asian origin in cardiovascular outcome trials of glucose-lowering therapies in Type 2 diabetes
Aims : Our aim was to investigate the proportional representation of people of South Asian origin in cardiovascular outcome trials of glucose-lowering drugs or strategies in Type 2 diabetes, noting that these are among the most significant pieces of evidence used to formulate the guidelines on which clinical practice is largely based. Methods : We searched for cardiovascular outcome trials in Type 2 diabetes published before January 2015, and extracted data on the ethnicity of participants. These were compared against expected values for proportional representation of South Asian individuals, based on population data from the USA, from the UK, and globally. Results : Twelve studies met our inclusion criteria and, of these, eight presented a sufficiently detailed breakdown of participant ethnicity to permit numerical analysis. In general, people of South Asian origin were found to be under-represented in trials compared with UK and global expectations and over-represented compared with US expectations. Among the eight trials for which South Asian representation could be reliably estimated, seven under-represented this group relative to the 11.2% of the UK diabetes population estimated to be South Asian, with the representation in these trials ranging from 0.0% to 10.0%. Conclusions : Clinicians should exercise caution when generalizing the results of trials to their own practice, with regard to the ethnicity of individuals. Efforts should be made to improve reporting of ethnicity and improve diversity in trial recruitment, although we acknowledge that there are challenges that must be overcome to make this a reality
Genotyping and drug resistance patterns of M. tuberculosis strains in Pakistan
<p>Abstract</p> <p>Background</p> <p>The incidence of tuberculosis in Pakistan is 181/100,000 population. However, information about transmission and geographical prevalence of <it>Mycobacterium tuberculosis </it>strains and their evolutionary genetics as well as drug resistance remains limited. Our objective was to determine the clonal composition, evolutionary genetics and drug resistance of <it>M. tuberculosis </it>isolates from different regions of the country.</p> <p>Methods</p> <p><it>M. tuberculosis </it>strains isolated (2003–2005) from specimens submitted to the laboratory through collection units nationwide were included. Drug susceptibility was performed and strains were spoligotyped.</p> <p>Results</p> <p>Of 926 <it>M. tuberculosis </it>strains studied, 721(78%) were grouped into 59 "shared types", while 205 (22%) were identified as "Orphan" spoligotypes. Amongst the predominant genotypes 61% were Central Asian strains (CAS ; including CAS1, CAS sub-families and Orphan Pak clusters), 4% East African-Indian (EAI), 3% Beijing, 2% poorly defined TB strains (T), 2% Haarlem and LAM (0.2). Also TbD1 analysis (<it>M. tuberculosis </it>specific deletion 1) confirmed that CAS1 was of "modern" origin while EAI isolates belonged to "ancestral" strain types.</p> <p>Prevalence of CAS1 clade was significantly higher in Punjab (P < 0.01, Pearsons Chi-square test) as compared with Sindh, North West Frontier Province and Balochistan provinces. Forty six percent of isolates were sensitive to five first line antibiotics tested, 45% were Rifampicin resistant, 50% isoniazid resistant. MDR was significantly associated with Beijing strains (P = 0.01, Pearsons Chi-square test) and EAI (P = 0.001, Pearsons Chi-square test), but not with CAS family.</p> <p>Conclusion</p> <p>Our results show variation of prevalent <it>M. tuberculosis </it>strain with greater association of CAS1 with the Punjab province. The fact that the prevalent CAS genotype was not associated with drug resistance is encouraging. It further suggests a more effective treatment and control programme should be successful in reducing the tuberculosis burden in Pakistan.</p
Filling the Knowledge Gap in Diabetes Management During Ramadan: the Evolving Role of Trial Evidence
Postprandial hyperglycaemia: Red herring or red flag?
Several studies have suggested that postprandial rather than fasting hyperglycaemia may be a major determinant of cardiovascular risk. This article reviews the importance of postprandial hyperglycaemia and explores the benefits and risks of managing postprandial hyperglycaemia aggressively in order to prevent diabetes-related complications
